A simple, reproducible and reliable method is proposed for measurement of cotyloid version. Using a standing frontal image of hip, the cotyloid borders are identified and a single-plane geometric projection determines the precise degree of version. This study can also be made from a conventional frontal pelvic image with bipodalic weight bearing. Knowledge of spatial orientation allows a better approach to head-cotyle biomechanical phenomena and thus improved understanding of the genesis of certain hip diseases.

Open reduction and internal fixation should now be considered the treatment of choice for most displaced fractures. The efficacy of this therapeutic approach is a function of improved fracture assessment coupled with advances in surgical exposure and stabilization techniques. A predictably high rate of good functional outcomes can now be expected, even in complex fracture patterns. It should be remembered, however, that this surgery is among the most demanding in orthopedics. Extensive study of general principles and techniques is necessary, and individual case preparation must be exhaustive. Early experience is probably best restricted to elemental fracture patterns. In this manner, both patient and surgeon may rely on a positive outcome.
To evaluate the role of CT in acetabular fractures, CT findings were compared in 45 patients with the information yielded by conventional radiography. In 16 cases the additional information obtained by CT was of importance; indications for CT in initial assessment of acetabular fractures are discussed.

A critical analysis of the results reveals the following major factors in determining the outcome of an individual acetabular disruption: The damage to the acetabular articular surface, which is reflected by the pattern of fracture and its degree of comminution, impaction, and osteoporosis, and associated damage of the femoral head The adequacy of the reduction, which is reflected by intraoperative inspection and postoperative radiographs The associated complications of the fracture and treatment (e.g., avascular necrosis, sciatic palsy, and heterotopic ossification) We recommend principles of treatment of acetabular fractures comparable to those employed for most other displaced intraarticular fractures, namely an anatomic reduction, stable internal fixation, and early motion, to realize reproducibly optimal results. The principal contraindications to open reduction are osteoporosis and generalized comminution to a degree that precludes a reasonable likelihood for the surgical team to achieve a stable anatomic reduction. We have provided guidelines for the surgical approach, techniques of reduction, and methods of fixation, and an analysis of results, which provide the basis for the successful treatment of these difficult intraarticular injuries.
