Growth rate analysis of lung metastases appearing 18 years after resection of cutaneous adenoid cystic carcinoma. Case report and review of the literature.

Growth rate analysis of lung metastases of a cutaneous adenoid cystic carcinoma, which appeared 18 years after the resection of the primary tumor from the scalp is presented. The doubling times of the metastases were long compared with that of other lung metastases. They were 22 months for the metastasis in the right lung and 70 months for the metastasis in the left lung, with a shortening of the doubling time in the left side to 10.4 months in the last 4 months of observation. Backward extrapolation showed that the metastases to the lung were disseminated before the diagnosis and surgical resection of the primary tumor. To our knowledge, this is the third reported case of lung metastases from a cutaneous adenoid cystic carcinoma out of 25 documented cases. We present a review of the literature and discuss the clinical implications of our findings.

[Investigation of two cases of adenoid cystic carcinoma of the esophagus].

A 59-year-old man and a 55-year-old man with adenoid cystic carcinoma of the esophagus are presented. They were also treated with esophagectomy but whose prognosis were significantly different. The former has been still alive for 3.5 years and the later died at 7 months after the operation. In the patient with good prognosis, the tumor was 1.6 x 1.2 cm in size and pathologically invaded muscularis propria (mp) with no metastases to lymph nodes (pathological stage I). It has been reported that the prognosis of adenoid cystic carcinoma of the esophagus is poorer than that of other histological type of carcinomas. However, it is suggested that if the tumor can been resected when it is small in size with no metastases, it will be able to expect for good prognosis as in this presentation.

Mandibular ameloblastoma and maxillary adenoid cystic carcinoma: case report.

We report the case of a 74-year-old woman who developed a follicular ameloblastoma of the right mandible and 22 months later developed a cribriform adenoid cystic carcinoma of the soft palate on the right maxilla. The ameloblastoma was treated by hemimandibulectomy, and the adenoid cystic carcinoma was managed by resection of the soft palate and the surrounding tissue and bone followed by a 6-week course of radiotherapy. Our review of the literature indicates that only one similar case has been previously reported where an odontogenic tumor and a salivary gland tumor involved two different anatomic locations in the same patient at nearly the same time.

[Adenoid cystic carcinoma of the head and neck: a review of 30 cases].

OBJECTIVES: To evaluate treatment results and identify prognostics factors which determine local and regional treatment failure and crude survival in adenoid cystic tumours (ACT) in the head and neck. METHODS: A retrospective study (1984 to 2001) of 30 cases of adenoid cystic tumour. The mean age was 56.9 years, with a sex ratio female to male of 2:1. Tumours of all sites were encountered, most (26.7%) being in the nose and sinuses and major salivary glands (26.6%). Tumours at the T4 stage were found in 14 cases, mainly situated in the nose and sinuses (50%). No patient had cervical lymphadenopathy, while two patients (6.7%) had bony metastases at the time of diagnosis. Two patients (6.7%) were treated by surgery alone, for T1 and T2 tumours; four patients (13%) had radiotherapy alone, and 24 patients (80%) had surgery with post-operative radiotherapy with a mean dose of 60.3 Grays (56-65 Grays). RESULTS: Local recurrence occurred in 30.8% (nine cases) after a mean interval of 43 months (1 to 10 years). Treatment of these recurrences was by revision surgery in six cases, leading to local control in one case, but in a patient with pulmonary metastases. No cases of lymph node recurrence were observed. 30% of patients developed metastases; these were pulmonary in 44% and bony in 33.3%. Mean follow-up was five years. Crude survival at 3 years was 91%, at 5 years 86%, and at 10 years 50%. The percentage tumour-free survival was 70% at 3 years, and 57% at 5 years. 22 patients (73.3%) are still alive, 17 of them recurrence-free. Analysis of the prognostic factors has shown tumour to be more aggressive in the nose and sinuses when they presented at an advanced stage (T4); recurrence was more common when excision was incomplete, or if there was peri-neural spread. Tumours of "massive" histological type carried a poorer prognosis than those of cribrigorm type (60% death compared with 10%). The development of metastases is independent of local recurrence. Metastases to bone appear to be more rapidly aggressive than pulmonary metastases, which may remain asymptomatic for some time. CONCLUSION: Combined radiotherapy and surgery have allowed improved local control of ACT, but the therapeutic challenge remains the multiply recurrent ACT, or those with symptomatic metastases, and this despite new research techniques (neutral therapy, immuno-histochemistry, molecular biology). ACT are rare tumours, whose prognosis remains poor.

[The results of tympanometry in children with adenoid hypertrophy and coexisting allergy].

UNLABELLED: Adenoid, nasal and middle ear diseases are frequent health problems for young children. Allergic diseases of the upper respiratory tract are becoming one of the leading factors in the etiology of Eustachian tube dysfunctions and this dysfunction is the most significant etiologic factor of secretory otitis (OMS). To examine the influence of atopy on the OMS we studied 89 children ages 4 to 10 years, with an average age of 6.2 who had adenoid hypertrophy and pathologic audiometry. In 36 children the skin tests showed an allergy, mostly to different kinds of pollen or house dust. There was a statistically significant difference in tympanometry by chi-square analysis, between allergy positive and allergy negative group. Type B tympanogram was found in 72% children whit coexisting atopy and in 21% without atopy however type C only in 19% allergic children and in 70% children without atopy. CONCLUSION: Children with allergic disorders of upper respiratory tract should be investigated by tympanometry for OMS.

[Analysis of differentially expressed lung metastasis-associated proteins in adenoid cystic carcinoma cell lines].

OBJECTIVE: To analyze differentially expressed metastasis-associated proteins in Adenoid cystic carcinoma cell lines of human salivary gland by proteomics. METHODS: Protein expression alterations of ACC-2 and ACC-M cells were described by 2-D gels. After image analysis by software, proteins of interest were excised from the gels and identified by matrix assisted laser desorption ionization time-of-flight mass spectrometer. RESULTS: 12 protein spots showed significantly differential expression patterns between two cell lines. In the identified protein candidates, transketolase, modulator recognition factor 2, Dim1p homolog, splicing factor (arginine/serine-rich 9) and v-Ha-ras l oncogene were all lowly expressed in the poorly metastatic ACC-2 cell and significantly upregulated in highly metastatic ACC-M cell, while type I collagen pro alpha and tumor necrosis factor (ligand) superfamily member 4 showed a high expression in ACC-2 cells and a low expression in ACC-M cells. Pirin (spot 6) just appears in ACC-2 cell and was not detectable in ACC-M cell, while retinal homeobox protein was just detected in ACC-M cell and did not appear in ACC-2 cell. CONCLUSIONS: The proteins may be involved in the adenoid cystic carcinoma lung metastasis through different mechanisms. Our work may contribute to discover diagnostic markers and therapeutic targets.

Sinonasal adenoid cystic carcinoma with widespread symptomatic bony metastasis.

Adenoid cystic carcinoma is a fairly uncommon salivary neoplasm of the head and neck. These lesions often progress insidiously, and they have a propensity for early perineural spread and bony invasion. Distant metastasis to the lungs is fairly common and is usually fatal. We report a rare case of adenoid cystic carcinoma in a patient who on initial evaluation had widespread bony metastasis but no pulmonary involvement.

[Successful treatment of multiple pulmonary metastases of adenoid cystic carcinoma of the external auditory meatus with TS-1].

The patient, a 67-year old man, was referred to us with refractory otorrhea of the left ear. The diagnosis was adenoid cystic carcinoma of the external auditory meatus, so the primary focus was excised. During the postoperative observation period, multiple metastases to the lung were noted, for which TS-1 (120 mg/day, given for 28 consecutive days, each course followed by a discontinuation of 14 days) was administered. After the completion of 2 courses, reductions in the metastatic pulmonary lesions and the amount of pleural effusion were noted, while adverse effects were limited to mild anorexia. Thus, it was possible to continue medication for 15 months on an ambulatory basis. It was indicated that TS-1 may be effective in the treatment of multiple pulmonary metastases of adenoid cystic carcinoma.

[Adenoid cystic carcinoma of external auditory canal: a report of 7 cases].

OBJECTIVE: To detect the clinical and pathological features of the adenoid cystic carcinoma of external auditory canal, and to improve its diagnosis and treatment level. METHOD: A retrospective analysis was performed on 7 patients with ACCEAC by their clinical manifestation, the therapy and follow-up results as well as the combination with the review of related literature. RESULT: The period from the appearance of primary symptoms to definite diagnosis averaged 5.7 years. All patients were treated by surgical excision and followed up from 2 years 8 months to 9 years 11 months. Five of them were recurrent after operation, 2 of which died. CONCLUSION: Adenoid cystic carcinoma of external auditory canal, which is rarely seen in clinic, is not easy to exact diagnosis in early stage. It progresses slowly and tends to invade parotid and even intracranial cavity. It also has the tendency of recurrence and pulmonary metastasis. So, its necessary to do both aggressive surgery and postoperative irradiation. The long term follow-up indicates that the tumor has a poor prognosis.

[Adenoid cystic carcinoma with striking intracranial extension: a case report].

We report a case of intracranial adenoid cystic carcinoma in the middle cranial fossa. A 40-year-old woman had noticed a tumor in the left external auditory canal 8 years previously. It was totally resected at that time. Histologically, the tumor was diagnosed as an adenoid cystic carcinoma. Recently the patient presented with loss of consciousness, suffering from mild recent-memory disturbance. Neurological examination revealed no other abnormalities. Magnetic resonance imaging (MRI) demonstrated an enhanced solid mass with a multicystic component in the left temporal lobe. She underwent subtotal removal of the tumor. It was well-demarcated, and was confirmed to be adhesive to the dura matter of the middle cranial fossa. Histology of the tumor was the same as it was 8 years ago. She was discharged from our hospital without neurological deficits. Two months after surgery, gamma knife radiosurgery was performed against the residual tumor.

[The expression of PTEN and PKB in salivary adenoid cystic carcinoma and its clinical significance].

PURPOSE: To investigate the expression of PTEN and PKB in salivary adenoid cystic carcinoma (SACC) and its clinical significance. METHODS: The expression of PTEN and PKB in 63 cases SACC were evaluated by SP immunohistochemistry. SPSS 11.5 software was used for chi-square test. RESULTS: In 38 cases, both non-cancer tissue near by cancer and cancer tissue, 21 (55.26%) cases cancer tissue PTEN expression positive were found, 32 (84.21%) cases the non-cancer tissue near by cancer PTEN expression positive were found. There was significant difference in cancer tissue and non-cancer tissue near by cancer (P<0.05). 31 (81.58%) cases cancer tissue PKB expression positive were found, 20 (52.63%) cases the non-cancer tissue near by cancer PKB expression positive were found. There was significant difference in cancer tissue and non-cancer tissue near by cancer (P<0.01). In 25 cases with metastasis, 9 (36%) cases PTEN expression positive were found; in 38 cases with non-metastasis, 26 (68.42%) cases PTEN expression positive were found; there was significant difference in cases of metastasis and non-metastasis (P<0.05). In 25 cases with metastasis, 24 (96%) cases PKB expression positive were found; in 38 cases with non-metastasis, 27 (71.05%) cases PKB expression positive were found; There was significant difference in cases of metastasis and non-metastasis (P<0.05). PTEN expression and PKB expression were no significant difference between the different sex, age and sorts of pathology. PTEN and PKB were negatively correlated (P<0.01). CONCLUSIONS: The expression of PTEN and PKB may be related to the occurrence and progress of salivary adenoid cystic carcinoma,PTEN and PKB may have effect on metastasis.

[The adenoid cystic carcinoma].

The adenoid cystic carcinoma arising in the external auditory canal is a rare tumor, as other malignant neoplasias of the EAC and middle ear. The early diagnosis of these malignant tumors of the ear is very important to improve the prognosis. In this sense, the clinic experience is frequently the most useful diagnosis instrument to suspect this pathology when we have a confrontation with chronic otorrea and otalgia, common presentation form of this disease. In this report we show a new case of adenoid cystic carcinoma of external auditory canal and review the most significant medical literature existing in relationship with this pathology.

[Dynamic observation of lung metastasis model of adenoid cystic carcinoma].

OBJECTIVE: To form ACC-M-GFP cells by transfecting pEGFP-1 into ACC-M cells, and to build up lung metastasis model of adenoid cystic carcinoma for dynamic observation in simulated lung environment. METHODS: pEGFP-1 was prepared and then transfected into ACC-M cell lines by using cationic lipid-based gene transfer technique. After successive selection, the ACC-M-GFP cells were collected and inoculated in BALB/C mice. The simulated lung environment was built up, and dynamic observation was performed by laser confocal microscopy within four weeks after transfection. RESULTS: The amount of fluorescent tumor cell colonies and the intensity of fluorescence gradually increased from the second week to the forth week after transfection. In the meantime, only a very small number of tumor cells had the ability to form clones. CONCLUSIONS: We successfully built up a lung metastasis model of adenoid cystic carcinoma for dynamic observation. The model is suitable for capturing and analyzing metastatic tumor cells in early stage.

[Aspects of biological activity of T and NK lymphocytes in children with adenoid hypertrophy and concomitant otitis media with effusion].

UNLABELLED: The aim of this study was to inquire whether it has got an influence on the number and activity of peripheral blood T and NK cells. MATERIAL AND METHODS: The examined group consisted of 51 children with adenoid hypertrophy. The first group consisted of 17 children with only one episode of otitis media with effusion (ome). The second group consisted of 14 children with more than 4 episodes of ome and the third group consisted of 20 children with adenoid hypertrophy without ome. Evaluation of percentage of CD4+, CD8+ and NK subsets with co-expression of CD69, HLA-DR molecules in peripheral blood was performed by flow cytometry method. In examined groups with ome the decrease of CD4 number was compensated by increase of the number of NK and CD19 cells. In these groups the increase of CD69 and HLA-DR expression on the examined subsets was observed. The results suggest that in recurrent otitis media with effusion may be one of the factors causing T lymphocytes deficiency. Obtained results might be one of the criteria of using adenoidectomy.

[Olfactory sensitivity in adenoid hyperplasia].

Olfactory sensitivity was assessed in 37 children aged between 5 and 12 years before and after adenectomy. Olfactory detection thresholds for 2-phenylethanol and dimethyldisulphide, as well as the ability to distinguish 8 other odours, were measured using a squeeze bottle test. The results suggest that preoperative olfactory detection thresholds in subjects with severe nasal obstruction attributable to adenoid hypertrophy are higher than in those with less severe nasal obstruction. The olfactory detection thresholds improved after adenectomy in cases with high preoperative nasal obstruction indices. The olfactory detection threshold remained relatively unchanged in cases with less severe adenoid hyperplasia. In contrast, improvement in olfactory discrimination was observed in all children, irrespective of the degree of preoperative nasal obstruction.
