[The new problems in psychiatry (author's transl)].

Modern psychiatry, born of a movement created by Pinel in France and Tuke in England, has set aside the rejection of mental patients, and centred the study of mental diseases in France on the predominance of periodic psychoses, and therefore curability. Another movement, however, arising from the discovery of biological psychiatry has focused psychiatry on schizophrenia and incurability. This is a serious and damaging error. Furthermore, the discovery of toxic causes by Moreau de Tours, and more recently, of experimental catatonia, has enabled psychopharmacology to widen etiological and therapeutic fields. Even so, psychiatry is menaced by a return to the past, perhaps to the point of rejection of mental patients, and this has been accentuated by psychoanalysis, which rejects (psychotics) in favour of neurotics, and by the concept of blocking in incurability. This regression is beginning to affect legislation, with the threat of a return to the arbitrary, and to the social decline of patients, following the recent law on the legally incapacitated.

Monosymptomatic hypochondriacal psychosis manifesting as delusions of parasitosis. A description of four cases successfuly treated with pimozide.

A recent article in the Archives described the severe problems encountered in trying to treat patients with delusions of parasitosis. The present article describes four such cases, which fall into the larger category of monosymptomatic hypochondriacal psychoses, conditions that appear to be related to the paranoid disorders. Evidence is accumulating that such cases may respond very specifically to treatment with pimozide, a relatively new psychotropic medication. This drug keeps the symptoms in remission but does not effect a cure; nevertheless, it may represent a highly important advance in therapy in a condition traditionally regarded as virtually untreatable.

The role of cerebral arteriovenous malformations in psychiatric disturbances: case report.

A patient is described who had congenital vascular malformation of the vein of Galen, with several psychiatric, as well as neurologic, manifestations. Prior to this patient's death, there was an increase in some psychiatric manifestations in response to suggestions of surgical therapy for the cerebral venous anomaly. Other symptoms appeared to represent direct organic effects of the lesion. The medical literature on the anomalies of the great vein of Galen reveals a frequent but not specific relationship to concomitant psychiatric manifestations.

[Protracted psychoses in juvenile psychopath-like heboid schizophrenia (atypical variant of its course)].

A group of 22 patients selected as a result of follow-up examinations of 104 patients with juvenile psychopathlike heboid schizophrenia is described. A specific feature of the disease course in the patients observed consisted in appearance of atypical protracted psychoses after a long-time period of psychopathlike heboid disorders. Peculiarities differentiating this form from the slow-progressive variant of psychopathlike heboid schizophrenia and from paroxysmal schizophrenia with heboid initial stage are presented. The group described occupies an intermediate place between these forms of schizophrenia. A question on the role of aggravation of the disease course with various exogenous adverse factors (noted in all the cases described) is discussed.

[Initial mental disorders in rheumatism].

The paper deals with 11 cases of mental disorders appearing during the initial phases of rheumatism. Their duration and psychopathological traits were related to the development acuity and expressiveness of the clinical picture of rheumatism. The author indicates special traits of mental disorders in latent rheumatism, the beginning of which are at times difficult to estimate. Factors promoting the appearance of relapses of mental disorders are being evaluated.

[Psychogenic reactions in cancer patients].

An overall (at random) study of 183 patients with cancer of the stomach or rectum at an oncological hospital showed that all cases had certain psychogenic reactions, which differed both by their clinical expression and severity. The type and severity of psychogenic reactions correlated with the stage of treatment (diagnostic, preoperational, etc.) sex, age, education and the localization of the tumor and premorbid personality traits. The paper offers data on the use of combined treatment (psychopharmacological and psychotherapeutic impact) aimed at removal of psychogenic disorders.

[Visual perception disorders in depressive syndromes].

A wide range of psychopathological visual phenomena, part of which are traditionally considered as derealizational disturbances, are described in the structure of depressive states. Five main types of disordered visual perception are singled out, which differ by their structure and degree of correlation with the depressive affect and symptoms of depersonalization in a wide sense. The results point to certain dynamics in the described phenomena, their sensory basis. Electrooculographic studies of eye movements indicating a peculiar formation of a visual image in different forms of depressions are demonstrated.

[Typical forms of schizophrenia in old age].

Aside from typical forms of late schizophrenia which generally conform to the definition given by M. Bleuler, there also are psychoses appearing in old age which differ significantly from the atypical symptoms and consequently present certain diagnostic difficulties. This report contains descriptions of late manifestations in schizophrenic psychoses, which develop with a continuous or assault-like course with a prevalence of parnoial disorders. Paranoid delusions, in such cases, are characterized by aging traits (concrete and short-term delusions, exaggeration of the degree of superficial persecution and prejudice, and a limited number of people involved in the delusions). The development of such forms of late schizophrenia takes a slowly progressing course. The results of these studies, especially the psychopathological symptomatology, the genetic-constitutional background and the development and outcome of these psychoses are analyzed in detail. The data permit to consider such forms of psychose as atypical variants of late schizophrenia.
