Tick zoonoses in the southern part of Switzerland (Canton Ticino): occurrence of Borrelia burgdorferi sensu lato and Rickettsia sp.

The diversity and the distribution of tick species and their infection rates by the pathogenic micro-organism Borrelia burgdorferi sensu lato, the etiologic agent of Lyme borreliosis, and Rickettsia sp., were studied in Canton Ticino (the southern part of Switzerland). Ticks specimens collected from animals and humans were classified and analysed for the presence of both pathogens. In particular, PCR analysis was performed for the detection of Borrelia spirochetes in Ixodes ricinus and Ixodes hexagonus, and the hemolymph test was done on Rhipicephalus sanguineus for the detection of Rickettsia sp. PCR assays, performed on 424 of the 989 collected ticks, revealed a low rate of infection (around 2%) of both vectors I. ricinus and I. hexagonus by B. burgdorferi sensu lato. These results are in agreement with the modest number of Lyme borreliosis cases yearly recorded in Ticino. Further, through analysis of DNA sequences, the strains carried by the infected ticks were identified as belonging to the genomic group VS116. The widespread finding of the Mediterranean species Rhipicephalus sanguineus in different locations from July 1994 to October 1995 demonstrates its establishment in Ticino. Of the 210 specimens collected, 70 were analysed and one was infected by Rickettsia sp.

HIV testing and retesting for men and women in Switzerland.

This study was conducted to describe voluntary HIV testing in the general population in Switzerland and to estimate yearly HIV test incidence. In 1994, a representative telephone survey of individuals aged 17 to 45 years obtained self-reported information on HIV testing. In addition to describing cumulative HIV test incidence, yearly HIV test incidence over time was estimated by a Bayesian hurdle model allowing for the plausible scenario of test consumption differing between first test and subsequent retests. Overall, 33% of the Swiss population (age 17 to 45 years) has been tested at some time for HIV on a voluntary basis (30% men, 36% women). For the time period 1990-1994, the result showed for 35-year-old individuals with supposedly low risk behavior, that 1) annual test incidence (first test or retest) showed a greater increase for men (4.2 to 5.9%) than for women (5.0 to 6.0%); 2) annual first test incidence increased moderately and differed for men and women (2.9 to 3.4% for men, 4.6 to 5.2% for women), and 3) annual retest incidence was twice as high for men (17.6%) as for women (8.6%). In conclusion, a substantial part of the Swiss population has been tested at some stage for HIV on a voluntary basis, and differences exist for testing and retesting between men and women.

Incidence, mortality and survival from prostate cancer in Vaud and Neuchâtel, Switzerland, 1974-1994.

BACKGROUND: Prostate cancer incidence has been increasing in most developed countries in the absence of similar trends in mortality, and with variable patterns in different areas of the world. MATERIALS AND METHODS: Trends in incidence and mortality from prostate cancer for the period 1974-1994 were analysed using data from the Cancer Registries of the Swiss Cantons of Vaud and Neuchâtel. Of 5,010 cases registered, 80% were histologically or cytologically confirmed. RESULTS: Age-standardized incidence rates increased from 33.1 to 48.6 per 100,000 (+47%). The upward trends were greater in the most recent calendar periods, and in the younger age groups (+77% at age 45 to 54; +57% at age 55 to 64). In contrast, mortality was stable, with an overall increase of only 3% in age-standardized rates (from 20.4 to 21.0 per 100,000), due to some increase in men aged 65 or above. Consequently, the incidence/mortality rate ratios increased from 1.6 in 1974-1979 to 2.3 in 1990-1994. Five-year observed and relative survivals increased from 26% to 41% and from 46% to 58%, respectively. Ten-year observed and relative survival for cases diagnosed in 1985-1989 were 19% and 42%, respectively. Survival improvements were greater below age 75. CONCLUSION: The pattern of trends in incidence, mortality and survival confirms the influence of improved diagnosis of prostate cancer over the last few years in this European population. Still, while Swiss prostatic cancer mortality rates are the highest in the world (20.3 per 100,000, world standard), i.e., about 30% higher than in the United States, all races combined, incidence rates are still half as much. On account of the steady increase of prostate-specific antigen testing in Switzerland, further incidence increases are likely.

Growing importance of capitation in Switzerland.

Since its liberalization the Swiss health insurance market has shown risk selection activities of the insurance funds, which call for risk adjustment. Because risk selection continues to be profitable under the current risk adjustment formula, fast growing HMO and PPO plans are (mis)used to attract good risks rather than to contain costs. For fear of being replaced by one centralised fund, social health insurers are themselves proposing improvements of the risk adjustment formula, to be applied to funds. The revised formula proposed in this paper, applicable among funds for risk adjustment and to gate-keeping models to calculate fair capitation, explains 12.4% of the variance of health care expenditure, halves profits from risk selection, and uses only the (few) data that are available in Switzerland.

Micronutrients and laryngeal cancer risk in Italy and Switzerland: a case-control study.

OBJECTIVE: To investigate the relation between various micronutrients and laryngeal cancer risk. METHODS: A case-control study was conducted in Italy and Switzerland between 1992 and 2000. Cases were 527 patients with incident cancer of larynx, admitted to the major teaching and general hospitals of the study areas. Controls were 1297 subjects admitted for acute, non-neoplastic diseases to the same network of hospitals. Dietary habits were assessed using a validated food-frequency questionnaire. Odds ratios (OR) and their corresponding 95% confidence intervals (CI) were computed using multiple logistic regression. RESULTS: Significant inverse relations emerged between laryngeal cancer risk and intake of vitamin C (OR = 0.2, for the highest versus the lowest intake quintile; 95% CI: 0.2-0.4), beta-carotene (OR = 0.2; 95% CI: 0.2-0.4), alpha-carotene (OR = 0.3; 95% CI: 0.2-0.5), lutein/zeaxanthin (OR = 0.4; 95% CI: 0.3-0.6), vitamin E (OR = 0.4; 95% CI: 0.3-0.6), beta-criptoxanthin (OR = 0.4; 95% CI: 0.2-0.5), folic acid (OR = 0.4; 95% CI: 0.2-0.6), thiamin (OR = 0.4; 95% CI: 0.3-0.6), glutathione (OR = 0.5; 95% CI: 0.4-0.8), reduced glutathione (OR = 0.6; 95% CI: 0.4-0.8), vitamin B6 (OR = 0.6; 95% CI: 0.4-0.9) and potassium (OR = 0.6; 95% CI: 0.4-0.9). Direct associations were found with zinc (OR = 1.5; 95% CI: 1.0-2.2) and vitamin D (OR = 1.8; 95% CI: 1.2-2.6). Combining low intakes of vitamin C, carotene, vitamin E, and folate with heavy smoking and drinking led to ORs between 80 and 170. CONCLUSIONS: This study provides further support that, independently from smoking and alcohol consumption, the intake of several micronutrients, including selected antioxidants, is inversely related to laryngeal cancer risk.

Struggling with imaginaries of trauma and trust: the refugee experience in Switzerland.

This article discusses some effects of migration politics on asylum seekers and refugees and on the Swiss health services. It is based on multisited ethnographic research that tracked interpretative concepts of the refugee experience. Following a grounded theory approach, it identifies imaginaries of trauma and trust as key categories in the field of transnational migration and health. The psychiatric concept of trauma and a more popularized discourse of traumatic memory are strongly emphasized in all of the investigated field sites: the providers of primary health care and psychosocial services and representatives of social welfare agencies and law-making bodies use this "diagnosis" extensively. This leads refugees to develop tactics of a) identifying with the trauma discourse in order to become "good refugees" and achieve legal status in Switzerland; b) struggling with the ascribed pathologies and suffering from retraumatizing effects of these predominant trauma policies; and c) trying to refuse or subvert them by emphasizing the existence of structural violence in the receiving countries. An analysis of the interactions of health providers and refugees shows that it takes place in an environment of social and economic insecurity and in a shared imaginary of (mis)trust, putting at stake the moral economy of recent migration politics and the refugee experience.

Needs and use of drug information sources in community pharmacies: a questionnaire based survey in German-speaking Switzerland.

OBJECTIVES: To explore the types of drug information used by pharmacists in daily practice, their needs and wishes concerning drug information and their opinions about future changes of pharmacy practice. METHODS: Postal survey to a random sample of 223 (26% out of 859) community pharmacies from the German-speaking part of Switzerland. The 48-item questionnaires were processed automatically with the Cardiff TELEform-Software. In addition, a telephone-survey to a random sample of 20 non-responders was performed in order to test for non-response bias. RESULTS: A total of 108 pharmacists (response rate 48%) reported that the official Swiss drug reference book is still the most popular source of drug information used to solve all kinds of drug related problems. The Internet as a source of drug information is of minor importance, even though 88% of the pharmacies have Internet access. Deficits in drug information were reported for paediatrics, phytotherapy, drugs during pregnancy/lactation and for therapy guidelines. According to 35% of the pharmacists, the importance of offering drug information to customers will increase in the future. Most of the pharmacists are not afraid that Internet pharmacies would replace them. CONCLUSIONS: The results show that the majority of the community pharmacists are only partially satisfied with the sources of drug information currently available. The Internet still plays a minor role for solving drug-related problems in daily practice, even though the available infrastructure makes the community pharmacies able to use the Internet more frequently. The pharmacists need more websites tailored to their needs. The pharmacists have clear visions about possible future developments. They do, however, have to adopt quickly to the changes ahead in order to remain competitive.

[Feline spongiform encephalopathy: first clinical case in Switzerland].

A six-year-old female Birman cat was referred to our clinic because of chronic progressive changes in behavior. Additionally, generalized vestibular ataxia and psychomotor seizures were noticed. A multifocal lesion in the forebrain as well as brainstem was suspected. Ancillary investigations such as complete blood cell count, serum biochemistry profile, urinalysis and cerebrospinal fluid examination revealed no significant abnormalities. Electroencephalography showed diffuse changes in the cortical activity. Feline spongiform encephalopathy was confirmed by histological brain examination and positive immunohistochemistry for PrPSc. This is the first time that a case of feline spongiform encephalopathy is diagnosed in Switzerland.

[Geographic distribution of BSE in Switzerland].

Visual evaluation of the occurrence of BAB (born after ban) cases pointed towards spatial clustering. Therefore a statistical analysis of spatial case clustering was conducted using GIS technology. In the initial analysis, all 376 cases (135 BAB, 241 BBB/born before ban) diagnosed through mid of March 2001 were investigated using the spatial scan statistic. Two clusters of BBB cases were identified in the western part of Switzerland, and two clusters of BAB cases in the eastern part. Epidemiological investigations performed within the BAB clusters showed an increased pig density in these cluster regions. Pig density is considered an indicator for the probability of contamination of cattle feed with feed containing meat-and-bone meal that is intended for other species (cross-contamination). In an updated cluster analysis including all cases reported until end of June 2002 (data set B), clusters were identified in the same regions as previously. It was shown that the BAB clusters occurred in different time periods. The small scale differences in cluster size and location are discussed, with an emphasis on the implications for following epidemiological investigations.

[Housing and use of horses in Switzerland: a representative analysis of the status quo].

Out of a representative sample of 1861 Swiss horse yards, 622 horse keepers (35.2%) with 2536 horses, ponies and donkeys sent back a questionnaire about general farm characteristics, their horse-specific training, horse stock, housing conditions, intraspecific social contacts, feeding, ability to move freely, use of the horses and management. 83.5% of horses were kept individually (thereof 18.3% tethered, 32.3% in a loose box indoors, 28.4% in a loose box outdoors, and 4.5% in a loose box with permanent access to a paddock or an individual pasture), and 16.5% lived in group housing systems. 36% of the horses had daily access to a pasture, 62.8% depending on the weather, and 1.2% never at all. Individually housed horses pastured significantly less than horses kept in groups. The mean use per week was 4.5 hours, depending on breed, type of use and housing system. Only 50% of the horse keepers were trained with respect to horses or farm animals (e.g. professional training graduation, license, military service, veterinarian, farmer). In Switzerland, horses are still housed and fed in a traditional way, despite considerable recent changes in the amount and type of use (65.1% leisure horses, 34.9% sport and working horses); this may result in health and behavioural problems. Therefore, information and education of horsewomen and horsemen should be emphasized in the future. For the transfer of knowledge, veterinarians are of high importance.

[Porcine malignant catarrhal fever: diagnostic findings and first detection of the pathogenic agent in diseased swine in Switzerland].

For the first time Ovine Herpesvirus 2 (OvHV-2) was identified in Swiss pigs as the causative agent of Porcine Malignant Catarrhal Fever (MCF). Diseased animals from two farms were observed to show weakness, anorexia, fever up to 41 degrees C, and neurological symptoms, i.e. ataxia, convulsions and hyperesthesia, erosion on the snout and in the oral and nasal mucosa, as well as multiple skin lesions. Histopathological findings included severe non-purulent inflammation with mononuclear cell infiltration in several organs. Most dominant were meningo-encephalitis, disseminated nephritis as well as purulent catarrhalic bronchopneumonia. The findings were quite reminiscent of the lesions due to MCF in cattle and give therefore substantial proof to use Porcine Malignant Catarrhal Fever as the term for the disease. Identification of the causative agent was done with a quantitative PCR specific for OvHV-2. Different tissues from diseased animals were positive. Furthermore, one animal which had been ill for more than five days tested positive for antibodies against an epitope conserved among MCF viruses. Serum samples from diseased animals reacted negative towards Classical Swine Fever- and Pseudorabies virus antigen. A weakly positive reaction against porcine enterovirus type I argued against the involvement of enteroviruses in the observed disease. Moreover, by means of different conventional PCRs, we detected the newly discovered porcine lymphotropic herpesviruses for the first time in Switzerland and could at the same time exclude their involvement in Porcine Malignant Catarrhal Fever.

[Prevalence of Fasciola hepatica and Dicrocoelium dendriticum in the cow: inspection in an east Switzerland abattoir].

On the basis of meat inspection in an abattoir in the eastern part of Switzerland, the incidence of liverflukes in cattle was recorded weekly during one year. The prevalence was compared with surveys from the seventies and nineties. The average incidence of Dicrocoelium dendriticum infection did not change. Although the prevalence of Fasciola hepatica apparently slightly decreased, an actual decrease of liverflukes could not be demonstrated.

[Chronology of horseshoes found in Switzerland].

The horse was domesticated by a people in Central Asia. However, it is not known where and who invented horse shoeing, an invention that increased the possibilities for using horses. The word "horseshoe" was first mentioned in a document from 826 p.c. Up to now an archaeologist couldn't date an excavated horseshoe. I succeeded now to establish a chronology for horseshoes found in Switzerland. The first realisation is that in the course of more than thousand years of horse shoeing history three basic forms followed each other. Moreover, the existing horseshoes can be allocated to periodical types, each in use for approximately a quarter of a century.

Porcine circovirus as a possible cause of postweaning wasting in pigs in Switzerland.

Postweaning wasting is a major worldwide problem in pig production, particularly with respect to the disease termed postweaning multisystemic wasting syndrome (PMWS). In addition to wasting, PMWS symptoms include respiratory distress, diarrhoea, pallor and occasional cases of jaundice. The causative agent is porcine circovirus type 2 (PCV-2). The objective of the present study was to determine the significance of PMWS and similar conditions in Switzerland. A total of 72 weaned piglets from 26 farms showing wasting were examined for the presence of PCV-2 by immunohistochemical and histological analysis and 57 piglets from 21 farms were examined serologically. Possible causes for wasting other than PCV-2 were investigated by macroscopic, histological and bacteriological methods. PCV-2 antigen was identified immunohistochemically in the lymphatic organs in 11 of these 72 piglets. However, only 4 animals showed histological changes typical of PMWS. PCV-2 antibodies were found in 70% of the piglets. Piglets with wasting syndrome not associated with PCV-2 infection suffered from conditions including porcine proliferative enteropathy, gastric ulcers, polyserositis and polyarthritis. The most frequent condition was chronic enteritis not associated to circovirus infection. The results from the serological analyses indicate a wide distribution of PCV-2 in the Swiss pig population. However, confirmed cases of PMWS were rare in the investigated piglets.

[Outbreak of sheep scab on two adjacent common alpine pastures in the canton of Schwyz (Switzerland)].

Sheep scab, which is caused by the mite Psoroptes ovis, is a notifiable disease in Switzerland. In the framework of an epidemiological study in the canton of Schwyz, a sheep scab outbreak on 2 adjacent alpine pastures with 62 flocks (1770 sheep) was followed up clinically, parasitologically and serologically. No mass treatment of the flocks with organophosphates or avermectins had been undertaken before the flocks were taken up to the alpine pastures, but they were treated according to their serological status. Supervised treatment of seropositive judged flocks (at least one seropositive or at least 2 serologically equivocal animals per flock) with Doramectin was undertaken, whilst seronegative judged flocks remained untreated. Sheep returned from the summer pastures in early September 2001 and within three months scab infestation was diagnosed in 53 flocks (85.5%). These infested flocks were scattered all over the canton. Furthermore, the transfer of sheep from 6 infested flocks resulted in the transmission of the disease to 10 new flocks. Infested flocks were kept in quarantine and sanitized. The complexity of the traditional use of shared alpine pastures and the intense, uncontrolled trading with animals render the control of sheep scab difficult.

[Abortion in sheep: epidemic Salmonella abortusovis outbreak 2005 in Switzerland].

In spring 2005, the outbreak of contagious abortion caused by Salmonella Abortusovis in 6 sheep flocks in Switzerland led to considerable economic losses. The Swiss small ruminant health service (BGK) evaluated this case. The aim was to identify the source of the epidemic in order to avoid further spread of infection and to evaluate the possibility of using vaccination. Moreover, a strategy for prevention of future outbreaks was developed. This article aims to increase disease awareness of food animal practitioners for Salmonella Abortusovis abortion in sheep.

[Autochthonous infections with Angiostrongylus vasorum in dogs in Switzerland and Germany].

Angiostrongylus vasorum is endemic in foxes an other carnivores in the South and south East of France, Denmark and Great Britain. The reddish nematode is present in the Arteria pulmonalis and the right side of the heart and causes respiratory and cardiovascular symptoms. From 1999 to 2004, A. vasorum was diagnosed in 5 dogs from northern Switzerland, in 1 dog from southern Germany and in 3 dogs from south Ticino. Clinical signs in the affected dogs varied and ranged from cough, tachypnoea and dyspnoea to neurological symptoms in 2 of the dogs. Four dogs died and in 3 of the 4 dogs adults and larvae were found after digestion of formalin-fixed tissue. Diagnosis in the other 5 dogs was achieved by detecting the larvae coproscopically. Based on anamnestic data, these 9 dogs are probably autochthonous cases.

[Phytomedicine in dogs and cats: web-based survey among veterinarians in Austria, Germany and Switzerland].

In the last decades phytopharmaceuticals have stabilized their significance in human medicine. The aim of this study was to obtain information about the present situation of phytomedicine in three German-speaking countries. For that purpose questionnaires were sent via internet to 2675 veterinarians in Austria, Germany and Switzerland and the collected 189 answers were statistically evaluated. As shown in this paper, there is an increasing interest in veterinary phytotherapy. Presently three-quarters of the veterinarians are using phytoproducts in clinical practice. Herbal medicinal products are only seldom used in eye and ear infections as well as in endo- and ectoparasitoses but more often in diseases of the liver, joints, kidneys, heart and skin. Generally, phytodrugs are more frequently applied in chronic diseases as well as secondary medication, whereas the time of treatment usually exceeds one month. Basic advantages of medicinal phytoproducts have been shown to be the low side effects and the acceptance of the pet-owners and the only disadventage has been reported to be the lack of information. This study clearly shows the upward trend in using herbal drugs by veterinarians but the results also point to the lack of scientific knowledge about phytotherapeuticals in animal health.
