[Peripac and Histoacryl as bandaging materials after removal of dental calculus and subgingival curettage].

Scaling and subgingival curettage were performed on the upper and lower front teeth of 25 patients in the age of 25 to 35 years. Peripac was applied to the periodontal tissue of three of the six teeth treated per subject and Histoacryl to the periodontal tissue of the other three teeth. Prior to and three months after the beginning of therapy, pocket depth, sulcus bleeding tooth mobility were determined. It was found that the use of Histoacryl as a dressing material is not a disadvantage as compared to the conventional dressing Peripac as far as therapeutic success in terms of declining pocket depth and SBI is concerned.

[Permanent retention: a justified orthodontic compromise?].

The conditions on which an orthodontic case should be treated to become as stable and as self-retaining as possible are briefly discussed. The correct indications for permanent retention could be found in certain types of malocclusion in which these principles cannot be applied. They are mainly encountered in the treatment of adults and functionally deviating patterns. Moreover, some major problems and disadvantages of permanent lingual retention are mentioned. The terminal responsibility of this procedure towards the referring general practitioner is evaluated and warned for. A short overview of the technical evolution in the field of lingual retention leads to the presentation of a new, more durable modification of the flexible wire system. Some cases treated with this "meshflex retainer" are presented.

[Reaction of the periodontium to the wire ligature splint (Stout-Obwegeser)].

The direct traumatising effect of the Stout-Obwegeser wire ligature splint on the marginal periodontium is minimal and need not be considered. The increase in depth of pockets, in the index of sulcus bleeding and in tooth mobility is due to poor oral hygiene. In 16 patients (orthodontic operation and jaw fractures) periodontal measurements were obtained on 369 teeth in 30 jaws before application of the splint, 1 and 6 weeks later and as 6 weeks after removal of the splint. All gingival indices determined were at first raised. The last measurements however showed, better values than those at the beginning of treatment. Inflammatory changes regressed near all teeth whether they were included in a ligature or not. This appears to prove that oral hygiene alone is decisive. The question whether the "curettage" effect of splint application has a therapeutic action is left open.

Periodontics. Diagnostic, prognostic and treatment planning considerations.

The periodontal disease syndromes present the practitioner with an extensive array of clinical conditions. However, by performing a careful examination and recording of findings, the clinician will provide order to this seemingly complex picture. This careful and reasoned approach is based on a logical sequence starting with clinical observations that lead to a diagnosis of the disease entity present. This correct diagnosis in turn will lead to establishment of the prognosis and the therapeutic approach. By following these precepts, the practitioner ensures a sound biologic basis for his or her efforts. The end result is the realization of the goal of dental therapy that her is for the patient to maintain dentition in health, comfort, and function for a lifetime and minimize the potential for urgent periodontal needs.

A comparison of the periodontal ligament injection using 2% lidocaine with 1:100,000 epinephrine and saline in human mandibular premolars.

The purpose of this study is to evaluate, with the electric pulp tester, the anesthetic efficacy of the periodontal ligament injection using 2% lidocaine with 1:100,000 epinephrine and saline in human mandibular premolars. The periodontal ligament injection using 2% lidocaine with 1:100,000 epinephrine was found to be an effective technique for anesthetizing mandibular first premolars. However, the duration of profound pulpal anesthesia was approximately 10 minutes. The periodontal ligament injection using sterile saline was not an effective technique for anesthesia. Teeth mesial and distal to the injected tooth may also become anesthetized with this injection technique. The initial needle penetration and injection of the anesthetic solution in clinically healthy teeth were only mildly discomforting. No increase in tooth mobility was observed 45 minutes after the periodontal ligament injection. No clinically observable pulpal or periodontal damage was seen at 3 weeks after the injection.

Clinical evaluation of intra-alveolar transplantation of teeth with cervical root fractures.

Transverse and oblique intra-alveolar fractures constitute a major therapeutic problem, particularly in young dentitions. Very few treatment alternatives apart from extraction have been available. This report is a clinical and roentgenological evaluation of a series of intra-alveolar transplantations which were carried out on 56 teeth with complicated crown-root fractures. The patients ranged in age from 9-33 years and were observed over a period of 4 years. Of the 8 teeth which had to be extracted due to further traumatic/prosthetic complications, 7 were retained for histologic examination. All teeth exhibited successful healing without ankylosis. In 12 per cent of the cases, unhealed resorptions could be observed within the apical area. Two to nine weeks post-operatively, endodontic and/or prosthetic treatment could be carried out, after which the teeth have continued to function well in terms of both aesthetics and performance. No negative effects in the adjacent teeth have been observed. Cases with multiple crown-root fractures and one case with internal resorption have been described. The intra-alveolar transplantation technique provides not only an alternative method of treatment, but also an opportunity for deep root injuries to be diagnosed.

[Clinical study on the effectiveness of Insadol].

The effectiveness of Insadol (Laroche-Navarron, Paris) was submitted to a clinical test. The following indexes and measurements were considered: Sulcus fluid, P-Index, Plaque-Index, Pocket Depth, Tooth Mobility, Bone Loss. The measurement of the Sulcus Fluid revealed a small, but significant decrease in the medicament group. The other considered parameters showed no change in the medicament group, which makes any therapeutical effect appear doubtful.

[Long-term experience in endosseous endodontic implantation].

The results of 9 years experience with transfixed abutment-teeth for bar-joint dentures in the lower jaw were collected. The method has been slightly altered since the beginning, and these changes are described in order to complete earlier publications: there are special chapters dealing with the implant-material Syntacoben, which is used in human and veterinary medecine mainly, and with the transfixation technique. By measuring the tooth mobility over a period of 20 months in a group of subjects, one has shown that the stabilisation (4.5 +/- 1.0.10(-2) mm) of loosened lower anterior abutment teeth combined with a bar-joint denture is lasting, even under masticatory pressure. The long-term results (mean wearing-time is 5.5 years) are gathered in a summarized survey. In 62 patients with 100 transfixed abutment-teeth (making a total of 159 abutment-teeth) there was a success of 62%, 15% partial success, and 23% failure. 5 success-cases are also radiologically shown. Within strict indications, transfixation can be recommended today to the practitioner.
