Follow-up investigation of reconstruction of the alveolar process in the atrophic mandible.

In this article the results are presented of the reconstruction of the alveolar process with iliac crest bone grafts in 14 patients. In all cases in a second operation a vestibuloplasty and a floor-of-mouth plasty was done. After an average follow-up period of 39 months, almost complete resorption of the bone grafts was observed. The clinical results varied. To obtain better results and avoid complications like pain at the donor and graft sites, mental nerve disturbances and other major problems associated with bone grafting, we prefer to perform the visor osteotomy in the future.

[Physicochemical research on the bone tissue of the alveolar process in experimental periodontitis].

The thermal behavior of inorganic and organic components of the osseous tissue of the mandible alveolar process from experimental animals was first examined in health, elementary periodontitis and during its magnetic laser therapy. The derivatograms of the diseased osseous tissue were characterized by shifted maximum of the thermal effects observed towards the high temperature region and new peaks at 390 and 500 degrees C, which showed that highly boiling polypeptide fractions resistant to oxidation were prevalent in the organic matrix of osseous tissue. The abnormal process in the osseous tissue was largely explained by the conformational rearrangement of protein molecules, which in turn causing changes in their functional properties associated with formation of insoluble inorganic phosphate of routine composition and structure. As a result of magnetic laser therapy, thermic transformations in the examined samples followed the scheme that was close to thermal analysis of intact osseous tissue.

[The clinical picture and surgical treatment of the residual defects and deformities of the maxillary alveolar process after cheilo- and uranoplasty].

The authors have developed a new working classification of residual defects of the maxillary alveolar process, anterior segment of the hard palate and oronasal anastomosis, based on the results of cheilo- and uranoplasty in 125 patients. The choice of present-day methods of surgery is based on the suggested working classification scheme. The known methods of surgical management of the residual defects are described and new methods developed by the authors suggested, permitting an effective shortening of the terms of surgical rehabilitation and yielding stable functional and aesthetic results.

[Microphotometric research on roentgenograms of the alveolar processes in chronic gingivitis in children and adolescents].

Microphotometric examination of roentgenograms has shown a progressive reduction of mineral salt levels in alveolar process bone tissue in chronic, catarrhal, and hypertrophic gingivitis in children and adolescents. The authors recommend including calcium chloride magnetophoresis in multiple-modality treatment of chronic gingivitis patients. This treatment modality stimulates metabolic processes in the tissues at the expense of alternating magnetic field effect and attenuates the inflammatory reaction due to administration of calcium salts.

[Abnormal inclinations of the alveolar process; incidence of total forms].

Statistics from 1500 orthodontic cases confirm the frequency of total clinical forms of inclination abnormalities of the alveolar processes. The results obtained emphasize the importance of teleradiography of the head in a vertical position, the X-rays being directed perpendicularly in the direction of the plane of bite of the molars, completing in a transverse direction the morphological examinations begun by teleradiographies of the side of the face.

The "FAD" (Floating Alveolar Device): a bidirectional distraction system for distraction osteogenesis of the alveolar process.

PURPOSE: The following study illustrates preoperative and perioperative vector management in alveolar distraction using a new distraction system--the "Floating Alveolar Device" (FAD). The FAD is a bidirectional alveolar distractor that allows augmentation of an atrophic alveolar process in several planes, assures easy intraoperative positioning of the planned vector of distraction, and provides correction of the horizontal position of the transported segment during and after vertical distraction. PATIENTS AND METHODS: The FAD is composed entirely of stainless steel and has the following basic components: an upper member, a distraction rod, a lower base plate supporting the vertical force of the distraction rod, a jointed hinge that connects the upper and the lower members, and a tightening rod that provides blocking of the hinge. The clinician can manipulate and adjust the tightening rods, allowing a change in the angle of the hinge, thereby altering the transverse dimension of the vector of distraction. A total of 4 patients aged to 19 to 40 years underwent bidirectional alveolar distraction. All procedures were performed in the mandible. RESULTS: In all treated patients, planned distraction height and direction were achieved. In all cases it was possible to place implants at the planned time. CONCLUSIONS: The most common complication, axial displacement, is easily eliminated by moving the bone with the "floating" rod of the FAD during or immediately after the distraction period, according to the principles of the floating bone concept.

Secondary bone-grafting for repair of residual cleft defects in the alveolar process and hard palate. A new surgical technique.

A new surgical technique for secondary bone-grafting for closure of residual clefts in the alveolar process and hard palate is described. The technique improves the anatomy in the cleft region and, thus improves the possibility of total dental rehabilitation. The treatment results based on experiences with 293 patients will be published in a future paper, and must be estimated as promising. The technique minimizes hospitalization as well as post-operative discomfort.

Fracture of the alveolar process in a 2-year-old child: a report of an unconventional immobilization.

This is a report of fracture of the maxillary anterior segment of the alveolar process of an energetic 2-year-old child. The fracture was reduced under general anesthesia, and the fractured segment was immobilized using only 2-0 chromic gut in order to avoid a second surgical episode to remove traditional fixation after healing. The patient was followed for one year and healed well with no evidence of the lack of vitality of the involved teeth.

[A nonpigmented melanoblastoma of the mandibular alveolar process].

A 64-year-old male patient consulted a doctor for a tumor-like formation in the left submandibular area. The dentist detected ulceration of the alveolar process from the V to the VII tooth. Histological examination diagnosed pigment-free melanoblastoma, a tumor extremely rare in the oral cavity; the prognosis in such a tumor is much worse than in skin melanomas. This case confirms a viewpoint that tumors in the oral cavity are difficult to diagnose and they are usually diagnosed after they manifest themselves by metastases.

Surgery of the alveolar process as related to prosthesis.

The healing of an extraction wound is taken as starting-point, and complications and their avoidance are discussed insofar as they affect the shape of the alveolar process. Surgical methods of treating cysts and other tumours are reviewed, and it is shown how these may be carried out with little damage to the form of the denture-base. Every dentist should bear in mind subsequent prosthetic care whenever undertaking surgery of the oral cavity.

[The use of a cyanoacrylate-based composite material in operations on the alveolar processes of the jaws].

Improvement of methods for preventing inflammatory complications after surgery on the jaw bones and induction of predictable regeneration of bone tissue at the site of intervention is an important problem. Numerous experimental and clinical studies in Russia and abroad demonstrated the efficacy of biodestroyed materials on the basis of rapidly polymerized monomers--cyanacrylates. The products of polycyanacrylate biodestruction are nontoxic and are not accumulated in the organism. Addition of antiseptics and antibiotics in sufficient amounts to polycyanacrylates prevents the development of inflammatory processes at the site of the composite application. A new Russian composite material MK-9M was used in 44 patients operated on the alveolar process bone at Department of Oral and Maxillofacial Surgery of Moscow Medical Stomatological Institute in 1997-1998. Pronounced anti-inflammatory and osteoregenerating effect was attained in all cases.

[Changes in compensatory inclination of the alveolar processes (author's transl)].

When facial growth occurs in a manner outside general rules, progressive adaptation of the dental system to slow modification in the anatomical relations of the upper and lower jaws occurs by means of progressive compensatory inclination of the alveolar processes. This results, according to the direction of movement, in an increase or decrease in the space available for the teeth. In the two cases presented as examples, teleradiography of the head in lateral and vertical views demonstrated changes in the length and width of the dental arcades.
