[Status of the main functional systems in humans after long-term exposure to hyperbaric medium].

The exposure of divers to hyperbaric conditions under pressure of 4.6 MPa induced development of astheno-neurotic disorders with cardiovascular component against a background of pronounced fatigue. A decrease of the pulmonary ventilation function in the form of bronchial obstruction syndrome was observed as well. On the 10-15th day of the postdiving period asthenia phenomena extinguished and the pulmonary function tended to restoration. On the 30th day of postdiving period all the functional systems were normalized. The professional activity of divers for many years gives rise to the formation of adaptive reaction (increase of vital capacity of lungs and maximal ventilation of lungs), on the one hand, and to pathological disorders, such as arterial hypertension and bronchial obstruction syndrome, on the other hand.

[Psychosocial and clinical predictors of the disease course in patients with cardiac phobia and panic attacks].

Patients suffering from cardiac phobia or neurocirculatory asthenia (also known as cardiac neurosis or effort syndrome) represent a uniform disease group due to the typicality of the signs and symptoms and of the course of the disease; this group should be distinguished from the broad spectrum of cardiovascular disorders. There is close agreement with panic disorder according to DSM III, one of the reasons being that cardiac phobia was originally the model pattern for defining that disorder. In the present prospective study the significance of psychosocial and clinical factors for the prediction of the further course of the disease was studied in 36 patients with cardiac phobia treated on an inpatient basis for the first time, 29 of whom could be followed up by a checkup 2 1/2 years later. The duration of the disease before treatment, as well as difficulties in various social spheres, especially in partnership problems, were found to be variables exercising a particularly strong influence on the course of illness. The variables concerning the marital status and school education were also found to be statistically relevant predictors. Familial stress and the appeal-for-help behaviour of these patients were also recorded. The results of the study point to the importance of psychosocial factors as major coursemodifying factors and underline the need to pay particular attention to this part of the treatment.

Psychosocial support for cosmonauts.

Based on a meeting with members of the Psychological Support Group for cosmonauts, along with several referenced documents, the author summarizes the Soviet experience in dealing with psychological and interpersonal factors related to long-duration space missions. Cosmonaut selection and training procedures following the principle of "neuropsychological resistance" formulated by F.D. Gorbov. Inflight monitoring uses the macroanalysis of crew speech characteristics as an indicator of psychological state. Psychosocial problems include "asthenia" and interpersonal tension. Support efforts focus on enhancing behavioral and autonomic adaptation, planning flexible work/rest schedules, improving the space station environment, arranging relevant free time activities, and helping crews readjust to Earth postflight. Attention is paid to changes in cosmonaut preferences and sensitivities as the mission progresses. The use of support activities is believed to be positively associated with the health and performance of cosmonaut space crews.

[A case of sterility treated with sulpiride (author's transl)].

Oligospermia associated with asthenia is a frequent cause of sterility. Sulpiride, which is a regulator of autonomic nervous system function, was prescribed alone in such a case and pregnancy resulted on two occasions. The relationship between the normal duration of spermatogenesis and the periods during which sulpiride was prescribed was such that the hypothesis of a hormonal effect could be rejected and strongly suggests the existence of psychological factors in some patients with oligospermia.

[A method for determining the thermal lesion in workers in the hot shops of the metallurgical industry].

Results of examinations obtained by means of an asthenia scale allow to evaluate the level of thermal lesions in workers of hot shops of the metallurgical industry. The method was employed in 601 workers of hot shops of the metallurgical enterprises including 18 professional groups. This method makes it possible to realize adequate mental hygiene and psychoprophylactic programs in metallurgists of hot shops.

[Essential mixed cryoglobulinemia with liver involvement: a still open problem].

Essential mixed cryoglobulinemia (EMC) is a syndrome characterized by cryoglobulinemia and clinical features including purpura, arthralgia, asthenia (Meltzer-Franklin syndrome) without evidence of any systemic disease Liver involvement in the course of EMC is described in 50-84% of patients. It consists of mild silent hepatosplenomegaly and slightly rise of serum amino transferase. Eleven patients with clinical and laboratory findings suggestive for EMC (five type II and six type III) underwent percutaneous liver biopsy to evaluate the degree of liver involvement. Two liver cirrhosis, two chronic active hepatitis, one chronic persistent hepatitis and a case of hepatic steatosis were found. A type III cryoglobulinemia was present in four of the six patients with liver involvement. All the patients were Hbs Ag negative but three of them were Hbs Ab positive. The pathogenesis of liver involvement in the course of EMC is still now uncertain. The authors believe that a previous HBV infection plays no role in the pathogenesis of EMC syndrome. This syndrome must be considered different from mixed cryoglobulinemia secondary to chronic liver disease. They suggest that liver biopsy is mandatory during the course of EMC even when clinical and laboratory data are silent.

[Characteristics of neurotic state and transmembrane transport of sodium in erythrocytes in hypertensive and cardiac variants of neurocirculatory dystonia].

The paper provides findings from an examination of 26 patients with neurocirculatory dystonia (NCD) with the hypertensive syndrome, 18 with NCD of the cardiac type (neurocirculatory asthenia), and 10 healthy subjects. Application of original psychological questionnaires made it possible to detect anxiety neurosis in a cardiac variant of NCD and neurosis with marked conversion in a hypertensive variant of NCD. In the latter, there was an increase in Na+/H+ and Li+/Na+ countertransport rates in the erythrocyte membrane, which was typical of essential hypertension. In the former variant, the Na+/H+ countertransport rate was lower in the erythrocyte membrane than that in the controls, whereas no differences were found in Li+/Na+ countertransport rates between the cardiac NCD patients and the controls.

[The diagnostic spectrum of the general malignancy syndrome].

The study presents 391 patients with malignant general syndrome (asthenia, anorexia and involuntary weight loss) among the interns in a Internal Medicine Department, and it settles a diagnostic relation. In comparison with other studies it the diagnostic spectrum is very similar to that founded in other countries, in spite of their own characteristics.

[10 years' incidence of positive diagnostic decisions in functional heart disease in acute clinic treatment].

An unexpected low percentage of 0.89 per thousand (43 Patients) with the diagnosis of neurocirculatory asthenia could be documented in a clinical database of 48.114 patients with representative data of diagnostic and therapeutic procedures in West German hospitals. As expected, mean manifestation age in male patients was the age group of 20 to 39 years. Only 4.1% of the neurotic patients were committed to the hospital with this diagnosis. These data document the representative diagnostic behaviour towards neurotic chest pain patients in acute hospitals and probably reveal a serious underestimation of the real prevalence. There is evidence that for these patients the diagnostic strategy is weak and distorted by some systematic bias.

[Comparative study of individual characteristics of patients with borderline arterial hypertension and more severe forms of hypertensive disease].

In order to compare individual health characteristics of healthy people and patients with borderline arterial hypertension (BAH) and hypertensive disease (HD) 867 TASS employees aged 18-68 (365 men and 502 women) were examined. Abridged variant of a multifactor questionnaire MMPI was used for the determination of the psychologic profile. Those examined were divided into 3 groups, namely, the first group comprised 96 patients with BAH; the second group 41 patients with HD of the 1st stage, and the 3d group consisted of 70 persons with a normal level of arterial pressure (the control group). BAH was detected in 146 persons (17.1%) and HD of the 1st and IInd stages in III persons (13.0%). The analysis of individual characteristics showed that an average profile of the male population with normal blood pressure had no distinct peaks by separate scales, whereas the combination of the characteristics in patients with HD (a rise on the 1st and the 3d scale and a decrease on the 9th one) was regarded as R-factor. The latter revealed a decrease in general activity and the presence of depression and asthenia The above changes were also registered in patients with BAH but they were less distinct and characterized by intermediate indices in patients with HD and the healthy people. No reliable differences were noted among females.

[Treadmill exercise echocardiography: quantitative analysis of regional left ventricular wall motion by computer graphics].

To detect significant coronary lesions based on exercise-induced reversible asynergy, two-dimensional echocardiograms were recorded before and immediately after treadmill exercise test in 15 patients with angina pectoris (AP) and six patients with neurocirculatory asthenia (NCA). Short-axis views of the left ventricle were analyzed quantitatively, using the following indices: segmental area change (%A); segmental wall thickness change (%Th); and relative curvature (rC) which was the product of curvature of each segment multiplied by end-diastolic circumference. The results were compared with those of coronary angiography, T1-201 myocardial emission computed tomography, and exercise electrocardiography. 1. The results of comparisons of quantitative analysis by %A, %Th, and rC with qualitative (visual) analysis were as follows: the sensitivities were 57%, 74% and 91%; the specificities were 75%, 75% and 90%; and the accuracies were 68%, 75% and 90%, respectively. 2. To detect coronary stenosis of more than 75%, visual assessments and assessments by rC were superior to assessments by %Th or %A. The accuracies were 93%, 91%, 73% and 61%, respectively. 3. In five cases with three-vessel disease, all diseased coronary arteries were detected by treadmill exercise echocardiography using rC as an index. However, by treadmill exercise T1-201 myocardial emission computed tomography, the diagnosis of three-vessel disease was possible in only one case. 4. The results of treadmill exercise electrocardiography were positive in 11 of 15 the AP patients and in all six NCA patients. The results of exercise echocardiography using rC as an index were normal in all NCA patients and abnormal in 14 of the 15 AP patients.(ABSTRACT TRUNCATED AT 250 WORDS)

[Borderline mental disorders in workers in chemical industry plants].

A total of 550 workers at a chemical plant have been examined for symptoms of borderline neuromental disturbances. The ratio was as follows: 26.8%, normal subjects; 33.7%, predisease state; 10.3%, neurotic reactions; 1.8%, neuroses; 22.7%, neurosis-like states; 4.7%, chronic alcoholism. A normal person presents 2.7 symptoms, a neurotic one, 29.3. The rate of somatic diseases per subject is 1.6 and 3.6 respectively. Depressive and neurotic reactions in the form of neurocirculatory asthenia are quite frequent, being predominant in individuals with a shorter length of service, and are associated with financial and household difficulties and dissatisfaction with the present job.

[Dynamics and mental disorders in patients with portal hypertension after surgical treatment].

On the basis of the clinical, experimental-psychological and electroencephalographic examinations of 173 patients with portal hypertension (intrahepatic--110 and extrahepatic--63) the authors elucidated the regularities of the formation of mental disorders, their clinical characteristics and the course following surgical treatment. The asthenic syndrome with marked vegetative disturbances was the most prominent feature in the clinical picture. Patients with hepatic cirrhosis were characterized by a more severe asthenia and greater intensity of vegetative symptomatology as compared to patients with an extrahepatic form of portal hypertension. Characteristic of psychotic disorders in liver cirrhosis was severity of the psychoorganic syndrome. Personality shifts with the predominance of explosiveness, epilepsy- and paranoia-like tendencies grow with disease progression. Surgical treatment by constructing vascular anastomoses occasionally leads to intensification of the psychoorganic syndrome in cirrhosis patients and to its appearance in patients with extrahepatic portal hypertension, which calls for the development of more clear-cut indications for such surgery.

[Electroencephalographic characteristics of intellectual activity in astheno-adynamic subdepressions].

While performing associative verbal and arithmetic tasks patients with atypical subclinical astheno-adynamic subdepression with the predominance of asthenia-like thought disturbances presented characteristic disorders of reactive changes in several EEG factors. They appear to reflect non-reactivity and functional dissociation of the processes of activation, hyperactivity of the processes of cortical excitation, general blending of interzonal differences, elevated reactivity of the right hemisphere and parietal lobes of the brain.

[Evaluation of clinical symptoms of Addison's disease].

A total of 103 patients with Addison's disease were examined. The task was to determine the information content of the clinical symptoms of Addison's disease in their interrelationship. The theorem of the multiplication of probability of independent events and probability of the appearance of at least one event were used. On the basis of the above mathematical method it has been established that the most informative syndromes of Addison's disease are the combination of asthenia and adynamia with mass deficiency, arterial hypotension, skin pigmentation and nervous-psychic break-down. In comparing clinical and mathematical results it has been shown that a simultaneous appearance of the signs of the dyspeptic syndrome, its combinations with the pain abdominal syndrome and mass deficiency point out to a possibility of the Addisonian crisis or the onset of its development. At least one of three signs of muscular or articular involvement is predicted in each patient with Addison's disease. A table given in the paper makes it possible to determine the information content of numerous complexes of symptoms of Addison's disease.

[2 cases of amineptine dependence].

We relate two cases of amineptine (Survector) overconsumption by patients cured for atypical depression with asthenia and activities deficit as the prevalent symptoms. Prescription of two tablets a day (0,200 g) was respected in one case during six months, and in the other case during two years, with therapeutic benefit on apragmatism. To no obvious reason, within few months both patients had gradually raised the doses to twenty tablets (2 g) and thirty tablets (3 g) respectively: we observed subexcitation, insomnia, sensorial hyperaesthesia, irritability, tachyphemia with dysarthria, anorexia with weight lost of more than 10 kg and amphetamine-like troubles without confusion or delusion, as a result of which both patients were treated for their addiction, in hospital. Treatment with clorazepate perfusions did not cause any physical dependence problems. However, psychological dependence was strong enough for one of the patients to go out, on the third day, against medical decision. As far as we know, in France, only one such case of addiction use at high doses and in single intakes is mentioned in the existing literature. However, our observations suggest that it might be necessary to re-assess the place of amineptine among new antidepressive molecules with psychostimulant abilities.

[Low 25-hydroxy-vitamin D (25-OH-D) plasma levels in pathology (author's transl)].

Between october 1976 and march 1978, 160 patients with low 25-OH-D plasma levels were observed (25-OH-D less than 7 ng/ml). In this population, a high frequency of asthenia, anorexia, vertebral demineralization were noticed. Mean plasma level of calcium was significantly lower than controls. Mean plasma level of alkalin phosphatases was significantly higher than controls. These patients presented a varied pathology. These diseases were probably responsible for the low 25-OH-D plasma level. The mechanisms were varying, but anorexia and sun exposure defect were likely the main cause of the low 25-OH-D plasma level.

[Following up alcoholics. Comparative results two years after being treated for their alcoholism (author's transl)].

The authors are reporting about a two years experiment, following up alcoholics after their withdrawal treatment. Their work estimates the comparative results of two homogeneous groups : two-hundred patients, undergoing the same supporting psycho-therapy but receiving a different drug treatment. Reviewing their experiment, they evaluate the improvements they have achieved concerning anxiety, asthenia and depression on the one hand ; and on the other hand the alcoholics' ability to reintegrate into normal life, from a family, conjugal, sexual, professional and social point of view. The overall results are expressed in terms of abstinence, relapse, hospitalization and normalization of their behavior and character.
