A nursing partnership: the forces of magnetism guiding evidence-based practice in the Republic of Armenia.

A team of nurses at North Shore University Hospital in Manhasset, New York, has been participating in a global collaboration with nurses at Erebouni Medical Center in Yerevan, Armenia. During this collaboration, they have built on work initiated by representatives of the American International Health Alliance and the American Nurses Credentialing Center (ANCC) to develop application of evidence-based practice in the Armenian nurse's work environment. Their collective efforts have resulted in initiating nursing assessments and plans of care, position descriptions and performance appraisals, competency assessment tools, and performance improvement tools. Overall, the respect for nurses at Erebouni Medical Center has significantly improved. In October 2004, nurses at Erebouni Medical Center were recognized by the ANCC with the Journey to Nursing Excellence Award.

Telemedicine and international disaster response. Medical consultation to Armenia and Russia via a Telemedicine Spacebridge.

UNLABELLED: The Telemedicine Spacebridge, a satellite-mediated, audio-video-fax link between four United States and two Armenian and Russian medical centers, permitted remote American consultants to assist Armenian and Russian physicians in the management of medical problems following the December 1988 earthquake in Armenia and the June 1989 gas explosion near Ufa. METHODS: During 12 weeks of operations, 247 Armenian and Russian and 175 American medical professionals participated in 34 half-day clinical conferences. A total of 209 patients were discussed, requiring expertise in 20 specialty areas. RESULTS: Telemedicine consultations resulted in altered diagnoses for 54, new diagnostic studies for 70, altered diagnostic processes for 47, and modified treatment plans for 47 of 185 Armenian patients presented. Simultaneous participation of several US medical centers was judged beneficial; quality of data transmission was judged excellent. CONCLUSION: These results suggest that interactive consultation by remote specialists can provide valuable assistance to on-site physicians and favorably influence clinical decisions in the aftermath of major disasters.

Airborne field hospital in disaster area: lessons from Armenia (1988) and Rwanda (1994).

The outcome of survivors within disaster areas largely depends upon the quick reallocation and operation of logistic and medical support systems. Enthusiastic media equipped with advanced communication systems, reveal mass human suffering in real time. But, the response period required for the organization of rescue systems is much slower and is most frustrating. In this article, we present our experience in quick deployment and operation of airborne field hospitals gained following the earthquake disaster in Armenia in 1988 and the civil war in Rwanda in 1994. Deployment of improvised, volunteer-based, military field hospitals was feasible within 24 hours after the decision was made. A multi-disciplinary structure enabled an effective, flexible mode of operation and reduced the dependency on mericulous, time-consuming assessments of requirements prior to deployment. These missions are a paradigm for the successful incorporation and integration within the capabilities of military infrastructure of volunteer professionals drafted from civil medical facilities. Such field hospitals could provide backup for primary care medical systems in disaster areas and substitute or take some pressure off of local hospitals, particularly when evacuation systems are insufficient.

[Efficiency of tuberculosis control measures and their impact on epidemiological situation in Armenia in 1987 and 1997].

Examining epidemiological and qualitative data on the prevention, detection, and treatment tuberculosis in Armenia in 1987 and 1997 revealed 58.6 and 24.0%, rises in tuberculosis incidence and prevalence, respectively; its mortality has increased by 2.5 times. The qualitative indices have become worse: the proportion of destructive pulmonary tuberculosis has risen from 58.0 to 62.4%, cavernous closure has decreased from 78.0 to 57.8%, and cessation of bacterial isolation lowered from 85.0 to 74.5%. Social and organizational reasons of the above worse indices are presented. A package of tuberculosis control measures, which has been adjusted to the present conditions, is recommended.

Health care legislative reforms in Armenia: preparations for a purchaser-provider split.

Armenia, the former Soviet republic, is switching its economy to a more market driven system. Where health care is concerned, the previous government planned to introduce an independent State Health Agency managing the available funds. At the same time, this Agency would be authorised to contract with former state hospitals and individual providers. The underlying idea was a partial withdrawal by the state from both the provision and financing of health care. However, since the financing system continued to be based on general taxation, the state's role has remained largely unchanged in this respect. This situation has created new difficulties. To solve the variety of emerging problems, the Armenian government requested technical support from the World Bank. As a member of a multi-disciplinary team, the author will describe some major legal aspects of the underlying health policy reform plan and will conclude that the Agency's establishment will give an important impetus to the Armenian health care legislative reform process.

[Cytogenetic features of the blood-sucking blackfly Wilhelmia paraequina Puri (Diptera: Simuliidae) from Armenia].

Three blackfly Wilhelmia paraequina populations of Armenia (rivers Debet and Megriget, channel Megri) were studied. 2n = 6: IS + IIL, IIS + IL, IIIS + IIIL. Cytological maps of polytene chromosomes were constructed. High inversion polymorphism (95.63%) was observed, with 2.6 inversions per individual. Three inversions proved to be associated with male development: Y1 was characterized by a combination of two heterozygous inversions, IIL-3 + IIL-1, while Y2 had another combination of heterozygous inversions, IIL-5 + IIL-3. The X chromosome had a standard homozygous IIL disk sequence. The populations were shown to be similar in autosomal polymorphism. A tendency for differentiation was observed with respect to frequencies and types of sex-linked inversions: the sex determination system was Y1X-XX in the Debet and Megriget populations and Y2X-XX in the channel (Megri) population. On the strength of these findings, one W. paraequina morphotype was assumed to involve two cytotypes, A (Debet and Megriget populations) and B (Megri population).

[Regulatory issues of biologically active supplements in Armenia].

Since 1996 by Drug and Medical Technology Agency of the Ministry of Health of the Republic Armenia there has been a record of biologically active supplements, and up to now it numbers 61 products. There has been created a database on biologically active supplements containing relevant information on producers, composition of the product, indications for applying etc. There have been cited some data on examining biologically active supplements and on criteria of such products appraisal. The parameters of not approved Dietary supplements Applications are included and the requirements for Dietary supplements Labels are described in it. There has been cited the division of registration biologically active supplements as per active ingredients and recommendations on use. According to the results of special questionnaire for the Yerevan policlinic doctors, there have been analyzed the spreading area and dynamics biologically active supplements prescription Also there has been estimated the adequacy of their prescription and doctors opinion this product.

Nutritional needs surveys among the elderly--Russia and Armenia, 1992.

The ongoing social, political, and economic changes in the 15 republics of the former Soviet Union have resulted in hyperinflation of the Soviet Union ruble (SUR), regional conflicts, and other hardships for the populations of these republics (1). In January 1992, a public health assessment in Russia indicated that the elderly--most of whom subsist on fixed incomes and among whom the prevalences of decreased mobility or chronic illnesses may be substantial--are at greatest risk because of declining social support. During March-May 1992, CARE, in collaboration with CDC, conducted three surveys in Russia and Armenia to assist in targeting the delivery of food and medical humanitarian aid to the most needy among the elderly. This report summarizes findings from these surveys.

[Exploration of natural foci of tularemia and plague in Armenia using the serological examination of bird droppings and excrements of predatory mammals].

Forty strains of tularemia and 619 of plague microbes were isolated in 1974 in bacteriological examination of natural plague and tularemia foci from a great number of small mammals and their ectoparasites. At the same time in serological examination (in the antibody neutralization test) of bird pellets, 52 mummified cadavers, and 34 excretion samples of mammalian beasts of prey collected in Armenia (its central and North-Western part) in 1973 the antigen of tularemia microbe was revealed in 73, 8, and 3, and of plagye--in 42, 5, and 1 cases, respectively. In one of the sites examined the number of positive findings failed to exceed 10--17%, this indicating a low intensity of the epizootic in the majority of the places. Judging by the mean titres of the serological test, which varied from 1:12 to 1:1428 in examination for tularemia and from 1:12 to 1428 in examination for plague, it was possible to detect both epizootics coursing during the examination, and those which occurred several monts ago. Tularemia and plague foci were not infrequently present at the same territories, and these diseases could involve the same individual animals of Microtus arvalis (Pall.). The data obtained pointed to the greater effectiveness of examination of bird's pellets and excretions of mammalian beasts of prey for the reconnaisance investigation of the natural foci of plague and tularemia in comparison with the bacteriological methods applied usually.

[Clinical and epidemiological characteristics of tuberculosis in children and adolescents in Armenia].

In Armenia, the incidence of tuberculosis among children aged less than 18 years increased by 53.2% (from 7.9 to 12.1% per 100,000 inhabitants) in 1993 to 2003. On the contrary, the proportion of children and adolescents among all the patients decreased by almost twice (from 13.4 to 8.1%), which is largely due to the reduction in birth rates and to the inadequate detection of minor and local forms of primary tuberculosis. The clinical composition of tuberculosis in 1270 examined patients was the following: tuberculosis of respiratory organs (82.9%); its extrapulmonary forms (14.4%); tuberculous intoxication (2.7%); a concomitance of pulmonary and extrapulmonary tuberculosis (5%). The boy-girl ratio was 2:1. Tuberculosis of intrathoracic lymph nodes was predominant in children aged less than 14 years; infiltrative tuberculosis was prevalent in adolescents. Lung tissue destruction was observed in 15.7%; Mycobacterium tuberculosis was found in 10.2% of the patients. 49.9% of the patients were from the foci of tuberculosis; half of these cases were from the hidden foci of this infection.

[Seroepidemiological characterization of virus hepatitis in the Republic of Armenia].

The survey of the population immunological structure with respect to parenteral hepatitis showed awide circulation of hepatitis B (HB) and hepatitis C (HC) viruses among the adult population of Armenia. During the 5 year period of observation the number of persons having antibodies to HC virus increased 2.7-fold. High occurrence of antibodies to HBsAg of HB virus among the healthy population in 2002 (12.0%) in comparison with 1997 (5.4%) reflected a decreased infection rate with HB virus as well. Antibodies to hepatitis A (HA) virus were isolated, on the average, in 64 % of persons. Simultaneously with a decrease in the proportion of HA cases an increased number of HC patients was registered. No circulation of hepatitis E virus was detected. A high percentage of hepatitis cases of mixed etiology was established, as well as an increased number of combined parenteral hepatitis cases was registered (57.1%).

[On the occasion of the 20th anniversary of the accident on the Chernobyl NPP: medical consequences in Armenia].

The aim of this work is the analysis of the indices of the health and of the structure of the sicknesses of the inhabitants of Armenia who took part in the liquidation of the consequences of the Chernobyl accident. Also it is the determination of possible dependence of the frequency of diseases for the most widespread classes of sicknesses on the received dose of the irradiation, according to the data of the clinical examination and dispensarysation; and also it is the revelation of the role of other factors influenced on the health indexes.

[The dynamics of the wound microflora in the victims of the earthquake in Armenia].

The data on the composition of wound microflora, as well as changes in the contamination index over the course of treatment of patients with the syndrome of prolonged compression, who were brought to the institute after the earthquake in Armenia, are presented. The authors characterize the structure of the causative agents of wound infection in the patients treated at the traumatological department of the institute over several years. The epidemiological situation in the hospital in the period when the earthquake victims were treated there is analyzed.

[The effect of the earthquake in Armenia on the mental health of the pediatric population of the damaged regions].

The authors relate neuropsychic disorders in children and adolescents of Armenia in the acute period and 3 months after the earthquake. The pathological disorders were dependent on the individual reactivity of the child and environmental factors. 3 months after the earthquake 31% of the examines demonstrated marked mental disturbances. The prevalence of mental pathology among the population groups living in the regions damaged by the earthquake demands organization of specialized assistance for the population.

Reactions to disaster at a distance. The first week after the earthquake in Soviet Armenia.

Armenian-American adolescents from a Los Angeles school were interviewed during the first week after the massive earthquake in Soviet Armenia in December 1988. The authors describe their observations: All the adolescents strongly identified with the victims and felt that the catastrophe personally affected them; they all rushed into hectic activities to escape intolerable grief reactions; there was strong group pressure to accelerate helping efforts; the catastrophe was experienced as fortifying group cohesion; and there was a distinct preference to preserve and strengthen communal values, even over and above helping the earthquake victims. In particular, the authors observed the phenomenon of "participation envy," a special form of survivor guilt involving envious resentment at being excluded from a unique and rallying communal experience.

[Hibernation of Dermacentor marginatus ticks in alpine Armenia].

Only mature ticks, engorged females and hungry imago hibernate in mountain regions of Armenia. According to the author's observations eggs of ticks as well as their larvae are not capable of hibernating. In females of spring satiation the development of eggs proceeds in most short periods from May to October. The longest period before the beginning of egg laying is observed in ticks of summer and autumn satiation. It becomes prolonged, in general, on account of hibernation of satiated females. Females of August-September satiation hibernate more successfully than females satiated in June and July.
