[Content of the heavy metals, Zn, Cu, Cd and Pb in the organs of lympholeukemic cows].

Zn, Cu, Cd and Pb contents in liver, spleen and lymph nodes of 30 cows with lymphatic leukemia were determined using polarographic method. In leukemic tissues more or less prominent increase of mentioned metals was observed. Results of the examinations would indicate a partical disappearance of natural antagonism between particular elements in neoplastic tissues. Metals accumulation in leukemic tissue is prabably a secondary phenomenon in relation to virus infection. Alterated by virus physicochemical cell properties promote not only accumulation bat also perversion of heavy metals to tissues.

A note on narcissism.

The connection of the concept of narcissism with the concept of the self is re-examined in the light of Hartmann's formulation of narcissism as cathexis of the self. While the formula is valid, its converse remains open to question. Implications for a psychology of the self independent of (but not divorced from) the dynamics of narcissism are suggested in regard to the understanding of perversions, development, narcissistic object relations, and self-esteem. The viability of a separate psychology of the self in each area is proposed.

The nature and function of a pathological oedipal constellation in a female patient.

The first analysis of a female patient dealt essentially with separation-individuation and narcissistic issues. During the second analysis a dynamic constellation of narcissistic character pathology, some psychopathic tendencies, perverse fantasies, and enactments emerged. Prominent was an unconscious fantasy of stealing the sexual organs of one parent to sexually satisfy the other parent. The etiology of this constellation and its transmutations in analysis are described.

Violent sex and suicide.

Early literature on sexual disorders and suicidiology are reviewed in this paper in the light of relationships between sexual problems, especially violent sexual ones, and suicide. The relationship between guilt feelings and suicide is viewed from the standpoint of current observations about sexually violent behavior and suicide. It was postulated that sexual perversions constitute a defensive means of coping with internal stress. Suicide is viewed as serving the same aim. Specific violent sexual behaviors such as sexual asphyxia and rape are discussed in terms of suicide. Transsexuality and transvestism are treated similarly. It is concluded that sexual violence and preversion in general are forms of self-destruction in themselves.

[The characteristics of diagnosing homosexuality in examining a special population contingent].

In social terms homosexuality is one of the most dangerous sexual perversions, for it is subject to a strong social disapproval, affects the personality core, leads to social disadaptation, growth of anxiety, development of neurotic and depressive states, suicidal inclination, is conducive to the spread of syphilis and AIDS, forms the basis for numerous crimes (violence, aggression, murder). Homosexuality virtually cannot be corrected and is carefully concealed. In single-sex groups (soldiers, students of military schools, convicts) the prevalence of homosexuality may be as high as 30 to 50%. The proposed complex method for the diagnosis of homosexuality in men has been developed on the basis of methods of sexology, anthropometry, verbal and nonverbal psychodiagnosis, multidimensional mathematical statistics. The method consists of several independent blocks representing different aspects of sexuality. It is conductive to a flexible and rapid solution of numerous practical problems from singling out a risk group in screening examinations to full expert evaluation of the sexual sphere with a resulting differential diagnosis.

[Concepts in the assessment of "other severe psychological deviance"].

Rendering an expert opinion on offenders with personality disorders is one of the most frequent tasks in forensic psychiatry. In Germany, the juridical term "severe mental deviance" for impaired responsibility, caused by severe personality disorder or sexual perversion, causes a lot of problems, because there is no obligatory psychiatric interpretation of this term. This paper gives a review of the different clinical transpositions by German forensic psychiatrists and discusses the model of "progredient psychopathological development." Based on the common elements of these solutions, the paper proposes a clinical interpretation of "severe mental deviance", which is derived from the diagnostic and clinical practice with non-delinquent patients with severe personality disorders.

[Three cases of matricide].

The matricide is defined as the murder of the mother by her son or her daughter. The term of parricide is also used. Matricide is not a specific mental disease. The study of three original cases leads to classify two of them as psychiatric disorders, legal authority is applicable to the last. The first category contains a wide spectrum of psychiatric diagnosis and could be successfully controlled by treatment. Those cases fall in the article 64 of the French Law at the moment of the murder. The second category consists of perverse individuals responsible of their actions and hardly curable by treatment. These cases are abandoned to Justice. The "parricide" represents 2.54% of murders, the matricide 0.68%.

[Sexuality in Ancient Egypt].

The present article explores the sexuality in ancient Egypt. In particular in this article are presented the ways of concubinage (marriage, concubinage, adultery), the incest, loves of the pharaohs and of the common people, the freedom of choice in garments, the status of the hetairas and of the whores, the sexual perversions (male and female homosexuality, necrophilia, sodomism, bestiality, rape, masturbation, exhibitionism), the operations of the genitals (circumcision, excision, castration) and finally the level of knowledge in gynaecology, fertility, contraception and obstetrics that even today demands our admiration.

[Incest: murder of the identity].

The author begins by describing incest in the context of a change in attitude toward it and a substantial increase in forensic referral. This is followed by the description of the psychological profile of abusing fathers and how the traumatic experience severly shatters the sense of identity in the victims. Some clinical examples illustrate the role of negative hallucination, basic violence and the phenomenon of the double. The author also approaches the mental functioning of the abusers through the angle of perversion and attempts to define some necessary therapeutic conditions.

The effect of intranasal azelastine and beclomethasone on the symptoms and signs of nasal allergy in patients with perennial allergic rhinitis.

A double-blind, randomized, parallel-group, placebo-controlled study involving 130 patients was conducted at 9 centres in the U.K. to assess the effect of 6 weeks of treatment with azelastine nasal spray (azelastine) and beclomethasone dipropionate nasal spray (BDP) on the symptoms of perennial rhinitis. Efficacy was assessed by patients recording daily the severity of the symptoms of rhinitis on 10-cm visual analogue scales. Analysis of this diary data showed significant reductions in sneezing, blocked nose, running nose, and itching nose during azelastine treatment. Patients on BDP recorded a consistent reduction in rhinitis symptoms, but these reductions were significant only for sneezing on treatment day 7. When rhinitis symptoms were assessed by clinical investigators on a 4-point scale, the scores obtained following treatment with the 2 study medications showed little change from baseline or "active" treatment scores. There was no evidence of a consistent change in nasal airway resistance, measured using anterior rhinomanometry, following treatment with either BDP or azelastine. Azelastine nasal spray and BDP nasal spray were well tolerated by the patients and the relative incidence of adverse events was similar in the azelastine and placebo/azelastine treatment groups, except that taste perversion occurred more frequently during azelastine treatment than during placebo/azelastine treatment. There was no evidence of an increased incidence of somnolence or fatigue in patients who received azelastine nasal spray. Overall, the results of this study indicate that azelastine administered twice daily as an intranasal spray is a safe and efficacious treatment for the symptoms of rhinitis in patients suffering from mild to moderate perennial rhinitis.

Tolerability, central effects and pharmacokinetics of intravenous ketorolac tromethamine in volunteers.

The central effects, tolerability and pharmacokinetics of multiple intravenous doses of the analgesic ketorolac tromethamine (30 mg 4 times daily for 5 days) were studied in male volunteers. In this double-blind, randomized, parallel group study, 13 subjects received ketorolac tromethamine and 7 subjects received placebo (vehicle). To determine the effects of withdrawal all subjects were then given further dosing with placebo (4 times daily) for 2 days while maintaining the double-blind nature of the previous drug assignment. Physical examinations and laboratory tests were obtained prior to the drug administration and after completion of the study. Scales for assessment of anxiety, depression, sleep and opiate withdrawal were presented to the subjects on day 2, 5, 6, 7 and 8 of the study. After 5 days of multiple intravenous doses ketorolac showed overall good systemic tolerance and safety in comparison with placebo. Myalgia and taste perversion were more frequently reported in the ketorolac group. The frequency of injection site complaints, mostly transient pain, was about 80% for both ketorolac and placebo, indicating these were likely caused by the vehicle. There were no significant changes in the scales assessing anxiety, depression, sleep and opiate withdrawal during treatment with ketorolac and after its withdrawal, suggesting that the drug has neither any major central effects nor any clear addiction potential in this dose schedule. A classic revisited: Kurt Eissler's 'the effect of the structure of the ego on psychoanalytic technique'.

Eissler's influential contribution, which introduced the idea of the parameter of technique, extended Freud's later contributions so that they would apply to thoughtful analytic work with hard-to-treat patients. Aimed at sharpening the baseline of psychoanalytic technique-interpretation--and differentiating ego-psychological work from Franz Alexander's modifications in particular, Eissler's 'parameter' was progressively and perversely made into a coercive, if not punitive, concept. Looking back on it now, one can see that, in addition to its merits, the paper advocates an orientation that is no longer beyond dispute. In contrast to the present pluralistic state of psychoanalytic approaches to treatment, certain aspects of Eissler's paper seem unacceptably finalistic, restrictive and uncritically committed both to assumptions about continua in development and pathological states and to an operational approach to diagnosis.

Is health care a commodity: how will purchasing improve the National Health Service?

Through the reforms to the National Health Service in 1990 an internal market for public health care provision in the United Kingdom was introduced. As part of this new system of care, responsibility for purchasing of health care was separated from that for its provision. The new purchasing function, undertaken by district health authorities and fundholding general practices, has created the opportunity for improvements in health and higher standards of care to be achieved through new and explicit mechanisms. However, the purchasing function has not yet realized its full potential to achieve beneficial change, partly because traditional behaviours have not yet adapted to the new system of care and partly because specific aspects of the internal market are creating barriers and perverse incentives. This paper discusses these issues and identifies the important barriers which have still to be overcome if purchasing is to be the driving force for change in the new National Health Service.

Health reform: the new reality.

In 1965 the poor and elderly did not have access to health care. To alleviate this problem Medicare was implemented. Major changes in health care occurred as a result. Initially, payers paid whatever was billed, which led to perverse incentives toward overutilization and inefficiency. It has been estimated that by the year 2000 the total health care expenditure will reach $2 trillion. Currently, we are again faced with approximately 37 million people who are uninsured and who do not have access to health care. It is not just the poor and elderly who have no access, but rather a broad spectrum of the American people. The current system cannot be sustained. Much has been written and proposed to support health care reform. It is imperative that nursing leaders have a clear understanding of what is being proposed and how that will impact hospitals, physicians, and nursing.

[Naltrexone in the therapeutic relation in cases of opioid abuse and dependence].

The paper describes the use of naltrexone, an opium-receptor antagonist, in the multimodal treatment of drug addiction and in particular in preventing the recidivation of heroin abuse. Naltrexone appears to lack the ambiguous characteristics of methadone which, in spite of not provoking all the effects of heroin, may be seen by the drug addict as a reasonable substitute, in particular if it is difficult to find. Since naltrexone is clearly a member of the drug category, and does not overlap into the pleasure category, it is possible to break the perverse triangle which is often created between the drug addict, doctor and methadone, thus making it possible to install a more authentic relationship between operators and the users of Ser.T.

Personal medical and social data: their processing and legal protection.

Computerized tools are, just like other machines, very useful to man. However the development of data processing requires regulation especially oriented towards the principles of the protection of private life and of the guarantees attached thereto. An analysis of the international legal tools relating to the protection of human rights confirms the constant concern for individual rights and the interests of society. Belgian legislation relating to the computerized processing of medical and social personal data, in some respects, reveals the dangers of a lack of protection of the individual's private life. In conclusion, this article points out that in a modern state one should remain vigilant because any blow to human rights is a perversion leading to the decay of law.

Sexual asphyxia: a lesser epidemic.

Asphyxiophilia is a paraphilia of the sacrificial/expiatory type in which sexual arousal and the attainment of gratification depend on hypoxia induced through either strangulation, smothering, chest compression and/or the inhalation of volatile substances. A peculiar, auto-erotic variant of this perversion is described, one in which the participant accidently died. Numerous female prostitutes have been canvassed in order to ascertain the prevalence in Durban, Republic of South Africa. The figures are an order of magnitude higher than was hitherto imagined. Insurance companies have been loathe to compensate the beneficiaries of such victims fully. How they and the courts view pivotal concepts such as 'mental infirmity', 'accident', 'natural consequences', and the legitimacy of psychiatry presenting itself as a unified body of knowledge sets precedents, and sets in motion processes whereby these decisions permeate into the general population and become--in time--accepted as norms. Thus, the medicolegal ramifications of sexual asphyxia affect not only the police and the courts, but also our society in general.

[Does aesthetic surgery form part of mainstream surgery or is it an entirely separate sector? Reflections and proposals for better protection of the public].

In contempt of the laws and regulations in force, several thousand unqualified practitioners carry out aesthetic surgery although only a mere few hundred are legally entitled to practice this surgical specialty. We are aware, in the present system which has no efficient control systems, that the public is no longer able to identify this small group of charlatans and incompetents. We call on massive concerted effort by the responsible public authorities to ensure that the general public receives the necessary protection. Amongst others, we demand that: The ministry of health, integrates all medical products and medical devices aimed at the aesthetic sector into the list of medical products and medical devices to marketing authorization (authorization de mise sur le marché--AMM) or to endorsement in compliance with the Huriet Law of 10.12.1988. The conseil national de l'ordre des medecins, (national council of doctors advisory board), ensures that the laws and regulations in force governing the value of diplomas, qualifications and competences are respected, be aware of the obsolescence of the general nature of the medical degree, inapplicable due to the efficiency and thus the dangerous nature of modern medicine. The ministry of justice, clearly defines the nature of the informed consent demanded from the patient prior to any therapeutic treatment, ensures the conditions required for legitimate compensation for prejudices caused by therapeutic risks, specifies the doctor's responsibility without malpractice conditions, be aware of the perverse effects caused by the abandonment of the obligation of means and its replacement by an obligation of results demanded from the doctor, to correct the unjust and anomalous situation which opposes the ten year responsibility of the medical product or medical devices manufacturer with the thirty year responsibility of the doctor using these products or equipment.
