Silicone band technique for laparoscopic tubal sterilization in the gravid and nongravid patient.

Laparoscopic tubal sterilization by the Falope ring technique was performed on 196 patients. Sixty-four patients underwent laparoscopic tubal sterilization at the time of gestation termination. One hundred thirty-two patients underwent interval laparoscopic sterilization. The complication and failure rates in the group that had both laparoscopic tubal sterilization and gestation termination was lower than in the group that had laparoscopic tubal sterilization alone. The Falope ring sterilization technique is a safe method when combined with gestation termination, and spares the patient the inconvenience and increased hazard of 2 separate procedures.

Alcohol abuse in mothers during gravidity and breastfeeding brings changes of hippocampal neurons in their offspring.

Neurotoxic effect of ethanol on the CNS of laboratory rats in the prenatal and postnatal period was studied. Another aim of the experiment was to analyse structure of the hippocampus after the prenatal and postnatal exposure to alcohol and to identify the most vulnerable hippocampal regions. Pregnant Wistar rats of our own breed received 20% alcohol p.o. ad libitum every day since the conception to the 18th day of postnatal life of their offspring. Since the birth (the day 1) till the age of 18 days offspring were kept together with their mother and were exposed to postnatal alcohol effect (alcohol in the breast milk). At the age of 18 days animals were perfused under deep thiopental anaesthesia with buffered solution of paraformaldehyde. Serial sections were stained with Fluoro-Jade B and DNA specific dye bis-benzimide (Hoechst No 33258). Brains of young rats aged 18 days were analysed under the light microscope Olympus Provis AX-70 with epifluorescence. In CA1 and CA3 areas and in Gyrus dentatus of the hippocampus, groups of degenerating cells were observed. In all offspring some cells with fine granulated karyons were identified, which were accompanied with high numbers of glial cells. Our results demonstrate the neurotoxic effects of alcohol and the high vulnerability of the developing CNS. The identification of cells with segmented karyons indicates the role of apoptotic mechanism in the cell death.

Alternate evaluations of proteinuria in the gravid hypertensive patient.

OBJECTIVE: To determine if the 12-hour urine total protein value correlates with the 24-hour value and to evaluate the random protein:creatinine ratio as a predictor of significant proteinuria (> or = 300 mg/24 h) for use in diagnosing preeclampsia. STUDY DESIGN: The study population included 15 patients with hypertensive disorders of pregnancy. The patients' urine was collected over 24 hours in 2 12-hour aliquots. The urine volume, total protein and creatinine were measured. The patients' initial voids were collected and evaluated for random urine protein and creatinine and calculation of the protein:creatinine ratio. The protein:creatinine ratio and 12-hour results were compared to the 24-hour results using simple regression analysis. RESULTS: Of the 15 patients, 6 had no proteinuria, 5 had mild proteinuria, and 4 had severe proteinuria (60% with significant proteinuria). The 12-hour protein results correlated with the 24-hour results for patients with mild disease (p = 0.00007, first 12 hours, and p = 0.012, second 12 hours) and severe disease (p = 0.014 and p = 0.007). The results for no disease were mixed: for the first 12 hours there was a poor correlation, but the results for the second 12 hours correlated well. The protein:creatinine ratio had a significant correlation (p = 0.02), using a cutoff of 0.15, returned specificity of 50%, sensitivity of 100%, positive predictive value of 75% and negative predictive value of 100%. CONCLUSION: Total protein values for 12- and 24-hour urine samples correlate well for the diagnosis of preeclampsia. A protein:creatinine ratio of < or = 0.15 rules out significant proteinuria. In combination, these 2 tests may allow more rapid diagnosis of preeclampsia.

[Rheology and gravidic hypertension].

Gestational hypertension is the development of hypertension and proteinuria after the 20th week of gestation. The most common causes of increased peripheral resistance are the vasoconstriction and hemoconcentration with plasma volume contraction. Additional rheological parameters are an elevated red blood cell aggregation and impaired erythrocyte deformability. Preeclamptic patients showed a significantly low cardiac output and central venous pressure than normal pregnant women. It has already been shown by the studies by Hytten and Paintin (1963) and also by the subsequent studies by Garn et al. (1981), Murphy et al. (1986) that a strong correlation exists between newborn weight and plasma volume. Other authors (Gallery et al. (1979/1981)) show the possibility that plasma volume contraction plays an even larger role than vasoconstriction in the fetal growth retardation that often accompanies maternal hypertension. This possibility is supported by the finding that hypertension and perinatal complications can be reduced in some pregnant women by the admission of oncotic solutions (i.e. hydroxyethyl-starch) that expand plasma volume. Volume expansion with hydroxyethyl-starch appears to be of therapeutic benefit for hypertensive patients and patients with fetal growth retardation with low cardiac output.
