Wobenzym in treatment of recurrent obstructive bronchitis in children.

A total of 27 patients with recurrent obstructive bronchitis (at least 3 episodes of obstructive bronchitis per year) of 5-15 years of age were studied. Serum IgE level (by ELISA method) and pulmonary function indices were determined together with the full clinical examination. The systemic enzyme preparation Wobenzym was administered with a dosage of 1 tablet for 6 kg weight. Wobenzym was administered for 3 months. Effectiveness of the treatment was assessed by special questionnaire, in which existence of the main symptoms were recorded. Analysis of the data, obtained after treatment, demonstrated decrease of the Daily Symptom Score and an increase of Symptom Free Days, as well as an improvement in spirometric indices (FVC, FEV, PEF). According to these data, it was concluded, that systemic enzyme preparation Wobenzym should be used as a supporting measure in combination treatment of recurrent obstructive bronchitis.

[Effects of salbutamo and ipraopium bromide inhalation on pulmonary function in young children with asthmatoid bronchitis].

OBJECTIVE: The efficacy of bronchodilator in asthmatoid bronchitis remains controversial. This study was designed to investigate the effects of bronchodilators, salbutamo and ipraopium bromide, on the pulmonary function in young children with this disease. METHODS: Pulmonary function tests were performed in 20 children with asthmatoid bronchitis (2 months-2.5 years of age) before and 30, 60, and 120 minutes after salbutamo and ipratropium bromide inhalation. The indexes of pulmonary function measured included tidal breathing flow volume (TBFV) loop, percent of tidal volume to peak tidal expiratory flow (%V-PF), terminal flows per peak expiratory flow (25/PF), peak tidal expiratory flow (PTEF), rate of mid-expiratory to mid-inspiratory flow (ME/MI), respiratory rate (RR) and tidal volume per kilogram (TV/kg). RESULTS: Before drug inhalation, the descending branch of the TBFV loop was depressed. The PTEF shifted forward and %V-PF (0.19 +/- 0.04) and 25/PF (0.42 +/- 0.11) decreased. These changes did not improve and the remaining indexes, RR, ME/MI and TV/kg, 30, 60, and 120 minutes after drug inhalation also remained similar to before inhalation. CONCLUSIONS: Salbutamo and ipratropium bromide inhalation did not improve the airway resistance and ventilation function in children with asthmatoid bronchitis. This suggests that the efficacy of bronchodilator in the treatment of this disease is doubtful.

[Clinical features and surgery in children with plastic bronchitis].

OBJECTIVE: To review the clinical features and therapeutic experience in children with plastic bronchitis. METHODS: Fourteen children with plastic bronchitis were reviewed retrospectively, 12 of which were under two years old. The clinical features are characterized by sudden onset, episodes of profound hypoxia and respiratory tract obstruction. SaO2 was between 0.70 and 0.80 even with mask oxygen inhalation. Eight cases were pyretic, 4 cases expectorated jel-like bronchial casts. The chest X-ray picture showed patchy consolidation or atelectasis unilaterally (10 cases) or bilaterally (2 cases). Pulmonary marking thickening and patchy shadow were observed in 2 cases. Twelve cases underwent rigid bronchoscopy and the bronchial casts were removed. Two cases underwent endotracheal intubation. RESULTS: Eight cases of 12 children received therapeutic bronchoscopy were cured. Other 4 cases had second therapeutic bronchoscopy and bronchial casts were removed again in 3 cases, one died from pulmonary hemorrhage. Two cases who underwent endotracheal intubation died from the multiple organ failure (MOF). Pathologic results showed:the bronchial casts were composed mainly of mucus and fibrin, inflammatory cell infiltrate were observed in 6 cases (Type 1, inflammatory), no cellular infiltrate occurred in 8 cases (Type 2, acellular). CONCLUSIONS: Plastic bronchitis is a severe and dangerous disease. The branching plastic casts may obstruct part or the entire tracheobronchial, causing respiratory failure. Bronchoscopy and pathologic examination are essential for it's diagnosis and treatment.

[Functional activity of the neutrophils in the bronchoalveolar space in chronic bronchitis and bronchiectasis].

Bronchoalveolar lavage fluid neutrophils were studied by the cytologic methods in 58 patients with chronic bronchitis, 63 ones with bronchiectasis, and 8 normal controls. The study included cytospectrophotometry of myeloperoxidase and alkaline phosphatase activity and estimation of active oxygen-producing cells in the NBT test. Neutrophilic functional activity was different in the patients with chronic bronchitis and bronchiectasis. Neutrophilic myeloperoxidase and alkaline phosphatase activities were lower in the patients with chronic bronchitis than in those with bronchiectasis, whereas the counts of cells active in the NBT test were low in both patient populations.

[Clinical and experimental research on chronic bronchitis treated with ke chuan ping decoction].

This paper presents the results of clinical observation and experimental research of ke chuan ping (KCP) in treating 31 cases of chronic bronchitis compared with that of traditional prescription qing jin hua tan decoction (QJHT) in other 31 patients with the same conditions randomized as control. The total effective rate of KCP group was 93.55% including 67.74% of basically cured and markedly effective cases. The total effective rate of QJHT group was 74.19% including 41.94% basically cured and markedly effective cases. There were significant differences between them (P less than 0.05). According to laboratory observation KCP had the ability to dilute sputum viscosity, promote pulmonary ventilation function, decrease WBC and raise PO2. Animal experimental research suggested that KCP had the effects on reducing sputum, relieving cough, modifying stridor and inhibiting bacteria. It was proved that KCP is a highly effective recipe for patients with chronic bronchitis. It was also shown that clearing away heat and dispersing phlegm therapy is an important measure for chronic bronchitis with Biao Zheng.

[The "Vaccination Reaction" syndrome of broilers after vaccination against Newcastle disease and infectious bronchitis (author's transl)].

In a part of the broiler flocks vaccinated against Newcastle disease (N.C.D) and infectious bronchitis (I.B.), disease symptoms of lingering nature have been observed, generally in the second half of the rearing period. In a practical investigation with weekly examinations of chickens, supplemented by a serological examination of twenty-four animals per flock at the age of six weeks, it was hoped to establish the factors responsible for this "vaccination reaction". In the district under notice the vaccination reaction syndrome had been responsible for widspread abandoning of twice spraying against N.C.D. in the first and fourth week in favour of drinking-water vaccination (generally combined with I.B. vaccination) in the second week and spray-vaccination in the fourth week or of combined drinking-water vaccination in the second week, either with or without N.C.D. drinking-water vaccination in the fourth week. Admittedly the incidence of vaccination reaction in flocks vaccinated exclusively via the drinking water was less frequent (32%) than in flocks in which the second vaccination was administered as a spray (48%), but this difference was largely accounted for by infection with Mycoplasma gallisepticum and/or Mycoplasma synoviae, complicated by infection with Escherichia coli. Of the thirty-three flocks free of mycoplasmosis, 24% exhibited the vaccination reaction, while the incidence to the fifteen infected flocks was 73.4%. In flocks infected with mycoplasmosis the course of E. coli infections was serious in 46.7% of the birds, while this figure was 18.2% in flocks free of mycoplasmosis. In flocks free of mycoplasmosis, the percentage of serious E. coli infections was lower after spray vaccination (14.3%) than after drinking-water vaccination (21%). The strains of E. coli brought in by one-day chicks from the hatchery disappear rapidly and play no role of any significance in the problem of colibacillosis observed at an age of three weeks or older. Of the 310 isolated strains of E. coli, 52 could not be typed and the others belonged to eighty different serotypes. With respect to the effect of infectious bronchitis the investigation does not provide sufficient evidence to permit of drawing conclusions. A significant role in the occurrence of the syndrome was played by coccidiosis and Gumboro's disease. With respect to environmental factors the available data did not allow of drawing conclusions. The authors recommend continuing with all available means to free the breeding animals from M. gallisepticum and M. synoviae, undertaking scientifically based research into the role of infectious bronchitis in the "vaccination reaction" syndrome, an effective programme of hygiene to control E...

[The effect of rehabilitation procedures on pulmonary function in patients with chronic obstructive bronchitis].

Modern treatment of patients with chronic obstructive bronchitis consists of a parallel and simultaneous carrying out of medicinal treatment and appropriate rehabilitation. The authors studied the effect of rehabilitation on the pulmonary function in 31 patients (rehabilitation group A-1) with chronic obstructive bronchitis within three weeks (position drainage on the oscillation table and vibro-message for 15-30 min. after aerosol-therapy, and respiratory diaphragmatic exercises). The control group was composed of 30 patients (medical group A-2) with chronic obstructive bronchitis treated with medicaments only. On the basis of the results before and after the treatment, the authors established that only group A-1 patients showed, at the end of treatment, a statistically greater forsed expiratory volume in the first second, reduced thoracic gas volume, reduced specific resistance and increased lung conduction in airways.

Effect of salbutamol and ipratropium bromide on airway calibre and bronchial reactivity in asthma and chronic bronchitis.

Bronchial reactivity to agonists such as histamine is seen in both chronic bronchitis and asthma, two conditions with different pathological changes in the airways. Salbutamol, when given acutely, reduces bronchial reactivity in patients with asthma, but the mechanism has not been clarified. To determine whether the effect of salbutamol depends on the pathological changes underlying the increased reactivity, we have compared the effect of salbutamol and ipratropium on bronchial reactivity in nine patients with asthma and ten with chronic bronchitis. Each drug was given on separate days in increasing doses (5, 100, 750, 1,000 micrograms) according to a double-blind, randomized design. Changes in forced expiratory volume in one second (FEV1) and specific airways conductance (sGaw) were measured after each dose, and the provocative concentration of histamine causing a 20% fall in the FEV1 (PD20) was determined after the last dose. Salbutamol and ipratropium were equipotent in the asthmatic subjects and caused a similar maximal increase in FEV1 (0.58 and 0.57 l) and sGaw (0.166 and 0.154 s-1.kPa-1). The two drugs also produced similar changes in the patients with chronic bronchitis, although the maximum response to both drugs was smaller (FEV1 0.29 and 0.32 l; sGaw 0.056 and 0.060 s-1.kPa-1; p less than 0.05). Despite differences in bronchodilatation the increase in histamine PD20 following salbutamol was similar in the asthmatic and bronchitic subjects (2.26 and 1.90 doubling doses (DD)) and greater in both groups (p less than 0.05) than the change in PD20 with ipratropium (0.84 and 0.56 DD).(ABSTRACT TRUNCATED AT 250 WORDS)

[The comparative pharmacokinetics of ceftriaxone in the blood and bronchial secretion in patients with different durations in the course of chronic bronchitis].

The study on ceftriaxone penetration into bronchial secretion showed that in patients with a short-term history of chronic bronchitis (no more than 3 years) ceftriaxone used in a dose of 1 g once a day intramuscularly was detectable in the bronchial secretion within 10 hours after the administration, its concentrations being 0.67-2.41 micrograms/ml in 3 hours, 15.87 micrograms/ml in 4.5 hours, 4.58 micrograms/ml in 6.5 hours and 2.29 micrograms/ml in 10 hours. In patients with a long-term history of chronic bronchitis (mean 10 to 20 years) the presence of ceftriaxone in the bronchial secretion was detectable in a concentration of 0.51-3.75 micrograms/ml only in 2 hours after its administration. Beginning from the 5th hour after the administration its detection failed. This is indicative of lower ceftriaxone penetration into the bronchial secretion of such patients. The duration of chronic bronchitis did not influence ceftriaxone pharmacokinetics in blood. The contents of the antibiotic in serum and bronchial secretion were determined by HPLC (the resolving power of 0.5 micrograms/ml).

A comparison of albuterol and erythromycin for the treatment of acute bronchitis.

BACKGROUND: Based on observations that pulmonary function tests of patients with acute bronchitis resemble those of patients with asthma, it was hypothesized that a bronchodilator may be an effective form of treatment for patients with acute bronchitis. METHODS: Albuterol was compared with erythromycin in a prospective, randomized, double-blinded fashion. Participants were patients who presented to family physicians with a history of having a productive cough of less than 30 days' duration, no history or evidence of pneumonia, and no other pulmonary or cardiac disease. Patients completed a 7-day symptom diary and returned to their physician after 1 week of therapy for reexamination. RESULTS: Patients treated with albuterol were less likely to be coughing after 7 days of treatment than patients treated with erythromycin (41% vs 88%, P less than .05). This was true for both smokers and nonsmokers and in patients with purulent-appearing sputum. Trends toward an earlier improvement in cough and an improved feeling of well-being also were observed in the albuterol group. No differences between groups were found as to the length of time before patients returned to work, the length of time until patients resumed normal activities, or the overall improvement in patient well-being. Minor side effects were equal in both groups. CONCLUSIONS: Oral albuterol may be more effective than commonly used antibiotics in relieving the symptoms of acute bronchitis.

[Laser therapy in dust bronchitis].

Laser therapies of 43 dust bronchitis patients (4 cases in 1st stage and 39 cases in 2nd stage) were performed in comparison with routine therapeutic procedures of 28 patients (6 cases in 1st stage and 22 cases in 2nd stage). 14 acupuncture points were selected. A good therapeutic effect was attained in 16.3% sample group and 7.1% control group patients, satisfactory results--in respectively 58% and 42.9% cases, insignificant results--in 25.6% and 50% cases. Along with the favourable therapeutic effect, a positive influence of laser therapy on the immune system was established, particularly on the immunity T-system, in dust bronchitis cases. Laser therapy was recommended as part of complex treatment procedures for dust bronchitis patients.

[Effect of jia wei yubingfengsan on bacterial adhesion to mouse tracheain chronic bronchitis model].

The experiment was designed to test the effect of Jia Wei Yubingfengsan (JW-YBFS) in chronic bronchitis of mice model. Two groups of mice model was used, one of which received JW-YBFS by oral administration another received only water as control experiment. The accurate quantification of bacterial adhesiveness by culture method showed that in JW-YBFS receiving mice viable counts of bacteria is much less than the control mice. In addition pathological examination showing the injury of tracheate mucosal epithelium was not found in JW-YBFS receiving mice while control mice showing typical chronic bronchitis lesion. The results demonstrated that JW-YBFS plays significantly role in bronchitis mice model. Our experimental data are in good agreement with Chinese medical conclusion, which means Fu Zheng Gu Ben.

Pathology of experimentally induced quail bronchitis.

Bobwhite quails (Colinus virginianus) were inoculated with 10(6) mean tissue-culture infective dose of quail bronchitis virus at 1, 3, 6, or 9 weeks of age by intratracheal, intraperitoneal, or subcutaneous routes. Quails developed necrotizing tracheitis, proliferative and necrotizing bronchitis and pneumonia; multifocal necrotizing hepatitis; necrotizing splenitis, with or without hyperplasia of splenic mononuclear phagocytes; bursal lymphoid necrosis; and bursal atrophy. Lesions were more extensive and severe in quails inoculated at 1 or 3 weeks of age than in older quails. Large intranuclear inclusions, characteristic of adenovirus infection, were identified in trachea, lung, liver, and bursa of Fabricius. This is the first report of the histopathology of experimentally induced quail bronchitis.

[The leukotriene B4 content of the bronchoalveolar lavage fluid and the function of the prostacyclin-thromboxane system in patients with variants of chronic bronchitis].

A study was made of the content of leukotriene B4, prostacycline and thromboxane A2 in the fluid of bronchoalveolar lavage in 62 patients with chronic bronchitis (CB) in the stage of exacerbation and remission. The time course of changes in the concentration of the eukosanoids was compared with the status of pulmonary local defense factors and cellular immunity. In catarrhal obstructive bronchitis, an important mechanism of a steady maintenance of bronchial obstruction involved a rise of the content of leukotriene B4 whereas in purulent obstructive bronchitis, it was an excess level of thromboxane A2. It is assumed that immunologically dependent activation of the leukotriene B4 and thromboxane synthetase capacity of alveolar macrophages may stipulate the clinical course of CB, modulating the bronchoconstrictor or inflammatory component of the disease. To correct phlogogenous function of alveolar macrophages, the use of immunomodulating therapy with a selective action on the suppressor component of immunity is desirable.

[Ambulatory treatment with cefuroxime-axetil of infectious bronchitis in patients sixty years of age or older: comparative study of the combination of amoxicillin and clavulanic acid].

The aim of this multicenter, prospective randomized trial was to compare the efficacy and safety of cefuroxime-axetil and amoxycillin/clavulanic acid for the treatment of infectious bronchitis in the elderly patient. Between January and April 1989, 157 out patients aged 60 years or more and presenting with infectious bronchitis were treated with either cefuroxime-axetil (250 mg bid), or the association amoxycillin/clavulanic acid (500 mg/125 mg bid). The two treatment groups were comparable at the time of inclusion; the mean age was 70 years, 82% of the patients were febrile, 75% presented purulent expectoration, 24% had a history of chronic bronchitis and 19% received symptomatic treatment was NSAIDs. The mean duration of treatment was 9 days. Clinical efficacy was assessed by the investigators. While fever and cough resolved similarly in the two groups, statistically fewer patients presented persistent purulent expectoration in the cefuroxime-axetil treatment group than in the group receiving amoxycillin/clavulanic acid (2% and 13%, respectively, p = 0.03). The proportion of patients who reported at least one side-effect was 3.6% in the cefuroxime-axetil treatment group against 21.6% of those who received the association (p = 0.006).

[Use of new mixtures of medicinal plants in the treatment of chronic bronchitis].

Three new prescriptions of drug plants were included into combined hospital treatment of 156 patients with chronic bronchitis. Each of the three mixtures was to treat a certain clinical form of chronic bronchitis being composed of plants with specific mechanism of action. The mixture was taken a tablespoonful 8-10 times a day. Pronounced and moderate effect of the therapy was achieved in 95.6% of those treated. This justifies the use of the mixtures as an adjuvant therapy in basic treatment of chronic bronchitis.

[Endoscopic and morphological characteristics of the bronchi and lungs in asbestosis and dust-induced bronchitis in asbestos-textile industry workers].

A bronchofibroscopy study of cases with histologic investigation of the bronchi and pulmonary mucous biopsy of 29 asbestos-textile workers, revealed diffuse bilateral endo-bronchitis (DBE) with concomitant tracheitis at different stages. No correlation was found between the endoscopy pictures and the clinical manifestations of asbestosis and dust bronchitis. The asbestosis diseased patients displayed pulmonary fibrosis++ without granulomatous inflammation in the asbestos dusts zone. In some cases, bronchial epithelium diffuse metaplasia was diagnosed with some dysplasia elements, as well as papillomatosis and malignant tumours in the lungs. Dust bronchitis++ and asbestosis can perform precancerous conditions in the lungs, what should be taken into account in cancer prevention measures for asbestos industry workers.
