[Medico-social problems in the recovery from stroke].

Until a few years ago physicians were not rehabilitation-oriented, but recent discoveries regarding the plasticity of the nervous system have made the patient with an acute cerebrovascular lesion a clinical model of great interest. Furthermore, the severity of the frequent residual invalidities and the high social costs are the reasons why this field of medicine is becoming increasingly important. The clinical study of stroke has not changed much during the past century. Perhaps our semeiological abilities are not the same but we have improved our knowledge of the neurosciences and we know that in the acute phase we have to look towards the patient's future and not only at his current condition. A certain degree of recovery occurs immediately after stroke thanks to the resolution of secondary events such as edema and ischemia. Other factors that may have important effects on recovery include the localization, nature, extension and degree of brain damage, the patient's sex and age, the duration of coma, the patient's original cognitive capacity, his personality and motivation as well as the duration and intensity of rehabilitation and the time before starting rehabilitation. Many of these variables are difficult to check in trials performed in humans. Statistical problems make it difficult to prove any beneficial effect of rehabilitation. Some recent trials have shown statistically significant improvements attributable to rehabilitation. For instance, exercise under visual stimulation around a scotoma has been shown to reduce the area of blindness. The improvements occurred only after exercise and never spontaneously. Solving an "appalling" problem: social reformers and the campaign for the Alberta Sexual Sterilization Act, 1928.

In this article, the author examines the campaign leading up to the passage of the Sexual Sterilization Act in Alberta in 1928. The author asserts that the passage of this Act was the result of the influence of a few elite individuals, particularly those involved with the United Farm Women of Alberta social reform movement, and may not have been reflective of widespread favourable public sentiment. While there were serious misgivings regarding the passage of the Sexual Sterilization Act, the legislation was ultimately successful because of the pressing problems of inadequate mental facilities and budgetary constraints. The author discusses the legislation's eventual repeal in 1972 due to public denunciation of eugenic measures, concerns about liability, and the threat posed to individual liberties. This article was the winner of the William Morrow Essay Contest in 1999.

[Medico-social problems of young pregnant women].

A total of 255 women aged under 17 and 320 pregnant women aged 18 were followed up in 1994. Medicosocial aspects of pregnancy were investigated using special questionnaires, medical aspects were analyzed using medical files. This enabled us to characterize from medical and social viewpoints young women, parent families, and socio-hygienic factors of pregnancy.

[Ecologic and social problems of reproductivity in the Irkutsk region].

The study covered quantitative and structural features, changes in functional parameters of female reproductive system, in disorders of pregnancy, childbed and newborns in Irkutsk region. The authors demonstrated role and relationships of some regional factors--natural, ecologic and technogenic, occupational and industrial, social and economic--in formation of reproductivity, disorders during 1980-1990s.
