Early extramedullary plasmacytoma confined to the lamina propria of the gastric mucosa: case report.

A case of early gastric plasmacytoma in a 51-year-old woman is reported. The demonstration of monotypic IgM-kappa immunoglobulin in biopsy specimens by an immunoperoxidase technique contributed to the histological diagnosis. The patient had non-ulcerative lesions (of approximately 15 x 16 cm) of a superficially spreading type of gastric plasmacytoma. Extensive histological analysis of the resected stomach showed the IgM-kappa monoclonal proliferation of plasmacytic tumor cells to infiltrate merely into the lamina propria of the gastric mucosa and not to extend deeply into the submucosa, proper muscle and serosa of the stomach.

[Extramedullary plasmacytoma of the nasopharynx. Typical versus atypical presentation].

Extramedullary plasmacytoma are rare plasma cell neoplasias. Eighty percent of these tumours grow in the ENT region. We report a typical case of a woman with a single tumour in the nasopharynx. Actually she is in complete remission after surgical and radiotherapic treatment. We also report an atypical case of a woman with a tumour in the nasopharynx associated with a cervical metastatic adenopathy and a plasmacytoma of bone. The treatment of this widespread disease was systemic chemotherapy. An update on this subject is done.

Primary extramedullary plasmacytoma of the small intestine. A case report and review of the literature.

A case of primary extramedullary plasmacytoma of the small intestine in a 73-year-old Japanese woman was reported. The patient underwent local resection of the tumor, and showed no signs of local recurrence or dissemination of the disease after 28 months follow-up. The tumor cells had relatively large nuclei with distinct nucleoli and wide and slightly basophilic cytoplasm with a high N/C ratio which showed the morphology of atypical plasma cells. Immunohistochemical examination revealed that the tumor cells contained IgG gamma-type immunoglobulin in their cytoplasm but they did not contain IgA, IgM, IgD, and kappa-light chains. The tumor cells were also positive for CD79a and CD138 and negative for LCA, CD20 and CD45RO. These findings clearly indicated this case to be plasmacytoma.

[Solitary costal plasmacytoma: a rare cause of subacute demyelinating polyradiculoneuropathy].

INTRODUCTION: The solitary plasmacytoma (SP) is a non-frequent, localized variant of multiple myeloma with a single bone lesion and earlier appearance. Polyradiculoneuropathy is the most frequent neurological manifestation. We report the case of a young male who developed a subacute demyelinating polyradiculoneuropathy as the initial symptomatology of a costal SP, which constitutes an extremely infrequent association, given age and site of the lesion. CASE REPORT: A 32-year old, previously healthy, man presented with a one month history of progressive symmetrical paraparesis and paresthesias in feet. The neurological examination revealed 4/5 paraparesis, global arreflexia and hypopalesthesia in legs. Cerebrospinal fluid examination revealed elevated proteins (83 mg/dl) with normal cell count and glucose content. Investigations showed high levels of beta(2)-microglobulin (3 mg/l), and a monoclonal IgG lambda gammapathy. The chest X-ray and thoracic CT revealed an osteolytic lesion in the left third rib. Nerve conduction studies showed sensitive and motor polyneuropathy with secondary axonal degeneration. Bone marrow biopsy was normal. Second and third left ribs were excised, revealing a pathologic diagnosis of plasmacytoma. The patient became asymptomatic after corticosteriod administration lesion excision and local radiotherapy. Conclusions. Subacute demyelinating polyradiculoneuropathy can be due to multiple causes. SP can be one of its etiologies and occur at infrequent ages and localizations.

[Four cases of an extramedullary plasmacytoma of the head and neck].

From 1987 to 1988, four patients with solitary, extramedullary plasmacytomas of the head and neck were seen at the Gunma Cancer Center Hospital. With reference to such plasmacytomas, we have reviewed the medical literature regarding these tumors and mainly present the results of radiation therapy with respect to these patients. One patient, who had a local recurrence after surgical treatment, underwent radiation therapy (36Gy in four weeks) that resulted in good local control of the disease. Two patients developed multiple painful bone lesions during their long follow-up periods without manifesting any abnormal laboratory findings, suggesting multiple myeloma. It was found that radiation therapy (less than 30 Gy) was useful for the palliation of painful lesions.

Extramedullary plasmacytoma of the nasal cavity: treatment perspective in a developing nation.

Extramedullary plasmacytomas are uncommon tumors, with a worldwide annual incidence of 3 per 100,000 population. They account for 1% of all tumors of the head and neck and 4% of all nonepithelial tumors of the nasal tract. A variety of treatment options has been suggested. These treatments vary according to the site of presentation, the presence of locoregional spread, and the histologic picture. Radiotherapy has been widely used as a treatment modality, but little has been written about surgery as a single management modality. However, such an option assumes importance in a developing nation, where patient follow-up is erratic and treatment costs must be kept low. We discuss the feasibility of surgery in such a circumstance, and we describe our surgical treatment of a case of extramedullary plasmacytoma in an elderly woman who presented to our hospital in India.

IL-2 gene transfer in immunotherapy of cancer: local administration of IL-2-activated lymphocytes and X63-m-IL-2 cells constitutively producing IL-2 inhibits growth of plasmacytomas in syngeneic mice.

Peritumoral administration of X63-m-IL-2 cells, transformed by IL-2 cDNA and constitutively producing large quantities of IL-2, was effective in mediating regression of X63-Ag8.653 plasmacytomas transplanted in syngeneic mice. Injection of killer cells with LAK activity substantially enhanced the tumor-inhibitory effect of IL-2-producing cells. In vitro activation of murine bone marrow and spleen cells with IL-2 produced by the X63-m-IL-2 cell line resulted in generation of lymphocytes with oncolytic activity against X63-Ag8.653 plasmacytomas and a variety of other target cell lines. The cytolytic activity of adherent IL-2-activated killer cells (A-LAK) was substantially higher than that of a nonadherent subset. Bone marrow and spleen cells from both healthy control and tumor-bearing mice contained LAK precursors, but the cytolytic activity derived from tumor-bearing mice was lower than that from healthy controls.

A new variant 15; 16 translocation in mouse plasmacytoma leads to the juxtaposition of c-myc and immunoglobulin lambda.

Mouse plasmacytomas (MPCs) induced by pristane oil, or by a combination of pristane oil and Abelson virus, carry one of two chromosomal translocations. The typical 12; 15 translocation leads to the juxtaposition of c-myc and immunoglobulin heavy-chain sequences, whereas the 6; 15 translocation links the kappa light-chain locus with the pvt-1 (plasmacytoma variant translocation) locus, located at least 75kb 3' of c-myc [Cory, S., Graham, M., Webb, E., Corcoran, L. & Adams, J. (1985). EMBO J., 4, 675-681]. Unlike the human Burkitt's lymphoma-associated translocation, the lambda/myc juxtaposed variant translocation has not been found previously in MPCs. Using unconventional MPC induction systems in which the tumor precursor cell was induced to proliferate in a secondary host, we have recently identified a 15; 16 translocation in six of the derived MPCs [Wiener, F., Silva, S., Sugiyama, H., Babonits, M. & Klein, G. (1990). Genes Chromosomes Cancer, 2, 36-43]. Chromosome 16 harbors the lambda light-chain gene. To explore whether the 15; 16 translocation represents the lambda/myc juxtaposition, we have mapped the breakpoints on chromosomes 15 and 16 by pulsed-field gel electrophoresis (PFGE). The pvt-1 region was mapped to approximately 220 kb 3' of c-myc. The breakpoint on chromosome 15 in ABPC-Ch-163-10, one of the six 15; 16 translocation-carrying MPCs, was situated approximately 80 kb 3' of c-myc and 140 kb 5' of pvt-1b, the major breakpoint cluster region of the previously analysed 6; 15 variant MPCs. The breakpoint on chromosome 16 was found to cut between the V1 and C3 regions of the lambda locus. Co-migration experiments showed that the C3 and the myc gene were juxtaposed head to tail on the 15; 16 translocation chromosome. On the reciprocal product V1 was juxtaposed to pvt-1.

A case of gastric plasmacytoma: genetic analysis and immunofixation electrophoresis.

A case of early gastric plasmacytoma in a 66-yr-old woman is reported. The demonstration of monotypic IgM lambda immunoglobulin in plasma cells infiltrating the gastric antrum contributed the histological diagnosis. Genetic analysis of the tumor cells confirmed the presence of a monoclonal neoplastic population of plasma cells in the tumor. Monoclonal immunoglobulin (IgM lambda) was demonstrated in serum, not by conventional immunoelectrophoresis, but by a highly sensitive immunofixation electrophoresis technique. Serum monoclonal immunoglobulin disappeared after subtotal gastrectomy. The value of immunofixation electrophoresis of serum protein and DNA analysis of tumor cells was emphasized for the diagnosis of gastric plasmacytoma.

Extramedullary cutaneous plasmacytoma.

Neoplastic degeneration of plasma cells can show different clinical features, including multiple myeloma and plasma cell leukemia. Skin involvement is not rare in patients with multiple myeloma, and represents a poor prognosis. Appearance of a cutaneous plasmacytoma without the involvement of bone marrow is very rare. We describe a patient who had cutaneous plasmacytoma, with two solitary lesions of the oral cavity. The patient also showed a cerebral involvement, but no evidence of multiple myeloma, with normal-appearing bone marrow.

Extramedullary plasmacytoma of the soft palate.

A 59-year-old white male presented with symptoms of an upper respiratory infection and the sensation of nasal fullness or obstruction. There were no constitutional symptoms or history of previous oropharyngeal neoplasms. Examination revealed a fleshy tan-pink 2 cm. x 1 cm. pedunculated lesion at the base of the uvula posteriorly, extending into the posterior soft palate. Indirect laryngoscopy confirmed extension of the lesion base onto the posterior soft palate. There was no palpable cervical adenopathy. CT scan of the sinuses and neck revealed no abnormality except the soft tissue mass in the area of the uvula and soft palate. Excisional biopsy of the lesion revealed a plasmacytoma which produced a monoclonal lambda chain immunoglobulin. Hemoglobin was 17.2 g/d. Serum electrophoresis and immunophoresis, Bence-Jones protein, bone marrow, and bone scan were normal. Twenty-six months later, a 1.5 cm. pedunculated lesion was noted at the site of original tumor, which was demonstrated to be recurrent plasmacytoma on excisional biopsy. No other lesions were identified on direct laryngoscopy and skeletal survey was normal. Repeat laboratory studies were normal except for a slight hypergammaglobulinemia (total protein 6.4 g/d., albumin 51.5 percent, alpha 1 globulin 4.0 percent, alpha 2 globulin 11.4 percent, beta globulin 12.8 percent, and gamma globulin 20.2 percent). The patient refused further treatment and has had no further recurrences at one year.

Pericarditis and pleuritis caused by extramedullary plasmacytoma.

The following illustrates a case study of a 9 years-old girl with combined pericarditis and pleuritis caused by solitary extramedullary plasmacytoma. Pericardiocentesis and permanent thoracocentesis were performed, both yielded serohemorrhagic and serous fluid in succession. In the beginning etiological diagnosis was made on the basis of the clinical pattern for tuberculosis infection and growth of three species of bacteria for bacterial infections and candida species for candidiasis. The initial treatment was in accordance with the etiological diagnosis mentioned above. The final diagnosis was establish in the fourth month after the discovery of plasmacytoma in the pleural fluid and CT scan examination disclosing masses in the right lung. Accordingly, cytostatic therapy was started. The result of therapy was very good, exudation into the pleural and pericardial sac regressed gradually and eventually ceased completely. According to the literature the prognosis of these neoplasma is good.

The effect of adherent peritoneal exudate cells (APECs) and their products on the RPC-5 plasmacytoma growth in vitro.

The ability of peritoneal exudate cells (PECs) and their adherent (APECs) and nonadherent (NAPECs) fractions to enhance RPC-5 plasmacytoma growth in vitro was studied. The capability of these cells to bind RPC-5 cells and influence of the binding on cytolysis tumor cells by activated with C. parvum macrophages was also determined. The effector cells were harvested from mice injected i.p. with pristane, thioglicollate medium or C. parvum or from intact mice. The effect of supernatants from the in vitro cultured PECs, APECs or NAPECs on growth RPC-5 cells were also tested. It was found that the RPC-5 plasmacytoma growth was enhanced only by cells obtained from mice treated with pristane, or by supernatants from cultured PECs and APECs derived from pristane treated mice. The adherent cells from pristane treated mice were able to bind tumor cells. The tumor cells preexposed to adherent cells from pristane stimulated mice were resistant to lysis by activated with C. parvum macrophages.

[A solitary extramedullary plasmacytoma of the nasopharynx].

A 72-year-old male, complaining of epistaxis, was referred to our hospital where a physical exam revealed a tumorous mass in the nasopharynx. A subsequent immunohistopathological investigation found that the tumor mass was a plasmacytoma with IgG and a lambda chain. Electrophoresis of a serum sample revealed a normal value and there was no signs indicating a multiple myeloma on x-ray surveys of the skeleton, or from bone marrow aspiration or scintigraphy. Thus the diagnosis was an extramedullary plasmacytoma and the patient received a combined treatment of chemotherapy, radiotherapy, and surgery. Since surgery, the patient has had neither a recurrence or a metastasis of the tumor for four years and three months.

Solitary conjunctival extramedullary plasmacytoma.

We describe two patients with different adnexal locations of localized extramedullary plasmacytomas, one under the conjunctiva of the caruncle and the other under the tarsal conjunctiva. In both general physical examination and ancillary tests, including bone marrow aspiration, were normal. Because immunohistochemical studies of the tumor showed no immunoglobulin secretion and electrophoresis and immunoelectrophoresis of the serum and urine were normal, we diagnosed solitary conjunctival extramedullary plasmacytoma in each case.

[Changes in the transcription activity of c-myc genes and heat shock proteins (HSP 70) after incubation of mouse plasmacytoma cells with dexamethasone].

The transcription of genes c-myc and hsp70 following inhibition of plasmacytoma cells proliferation by glucocorticoid hormone dexamethasone has been investigated. The dynamics of [3H] thymidine incorporation into the plasmacytoma cells DNA after incubation with dexamethasone has been studied. It declines after 8-35 h incubation with dexamethasone. The transcriptional activity of c-myc gene decreased after 3-12 h of incubation with hormone and restored to the original level at 24-30 h. The level of transcription of hsp 70 gene decreased after 12-48 h of introduction of hormone. Heat-shock treatment resulted in decrease of c-myc expression and to some increase of the transcription of hsp 70 gene. So, the dynamics of transcriptional activity of the genes c-myc and hsp70 are different and independent.

Isolation and characterization of in vitro and in vivo functions of a tumor-specific T suppressor cell clone from a BALB/c mouse bearing the syngeneic ADJ-PC-5 plasmacytoma.

A spontaneously transformed T suppressor (Ts) clone, A12-D11/t, is described which arose from the antigen-specific Ts clone A12-D11 isolated from cells of the inguinal lymph node of a BALB/c mouse bearing the syngeneic plasmacytoma ADJ-PC-5. A12-D11/t Ts cells suppress in vitro specifically a primary syngeneic cytotoxic antitumor response with the consequence that no ADJ-PC-5-specific cytotoxic T cells can be generated. The in vivo effects of A12-D11/t Ts cells were studied by injecting them into BALB/c mice. Spleen cells from those mice subsequently failed to respond against ADJ-PC-5 plasmacytoma cells, whereas their response against other syngeneic BALB/c tumors remained unaffected. By using a model system which allows us to study the host's immune reactions to the antigenic load corresponding to initial stages of tumorigenesis, it has been shown previously that ADJ-PC-5-specific Ts cells are activated before the antigen threshold for the activation of cytotoxic T cells is reached. Regarding its phenotype and specificity, the A12-D11/t Ts clone seems to be the exact counterpart of such a Ts cell, and is therefore of special interest in the study of the role of Ts cells preventing immunity against a growing tumor.
