[Comparative evaluation of the incidence of premature labor among female workers in plants with different work profiles].

The incidence rate of premature births has been studied in 777 women employed at a chemical plant, 1430 women working in clothing industry, and 805 port women-workers. In the mentioned groups 35,41 and women, respectively, had undergone premature births. This constituted 4.5, 2.87 and 3.85%. However, in women who had delivered, these percentages have gone up to 5.55, 4.18 and 5.24. With reference to the first as well as the second findings the above differences are statistically insignificant. In the test plants no workplaces exposing to a greater risk manifested by frequent premature births have been disclosed. In all the three plants the first premature birth was recorded almost at the same rate in women who previously had no pregnancy or delivered on time, while the subsequent premature births and most frequently been directly preceded by a premature delivery. Spontaneous abortion preceded the premature births over five, and the pregnancy interruption procedure over six times less frequently than the normal parturition. The direct and most often conjectural causes of premature births mentioned by the examined subjects include: premature flow of the amniotic fluid, physical effort disconnected with the work performed, serological incompatibility involving Rh factor, psychic trauma and gestosis.

Characteristics of first pregnancy in relation to early breast cancer. A case-control study.

We examined some characteristics of first pregnancy in 314 female breast cancer patients whose disease was diagnosed at < 45 years of age and who were identified through a population-based cancer registry in New York State and in 314 control women identified through birth records. Preeclampsia was reported less frequently on the birth records of the first recorded pregnancy in the cases than in those of the controls, with a relative risk of 0.28 (0.05 < p < 0.10, from logistic model). We examined this finding on preeclampsia with particular reference to immunologic mechanisms, but further studies are needed on larger samples. Twinning and maternal Rh factor, also relevant to immunologic factors, were not significantly associated with a higher risk of breast cancer. Among the women whose first pregnancy resulted in a live birth, length of gestation (which is correlated with maternal estriol levels) did not differ between the cases and controls.

[Blood group, alcoholism and liver damage (author's transl)].

The distribution of blood group characteristics in the ABO system was examined in 339 alcoholics, men and women, who were subjected to laparoscopy and liver biopsy from 1973 to 1977. Compared with the normal distribution (Fischer) and an additional random sampling of 12 843 patients of all wards there was no statistically significant difference. More than chance interdependencies between the degree of liver damage and a blood group could also not be confirmed. There were no differences with regard to the Rh factor either.

Population genetics in the state of Nuevo León, Mexico. II. Genetic markers, frequencies and their possible association with chromosome damage in workers exposed to lead.

In order to find out genetic variability and its possible association with chromosomic damage, we studied the ABO, Rh and MN blood groups as well as the ability to taste PTC, ear lobe type and mid-digital hair, in 77 workers exposed to lead oxides. 23 people who worked near the source of lead (internal control) and 20 people not exposed to lead (external control). From each person a blood sample was taken for chromosome analysis in cultured lymphocytes and for lead determination, at the same time an urine sample was taken for delta-aminolevulinic acid (ALA) determinations; at the same time an urine sample increased chromosome damage, blood lead levels and urinary ALA, were found as compared with both controls. Differences were statistically significant. As for genetic marker frequencies differences were found only for the Rh factor, between the internal control and both the external control and exposed workers. No significant association was found between genetic markers and chromosome damage. Association was observed between blood lead levels, and urinary ALA with some genetic markers like ABO and MN blood groups and ear lobe type.

The last fifty years in obstetrics and gynecology.

The last fifty years in Obstetrics and Gynecology have been associated with some of the most outstanding changes in the history of this specialty. We have seen the discovery and use of antibiotics, the Pap smear, shortened hospital stays, the Rh factor, and the birth control pill. It is doubtful we shall ever see as many changes in an equal period of time.

Rejection management and long-term surveillance of the pediatric heart transplant recipient: the Loma Linda experience.

At Loma Linda University Medical Center, 210 heart transplant procedures have been performed on 207 newborns, infants, and children since 1985. Actuarial survival rate at 5 years is 72% for the entire population and 82% for those receiving a transplant during the first month of life. These patients have been managed with a regimen that minimizes long-term steroid use and emphasizes the noninvasive diagnosis of rejection. This article describes in some detail the mechanics of this process. In addition, the rejection history of 154 children undergoing transplantation from 1989 through 1992 was reviewed. The average number of rejection episodes was 1.67 (standard deviation 1.65; median, 1; mode, 0). The vast majority of rejections occur in the first 3 months after transplantation. Long-term freedom from rejection was 19% for newborn recipients, 42% for infants, 25% for older children. Donor/recipient mismatch for gender, race, blood type, Rh factor, and HLA typing did not correlate with rejection history. Older age at transplantation and cytomegalovirus disease were correlated with more frequent rejection episodes. Five patients had posttransplantation coronary artery disease. This was strongly correlated with greater rejection frequency and death from rejection. In addition, there was a trend toward less posttransplantation coronary artery disease with antibody induction therapy, younger age at transplantation, and absence of cytomegalovirus disease.(ABSTRACT TRUNCATED AT 250 WORDS)

[Electrocardiographic disorders during sanguinotransfusion].

Electrocardiographic findings are analyzed out of a total of 80 exanguinotransfusion done in 70 newborns complaining of hyperbilirubinemia due to isoimmunization to Rh factor, to blood group, to subgroup and to liver enzymatic immaturity. Twenty-six of these babies showed subnormal weights. The technique used was especially that of closed circuit with two vessels and continuous droping. Seventeen patients with concomitant respiratory insuficiency were exanguinated. Electrocardiographic disorders were found in 70% with predominance of hypocalcemia --19 cases--and tachycardia in 9 cases. There were no cases of true hyperkalemia, even in the group of patients who were given blood of over three days of extraction. There were 5 cases of hypokalemia; another 5 with overload in right cavities as possible response to hypervolemia. Disorders of rhythm, bradycardia, ischemia lesions and A-V blockage were present as features of poor prognosis in the only two patients who died in one of whom hyperkalemia and in the other one, hypokalemia were identified. Stress is placed on the greater number of disorders present in the group of infants with subnormal weights, as in those affected with added respiratory insufficiencies or with severe hemolytic diseases.

Activities of the 105th Croatian Army Brigade Medical Corps during the 1991-1992 war.

The organization and work of the medical corps of the 105th Croatian Army Brigade from Bjelovar, from October 18, 1991 to September 27, 1992, are presented. The brigade operated on the battlefields in East Slavonia, West Slavonia (Novska), and Posavina. Health care was also provided to the civilians living in the area. On their arrival at the battlefield, all members of the 105th Croatian Army Brigade underwent blood group and Rh factor determination and were vaccinated against tetanus. At the battalion and brigade stations, 4,220 diseased and wounded were examined, and 242 surgical procedures were performed, 189 of them under local and 20 under general anesthesia. Respiratory diseases were most common (27.2%), followed by locomotor (14.5%) and gastrointestinal (11.2%) disease. Wounding was recorded in 5.4% of the total number of patients, with 29.3 and 17.5% referring to explosive and gunshot wounds, respectively. Primary care of war wounds was provided in 42.5% of the patients. Total mortality was 2.2%. Wound infection or lethal outcome was not recorded during the triage, treatment, and evacuation.

[The clinical and morphofunctional characteristics of different types of stomach ulcer].

AIM: The study of a complex of anamnestic, clinicoendoscopic and functional-morphological characteristics in type I and II (according to Johnson) gastric ulcer. MATERIALS AND METHODS: Esophagogastroduodenoscopy, gastric secretion tests, determination of blood group and Rh factor were performed in 91 patients (52 patients with ulcer type I and 39 with ulcer type II). RESULTS: Ulcers type I have arisen in the presence of long-term chronic gastritis. They were associated with marked changes in the mucosa of gastric body, its atrophy and intestinal metaplasia, persistent recurrences in the same gastric zone. Ulcers type II are characterized by hereditary loading, 0(I) blood group, combination with gastroduodenal erosions, season occurrence, trend to migration and recurrence in different gastric or duodenal zones, HCl hypersecretion, high occurrence of Helicobacter pylori infection. CONCLUSION: It is thought valid to include types of gastric ulcer in current classification of ulcer.

Antileukemic effects of recombinant human tumor necrosis factor alpha (rh-TNF alpha) with cyclophosphamide or methotrexate on leukemia L1210 and leukemia P388 in mice.

We investigated the influence of recombinant human tumor necrosis factor alpha (rh-TNF alpha) administered as a single agent or in combination with cyclophosphamide (CY) or methotrexate (MTX) on the survival time of mice inoculated with lymphoblastic leukemia L1210 or lymphatic leukemia P388. The median survival time of leukemia L1210 bearing mice treated with rh-TNF alpha at doses ranging from 200 to 275 g/kg in daily i.p. injections was longer than that of control animals. Groups of mice with leukemia L1210 receiving rh-TNF alpha combined with either MTX or CY lived longer than animals treated with these agents individually. We observed only slight prolongation of life of animals inoculated with this tumor and treated with rh-TNF alpha at dose of 800 micrograms/kg in four injections on 2, 4, 6 and 8 day of experiment, and no effect when rh-TNF alpha was administered at dose of 200 or 400 micrograms/kg at the same treatment regime. In contrast no significant differences in lifetime were obtained from either simultaneous or sequential treatment of mice bearing leukemia P388. Groups of mice with this tumor treated with rh-TNF alpha in conjunction with either MTX or CY lived longer than controls, or rh-TNF alpha singly treated mice, but their survivals were not significantly prolonged compared with mice receiving cytostatics alone.

Adaptation of dry nephelometer measurements to ambient conditions at the Jungfraujoch.

In a numerical study the influence of relative humidity (RH) on aerosol scattering coefficients sigma was investigated. Based on a core/coating aerosol model, RH enhancement factors for scattering, xi(RH) = sigma(RH)/sigma(RH = 0), were calculated for the wavelengths lambda = 450, 550, and 700 nm for a summer and a winter case. The investigation was adapted to the situation (e.g., chemical composition, particle size distributions, hygroscopic behavior) of the high-alpine site Jungfraujoch (JFJ, 3580 m asl), where long-term measurements of dry aerosol scattering coefficients are performed at these wavelengths. The presented results are therefore representative of the lower free troposphere above a continent. The RH enhancement factors at a specific RH strongly depend on the average particle size. For example, at RH = 85% they vary between approximately 1.2 and approximately 2.7 in summer and between approximately 1.4 and approximately 3.8 in winter. It is shown that there is a strong relationship between xi and the Angstrom exponent å (based on scattering only) of the dry aerosol, which is directly derived from the dry scattering measurements. This allows for parametrizing xi for a specific wavelength and season with å and RH. The parametrization is applicable for RH up to approximately 90%--for higher RH the underlying hygroscopic models become unreliable--and for å between approximately -0.25 and approximately 2.75, which covers the range observed at the JFJ. Also addressed is a systematic error in the dry scattering coefficients measured with a nephelometer previously discussed in the literature, which arises from nonidealities in the angular intensity distribution of the light inside the instrument. This effect also depends strongly on the particle size and can be described by a correction factor C that can be parametrized with å. The scattering coefficient corrected for measurement artifacts at ambient RH for specific wavelength and season therefore can be estimated from the uncorrected dry nephelometer scattering coefficient sigma(neph) as sigma(å, RH) = C(å) x xi(å, RH) x sigma(neph). As additional information only ambient RH data are needed. The 95% confidence bound of this total correction ranges from less than 5% for low RH and large å up to approximately 40% for high RH and small å.

Frequency of the antipsychotic therapy acute side effects in the treatment of acute psychosis.

Antipsychotic drugs produce a wide spectrum of physiological actions. Some of these effects differ among the various classes of antipsychotics. This medications have indications in the treatment of acute psychotic disorders. The main goal of this investigation was to determine the incidence and prevalence of the neuroleptic therapy acute side effects. The reason for this epidemiological investigation performing was the lack of knowledge of the exact neuroleptic therapy side effects incidence. Qualitative study on this problem has not been performed yet. Antipsychotic therapy side effects prevalence rate according to the literature data is ranging from 24% to 74%. Different prevalence rate is a consequence of different antipsychotic drug usage, different drug administration method and different side effects identification. On account of all these facts, we put the hypothesis on the correlation between the antipsychotic therapy and occurred side effects. Our experiment included all patients hospitalised from December 31st 1999 to January 31st 2000 in Intensive Care Unit of Biological Psychiatry Department of Psychiatric Clinic in Sarajevo. All patients were divided in three groups according to the applied therapy. All of them met ICD-10 criteria for schizophrenia (F20-29). During our study the following examinations were performed: psychiatric interview, BRPS, scale of side effects, psychophysiological tests, general clinical impression, scale of appetite, carbon hydrate needs scale. Psychiatric and statistical evaluations were done as well. The evaluation of our examination is showing successful results in all groups of patients. The improvement of psychopathological symptoms was insignificant. Reported side effects were minimal with low incidence rate and relatively high prevalence rate. Statistical tests were calculated from the obtained data after what the null hypothesis was rejected. Consequently, an alternative hypothesis was confirmed and it indicated that the acute side effects incidence and prevalence were within the range of expectation. Intensity of the recorded side effects was moderate to mild. On the basis of the obtained data, it has been concluded that applied antipsychotic agents did not induce more psychophysiological function impairments in the treated patients. Psychophysiological functions remained in physiological range limits and their changes were not significant. Neuroleptic therapy side effects were minimal, meaning no toxic signs or therapy discontinuations were recorded.
