Sexual initiation, substance use, and sexual behavior and knowledge among vocational students in northern Thailand.

CONTEXT: Thailand has undergone dramatic social changes in the last two decades, yet little is known about factors related to sexual initiation among adolescents. METHODS: A survey using the audio computer-assisted self-interviewing method was conducted to assess social and demographic characteristics, substance use, sexual behavior, and knowledge of HIV and STIs among 1,725 vocational school students aged 15-21 living in northern Thailand. Gender differences for these factors were evaluated using chi-square and Mann-Whitney U tests. Multivariate survival analysis using Cox proportional hazards models assessed associations between these variables and sexual initiation for each gender. RESULTS: Males initiated sexual intercourse at an earlier age than females (median ages of 17 and 18, respectively). At any given age, sexual initiation was associated with having a nonagricultural background and using alcohol or methamphetamine (adjusted rate ratios, 1.3-2.9). For males, initiation was also associated with having parents who did not live together, having a friend as a confidant, tobacco use, high perceived risk for HIV and high STI knowledge (1.3-1.7). For females, other factors associated with earlier initiation were younger age at interview, living away from family, lacking a family member as a confidant, high perceived risk for STIs and ever having smoked marijuana (1.3-2.4). CONCLUSIONS: Interventions to ameliorate the adverse consequences of early sexual initiation need to address social influences such as parents and peer groups. Programs should identify and target high-risk subgroups, such as those who are sexually experienced at an early age and those engaged in patterns of generalized risk-taking.

Condom use among vocational school students in Chiang Rai, Thailand.

Condom use and its psychosocial correlates were investigated in a sample of 1,725 male and female vocational students (aged 15-21 years) in northern Thailand. Consistent condom use was relatively infrequent with all partner types (8.0% with recent steady partners, 28.5% with casual partners, and 30.7% with commercial sex partners), and only 24.3% reported condom use at first sex. These findings suggest that condom use, even with commercial partners, is not becoming widely established in the younger generation of Thai youths. Condom use with commercial partners was far below goals established by Thailand's 100% Condom Campaign, which was particularly significant for a population in Thailand's HIV epicenter. Consistent condom use with a steady partner was significantly related to condom use during first sex, which suggests the importance of establishing a "condom habit." Consistent condom use with casual partners was related to never having been pregnant (self or partner). Having used condoms at first sex also was associated with never having been pregnant (self or partner), as well as with a number of background social and psychological factors. Intentions to use condom were highly related to peer norms. Results suggest the importance of addressing peer norms concerning condom use, as well as the role of condoms in effective birth control.

[The Fundacao Oswaldo Cruz's Scientific Vocation Program (Provoc) as a valuable educational strategy].

The velocity and variety of scientific and technological advances have brought substantial changes to the processes of goods production and of social relations. A concern that inevitably arises within this context is the urgent need to train professionals to work in fields related to science and technology, especially in countries on the so-called periphery, where the quantitative growth of the universe of researchers is fundamental to competing on the world market. Since 1986, the Fundacao Oswaldo Cruz (Fiocruz) has been introducing students to the study of biomedical science through its Scientific Vocation Program (Provoc). The article examines this experience based on the results of a study conducted among high school student participants in Provoc. Placing the issue within the context of Brazilian reality, the article concludes by recommending that Provoc be intensified and expanded to include other areas of knowledge, in view of the program's excellent results in identifying and encouraging students with talents in the biomedical area.

Reciprocal peer management: Improving staff instruction in a vocational training program.

To test the feasibility and utility of involving peers as sources of feedback, 6 subjects, instructors in a vocational program for adults with mental retardation, participated in a staff training and management program. Subjects' teaching interactions were assessed during baseline, in-service training (on effective teaching), return-to-baseline, peer management, and follow-up phases. Peer management was introduced in multiple baseline fashion across pairs of subjects. Members of each pair were trained to monitor peer teaching, to record and graph data, to provide feedback, and to set goals with the peer. Each pair then performed these procedures on the job for several weeks, during which time 4 of the 6 subjects increased their use of effective teaching methods (over baseline). However, inconsistencies in the magnitude and durability of these increases require that the study be viewed as inconclusive, although it has heuristic value as a promising model for involving co-workers in staff management programs.

The call to vocation: life histories of elderly women religious.

Thirty elderly women religious (Catholic nuns) were studied using a phenomenological interview technique. This paper discusses the women's "call to vocation"--the remembered life events that influenced their decisions to enter the convent as young girls. The women talked about romantic and pragmatic motivations as well as spiritual direction. Remembrances of three of the study participants are used to demonstrate circumstances and emotions that led the women to choose their life course. The recollections have significance for the women religious in late life; they provide a source of satisfaction and commitment. They also offer researchers a means of viewing the worlds of this population of women.

Vocational adjustment and general well-being of young adults with IDDM.

The vocational experiences and general well-being of 58 young adult subjects (mean age 24.3 yr) with insulin-dependent diabetes mellitus (IDDM) diagnosed during their adolescence were compared with that of 55 healthy matched control subjects with linear logistic discriminant function analyses. Assessment measures included the Rand General Well-Being Scale and the Rand Functional Limitations and Physical Abilities Batteries. Diabetic subjects, on average, reported significantly lower general well-being than control subjects, particularly in terms of health-related fears and feelings of depression. However, diabetic subjects did not report a pervasive functional deficit relative to control subjects and experienced similar employment rates and problems in the workplace. These results suggest that this group of young adult diabetic subjects has adjusted well to the demands of the workplace despite lower reports of general well-being. The results are discussed in light of relevant sampling issues.

Physiognomic perception and educational-vocational preference.

The purpose of the present study was to determine whether physiognomic perception influenced community college students' selection of a variety of educational-vocational goals. Physiognomic perception, a cognitive style dimension through which people imbue objects with varying degrees of affect, was measured by a standardized and validated instrument known as the Stein Physiognomic Cue Test. The subjects were 183 community college students classified according to their stated choice of major field on a brief questionnaire. The hypothesis that students selecting various major fields of study would differ as a function of physiognomic tendencies was supported. The expectation that females would have greater physiognomic tendencies than males was not confirmed for the present sample.

Wais-III norms for working-age adults: a benchmark for conducting vocational, career, and employment-related evaluations.

The Wechsler Intelligence Scales are routinely used to assess threshold variables which correlate with subsequent job performance. Intellectual testing within educational and clinical settings accommodates natural developmental changes by referencing results to restricted age-band norms. However, accuracy in vocational and career consultation, as well as equity in hiring and promotion requires the application of a single normative benchmark unbiased by chronological age. Such unitary norms for working-age adults (18- to 64-yr.-olds) were derived from the WAIS-III standardization sample in accord with the proportional representation of the seven age-bands subsumed within this age range. Tabular summaries of results are given for the conversion of raw scores to scaled scores for the working-age population which can be used to derive IQ values and Index Scores.

Bridging the gap between hospital experience and general practice during vocational training.

This paper describes a strategy for learning that may effectively link the hospital and general practice parts of vocational training. It suggests the use of a locally developed study guide for each hospital post that is based on certain priority objectives for general practice training. The learning needs of individual trainees are the central focus of the strategy.

[Nurses' image perceived by academic and vocational high school teachers in Korea].

PURPOSE: The purpose of this study was to identify nurses' image and its related factors to make nurses' image among academic and vocational high school teachers in Korea. METHOD: Study samples were composed of 470 teachers who were from 12 high schools in G and J city located in southwestern part of Korea. Data were collected from August 26 to October 4, 2002. The internal consistency of the total scale was Cronbach's a =.940. RESULT: In order to make nurses' image, 76.4% of respondents were influenced by the experience of their hospitalization in visiting hospitals. The mean score of nurses' image in general was 3.19+/-0.55; in four subcategories, 3.46+/-0.60 for professional image, 3.28+/-0.69 for traditional image, 2.93+/-0.70 for social image, and 2.91+/-0.64 for vision of nursing career. The mean score of nurses' image in general was more positive significantly in the 50's age group (F=6.43, p=.002) and in male groups (t=2.92, p=.002). CONCLUSION: On the basis of these findings, nursing professionals need to improve their working conditions, aspiration in job, and autonomy of nurses in their practice fields, monitor the mass media and other publishing materials continuously, and share the appropriate information on nursing profession to the school teachers and the public.

Vocational outcome of intervention for low-back pain.

Practical management guidelines for occupational health physicians are needed for the individual support of employees with low-back pain. In this study the level of evidence regarding the efficacy of intervention with vocational outcome parameters was assessed. In a systematic literature search, 40 randomized clinical trials on different types of intervention were retrieved. Their internal validity and statistical power criteria were assessed. The randomization procedure, blinding of patients, and sample size were problematic in most studies. For patients with acute low-back pain limited or moderate evidence was found for the efficacy of no bed rest, a short period of bed rest, and spinal manipulation. For chronic patients limited evidence was found for the efficacy of antidepressants. For the other types of intervention, studies with sufficient statistical power were lacking. Such studies are needed before more-detailed evidence-based guidelines can be formulated for occupational health care.

Vocational training: its role in nursing.

The use of occupational standards in professional health care curricula is currently of great importance following support for their use from the NHS Executive and the employment department. In the first of two articles reviewing the role of National Vocational Qualifications (NVQs) in nursing education, the author explains the background and development of NVQs, and the introduction and function of the Care Sector Consortium, identifying the links that now exist with nursing education.

Functional disabilities of disabled workers in vocational aid facilities and a social welfare factory.

We evaluated functional disabilities of disabled workers at vocational aid facilities and a social welfare factory to reveal whether evaluation of the functional disabilities is useful for occupational health care activities. Subjects were 121 persons with a physical disability working at Fukuoka Colony (FC workers) and 35 persons with mental retardation working at Nanomi Kogei (NK workers). The Self-Rating Barthel Index, Self-Rating Frenchay Activities Index, and Satisfaction in Daily Life (SDL) were used for the evaluation of basic activities of daily living (ADL), lifestyle, and subjective domain of quality of life, respectively. The FC workers were almost independent in basic ADL: had a high capability in self-care activities and moderately disturbed capability in mobility activities, and had a more active lifestyle than the NK workers. The total SDL score indicated that the FC workers were less satisfied with overall daily life, although not significant, and were less satisfied with the SDL items of physical health, gait, house facilities, and having a job than the NK workers. Because these results indicated the features of disabled workers and gave an occupational health physician valuable information about disabilities, the evaluations used in this study can be regarded as useful for occupational health care activities.

The McCord-Christian medical Fellowship vocational training programme.

The first 3 years' experience of a vocational training programme for doctors based at McCord Hospital in Durban is described and reviewed. The stated aim of the programme is to equip doctors to 'serve the under-served according to the example of Christ' and to produce caring, competent and committed doctors for areas of need.

Restoration longevity and analysis of reasons for the placement and replacement of restorations provided by vocational dental practitioners and their trainers in the United Kingdom.

OBJECTIVE: This study examined the reasons given by vocational dental practitioners and their trainers for placement and replacement of restorations. METHOD AND MATERIALS: Each participant was asked to record the reason for placement or replacement of restorations. The age and class of the restoration being replaced were also recorded, as was the material being used and the material being replaced (if known). RESULTS: Details of the reason for placement or replacement was received on 9,031 restorations. Of the restorations placed, 53.9% were amalgam, 29.8% were resin composite, and 16.3% were glass-ionomer cement. The reasons for placement or replacement of the restorations were principally primary caries (41.3%), secondary caries (21.9%), tooth fracture (6.4%), marginal fracture or degradation (6.1%), and noncarious defects (5.8%). Of the amalgam restorations, most were placed to restore Class II and Class I preparations (65.8% and 29.9%, respectively). Of the composite restorations, most were placed in Class III and Class V cavities (35.5% and 26.3%, respectively). Glass-ionomer cement was used predominantly to restore Class V cavities (63.5%). CONCLUSION: Secondary caries was the most prevalent reason for replacement of restorations, regardless of material. Statistical analysis indicated that amalgam provided significantly greater longevity than composite or glass-ionomer materials.

Strong Vocational Interest Blank scores and differential attraction to in individualized curriculum.

Research suggests that preference for and effectiveness of instructional methods vary with individual differences in personality and motivation. Students who had taken the Strong Vocational Interest Blank (SVIB) and were completing an individualized course in preclinical dentistry made a hypothetical choice between individualized and traditional curricula. As compared with students preferring the regular option, those preferring the individualized optipn scored higher on interests in scientific, aesthetic-cultural, and adventuresome activities and on motivation for academic achievement and professional specialization. On occupational scales, they were more similar to those in biological science and social service and resembled less those in skilled trades, business and accounting, and sales. The groups did not differ in Dental Aptitude Test scores or grades. Findings from the SVIB and questionnaire data suggest that individualized programs are especially suited to students with high intellectual interests, academic motivation, self-discipline, and organization. Implications for individualized programs are discussed.

Reasons for the placement and replacement of restorations in vocational training practices.

AIM: The reasons for placement and replacement of restorations are central to clinical practice. It is the purpose of this paper to assess the reasons for placement and replacement of restorations in the practices of vocational dental practitioners (VDPs) and their trainers. METHOD: VDPs and their trainers were invited to take part in the study. VDPs were asked to record the principal reason for the placement or replacement of all restorations which they provided for a one-month period and, also, to record the age, Class and materials used for each restoration replaced (if known). Trainers were also asked to participate. The data thereby collected were analysed statistically. RESULTS: 56 VDPs and 17 trainers participated. Data were obtained on a total of 9031 restorations, 4423 (49%) being initial placements and 4608 (51%) replacements. Of the total number of restorations assessed, 54% were amalgam, 30% composite and 16% glass ionomer. Principal reasons for placement/replacement of the restorations were primary caries (41%), secondary caries (22%), tooth fracture (6%), margin fracture/degradation (6%), non-carious defects (6%) and bulk fracture (5%). The type of existing restorative material was recorded for 4196 restorations: secondary caries was the most common reason for the replacement of restorations--for 46.0%, 39.6% and 39.8% of amalgam, composite and glass-ionomer restorations respectively. VDPs placed more restorations as a consequence of caries than their trainers. CONCLUSION: Primary caries is the most common reason for the placement of restorations by the participating dentists, and secondary caries is the most common reason for the replacement of restorations of amalgam, composite and glass ionomer. VDPs placed more restorations as a consequence of caries than their trainers.

Federal old-age, survivors and disability insurance; determining disability and blindness; revision to medical-vocational guidelines. Final rules.

We are clarifying section 201.00(h) of the medical-vocational guidelines in appendix 2 of sub part P of regulations part 404. This section provides guidance for evaluating disability in individuals under age 50 who have a severe impairment(s) that does not meet or equal in severity the criteria of any listed impairment in appendix 1 of sub part P, but who have a residual functional capacity for no more than the full range of sedentary work and cannot do any past relevant work. The revisions only clarify the current rules.
