[Analysis of 216 cases of pleomorphic adenoma of the parotid gland].

Pleomorphic adenoma is the most frequent benign tumour of the salivary glands. The most common localization is the parotid gland. The authors present a clinical analysis of the group of 216 patients, who was treated in ENT Department and who was diagnosed as pleomorphic adenoma. The frequency of incidence of mixed tumours is 50.70% of all 426 cases of parotid gland's tumours. In 92, 13% cases the tumours was localized in the superficial part, only in 7.87% in deep part of the gland. Sixteen patient was operated on because of the recurrence of mixed tumours. In one case we observed the syndrome of the auriculotemporal nerve. The temporary paresis of facial nerve after operation occurred in 19 patients. We don't noticed the incidence of salivary fistula in our material.

The immunohistochemical outline of p27kip1, cyclin B1 and cyclin D3 in pleomorphic adenomas.

AIM: Pleomorphic adenomas of salivary glands are benign lesions which may sometimes relapse even after complete surgical removal. This risk has led to the search for methods to provide predictive data on the biological behaviour of such neoplasia. The authors intend to evaluate the degree of cellular aggression of these tumours by finding prognostic data using the antigens involved in cellular proliferative activity. Therefore they have chosen for this study: p27kip1, cyclin B1 and Cyclin D3. METHODS: Seventeen mixed tumours, 2 of them relapsed, underwent the direct immunohystochemical PAP technique for the determination of antigens p27kip1, cyclins B1 and D3 of the tissue. RESULTS: The results obtained show that the verification of these markers may reveal a potential risk of biological deviation and that their expression is independent of the degree of cellularity in neoplasias. CONCLUSIONS: On the basis of the results, the conclusion is drawn that there is no relation between the expressivity of the mentioned antigens and histological characters of pleomorphic adenomas.

[Histogenesis of pleomorphic adenomas of the salivary glands].

Histogenesis of pleomorphic adenomas was investigated by histological and electron microscopical methods. Histochemically alkaline phosphatase activity was revealed in transformed cells. Positive immunohistochemical reaction was shown in cells of pleomorphic adenomas with polyclonal antibodies against the smooth muscle myosin, human carbonic anhydrase III and monoclonal antibodies to cytokeratins N8, N17, N18, vimentin (clone NT-30) and to membrane epithelial antigen. It is concluded that these tumours are of the epithelial nature.

Phosphatase enzymes. Cytochemical study of pleomorphic adenoma and normal human salivary glands.

Human parotid glands, submandibular glands, and pleomorphic adenomas were examined by electron microscopic histochemistry. All epithelial cells of the normal salivary glands showed plasma membrane adenosine triphosphatase (ATPase) and inosine diphosphatase (IDPase) activity. However, myoepithelial cells reacted most intensely. Pleomorphic adenomas showed epithelial cells within solid and ductal portions of the tumors that were variably reactive for both ATPase and IDPase. Histochemical examination of the epithelial cells in the myxoid portions of the tumors did not provide conclusive evidence as to the nature of their progenitor cells. Surface-associated phosphatases (alkaline phosphatase, ATPase, and IDPase) cannot be reliably used as histochemical markers of salivary gland myoepithelial cells. Therefore, morphological and phosphatase histochemical studies that intend to examine the role of myoepithelial cells in salivary gland neoplasms must be interpreted with care.

Various expressions of modified myoepithelial cells in salivary pleomorphic adenoma. Immunohistochemical studies.

Variant expressions of modified myoepithelial cells in salivary pleomorphic adenomas are described as determined by immunohistochemical techniques which visualized the distributions of S-100 protein, intermediate-sized filament proteins (keratin, vimentin, and desmin), and contractile proteins (myosin and actin), as well as lysozyme and lactoferrin. Immunohistochemical staining patterns of S-100 protein were basically used to classify modified myoepithelial cells, along with histologic criteria. Histochemical modifications of myoepithelial cells in pleomorphic adenoma of salivary glands could be divided into a) reactive, b) transformed, and c) neoplastic myoepithelial cells. Reactive myoepithelial cells were stromal-like cells which displayed an intense S-100 protein reaction. Transformed myoepithelial cells were negative or slightly positive for S-100 protein; they were located in the outer zone of tubular or duct-like structures and were spindle-shaped. The inner round cells of tubular and ductal structures, which could be ductal origin, gave intense keratin staining, as well as marked reactions for lysozyme and lactoferrin. Neoplastic myoepithelial cells were plasmatoid or fibrous types of cells and contained abundant S-100 protein and vimentin. These cells were termed "myoepithelioma" as in classical diagnosis.

[Pleomorphic adenoma of the salivary glands. Impact of the myxoid component on the prognosis].

In a series of 570 pleomorphic adenomas of main and accessory salivary glands, we found 48 recurrences (8.04%). These recurrences occurred between several months and 19 years after surgical treatment. They were more frequently noted in parotid tumors. Comparatively to a control series, in recurrent tumors myxoid structures were more frequent than epithelial structures, as demonstrated by immuno-markings (anti-protein S 100 and anti-vimentin sera). Besides, in these tumors, extracapsular myxoid expansions were commonly seen. The proliferative potential of myoepithelial cells which are the main cellular component of myxoid areas, is thus suggested as a mainly factor of recurrence of pleomorphic adenomas.

[Histological and histochemical observations on the horny pearls of pleomorphic adenoma].

An unusual case of pleomorphic adenoma displaying numerous horny pearls is described. The tumour arose in the right lateral lingual border of a 33-year-old man. In many pearls the cores were organized in two separate portions with distinct structural and histochemical features. The narrow peripheral portion had a compact structure and was rich in keratin since it contained cystine and was coloured with acid dyes of intermediate molecular size. The central portion was characterized by a loose texture and its histochemical profile (affinity for acid dyes of large molecular size, weak alcianophilia, vivid binding of colloidal iron) resulted from the existence of acidic mucosubstances in the cell coating of the neoplastic keratinized cells. These observations suggest that certain similarities exist between the processes of normal keratinization and squamous metaplasia in pleomorphic adenoma.

[Surgery for pleomorphic adenoma of the parotid gland evaluated by long term follow-up].

Fifty six cases with pleomorphic adenoma of the parotid gland were evaluated on local recurrences following surgical treatments which were performed from 1970 to 1983. The surgical treatments for the tumors were 11 extirpations and 45 parotidectomies. The recurrence in each treatment was observed in one (9%) and in none (0%), respectively. The average follow-up period is 11 years (from 5 to 19). The treatment of choice for pleomorphic adenoma is the parotidectomy, of the superficial, deep and/or posterior, lobes.

Benign neurilemmoma (schwannoma) masquerading as a pleomorphic adenoma of the submandibular salivary gland.

A case of solitary benign neurilemmoma (schwannoma) arising in the submandibular region is presented. The tumor was mistaken clinically for an enlarged submandibular salivary gland. Fine needle aspiration cytology made an erroneous diagnosis of a pleomorphic adenoma, predominantly stromal in composition. Histology of the resection specimen resulted in the correct diagnosis of a benign schwannoma. Review of the needle aspirate demonstrated cytologic features that should enable both the correct diagnosis of this neoplasm and its distinction from pleomorphic adenoma, which it mimicked in this location.

[Recurring tumors of the parotid gland with special reference to pleomorphic adenoma].

161 patients with parotid tumors seen between 1974 and 1980 are analyzed. A recurrence rate of 20% was found for both benign and malignant tumors. 76% of the recurrent tumors were judged to have been inadequately excised at the first operation. Pleomorphic adenomas had a recurrence rate of 25%. More than half these tumors exhibited multinodular patterns of recurrence. Therefore, a recurrent pleomorphic adenoma should not be reoperated upon for at least one year.

[Histogenesis of pleomorphic adenoma of the salivary gland].

Six parotid pleomorphic adenomas were studied histochemically by electron microscopy after staining with ruthenium red. It was found that the myxoid region was filled with ruthenium red granules. Similar positive ruthenium red-stained intracellular granules were observed within the Golgi-derived vacuoles of the neoplastic myoepithelial cells in the myxoid region. Some cells located at the periphery of the double-layer ductal structure had the features of myoepithelial cell, they also showed similar positive ruthenium red-stained granules in the intracellular vacuoles and intercellular space. No ruthenium red-stained granules was obtained in neoplastic ductal epithelium and the mucus in ductal cavities were negative for ruthenium red stain. The observations indicate that the neoplastic myoepithelial cells in the myxoid region probably derive from the periphery of the double-layer ductal structure, they secrete the proteoglycans forming the myxoid region and play an important role in the histogenesis of pleomorphic adenoma.

[Dysfunction of the facial nerve following total conservative parotidectomy for pleomorphic adenoma].

Two hundred and forty seven previously untreated pleomorphic adenoma of the parotid gland were operated with a total conservative parotidectomy between 1965 and 1992. The overall incidence for temporary facial nerve paresis and paralysis was 63.1% and 5.2%, respectively. None of the following variables sex, age, tumor size, tumor location, tumor contact with the facial nerve, degeneration of the pleomorphic adenoma, inadvertent tumor spillage, use of Surgical, and postoperative hematoma were statistically related to facial nerve dysfunction. Kaplan-Meier actuarial analysis demonstrated that facial nerve dysfunction recovery was noted until the 18th postoperative month. The use of the linear regression model with Pearson correlation underscores that the recovery time of facial nerve function significantly statistically increased with the patient's age (p = .0017). The overall incidence for permanent facial paresis and paralysis was 3.6% and 0%, respectively. Implications of these findings are discussed together with a review of the medical literature.

[Crystalloids in salivary gland pleomorphic adenoma].

We examined crystalloids in salivary gland pleomorphic adenomas. The crystalloids were detected in 4 of 34 pleomorphic adenomas (11.7%). In three cases they were found in the minor salivary gland and in one case in the major salivary gland. Light microscopy revealed that all the crystalloids were in parenchyma. They were composed of eosinphilic structure and radially arranged clusters of needle-shape fibers. The central parts sometimes contained clear space. They were usually closely surrounded by neoplastic myoepithelial cells. All 4 cases containing crystalloids were million reaction-negative and positive to van Gieson, Mallory and Reticulin Silver Impregnation stains. Therefore all the crystalloids in our 4 cases were considered as collagen-rich.
