Endoscopic aesthetic facial surgery: technique and results.

The endoscopic approach to forehead and midface lifting has become popular method of face rejuvenation with minimal incisions. We have performed 67 endoscopic facelift procedures in the last four years. Forehead lifting technique included five small scalp incisions, wide subperiosteal elevation, endoscopic myotomy and forehead tissue fixation with srews, superficial temporal fascia (STF) suture to deep temporal fascia (DTF). Midface lifting technique included temporal 2.5 cm and 1.5 cm vertical intraoral incision, midface subperiosteal undermining and midface elevation with cable sutures Bichat's fat to DTF. Age mediana of patients who underwent endoscopic front lift was 46, patients who had endoscopic front lift and midface lift procedure age mediana was 40. Postoperative complication rate was 7.5% and included frontal branch weakness (n=2), hematoma (n=1), infraorbital nerve paresthesia (n=1) and asymmetrical smile (n=1). The main question is the quality of the results. We have reviewed 49 patients who were followed 6 months or more. Preoperative and postoperative life-size photographs were analyzed. The mean elevation mediana at medial canthus was 2.2 mm, at medial limbus 2.3 mm, at lateral limbus 2.5 mm, at lateral canthus 2.9 mm. Midface - lift effect resulted cheek elevation from 1.07 till 4.71 mm lip corner elevation 1.03 mm to 3.27 mm. We observed cheek elevation, improving nasolabial line, increasing volume of malar region, elevating lip angles in patients after endoscopic midface lift. We have found that important advantage of subperiosteal midface lift, when performed in conjunction with endoscopic brow lift, is its ability to move the cosmetic eye unit, proportionally, leading to a harmonious facial appearance. Endoscopic facelift is effective procedure for face rejuvenation especially for eyebrows and cheek elevation.

Prospects for elasticity reconstruction in the heart.

The elastic moduli in anisotropic media can be estimated using either direct mechanical or sound speed measurements. Here we compare moduli in the passive heart estimated with different methods and demonstrate that high-frequency (i.e., ultrasonic) sound speed measurements are inconsistent with static deformations and low-frequency shear wave results. Both tissue fixation and the high-operating frequency of ultrasonic measurements contribute to these discrepancies. Moreover, the precision of ultrasonic sound speed measurements required to estimate elastic moduli describing static deformations of a nearly incompressible anisotropic medium such as the heart appears to be beyond the scope of current methods. We conclude that an incompressible anisotropic elastic model is appropriate for elasticity reconstruction in the heart, in which three independent constants characterize small strain behavior, but four are needed for a fully nonlinear description of finite deformations.

[Effect of HuperzineA on neurophathologic change of mouse exposed by isocarbophos and phoxim].

OBJECTIVE: In order to assessment the Huperzine A (HupA) on CNS of mice exposed by phoxim and isocarbophos. METHODS: One group was given tween-80 or peanut oil as control. One group was given phoxim or isocarbophos alone as phoxim or isocarbophos group. The other group was given HupA 2h before phoxim or isocarbophos exposure as HupA protective group. Twenty-four hours after exposure, the whole body perfusion in situ technique was performed for tissue fixation. The whole brain was removed and adjacent coronal sections was obtained. The sections of brain were stained with hematoxylin and eosin (HE) and toluidine blue. Under optic microscope images were collected and analysed. RESULTS: The extensive lesions including cortex oedema, pyramidal cell oedema, and nissal body numbers reduced were observed in phoxim or isocarbophos group. The animals of HupA protective group were observed cortex oedema, pyramidal cell oedema, but the lesional degree was lighter obviously than phoxim or isocarbophos group. CONCLUSION: It was suggested that the HupA could have the protective effect of neuropathologic changes exposed by phoxim and isocarbophos.

[The molecular genetic assessment of prognostic factors in carcioma of the prostate: a pilot study].

The loss of the 8p22 chromosome part and amplification of the 8q24 region (c-myc oncogene region) are the most frequent chromosomal aberrations observed in prostate carcinoma. We aimed to demonstrate the frequency of these chromosomal abnormalities and to assess how they correlate with the prognosis in patients with prostate carcinoma. From a set of 130 patients, we selected a group of 17 who died within five years after the diagnosis had been determined. Due to the lack of tumor cells in puncture biopsies, we managed to carry out in situ hybridization and assess the result objectively for 9 patients only. In seven cases, amplification of the region for c-myc oncogene was found; however, in five of them, polyploidy of chromosome 8 was manifested simultaneously. Deletion of the 8p22 chromosome part (region for the LPL gene) was found in five cases. A normal finding was detected in two cases. However, the analysis was carried out on a small number of cells gained from paraffin slices, so it was impossible to meet the requirement to examine 300 interphase cell nuclei. Therefore, we recommend to always determine whether the material taken is representative enough for this methodology and whether the tissue fixation is not inadequate or insufficient.

[Viscerosynthesis: the realization of an old dream. A look at the 150-year development of surgical suture techniques].

In analogy with osteosynthesis in bone surgery, viscerosynthesis has been introduced as a term for intestinal tissue fixation by staplers. As a result of continuous research, the development of mechanical suture instruments started early in the 19th century. At that time the base of manual suture technique was described by A. Lembert (1826), which is still applicable today. Simultaneously F. Denans introduced a device for intestinal anastomosis. An impressive variety of instruments was developed since then, which resulted in 1893 in the construction of the successful J. Murphy anastomotic button. Derived from it several more elaborate instruments were constructed until recently for oesophageal and rectal anastomoses as well. Entirely new of conception was the stapler apparatus by H. Hültl in 1906, succeeded shortly after by von Petz' stapler clamp. Inspired by them the Russians developed since World War II a tremendous instrumentarium. From 1959 the Americans took over and delivered highly specialized and technologically outstanding stapler instruments which currently dominate the market. Referring to the preceding generations of surgeons, one might say that with the advent of those instruments an old dream has been realized.

[Morphological changes in the brain in schizophrenia].

In 1975-1985 in the mental hospital in Dobrany died 225 patients with chronic schizophrenia. The author presents the causes of death confirmed by necropsy. In all the brain was examined by a light microscope. Ordinary microscopy revealed that in chronic schizophrenia at the age above 65 years Alzheimer's disease is six times more frequent than in the case-records of the mentally healthy population. Electron microscopic examination performed in 22 cases of schizophrenia revealed changes on the synapses at all examined sites of the cerebral cortex--i.e. aggregations of synaptic vesicles. The most probable cause of aggregation of synaptic vesicles could be an impaired "immunological interaction" between synaptic vesicles which from the physiological aspect have at least three types of origin. The findings are supplemented also by data on the function of neurotransmitters and neuropeptides in the physiology and pathology of cerebral functions. The author draws attention to research focused on immunological reactivity in schizophrenia. During electron microscopic examination artefacts caused by premortal agony were eliminated as well as those caused by post-mortem changes and tissue fixation. As controls the author examined the cerebral cortex of five subjects who did not suffer from mental disease. The author also submits a of morphological incorporation of synapses in the structure of the cerebral cortex of five subjects who did not suffer from mental disease. The author to the possible relation to clinical manifestations of the disease.

The role of immunohistochemistry in the diagnosis of poorly differentiated malignant neoplasms.

The development and refinement of immunohistochemical methodologies over the past decade has had a major impact in many different areas of diagnostic pathology. The generation of increasing numbers of well-characterized polyclonal antisera and monoclonal antibodies directed to a variety of antigenic determinants has made the phenotyping of neoplasms a reality. The results of such immunohistochemical analyses have provided an important starting point for the further workup and management of patients with undifferentiated or poorly differentiated malignant neoplasms of unknown origin. It should also be apparent, however, that the results of immunohistochemical analyses in such clinical settings are subject to a large number of variables and that these procedures must be controlled rigorously. The results of extensive performance testing, particularly with new reagents, must be available in order to establish their specificities and sensitivities under different conditions of tissue fixation and processing. Such studies not only will establish more precise and reproducible diagnostic criteria but also will be important for the definition of new clinical and pathological entities and for the characterization of novel prognostic parameters.

Supravital microscopic fluorescent technique for the detection of tinea capitis.

A simple one-step, office technique for the identification of superficial mycoses is presented. Buffered acridine orange is used as a supravital stain for direct examination of specimens under a fluorescence microscope. Neither prior treatment with potassium hydroxide, nor tissue fixation is required. Results usually can be obtained in less than five minutes. Extensive training in use of the microscope is not required.

[Differential staining of calcium compounds in histologic sections].

An experimental protocol is proposed for the differentiation of calcium-hydrogen-phosphate, tri-calcium-phosphate, calcium-oxalate, calcium-urate and fatty acidic calcium. For tissue fixation calcium acetate-formaldehyde is recommended in which fatty acids and earth-metal urates form insoluble calcium-salts which can be demonstrated in paraffin sections.

[Treatment and reconstruction of open fractures of the forearm with tissue loss].

The importance of the forearm calls for attempts to preserve it and to restore its function even in severe injuries with extensive tissue loss. At early and comprehensive first surgical treatment providing sufficient perfusion good results can be achieved also in cases that do not appear promising at first sight. After treating the soft tissues fixation of the fracture is to be ensured externally and reconstitution surgery is to proceed according to the management plan. Of the reported series of 11 severe compound fractures the functional result of management was not favorable only in a single case.

Novel technology in microsurgery.

Several of the latest technical advances in microsurgical technology employing conventional engineering are briefly summarized. New scientific fields are introduced: injectable soft polymeric gels for cataract implants and vitreous substitution, controlled drug release and temporary tissue fixation via polymeric biodegradation, photodynamic therapy via intraoperative use of photosensitizers and surgical photoablation via short pulsed laser surgical instrumentation. Clinical examples covering the anterior and posterior segment of the eye are described. The potential uses of these technologies involve procedures for cataract, vitrectomy, strabismus, glaucoma and keratoplasty as well as therapeutics.

Alterations to subplasmalemmal leptomeres in adult canine myocytes during total in vitro ischemia.

Published evidence suggests that ischemia-induced cell swelling renders myocytes vulnerable to plasmalemmal disruption and consequent cell death. Alterations to the myocyte cytoskeleton may be involved in the pathogenesis of this plasmalemmal injury. One putative cytoskeletal structure in cardiac muscle that has received little consideration is the subplasmalemmal network of periodic densities with linking microfilaments termed leptomeres or leptofibrils. We demonstrate these structures in dog heart papillary muscle and describe the improvement in their definition brought about by tissue fixation at 37 degrees C in 2% glutaraldehyde with addition of 0.05 M lysine-HCl, followed by brief postfixation with osmium tetroxide. Alterations to leptomeres during ischemic injury were examined in myocardium subjected to total in vitro ischemia for 30-180 min at 37 degrees C. Leptomeres showed little morphological alteration during the first 90-120 min, after which leptomere periodic densities (striae) increased in size, from 10-20 to 50-80 nm, and were more densely stained. The leptomeres eventually (150-180 minutes) lose definition. The course of these alterations coincided with the appearance of ultrastructural evidence of irreversible ischemic injury to the myocytes.

Perigraft-reaction after implantation of vascular prostheses. Pathogenesis, clinical picture and treatment.

This report is based on 18 perigraft reactions in 2237 implanted vascular prostheses (1970-1985). The incidence of this complication is 6.8-9.5/1000 both for Dacron-double-velour and expanded PTFE-prostheses. The clinical picture is characterized by localized or extended fluid accumulation around the artificial graft (perigraft cysts or "swimming prosthesis"). Including 271 reports from the literature, 75% of vascular prostheses in extraanatomical positions were affected. The time interval between vascular substitution and clinical manifestation is on average 23.5 months. The pathogenesis of this aseptic late complication is still unclear. Several disposing factors are under discussion: (a) physico-chemical irritation of the surrounding tissue by the fabric itself (release of monomers, emulgators etc.), (b) mechanical trauma (continuous movement of the graft in the tissue bed, i.e. insufficient tissue fixation resulting in gaps and exudation of fluid round the prosthesis), (c) latent or manifest renal insufficiency. The partial or total removal of the affected prosthesis and its substitution by another type of vascular prosthesis offers the best chance of definitive cure. Repeated puncture of perigraft cysts produces frequent recurrencies and the danger of secondary infections. Therefore only an exchange of the graft can be recommended.

[Pharmacokinetics and metabolism of anthracyclines in man].

Although they differ only slightly from each other, the various anthracyclines have their own metabolic pathways and pharmacokinetic parameters. Doxorubicin and daunorubicin have a similar reduced metabolite which reaches low plasma levels in the case of doxorubicin, but is predominant in the case of daunorubicin: daunorubicinol. Aglycones are only formed in large quantities from aclarubicin: aklavinone. The plasma decay of doxorubicin levels is triphasic, with successive half-lives of 5 min, 1 h and 30 h. Unchanged daunorubicin is eliminated more rapidly from plasma, but its metabolite is eliminated more slowly. New anthracyclines such as pirarubicin are characterized by a large volume of distribution, suggesting a higher tissue fixation. These pharmacokinetic data must be kept in mind for the design of new protocols which are aimed to dose fractionation or tumor targetting.

The number and diameter distribution of axons in the monkey optic nerve.

Using an automated image analysis system, cross-sections from optic nerves of 17 normal cynomolgus monkeys were examined. The number of nerve fibers, their density, and the distribution of their diameters for whole nerves and for various regions of the nerve cross-section were estimated. The mean total number of fibers in the optic nerve was 1.2 million. The mean diameter of axons was 0.8 micron. The method of tissue fixation substantially affected the measurements. Histograms of fiber diameter suggested a trimodal distribution of fiber size with peaks at 0.5, 0.8, and 1.5 micron. The relative proportions of these fiber peaks differed significantly in different regions of the nerve. The highest proportion of large fibers was in the superior nerve periphery. The highest concentration of smallest fibers and the highest density of all fibers were located centrally in the infero-temporal quadrant. The observation that higher fiber density and smaller mean fiber diameter are skewed toward the inferior pole appears to coincide with the inferior position of the fovea with respect to the optic nerve head. This finding has importance for interpretation of pathologic changes in the optic disc.

[Neurohistologic and histochemical findings concerning the accessory fiber of several incapsulated receptors].

In order to clarify the role of catecholamines in encapsulated receptors, Pacinian corpuscles and the bulbs of Krause have been studied by the fluorescent method of Falck. Fluorescence, specific for catecholamines, has not been revealed in the receptors. Catecholamines do not seem to be neuromediators in stimulating sensitive nerve terminals. The regulatory role of catecholamines in the activity of the encapsulated receptors is discussed. The neurohistological method of tissue fixation in the mixture of zink-iodine -- osmic acid has revealed some additional nerve fibers (Timofeev's apparatus) in the bulbs of Krause of the cat lip and nose. In order to understand the nature of the additional fibres in the bulbs of Krause, a branch of the trigeminal nerve has been sectioned in the suborbital area. After sectioning the staining of both the main and additional fibres in the bulbs of Krause in the lip and nose has disappeared. The data obtained support the view on sensitive nature of the additional fibres (Timofeev's apparatus) in the sensitive bulbs of Krause.
