The article summarizes the results of epidemiological observation which was performed in one of the training units. The goals of this observation were to estimate the prophylactic efficiency of the selected application of Natrii nucleinas in persons with the lowered immunoresistance on the background of antigrip immunization. A high clinico-epidemiological effectiveness ws approved for 10-days administration of the Natrii nucleinas in persons with clinic manifestations of grip and other respiratory infections at the moment of vaccination. It is resulted in the diminishing of the morbidity index, duration and gravity of this pathology in the postvaccinal period. The administration of Natrii nucleinas was experimentally substantiated during this epidemiological observation.

BACKGROUND: The worsening evolution of patients undergoing surgical myocardial revascularization makes it difficult the stratification of the preoperative mortality risk, a correct evaluation of results and the comparison of results of different centers. The aim of the study was to evaluate the prognostic weight of comorbidity in surgical myocardial revascularization. METHODS: We evaluated the characteristics of preoperative morbidity in 4999 patients who underwent surgical myocardial revascularization during four different periods (1979-1980, 1991-1992, 1994-1998, 1999-2002). We also evaluated the in-hospital results. RESULTS: By comparing the four different periods, an increase in older age, female sex, comorbidity, three-vessel disease, and severe left ventricular dysfunction was observed. Surgical mortality decreased to 2.3%. Multivariate analysis of the 1999-2002 period showed that only renal insufficiency was a risk factor for in-hospital mortality. CONCLUSIONS: Although the preoperative risk is higher, nowadays hospital mortality is reduced thanks to new cardiac-surgical techniques and approaches that increasing the capacity of controlling comorbidity in the pre-, intra- and postoperative course. For a correct decision-making process it is crucial to assess how much comorbidity may influence the long-term follow-up in these patients independently of surgical myocardial revascularization.
A case of 4-year-old boy with Wildervanck's syndrome and associated profound sensorineural hearing loss is presented. The child was disqualified from cochlear implantation following the CT scan of petrous pyramids which revealed inner ear malformation. The authors emphasize role of CT imaging in the management of profound sensorineural hearing loss in children before final qualification for cochlear implantation.
Patency of the foramen ovale (FO) is a very rare complication after lobectomy. Completion pneumonectomy after FO reopening has never been described before. In the reported case, a patent FO was diagnosed in a 52-year old man 9 months after a left upper sleeve lobectomy for a squamous cell carcinoma pT2N1. At the same time, bronchoscopy showed an endobronchial recurrence in the left main bronchus. The foramen was closed percutaneously by a 35 mm Amplatzer prosthesis. Dyspnea improved dramatically in the following 4 weeks and no more oxygen therapy was required. After complete restaging, a completion pneumonectomy was performed without any postoperative complication. This case report suggests that pneumonectomy in such delicate patients is feasible.

An immunization advisory clinic was set up in mid-Essex in 1988 to provide a referral facility for professionals and parents who were unsure about the eligibility of certain children to receive immunization. This paper describes four typical cases. The history and management of each case are described and the fact that all the children were successfully immunized is highlighted. It is hoped that by sharing the experiences of the immunization advisory clinic with other professional staff, more positive decisions regarding immunizations will be made.
