[A new chiral derivatizing reagent for the resolution of amine and alcohol drug enantiomers by HPLC].

A new chiral derivatizing reagent (s)-alpha-methoxybenzyl isocyanate (MIB) was prepared from (s)-(+)-mandelic acid with a practical method, and its ability of liquid chromatographic resolution for drug enantiomers by pre-column chiral derivatization was evaluated with racemic amphetamine and mexiletine as well as phenylpropanol. The operation is simple and rapid. Separation is good for the amine isomers, but poor for alcohols and extensive work remains to be done. The solution of derivatization was directly injected into the chromatograph. Conventional silica gel column was used with light petroleum as the mobile phase.

[Olfactory neuroblastoma. A clinical and morphological study].

A clinical and morphological study about neuroblastoma with olfactory differentiation are made. This study includes: TAC, vanimil mandelic acid urine determination, optic microscope, immunohistochemical technique, neuropeptidal, in situ hybridization, DNA analysis by flow cytometry and electronic microscope.

[State of the neurohumoral regulatory system in circulatory insufficiency].

A moderate elevation of the daily excretion of free noradrenaline and adrenalin is observed in chronic circulatory insufficiency, beginning with Stage IIA. The catecholamines metabolism is elevated, as shown by the daily excretion of normethanpherine and methanpherine and of vanillyl-mandelic acid. The activity of renin and angiotensinases was growing along with the progressing cardiac insufficiency. The blood level of angiotensinogen was decreasing, especially in patients with Stage IIB and III of decompensation. The daily excretion of aldosterone was growing along with the development of cardiac insufficiency. The functional state of the glucocorticoid function of the adrenal cortex was of a phased nature in cases of circulatory insufficiency. The study of the functional state of the epiphysis was conducted by way of determining the blood level of melatonine and of its daily excretion. In Stages I and IIA the level of this hormone was clearly elevated, in Stages IIB and III -- decreased as compared with the initial and normal levels. The plasma level of the antidiuretic hormone was distinctly growing, beginning with Stage IIB, reaching its maximal values in Stage III.

The metabolism of styrene oxide in the isolated perfused rat liver. Identification and quantitation of major metabolites.

Isolated perfused rat livers rapidly metabolized 14C-styrene oxide. A large proportion of the administered radioactivity was excreted in the bile as a single compound that was identified as S-(1-phenyl-2-hydroxyethyl)glutathione by comparison with an authentic synthetic standard. The circulating perfusate was found to contain approximately equal amounts of styrene glycol, mandelic acid, and the glutathione derivative.

[Headache, hypertension].

Headache and hypertension were the main clinical symptoms in this slim 77-year-old lady. Because the hypertension presented as paroxysmal and was combined with palpitations, dizziness, and sweating the suspicion of a phaeochromocytoma arouse. The vanillyl mandelic acid in the urine was elevated. As further investigation revealed that this elevation was due to medication with L-dopa because of Parkinson-syndrome. A proper history of the patients' medication or alternative laboratory tests prevent false conclusions.

[Pheochromocytoma revealed by pulmonary edema].

We report the case of a young patient of 37 presenting with two attacks of pulmonary oedema which revealed an underlying pheochromocytoma. The diagnosis was suggested by the raised level of vanyl-mandelic acid in the urine and the abdominal CT scan. After an adrenalectomy the outcome was satisfactory. Pheochromocytoma should be considered in the differential diagnosis of the acute respiratory distress syndrome in the adult linked to the action of the circulating catecholamines on the pulmonary capillaries.

Mild hypertension in patients with suspected dilated cardiomyopathy: cause or consequence?

This study was undertaken to clarify the relationship between mild transient hypertension and dilated cardiomyopathy. Fifty-five patients were studied: group 1--controls (12 patients), group 2--hypertensives without clinical evidence of heart failure (14 patients), group 3--patients with hypertensive heart failure and diastolic blood pressure above 100 mmHg (10 patients), group 4--patients with possible dilated cardiomyopathy with mild hypertension, i.e. diastolic blood pressure of 90-100 mmHg (8 patients), group 5--patients with dilated cardiomyopathy and normal blood pressure (11 patients). The haemodynamic status and cardiac contractility indices were measured in each patient on admission, using M-mode echocardiography. Serum sodium and potassium as well as the urinary sodium, potassium and vanillyl mandelic acid excretions were also measured. The stroke volume, cardiac output and cardiac index fell with heart failure, but much more remarkably in group 4. The peripheral vascular resistance was higher in groups 2, 3 and 4 than in groups 1 and 5; so also were the aortic diameter, left posterior wall thickness and left ventricular mass. The plasma volume, aldosterone and cortisol levels were higher and the urinary sodium and potassium excretion lower in patients with heart failure (groups 3, 4 and 5). It is concluded that the raised blood pressure found in some patients suspected to have dilated cardiomyopathy is not due to the haemodynamic and biochemical changes that occur in heart failure. Such patients are 'chronic' hypertensives with hypertensive heart failure. Their presenting blood pressure is low because of their markedly reduced cardiac output.

Single-strand breaks in DNA of peripheral lymphocytes of styrene-exposed workers.

Single-strand breaks (SSB) were determined by means of the DNA unwinding technique in peripheral lymphocytes of styrene-exposed workers and referents. A slight increase in SSB was observed among exposed subjects in comparison with a control group of office workers. A correlation was found between SSB, the excretion of mandelic acid and the concentration of styrene glycol in blood. The present work suggests that the DNA unwinding technique can be used for screening workers exposed to genotoxic compounds.

Free 3-methoxy-4-hydroxyphenylglycol (MHPG) in human urine: metabolic routes, biochemical and clinical significance in healthy and obese children.

Conjugated MHPG is an indicator of the adrenergic tonus having clinical use in the follow-up of affective disorders, arterial hypotension, anorexia nervosa and recently of obesity. Little is known about the significance of the unconjugated, free MHPG in various body fluids. Nineteen healthy and 38 obese children were studied as regards the urinary excretion of free MHPG (MHPGF), conjugated MHPG (MHPGC), total MHPG (MHPGT) and also free noradrenaline (NA) valnillyl mandelic acid (VMA) and metanephrines (MN). Part of the obese received a short-term course of drug therapy (diethyl-propion-DEP or thyroid extract) and were subjected to a low-carbohydrate diet containing 800-1000 calories a day. The same urinary determinations were made after diet alone or after diet and drug therapy. MHPGC and MHPGT were significantly higher in the obese children. The level of MHPGF was essentially the same in both groups and did not change significantly following any drug or after diet alone. It was concluded that esterification mechanisms are not involved in the pathological states expressing high or low levels of MHPGC. Suggestion is made that these mechanisms may not be saturated unless huge amounts of MHPG outflow from the brain into CSF.

Decrease of urinary levels of adrenergic metabolites including conjugated MHPG after a short term course of diethylpropion in obese children fed a mixed, low-calorie diet.

A short term course (8-11 days) of diethylpropion 50 mg a day was administered to 12 obese children of both sexes fed a mixed low calorie diet containing about 1000 calories/day. A group of 10 obese children received the same diet and no medication. The sex-distribution was similar in both groups as well as the degree of overweight (appreciated by the Quettelet index). The urinary excretion of some noradrenergic derivative (adrenaline-A, noradrenaline-NA, vanillyl-mandelic acid VMA, metanephrines NM and methoxy hydroxyphenyl glycol-MHPG) and of homovanillic acid (HVA) was measured in both groups before and after drug therapy or diet, respectively. After the diethylpropion course, the urinary mean value of MHPG decreased (2.17 +/- 0.30, mean +/- SE vs 1.47 +/- 0.21) but the significance was borderline. The mean level excretion of NA, VMA, MN and HVA were significantly lower following diethylpropion. No significant change was observed in the diet-only group of patients as regards the same determinations. These observations are not entirely explained by the previous pharmacologic knowledge on diethylpropion and other sympathomimetic amines.

Quantitation of iodine-123 MIBG uptake by normal adrenal medulla in hypertensive patients.

Eighteen hypertensive patients with a clinical suspicion of pheochromocytoma and raised or borderline raised plasma catecholamine and urinary vanillyl mandelic acid (VMA) levels were studied by scintigraphy using 123I-labeled metaiodobenzylguanidine (MIBG). None of these patients had any scintigraphic evidence of pheochromocytoma at the time of study or on subsequent clinical follow-up. A quantitative approach was taken to calculate the adrenal medullary uptake of [123I]MIBG in these patients. Three different methods of quantitation were evaluated using data acquired from an anthropomorphic phantom and analysed by three independent observers. In the patient studies 34 out of 35 adrenal medullas were visualized with uptake in the range of 0.01-0.22% of the administered dose 22 hr postinjection which was calculated using the preferred quantitation method. This is an appropriate control group range for comparison with patients who have proven norepinephrine and epinephrine secreting tumors. A quantitative approach to [123I]MIBG imaging provides an important tool for studying adrenomedullary pathophysiology.

Recurrent operated malignant pheochromocytoma.

Pheochromocytoma was diagnosed in a female patient aged 26 with paroxysmal arterial hypertension (AH), profuse sweating, headache, anxiety, tachycardia, transit arrhythmia, high urinary norepinephrine (NE), epinephrine (E) and vanillyl mandelic acid (VMA). Urography with intravenous Odiston revealed a relatively large tumor at the upper pole of the right kidney. Complete removal of the tumor in a 2-step surgical procedure brings apparent clinical and hormonal recovery for 6 years, but then clinical and hormonal relapse occurs through ganglionary metastasis and tumor at the basis of the mesentery. Removal of the metastasis and tumor brings again improvement in the clinical and hormonal picture.

Severe arterial hypertension caused by chronical abuse of a topical mineralocorticoid.

A 24-year-old woman was referred to our clinic because of severe hypocalemic hypertension and latent myocardial insufficiency. Plasma renin activity and plasma aldosterone concentration were lowered. Plasma cortisol concentrations and the urinary excretion of vanillin mandelic acid were found within normal range. Some efforts to identify a specific adrenal enzymatic defect were unsuccessful. By combined therapy with spironolactone, methyldopa and clonidine, the blood pressure was significantly lowered, but not normalized. Triamterene had also an expressed hypotensive effect and induced orthostatic phenomena. After six months of hypotensive drug therapy, the patient's mother reported that her daughter consumed for more than seven years high doses of special nasal drops containing, beside ephedrine and naphazoline, the potent mineralocorticoid 9-alpha-fluoroprednisolone. A strong drug-dependence to the naphazoline component could be evaluated. The application of the mineralocorticoid was stopped, and only a slight low-sodium diet had to be added to restore the long-standing elevated blood pressure to normal
