Collecting cancer risk factor data from hospital outpatients: use of touch-screen computers.

The aim of this study was to assess the prevalence of selected cancer risk factor data from hospital outpatients and the proportion of "at risk" patients who would like help from hospital staff to reduce risk factors. A touch-screen computer collected data from outpatients in Newcastle, NSW, Australia. Eight hundred and nineteen outpatients completed the computerized interview. Of these, 35% were smokers, of whom 25% said that they would like help to stop smoking; 47% were overweight, with 48% indicating that they wanted assistance; 17% consumed harmful levels of alcohol, with 2% wanting help; 30% of eligible women were overdue for a Pap test, of whom 75% said that they wanted a referral to be screened, and 25% were overdue for a mammogram, of whom 83% said that they would like a referral for an examination. Touch-screen computerized health risk assessments are practical for collecting and monitoring valid cancer risk factor data for hospital outpatients.

High tech. High touch. John Naisbett's world view.

History will record that 20 years ago, a man named John Naisbett made a contribution which literally changed the world, with his book Megatrends. It changed the way that we think about the world. It changed the way that we think about ourselves. A distinguished scholar, John Naisbett has worked in many different settings, including at the White House under Lyndon B. Johnson. He spent many years at Harvard University, and is best known for introducing a quantitative analysis of the major trends affecting America. One of those, of course, was demographics. Another was the aging of the planet. Another was technology. He recently authored a new book, High Tech, High Touch. Home care has "high touch" down to a science: that's the kind of care for which home care is famous. It is also important, however, that we introduce technology. As technology changes and offers the ability to provide new and more efficient ways of providing care, we need to embrace that opportunity. History will record that 20 years ago, a man named John Naisbett made a contribution which literally changed the world, with his book Megatrends. It changed the way that we think about the world. It changed the way that we think about ourselves. He delivered the following speech at the 2001 National Association for Home Care Annual Meeting.

Multiple off-pump coronary revascularization with "aorta no-touch " technique using composite and sequential methods.

BACKGROUND: Although off-pump coronary artery bypass grafting (OPCAB) has been widely applied in patients who are considered high risk for cardiopulmonary bypass (CPB), there is still a risk of stroke during the operation because of the ascending aortic partial clamp for proximal anastomosis. In the present study, we report the initial results of an "aorta no-touch " technique using an in-situ graft and composite and sequential grafting methods. METHODS: Between March 2000 and April 2001, 120 patients underwent OPCAB with this technique. The age of patients ranged from 47 to 86 years, with a mean age of 65.7 +/- 8.7 years. On average, 3.12 +/- 0.77 grafts per patient were completed. More than 4 distal anastomoses were performed in 32 patients (27%). As in-situ grafts, 140 internal thoracic arteries (ITAs) and 9 gastroepiploic arteries were used. The radial artery (RA) was used as a Y composite graft in 91 patients, as an I composite graft in 25, and as a K composite graft in 2. Sequential bypass grafting was performed using the RA in 60 patients, the saphenous vein (SV) in 6, the ITA in 4, and the gastroepiploic artery (GEA) in 3. Arterial grafts were used in 92% (345/374) of total bypass grafts. Distal anastomosed sites were 119 left anterior descending arteries (LADs) (32%), 90 posterolateral branches (24%), 64 posterior descending arteries (17%), 49 diagonal branches (13%), 39 obtuse marginal branches (10%), and 13 right coronary arteries (3.5%). RESULTS: There was no operative death or stroke. Early postoperative angiography revealed 95.5% (321/336) graft patency with 100% patency (119/119) of ITA to LAD grafts. Graft patency of the ITAs and RAs (98.5% and 95.9%) were significantly better than that of the GEA (79.0%, p = 0.0064 and p = 0.030) and saphenous vein (82.3%, p = 0.011 and p = 0.048). CONCLUSION: OPCAB performed with the aorta no-touch technique using an in-situ graft and composite and sequential grafting methods provides excellent early clinical results and graft patency.

[Critical update on the no-touch technique in colonic cancer using the grasping tie].

The time honoured strategy of "no-touch" in the surgery of colon cancer does not stand an anatomical and patho-physiological criticism, also on the basis of some results of the literature. The personal experience of the problem leads to the proposal of an alternative realization of the procedure, that is an up-to-date of the classic method, and is really worth and easy to be overall adopted. The suggestion is to seal bipolarly the colon tract with the tumour by mean of the original grasping tie from the very beginning of the operation, and then to temporarily clamp the mesocolon close to the gut, before going on with the surgical dissection, lymphectomy and exeresis as usual. This procedure is able to completely isolate from the first steps of the operation the affected tract of the colon, so opposing the possible spreading of tumour cells in the portal and in the systemic venous stream during the following surgical manipulations. The rationale of the proposal is that of a more efficacy of the "no-touch" strategy based on strict anatomical and patho-physiological considerations.

[Determination of eleven elements in water-touched material by ICP-MS].

Water-touched material include water distribution equipment, protecting materials, and water treatment material. The security of the material plays an important role in everyone's daily life. So it' s necessary to determing metal elements in the material. A method for the determination of eleven elements, including Ag, As, Ba, Cd, Cu, Sb, Sn, Mn, Ni, Pb and Hg, in the water-touched material by inductively coupled plasma-mass spectrum (ICP-MS) is described. The plasma parameters were optimized as follows: The RF input power was 1350 W; flow rate of cooling gas was 13 L x min(-1); flow rate of assistant gas was 0.70 L x min(-1); flow rate of carrying gas was 0.8 L x min(-1); flow rate of atomization gas was 1.02 L x min(-1); rate of sampling was 1.48 mL x min(-1). The detection limits of these eleven elements were 0. 003-0.170 microg x L(-1). The relative standard deviations (n=6) were 0.6%-6.3%. The correlation coefficients were 0.9990-0.9996. The recoveries of the method were 92.4%-108.2%. This method is sensitive, accurate and simple compared with other methods, and has the advantage of wide linear range. The results were satisfactory.

Extraadrenal myelolipoma. Intraoperative cytodiagnosis on touch preparations.

A case of presacral myelolipoma is presented. The tumor was diagnosed by examining intraoperative touch preparations. Extraadrenal myelolipomas, rare tumors composed of mature fat and hematopoietic tissue, must be distinguished from mass-forming extramedullary hematopoiesis. The use of touch preparations is an important adjunct to the study of frozen sections in the examination of fresh tissue, especially when the tissue is technically unsuitable for cryostat sectioning, as in this case.

[Definitive diagnosis of early malignant melanoma lesions by the quantitative analysis of 5-S-cysteinyldopa in the tissue and touch fluorescence method].

Methods for making diagnoses of early lesions of malignant melanomas using a touch-fluorescence method from the cut surface of the lesions and quantitative analysis of 5-S-CD in the tissues were developed. 1) The touch-fluorescence method and analyses of 5-S-CD values in the tissues were extremely useful in making definitive diagnoses of early lesions of nodular melanoma (NM). 2) Only quantitative analyses of 5-S-CD values in the tissues were useful in making definitive diagnoses of early lesions of superficial spreading melanoma (SSM). 3) It is impossible to definitively diagnose lentigo maligna using either way mentioned above. From these results, it appears that quantitative analysis of 5-S-CD values in the tissue was the most useful method for making definitive diagnoses in case of early lesions of NM and SSM.

Formaldehyde-induced fluorescence method for touch preparations from amelanotic melanoma tissues: a simple and rapid method for the demonstration of melanogenic activities of amelanotic melanoma cells.

Touch preparations from four amelanotic melanoma tissue samples were studied with the formaldehyde-induced fluorescence method. Fluorescent amelanotic melanoma cells could easily be detected under a fluorescence microscope. The fluorescence microscopic pictures obtained by the touch-fluorescence method were equally evaluated with those obtained by the classical fluorescence method. The method made it possible to demonstrate the melanogenic activity of amelanotic melanoma cells within 30 min. The routine application of this method to the diagnosis of amelanotic melanoma is suggested.

Ocular melanoma: surgical experience with "no touch" enucleation.

There is a controversy among ophthalmologists concerning the management of ocular melanomas. The "no touch" cryoenucleation technic was designed to prevent dissemination of tumor emboli during enucleation. I have observed no significant complications in five years of experience with this technic. It is a simple, safe, and potentially lifesaving procedure. My method is observation of very small lesions compatible with melanoma until growth occurs. At the 8 to 10 mm size or larger, "no touch" cryoenucleation is used provided the fellow eye is seeing.

Touch imprints: their use in the frozen-section diagnosis of alveolar soft-part sarcoma.

A case of alveolar soft-part sarcoma was studied. It illustrates the use of touch imprint preparations on fresh tissue sections examined while frozen. The technique is simple to use and can be applied to a wide variety of neoplastic and inflammatory lesions. Touch imprints substantially contribute to accurate histopathologic interpretation of lesions submitted for frozen-section diagnosis.

Vitreous touch after phacoemulsification.

Three cases with touch of an intact anterior vitreous face to the cornea following phacoemulsification had moderate corneal edema. In each patient, a different method successfully removed the touch of the anterior vitreous face to the corneal endothelium with subsequent prompt clearing of the corneal edema. Although uncommon, an intact anterior vetreous face against the corneal endothelium is possible when a wide opening has been made in the posterior capsule and the underlying vitreous face remains intact. In one case the recognition of anterior vitreous face contact to the cornea was delayed by the severity of the corneal edema. Of the 3 methods of treatment used, the pars plana anterior vitrectomy approach seemed the most effective and the medical approach with 4% cocaine seemed to be the least predictable.

Innervation of the tylotrich-touch dome complexes in rat skin: changing patterns during postnatal development.

The tylotrich-touch dome complexes of the rat were studied in detail at thoracic level, with two objectives: to follow the pattern of innervation of the individual complexes from birth to maturity and to determine the extent of overlap of the segmental nerves supplying them. Techniques included light and electron microscopy and histological observations following section of intercostal nerves. The touch domes were nearly always supplied from a single stem axon; as expected, their terminals increased in number in association with the differentiation of target Merkel cells from the epidermis. In general, they were supplied from the nearest segmental nerves. The tylotrich follicles were each supplied by several stem fibres. The number of palisade terminals applied to the epithelial root sheaths reached a maximum during the 2nd and 3rd postnatal weeks and declined during the following 2 wk. This overshoot can be regarded as another example of hyperinnervation found in the juvenile peripheral nervous system. During the period of decline, the stem fibres extended their territory, resulting in considerable overlap of the territories of the segmental nerves. By the beginning of the 8th week, overlap was relatively scanty, with an irregular distribution.

[Clinical evaluation of the blood glucose monitors Accutrend, Companion 2, Glucometer 3 and One Touch II in comparison with the glucose oxidase reference method].

New small, light-weight and fast-acting meters for measuring blood glucose have been developed recently. To determine their accuracy and precision we compared Accutrend, Companion 2, Glucometer 3 and One Touch II with the reference glucose oxidase method. For determination of accuracy 150 measurements were performed on two meters of each brand, together with measurement on the Beckman 2 Analyzer, which served as our reference. Capillary blood samples were obtained from patients with type 1 and type 2 diabetes attending our outpatient clinic. All measurements were performed by one experienced technician. Precision in series was determined by 15 measurements of venous EDTA samples. The coefficient of variance was used for statistical analysis. Accuracy was evaluated according to recommendations of the American Diabetes Association and clinically useful criteria such as the error grid analysis. We found that One Touch II performed best overall, followed by Accutrend. Companion 2 and Glucometer 3 showed higher deviations in both accuracy and precision, but nonetheless met the clinical criteria of accuracy and reliability measured by error grid analysis in 87% and 90.5% of measurements, respectively. In conclusion, all four blood glucose meters can safely be used, after proper training, by patients and medical staff for self-measurement of blood glucose.

'No touch' dissection, antegrade-retrograde blood cardioplegia, and single aortic cross-clamp significantly reduce operative mortality of reoperative CABG.

BACKGROUND: We assessed the improvement in total and cardiac-related operative mortality in 131 consecutive patients after reoperative coronary artery bypass surgery (CABG). METHODS AND RESULTS: The patients were divided into two consecutive groups, A (1988 to 1989) and B (1990 to 1993), on the basis of the implementation of alternative techniques of myocardial protection. The techniques in group B included the "no touch" technique or minimal dissection before bypass, routine femoral artery and vein exposure, and frequent cannulation for cardiopulmonary bypass and antegrade and retrograde blood cardioplegia, with all vascular anastomoses (proximal and distal) performed with a single aortic cross-clamp and cardioplegic arrest. Cardiac mortality from low cardiac output or myocardial infarction was 15% versus 0%, P = .002. Multivariate analysis of demographics, perioperative risk factors, and myocardial protection techniques revealed that only membership in group A and the requirement for an intra-aortic balloon counterpulsation independently predicted mortality. By univariate analysis, group A had significantly more patients with three-vessel disease and patients who required urgent or emergent procedures, but analysis of patients in these subgroups also demonstrates a significant reduction in mortality in the "no touch" patients (group B). Although no technique was independently responsible for reduced operative mortality, all the myocardial protection techniques implemented in group B combined to reduce the risk of reoperative CABG. CONCLUSIONS: With appropriate myocardial protection techniques, the risk of reoperative CABG should be similar to that for a primary procedure.

Touch cytology. A quick, simple, sensitive screening test in the diagnosis of infections of the gastrointestinal mucosa.

To assess the role of touch cytology (imprint from endoscopic biopsy specimens) in the diagnosis of mucosal infections of the gastrointestinal tract, we reviewed all records and specimens of patients seen during a 30-month period. Touch cytology was performed by rolling biopsy specimens on glass slides. After air fixation, a rapid staining method similar to May-Grünwald-Giemsa was used. The following infections and pathogens were diagnosed (in decreasing order of frequency): Helicobacter pylori gastritis (n = 53), Candida albicans esophagitis (n = 40), Giardia lamblia (n = 13), Gastrospirillum hominis (n = 11), and Blastocystis hominis (n = 8). The smear was positive in 45 patients with H pylori, in 35 patients with C albicans, in nine patients with G lamblia, in 11 patients with G hominis, and in eight patients with B hominis. Cytology was the only positive test in eight, nine, four, seven, and eight patients, respectively, and increased thus the diagnostic yield obtained by histologic examination.

Pars plana vitrectomy in the management of vitreous touch syndrome.

The management of three patients with aphakic bullous keratopathy by means of pars plana vitrectomy is described. The patients had previously undergone uneventful intracapsular cataract extraction with intact vitreous face. Eventual herniation of the vitreous into the anterior chamber with resultant corneal endothelial touch caused the corneal edema in all three cases. Pars plana vitrectomy resulted in resolution of photophobia and irritation. The visual acuity improved from 20/200, 20/200 and 20/70 to 20/70, 20/60 and 20/40 respectively. The central cornea was cleared of edema in all three cases. The final vision was limited in two of the patients by the persistence of preoperative cystoid macular edema. Pars plana vitrectomy may be used in the management of selected cases of aphakic bullous keratopathy from vitreous touch.

Molecular diagnosis of p53 mutations in gastric carcinoma by touch preparation.

Thirty-one tumor samples from selected cases of gastric carcinoma were analyzed for mutations of the p53 tumor suppressor gene. Template DNA was prepared according to the touch preparation procedure, which allowed us to isolate clusters of neoplastic cells out of a stromal cellular background to be used as a template in the amplification of target exons of the p53 locus. In our present study, by polymerase chain reaction/single strand conformation polymorphism analysis we give evidence of p53 mutations occurring in the DNA-binding core domain of the protein (exons 5 through 9), which are clustered in stages III and IV of the disease (six mutations out of seventeen samples; 35%). No p53 mutations were detected in fourteen gastric cancer samples at I and II stages. Beside the use of conventional molecular scanning procedures, our study proposes the application of the touch preparation method to increase the detection of genetic alterations in human solid tumors.

One Touch II hospital system for neonates: correlation with serum glucose values.

A correlation study was conducted to examine the relationship between results from LifeScan's One Touch II blood glucose monitoring system and laboratory analysis of serum glucose values in neonates. A total of 327 blood samples was obtained from newborn infants in a Level III NICU. The comparison of the paired results showed that the r value was .902. The overall results suggest that the LifeScan One Touch II set in the neonatal mode gives results showing good agreement with standard serum glucose values in neonates with serum glucose values < 100 mg/dl (5.6 mmol/liter).
