Double-blind comparisons of a selective serotonin reuptake inhibitor, zimelidine, and placebo on quantified EEG parameters and psychological variables.

A double-blind crossover comparison of three different single doses of a selective serotonin reuptake inhibitor, zimelidine, and a placebo on quantified EEG parameters and psychological variables was carried out in ten healthy male volunteers. Each subject randomly received 100 mg, 200 mg and 300 mg of zimelidine as well as placebo. C4--P4, P4--O2, C3--P3, P3--O1 and Fz--O1 EEG samples were taken by means of collodium Ag/AgCl stick on electrodes. 3 min average Fast Fourier Transformation power spectra based on 5 sec epochs were established. Additionally, performance tests and a subjective self-rating were carried out. The measurements, performed according to a strictly controlled schedule, were repeated 1, 2, 4 and 6 h post baseline (i.e. 8 a.m.). The data analysis was performed by means of two-way variance analysis for repeated measurements. Administration of three different single oral doses of zimelidine was found to significantly stabilize the average vigilance behaviour in terms of reduced slow wave EEG activity and of increased monorhythmic alpha activity. Simultaneously, concentration ability was improved and mild feelings of activation were reported. The results are discussed in the light of biochemical aspects and with regard to neurophysiological vigilance concepts.

[Maintenance therapy with antiulcer drugs: review of 71 cases].

OBJECTIVE: To find the characteristics of our patients being treated with anti-ulcer medication and to evaluate whether the prescriptions were correct. DESIGN: A descriptive, crossover study. SETTING: Perpetuo Socorro Health Centre in Huesca. PATIENTS: 71 patients selected through their long-treatment booklets during october-november 1992. MEASUREMENTS AND RESULTS: Using as our data source the clinical records, we evaluated age, gender, toxic habits, associated chronic problems, year and diagnostic method, aptness of the therapy indicated, beginning of long-term treatment, the medication chosen and relapses occurred. A data base dBase III Plus was used, applying a descriptive statistical analysis with the #x2 test, comparison of means and variance analysis. The group's mean age was 61.53 and had a high percentage of associated chronic pathology (81.6%). 69.01% suffered from a duodenal ulcer and 30.99% presented other problems which were susceptible to maintenance treatment with anti-ulcer medication. Overall treatment indication was correct in 87.93% of cases. 83.09% of the patients had long-term therapy, the indication being correct in 74.14%. The proportion of correct treatment was greater in the at-risk group (p < 0.005). The drugs used were ranitidine, famotidine, omeprazole and almagate. CONCLUSIONS: It is important to identify correctly those patients at risk of developing ulcerous pathologies so that the maintenance therapy can be suitably adapted and the best choice of drug be made, with the medical interactions being borne in mind.

Concentration of 137Cs in dried Lentinula edodes (Shiitake) as an indicator of environmental contamination.

We analysed the concentration of radioactive cesium in dried Lentinula edodes (Shiitake) produced in Japan, and evaluated its use as an indicator of radioactive contamination. We collected Lentinula edodes (Shiitake) mushrooms grown on wood in 38 prefectures from north to south, and measured 137Cs, 134Cs and 40K concentrations with high purity Ge-detectors. The concentration ranges were as follows: 137Cs <0.003-61.1 Bq/kg, 40K 378.7-667.3 Bq/kg; 134Cs was undetectable. There was a positive correlation between 137Cs concentrations in Shiitake and those in rain and dry fallout (P < 0.05). They were also correlated inversely with atmospheric temperature (P < 0.01). The multiple correlation coefficient comparing 137Cs concentration with atmospheric temperatures, 40K concentrations and fallout levels was 0.56 which was statistically significant (P < 0.01). A variance analysis based on the 137Cs concentration classified by geographical area (Hokkaido-Tohoku, Kanto, Chubu, Kinki, Chugoku, Shikoku and Kyushu) showed that the variance between the groups was significantly greater than that within each group which, together with the above-mentioned information, suggests that the 137Cs levels are affected primarily by these environmental factors.

Reproducibility of a histopathologic grading system for adult soft tissue sarcoma.

Tumor grade has been proposed as an essential factor in the staging of patients with soft tissue sarcomas. In a previous study, a histopathologic grading system using the evaluation of tumor differentiation, mitosis count, and tumor necrosis was described. The current study was conducted to test its reproducibility. The pathologic sections of 25 soft tissue sarcomas were submitted to a study group composed of 15 pathologists who had not been involved in the development of the grading system. The results were compared with those of a panel group. The crude proportion in agreement observed between the study group and the panel group was 81% for the evaluation of tumor necrosis, 74% for tumor differentiation, and 73% for the mitosis count. The crude proportion in agreement for the tumor grade was 75%, which was significantly better than the crude agreement rate of 61% for the diagnosis of histologic type (P = 0.001). A kappa statistical analysis, to check the possibility of chance-related concordance, showed a proportion in agreement of 68%. A two-way variance analysis showed that the homogeneity of the evaluation of tumor grade is impaired by tumor-related and observer-related factors. However, an improvement may be obtained by better training of pathologists. We conclude that the tumor grading system developed inside the French Federation of Cancer Centers, although perfectible, already provides reliable prognostic information and its use in prospective clinical studies may provide more information about its clinical usefulness.

[Genetic analysis on flowering and boll setting in upland cotton (Gossypium hirsutum L.). III. Genetic behavior at different developing stages].

An additive-dominant genetic model with genotype by environment interaction was employed to analyze the genetic behavior of flowering and fruiting at different development stages, with 2-year data from 4 x 4 diallel crosses. The results of variance analysis indicated that flowering and fruiting behaviors were mainly controlled by dominant effects at early period, but by additive effects later on. Compared with genetic main effects, GE interaction effects playeda minor role on the flowering and fruiting behavior. The analysis of covariance between average flower number, boll number at different development stages and total number of bolls showed that there were significant positive dominant correlation and negative or zero additive correlation before August 1st, but having the opposite results afterward. Interaction correlation existed during the whole period. It was found, by further analysis of conditional variances, that different development stages have diverse gene action intensity. The last ten days of July and the first-middle ten days of August had the highest gene action level. Time interval of investigation played a great role on exploring the law of gene action during the flowering and fruiting period; when selecting the time interval of investigation, it is necessary to contemplate the purpose of the experiment, environmental condition, investigating traits and developing period simultaneously.

New data analysis and mining approaches identify unique proteome and transcriptome markers of susceptibility to autoimmune diabetes.

Non-obese diabetic (NOD) mice spontaneously develop autoimmunity to the insulin producing beta cells leading to insulin-dependent diabetes. In this study we developed and used new data analysis and mining approaches on combined proteome and transcriptome (molecular phenotype) data to define pathways affected by abnormalities in peripheral leukocytes of young NOD female mice. Cells were collected before mice show signs of autoimmunity (age, 2-4 weeks). We extracted both protein and RNA from NOD and C57BL/6 control mice to conduct both proteome analysis by two-dimensional gel electrophoresis and transcriptome analysis on Affymetrix expression arrays. We developed a new approach to analyze the two-dimensional gel proteome data that included two-way analysis of variance, cluster analysis, and principal component analysis. Lists of differentially expressed proteins and transcripts were subjected to pathway analysis using a commercial service. From the list of 24 proteins differentially expressed between strains we identified two highly significant and interconnected networks centered around oncogenes (Myc and Mycn) and apoptosis-related genes (Bcl2 and Casp3). The 273 genes with significant strain differences in RNA expression levels created six interconnected networks with a significant over-representation of genes related to cancer, cell cycle, and cell death. They contained many of the same genes found in the proteome networks (including Myc and Mycn). The combination of the eight, highly significant networks created one large network of 272 genes of which 82 had differential expression between strains either at the protein or the RNA level. We conclude that new proteome data analysis strategies and combined information from proteome and transcriptome can enhance the insights gained from either type of data alone. The overall systems biology of prediabetic NOD mice points toward abnormalities in regulation of the opposing processes of cell renewal and cell death even before there are any clear signatures of immune system activation.

Serial variation in histological character of articular soft tissue in young human adult temporomandibular joint condyles.

Histological variation was studied in serial sections, in contrast to previous studies which have generalized from representative sections. The sample consisted of consecutive serial sagittal sections from the central third of nine condyles, plus an accompanying stone cast showing the intact articular surface before sectioning. The thickness of the articular soft tissue and its fibrous connective tissue and cartilage components was measured, and the presence of undifferentiated mesenchymal (UM) cells was assessed by low-power light microscopy. Components of variance analysis showed that section-to-section variation in thickness was of the same order as differences between joints, each explaining approx. 50% of the variance in both connective tissue and cartilage thickness. The fibrous connective tissue contributed as much to the overall variation in soft tissue thickness as did the cartilage component (SD 0.0946 versus 0.0909 mm for the superior sector). Serial UM cell variability was common, and the UM cells were often distributed in islands rather than uniformly across the articular tissue. Condyles with the greatest surface irregularity were characterized by greater serial variability in fibrous connective tissue thickness, more frequent absence of cartilage, and more areas of UM cell depletion. These results suggest that serial variation in histological character may be more important than mean values in the description of surface contours and articular tissue relations in the temporomandibular joint. This should influence the design of future investigations.

Sonoclot coagulation analysis: a study of test variability.

OBJECTIVE: To test the reproducibility of Sonoclot coagulation analysis (SCA; Sienco Inc, Morrison, CO). The authors wished to determine if the mix/release of the preloaded celite activator in standard Sono-cuvettes could be responsible for the high variation coefficients for SCA parameters with citrated whole blood and if citrated whole blood is optimal for SCA. DESIGN: A prospective trial. SETTING: A large academic teaching medical center. PARTICIPANTS: Eight healthy volunteers. INTERVENTIONS: Repeated blood sampling was performed through indwelling radial artery catheters. Seven different Sonoclot analyzers were used to test seven different types of analysis procedures in the volunteers, involving activators of different types and amounts and in different forms, and the use of citrated or native whole blood. MEASUREMENTS AND MAIN RESULTS: Two-way and one-way ANOVA, variance, variance analysis, and Tukey's test were used to evaluate differences in SCA methods and volunteer influence. A high variance, with SDs up to 200% of the median values of the SCA parameters with recalcified citrated blood and the standard Sono-cuvette, was confirmed. SCA with native blood and/or the use of other types of preloaded activators, ie, kaolin, significantly (p < 0.05) reduced this variance. Repeated SCAs further reduced the variance to 10% to 35% of the variance for a single analysis (standard cuvette and native blood). CONCLUSION: Improvement of the activator in the Sonocuvette, use of native whole blood, and repeated Sonoclot analyses reduced the previously reported high variability of this instrument.

Cadmium, chromium and lead contamination of Athene noctua, the little owl, of Bologna and Parma, Italy.

A study was conducted to determine cadmium, chromium and lead concentrations in liver and brain of 52 little owls (Athene noctua) from two provinces of Emilia Romagna region, with the aim of furnishing indirect information concerning contamination of their habitat, also considering possible environmental dispersion of the metals. Metal analysis was performed by atomic absorption spectrophotometry with graphite furnace. Variance analysis with sampling area, gender and age shows that no statistical difference was found for gender, while a significant difference (P<0.05) was found for cadmium and lead, but not for chromium, when sampling areas and age were of concern. For all metals highest mean concentrations were found in liver (170 ppb for cadmium, 297 ppb for chromium and 312 ppb for lead). These levels can be considered as indicative of chronic exposure to low and "background" amounts of pollutants and they are of no toxicological concern, as they are always well below the toxic thresholds defined for each metal. The present study can be considered as a starting point for further analyses, aimed to the definition of any possible subtle effect (e.g. effects on enzymes activity) and of any possible correlation between levels of pollutants and appearance of possible adverse effects. It also furnished useful data for diagnostic cases and potentially for monitoring local contamination.

Performance of latex-fixation kits used for serologic diagnosis of rheumatoid factor in rheumatoid arthritis sera.

Ten latex-fixation kits (both slide and tube) were evaluated by use of a reference serum containing 250 IU of rheumatoid factor (Communicable Disease Center). All tests were performed with serial dilution of serum; adjacent tubes differed by a factor of 0.10 log10. Geometric titers, means, standard deviations, and variance analysis were used to evaluate commercial kits. Three levels of variance were applied: (1) 0.18 or less for intralaboratory variation when the same serum is tested with the same kit; (2) 0.18-0.35 when various lots or batches of the same serum were used; (3) 1.0 or less when various commercial kits were tested with the same serum. The sensitivity of each commercial kit was determined by assigning to it a value (the minimum detectable unit) arrived at by dividing the number of units in the standard by the titer obtained. The technic presented could be applicable to other serologic tests, and thus provide a general method for their standardization and quality control.

Effect of short-term cimetidine administration on fecal bile acid losses in cystic fibrosis.

Eight children with cystic fibrosis and exocrine pancreatic insufficiency were voluntarily hospitalized in a clinical research setting so that diet and enzyme supplements could be well controlled for consecutive 72-h balance studies. We previously reported on these patients, as part of a larger study group, because they showed significantly reduced fecal losses of fat and nitrogen at the highest dose of cimetidine added to the otherwise constant regimen. Analysis of specimens from this same study shows a cimetidine-related reduction in fecal bile acid loss. Numerous variables thought to influence fecal bile acid loss were closely controlled and/or measured in this clinical study setting. In order to potentially learn more about this complex phenomenon, we carried out a stepwise regression analysis of the data which indicated that half of the variance in fecal bile acid loss is still unaccounted for by the five "independent" variables tested in the regression. However, in this variance analysis system cimetidine assumes a larger relative role in modulating bile acid loss than it does in fecal losses of fat and nitrogen.

Quantification of venous reflux by means of some duplex scanner and light reflection rheography parameters and its correlation with chronic venous insufficiency symptoms.

BACKGROUND: The quantification of venous reflux is rarely made but it is valuable for studying the natural history of CVI, its prognostic implications and the therapy effectiveness. We have evaluated some parameters of chronic venous insufficiency, caused by valvular incompetence, by means of duplex scanner and light reflection rheography searching for a better correlation with the clinical stages of vein disease. METHODS: We have examined 107 patients (35 males, 72 females; aged 22-78, mean 61.5 +/- 14.3) with long saphenous insufficiency in different clinical stages and measured the venous reflux, the reflux duration, the reflux grade, the vein diameter near the saphenofemoral junction with the duplex scanner and the refilling time with the light reflection rheography. The reproducibility of the studied techniques has been determined by examining 20 legs four times each in the same day or on different days. Statistical analysis of the data was performed by means of the variance analysis followed by the Waller-Duncan test. RESULTS: The venous reflux, the venous diameter and the refilling time can easily range between mild and severe insufficiency: reflux is 6.3 +/- 2.5 ml/sec at stage I; 27.5 +/- 10.1 at stage III; venous diameter is 6.04 +/- 1.4 at stage I; 10.6 +/- 2.2 at stage III; refilling time is 23.6 +/- 9.6 at stage I; 5.4 +/- 2.4 at stage III. The reflux time and reflux grade are unable to separate the various CVI stages. CONCLUSIONS: The measurement of venous reflux, venous diameter and refilling time is simple, quick, reproducible; these indices are able to differentiate mild from severe CVI but unable to separate patients at the second stage of the venous disease from those at the third stage and then ineffective in the ulcer risk assessment but the reflux is highly predictable. The ulcer risk is practically absent for reflux < 12 ml/sec; highly probable for reflux > 15 ml/sec (20/46 cases; 43.4%).

[Nisoldipine in the therapy of stable exercise angina: evaluation of its efficacy in ergometric tests compared with propranolol].

The safety and efficacy of a new dihydropyridine calcium antagonist, nisoldipine, were studied in comparison with propranolol, in patients with stable angina. Following 2 weeks of wash-out of the current therapy and 1 week of placebo, 26 patients underwent two cycloergometer tests, before and 2 hours after placebo administration. Subsequently, 10 mg of nisoldipine twice daily or 40 mg of propranolol three times daily were administered over a 4 week period, in a randomized single-blind fashion. Cycloergometer tests were performed after 2 and 4 weeks, before and two hours after drug administration. Two patients, one out of the nisoldipine group and one out of the propranolol, were considered drop outs. Statistical analysis of the results was performed using two-way variance analysis. With both drugs, time to 1 mm exercise induced ST-segment depression (p less than 0.01), time to angina (p less than 0.01), and exercise tolerance (p less than 0.01) improved furthermore, ST-segment and heart rate recovery time decreased (p less than 0.01). Rate-pressure product values at ischemic threshold and at peak exercise remained unchanged with nisoldipine, while significant reductions were observed with propranolol (p less than 0.01). Maximum ST-segment depression was reduced (p less than 0.01) with both drugs and a further improvement was observed two hours after drug administration (p less than 0.05). At submaximum work-load, rate pressure product values and ST-segment depression were improved with both drugs (p less than 0.01); this improvement was more evident two hours after drug administration. No significant side effects were observed with either drug.(ABSTRACT TRUNCATED AT 250 WORDS)

A comparison of digital and film radiography in Dutch dental practices assessed by questionnaire.

OBJECTIVES: The purpose of this study was to assess the experiences of general dental practitioners (GDPs) with solid-state and storage phosphor digital sensors compared to film. METHODS: In a mail survey 578 questionnaires were sent to Dutch GDPs who were users of digital X-ray detectors and film. The questionnaire requested demographic data, information about the digital system used and the user-friendliness of the X-ray detector system. In the analysis of the data these variables were related to the type of system used and also to the demographic data. The data were analysed using descriptive statistics, variance-analysis (One-Way ANOVA) and nonparametric tests (Kruskal-Wallis, Mann-Whitney and Wilcoxon) (SPSS 9.0). RESULTS: Four hundred and twenty-four questionnaires were returned (overall response rate 73%). The user-friendliness of the handling of the different systems before exposing the radiograph is better for conventional film, whereas the handling after exposing the radiograph favoured the digital X-ray detector systems. CONCLUSIONS: The user-friendliness was best for film pre-exposure and digital sensors post-exposure.

[The characteristics of the autonomic regulation of the heart rate in patients with IHD and ventricular arrhythmias under obzidan therapy].

Propranolol hydrochloride was administered to 30 patients with stable angina pectoris suffering from coronary heart disease (CHD). Of them 18 patients had ventricular extrasystole, 12 had ventricular tachycardia. All the patients underwent resting ECG, bicycle ergometry, transesophageal pacing, 24-h ECG monitoring. Vegetative cardiac regulation was determined using mathematical analysis of at least 100 successive intervals R-R at rest before and after therapy (120 mg day of propranolol hydrochloride). The authors estimated characteristics of the variance analysis and two secondary values--stress index and centralization index calculated by spectral characteristics of the rhythm. Only in CHD patients with ventricular tachycardia the treatment shifted the vegetative tone in the direction of parasympathetic nervous system. It is concluded that propranolol hydrochloride affects cardiac rhythm depending on the initial condition of vegetative regulation of cardiac activity.

Analysis of molecular variance inferred from metric distances among DNA haplotypes: application to human mitochondrial DNA restriction data.

We present here a framework for the study of molecular variation within a single species. Information on DNA haplotype divergence is incorporated into an analysis of variance format, derived from a matrix of squared-distances among all pairs of haplotypes. This analysis of molecular variance (AMOVA) produces estimates of variance components and F-statistic analogs, designated here as phi-statistics, reflecting the correlation of haplotypic diversity at different levels of hierarchical subdivision. The method is flexible enough to accommodate several alternative input matrices, corresponding to different types of molecular data, as well as different types of evolutionary assumptions, without modifying the basic structure of the analysis. The significance of the variance components and phi-statistics is tested using a permutational approach, eliminating the normality assumption that is conventional for analysis of variance but inappropriate for molecular data. Application of AMOVA to human mitochondrial DNA haplotype data shows that population subdivisions are better resolved when some measure of molecular differences among haplotypes is introduced into the analysis. At the intraspecific level, however, the additional information provided by knowing the exact phylogenetic relations among haplotypes or by a nonlinear translation of restriction-site change into nucleotide diversity does not significantly modify the inferred population genetic structure. Monte Carlo studies show that site sampling does not fundamentally affect the significance of the molecular variance components. The AMOVA treatment is easily extended in several different directions and it constitutes a coherent and flexible framework for the statistical analysis of molecular data.

The consequences of fixed-pattern noise and image movement on electronic portal images.

Fixed-pattern noise in electronic portal images (EPIs) is normally eliminated by dividing raw image data by an open-field calibration image (OFCI). However, for successful elimination, there must be exact registration between the two image sets. Any movement within the imaging system, such as that which occurs with gantry angle, will result in misregistration and a subsequent increase in noise. This paper describes, both qualitatively, by way of example, and quantitatively, by way of variance analysis, the consequences of misregistration and its effects on image quality. Our results show that image quality is found to degrade significantly with change in gantry angle, when a single OFCI is used, with a loss of low-contrast, fine detail. Variance is observed to increase over 2.5-fold. A simple solution of using multiple OFCIs is described, along with a technique for optimizing the number of images required, and the gantry angles at which they are acquired. When five OFCIs are used, the variance changes with gantry angle are limited to less than 20%. These changes are observed in both long and very short exposures (5 MU or 1 s).

The relationship between workers' safety culture and accidents, near accidents and health problem.

One of the dimensions treated as part of a company's safety culture or climate is workers' attitudes towards risk and safety. In the present study these personal aspects are defined as workers' safety culture, which is understood as a way of acting focused on life and taking care of one's health. A questionnaire on safety culture was filled out by 200 employees of a metallurgical enterprise. Factor analysis was used to determine empirical scales of the questionnaire, whereas variance analysis was used to test hypotheses. The results confirmed the hypotheses that people who experienced accidents, dangerous situations, and--to a lesser extent--health problems had a lower level of safety culture. Nevertheless not all of the scales determined during factor analysis turned out to be significant as far as all kinds of those undesirable situations are concerned. Proposals for future studies are formulated in the conclusion.
