Reversed circadian blood pressure rhythm independently predicts endstage renal failure in non-insulin-dependent diabetes mellitus subjects.

To investigate the significance of reversed circadian blood pressure (BP) rhythm as a predictor for diabetic endstage renal failure, introduction of hemodialysis (HD) was determined as an end point in 325 noninsulin-dependent diabetes mellitus (NIDDM) subjects, in whom 24-h BPs had been monitored during their first admissions between 1988 and 1996. Circadian BP rhythm was analyzed by the COSINOR method, as previously reported. After exclusion of 68 dropout subjects, 257 were recruited for further analyses, in which 194 had normal circadian BP rhythms (N), and the remaining 63 had reversed rhythms (R). During this follow-up period, the numbers of HD-introduced subjects in N and R were 6 and 16, respectively, showing a higher prevalence in the latter (p < 0.001, chi2 test). Follow-up periods were significantly shorter in HD-introduced diabetic subjects of N and R than those in HD-free subjects of each group. In baseline characteristics, there were no differences in age, gender, or serum creatinine between HD-free and HD-introduced subjects of N or R. With regard to microvascular complications, the degree of retinopathy and nephropathy in N and R tended to be more pronounced in HD-introduced subjects than in HD-free subjects. Further, mean levels of circadian mean BP rhythms in HD-introduced subjects of N or R were similarly high, compared with those in HD-free subjects of each group, irrespective of circadian BP pattern. Unadjusted HD-free times were estimated by the Kaplan-Meier method, with a significant difference noted between N and R (p < 0.001; log-rank test). The Cox proportional-hazards model adjusted for circadian BP pattern, age, gender, blood pressure level, glycemic control, duration of diabetes, serum total protein, and serum creatinine demonstrated that circadian BP pattern, age, gender (female), blood pressure level (hypertension), and serum creatinine exhibited significant high relative risks. Thus, our data suggest that reversed circadian BP rhythm is an independent predictor of endstage renal failure in NIDDM subjects.

Determination of glycated proteins in biological samples based on colorimetry of 2-keto-glucose released with hydrazine.

We developed a novel method for measuring glycated (glc) proteins in biological samples, based on the colorimetry of 2-keto-glucose which is released from the glc protein (ketoamine) on heating with hydrazine. The ketoamine-induced coloration remained constant at room temperature (25-27 degrees C) for 1 h. The method gave reliable precision and accuracy. However, high concentrations of serum pigments caused positive interference, suggesting that hemolytic or hyperbilirubinemic serum would give false-positive results. The concentration of glc protein in clinical serum samples measured by the present method (y) correlated well with those (fructosamine values, x) measured by the nitroblue tetrazolium-reducing method: y = 1.27x-1.69 (r = 0.92, n = 93). The concentrations (microM, mean +/- S.D.) of glc protein in sera from normal and diabetic subjects were 275 +/- 37 (n = 32) and 403 +/- 98 (n = 32), respectively, and the concentrations (nmol/mg hair, mean +/- S.D.) of glc protein in back hairs from non-diabetic and diabetic rats were 3.7 +/- 0.3 (n = 10) and 8.6 +/- 1.5 (n = 10), respectively. Thus, the technique gave reasonable concentrations of glc proteins in humans and rats with diabetes mellitus, indicating it to be reliable and diagnostically useful.

[The chronological changes in the size of maxillary defect].

Use of a maxillary prosthesis is now considered one of the good methods to ameliorate the crippling caused by the defect in the maxillary palate area after an ablative procedure for a maxillary tumor and it consumes no physical resources from the patient. There has been debate on whether the prosthesis should be made in the very early postoperative days or after the wound is stabilized. In this study, changes in the area of the defect in the palate after surgery were investigated by using plaster models from clinical cases we have experienced. Twenty-one cases were investigated according to the following features: "maxillary sinus cancer "+" more than 1.5 years of follow-up for maxillofacial prosthetics "+" presence of more than two casts of the case". These casts were photographed from a distance of 1.5 m, and the photographs were enlarged to the original size of the cast, then the defect area was measured with a Planimeter (Uchida Co. Ltd). The area of the defect had shrunk in 16 cases. It took over 1 year to get stabilized in 80% of the cases. It is believed to take around 3.5 years for all cases to become stable. This tendency had no relationship with the pattern of the defect, the stroke of the mouth opening, the pattern of the rest of the teeth, or the defect area itself. These findings suggest that it may be advisable to start the prosthetic work as early as possible rather than waiting for stability of the defect wound, to improve the patients's QOL.

A model of localized osteolysis induced by the MBT-2 tumor in mice and its responsiveness to etidronate disodium.

A new experimental method to test the effect of drugs on tumor-induced osteolysis using a bladder tumor in mice has been designed. The method consisted of inoculating MBT-2 tumor cells s.c. over the calvaria in mice, resulting in a local tumor causing fragmentation of the bone. This was accompanied by adjacent osteoplastic changes, which were evaluated by X-ray and histological examination. Etidronate disodium (EHDP), a diphosphonate derivative, at a dose of 3 mg/kg to 30 mg/kg s.c. protected the bone by decreasing the extent of osteolysis as judged by the same criteria. Therapy with EHDP prolonged the survival period. This inhibition was obtained with no apparent effect on the growth of the MBT-2 tumor.

Granulocyte colony-stimulating factor receptor expression on human transitional cell carcinoma of the bladder.

Receptors for granulocyte colony-stimulating factor (G-CSFRs) have been confirmed on the cell surfaces of several non-haematopoietic cell types, including bladder cancer cells. This observation has naturally led to the hypothesis that the expression of G-CSFR on these cells may enhance their growth by G-CSF. In this study, the expression of G-CSFR was determined in both established human bladder cancer cell lines and primary bladder cancers. We studied five different human bladder cancer cell lines (KU-1, KU-7, T-24, NBT-2 and KK) and 26 newly diagnosed bladder tumours. G-CSFR mRNA expressions on cultured cell lines were determined using the reverse transcriptase polymerase chain reaction (RT-PCR) method. Furthermore, the G-CSFR binding experiments on the cultured cell lines were conducted using the Na(125)I-labelled G-CSF ligand-binding assay method. Moreover, the G-CSFR mRNA expressions on primary bladder tumour specimens were assessed using the in situ RT-PCR method. Three out of the five cultured cell lines (KU-1, NBT-2 and KK) exhibited G-CSFR mRNA signals when the RT-PCR method was used. The G-CSFR binding experiments showed an equilibrium dissociation constant (K[d]) of 490 pM for KU-1, 340 pM for NBT-2 and 103 pM for KK cells. With in situ RT-PCR, the tumour cells of 6 out of 26 primary bladder tumour specimens (23.1%) presented positive G-CSFR mRNA signals. Thus, in this study, G-CSFR expression was frequently observed on bladder cancer cells. Therefore, the clinical use of G-CSF for patients with bladder cancer should be selected with great care.

[The effect of the odour of mother's milk and orange on the spectral power of EEG in infants].

Few studies have been found on the EEG responses to odours in infants. Most of the studies done regarding infants up until now have focused only on the psychological and behavioral reaction of infants to odours. The purpose of the present study is to investigate whether or not relative differences in EEG response are seen before and during the odour presentation of mother's milk and orange in infants, and if they do respond to these odours, are there any differences in the EEG responses? EEG responses to these two odours were studied in 11 healthy mixed fed infants aged from 3.3 to 4.2 months during sleep. Changes in the sleep stage 2 and slow wave sleep EEG's were analyzed by the fast Fourier transformation method. While the mother's milk was presented, a significant decrease in the amplitude of EEG was observed in the delta and theta bands in the bilateral frontal and central regions of the brain, but on orange odour presentation, an increase of delta bands was seen in the central and right parietal regions. Detailed statistical analysis (ANOVA) revealed that the EEG responses to the two odours were significantly different from each other in the delta and theta bands. The present results suggest that these two odours seem to cause a different psychophysiological change in infants.

Immunization of young chickens by trickle infection with Eimeria tenella.

Immunization of chickens was attempted with low levels of Eimeria tenella oocysts (50 oocysts per day) over the first 1 or 2 weeks of life--the "trickle infection" (TI) method. When chickens were immunized by TI at 0-13 days of age, no cecal lesions and a reduced number of oocysts in ceca were observed after challenge at 17 days of age. TI at 0-6 days of age conferred better protection against challenge with E. tenella than did TI at 7-13 days. However, cecal lesions were observed in almost all of these chickens. These findings indicated that TI for 2 weeks (0-13 days of age) provided better immunity than TI for 1 week (0-6 or 7-13 days of age).

Molecular cloning and functional expression analysis of a cDNA for human hepassocin, a liver-specific protein with hepatocyte mitogenic activity.

By means of differential cDNA expression cloning, we earlier isolated a novel rat cDNA and its protein, named hepassocin, which is upregulated during liver regeneration. Using the rat cDNA as a probe, we have now isolated human hepassocin cDNA encoding a protein of 312 amino acids, which has 81.4% and 83.8% identity, respectively, to rat hepassocin before and after elimination of its signal peptide. Dot blot analysis revealed that hepassocin mRNA was strongly expressed in adult liver, fairly strongly in fetal liver, and weakly in pancreas, but not in other tissues. Recombinant human hepassocin produced in Chinese hamster ovary (CHO) cells by the dihydrofolate reductase-methotrexate (DHFR--MTX) gene amplification method is a homodimer (68 kDa) and has mitogenic activities in hepatocytes of various animal species including rat, mouse, rabbit and dog, and the activity was lost with 2-mercaptoethanol treatment. These results suggest that hepassocin is a potent regulator in liver cell growth not only in rats but also in humans. Computer searches revealed that human hepassocin as well as rat hepassocin has a characteristic disulfide structure close to that of fibrinogen-gamma. We assume that this newly identified growth factor exerts functions in association with an extracellular matrix such as fibrinogen.

Estimation of the number of polyhydroxyalkanoate (PHA)-degraders in soil and isolation of degraders based on the method of most probable number (MPN) using PHA-film.

A new method to estimate the number of polyhydroxyalkanoates (PHA)-degraders in soil and to isolate degraders, called the film-MPN method, is proposed. The incubation time was measured by the first order reaction (FOR) model. This method was used to estimate numbers of poly(3-hydroxybutyrate-co-3-hydroxyvalerate)[P(3HB-co-3HV)]- and poly(3-hydroxyvalerate-co-4-hydroxybutyrate)[P(3HB-co-4HB)]-degraders in garden soil (4.30 x 10(5) and 2.15 x 10(5) aerobic degraders per gram of dry soil, respectively). The number of P(3HB-co-3HV)-degraders in paddy field soil was 5.06 x 10(5) aerobic degraders per gram dry soil. Also, several P(3HB-co-3HV)-degraders were isolated directly from positive-growth tubes of high dilution.

Risk analysis of low cardiac output syndrome after valve replacement.

In order to obtain a better understanding of the pathogenesis of the postoperative low cardiac output syndrome (LOS), a multivariate regression analysis was performed, evaluating predictive risk factors quantitatively as a function of plural preoperative variables. A total of 145 cases including 76 MVR (MS 36 MR 40), 42 AVR and 27 DVR were analyzed in this study. Ten historical, 10 hemodynamic and 4 operative risk factors were collected to compose the data file with corresponding status of postoperative cardiac function which was classified as follows. Patients who were not associated with postoperative LOS (Score 1), associated with the LOS which required and responded to ordinal dosage of a catecholamine (Score 2) associated with the LOS which required and responded to maximal dosage of the catecholamine and/or mechanical circulatory support (Score 3), and died of LOS (Score 4). Variables with significant relationships to postoperative LOS, regression equation to LOS score and their multiple correlation coefficients (R) of each group were as follows. MVR group: technical trouble (TT), extracorporeal circulation time (ECCT), change of myocardial preservation methods, delta LVSWI/delta LVEDP, diseased duration, aortic cross clamping time, CTR, Y = -1.16 + 1.01 (TT) + 0.05(ECCT) + 0.16(delta LVSWI/delta LVEDP) + 0.02(CTR), R = 0.76. AVR group: LVMMI, ECCT, cardiac failure, NYHA, Y = -0.71 + 0.03(LVMMI) + 0.004 (ECCT) + 0.22(NYHA), R = 0.78. DVR group: delta LVWI/delta LVEDP, LVMMI, NYHA, LV diastolic eccentricity ratio, ECCT, Y = -0.50 + 0.60 (delta LVWI/delta LVEDP) + 0.003(LVMMI) + 1.18(NYHA) + 0.38(delta LVSWI/delta LVEDP) + 0.003(ECCT), R = 0.87. It was demonstrated that preoperative ergometer exercise study during cardiac catheterization was useful in prediction of postoperative outcomes, especially in the MVR (MS) group.

[Significance of measuring 5-fluorouracil incorporated into RNA of tumor tissue as a parameter for the antitumor activity of 5-fluorouracil and its analogs].

A sensitive method for determination of incorporation of non-radiolabeled 5-fluorouracil (FUra) into RNA (F-RNA) of tumor tissue samples was established and was shown to be applicable to clinical materials. RNA fractions containing incorporated FUra were extracted from perchloric acid precipitates of tissue sonicates by KOH hydrolysis. FUra in RNA fractions was released by HCl hydrolysis at 100 degrees C and its levels were determined by GC-MS. The sensitivity of this method was 100 fold higher than that of HPLC method. F-RNA levels were dose dependent and correlated well with antitumor efficacy of the drugs. F-RNA levels in FUra resistant murine tumors were significantly lower than that in parent tumors despite of the lack of difference in FUra concentrations in tumor tissue. Based on these results, it is suggested that measurement of F-RNA levels together with the determination of FUra concentration and the inhibition rate of thymidylate synthase should be considered as a good parameters of the evaluation of antitumor efficacy of FUra and its analogs both in experimental and clinical settings.

[Urogenital cancer].

Treatment of urogenital cancer has made great progress due to early detection by various imaging techniques. Renal cancer is a good example of cancer that has been diagnosed in the earlier stages by the routine imaging methods. BRM therapy of interferons and interleukin II has been proved useful for renal cancer. It has been difficult to diagnose the wall invasion of the bladder cancer. However, ultrasonic imaging has made it possible to detect precisely the invasion into the vesical muscles and adventitia. Also, prostatic acid phosphatase and prostatic specific antigen have become more specific markers for prostatic cancer. Various treatments for this malignancy, including operative surgery, have made progress. The results of chemotherapy for testicular cancer have become fruitful by the use of cisplatin.

Transfer function analysis of positron-emitting tracer imaging system (PETIS) data.

Quantitative analysis of the two-dimensional image data obtained with the positron-emitting tracer imaging system (PETIS) for plant physiology has been carried out using a transfer function analysis method. While a cut leaf base of Chinese chive (Allium tuberosum Rottler) or a cut stem of soybean (Glycine max L.) was immersed in an aqueous solution containing the [18F] F- ion or [13N]NO3- ion, tracer images of the leaf of Chinese chive and the trifoliate of soybean were recorded with PETIS. From the time sequence of images, the tracer transfer function was estimated from which the speed of tracer transport and the fraction moved between specified image positions were deduced.

Transient brain activity used in magnetic resonance imaging to detect functional areas.

Functional areas were detected with short stimuli eliciting transient brain activity using the method of 'transient' regions of interest (ROIs) and functional magnetic resonance imaging (fMRI). This method was validated by comparing the results with sustainedly activated areas identified conventionally. Eighty-eight and 89% of the total areas of transient ROIs derived from 0.2 and 2 s stimulation, respectively, were identified at 5-7 s and 5-9 s, respectively, after stimulus onset. Eighty-eight and 76%, respectively, of these areas overlapped 'conventional' ROIs derived from 20 s stimulation. These results suggest that the delineation of transient ROIs, by targeting a period approximately 7 s after transient neural activity, can be useful for fMRI studies of cognitive functions.

Vogt-Koyanagi-Harada disease in identical twins.

BACKGROUND: Certain aspects in the development of Vogt-Koyanagi-Harada (VKH) disease are documented, and immunogenetic studies have revealed a high relevance of some HLA types. Roles of possible environmental factors in disease onset are not fully understood, however. METHODS: Two cases involving monozygotic twin sisters who were diagnosed as having VKH disease and followed for more than 2 years are presented. RESULTS: Both patients showed the diffuse panuveitis that is characteristic of VKH disease and shared the same HLA typings. However, there was a 16-year time lag between onset of disease in the two patients, and many differences in their lifestyles before disease onset were noted. CONCLUSION: Although certain endogenous factors play an important role in the development of VKH disease, some exogenous factors may also affect the onset of the disease.

[ST segment re-evaluation immediately after successful recanalization in acute myocardial infarction: predictor for poor recovery of left ventricular regional wall motion].

The value of recanalization therapy in restoring risk area after acute coronary occlusion is well documented. However, some cases show poor recovery of left ventricular function regardless of early reperfusion. This study investigated the use of ST segment re-elevation immediately after recanalization in patients with acute myocardial infarction as a predictor of recovery of regional wall motion and risk area. ST segment change and regional wall motion were compared in 16 patients with [ST(+)] and 8 patients without ST segment re-elevation [ST(-)] after successful recanalization within 6 hours from onset. ST segment re-elevation was defined as 0.2 mV or more in at least two contiguous leads immediately after recanalization. Wall motion was measured from single-plane ventriculograms performed in the acute and chronic (3-4 weeks later) phases in the infarct regions by the centerline method. Hypokinesis was defined as more than -2SD/chord(c) below normal and expressed as SD/c for the severity of regional wall motion and chord number(CN) for risk area. Time from symptom onset to recanalization did not differ between the two groups [3.8 +/- 1.2 hours for ST(-), 3.9 +/- 0.9 hours for ST(+), not significant]. In the ST(-) group, regional wall motion improved from -2.92 +/- 0.33 to -1.45 +/- 0.81 SD/c (p = 0.0005) and risk area decreased from 29.8 +/- 16.6 to 9.5 +/- 15.7 CN (p = 0.005) in the acute and chronic phases, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

[Determination of serotonin in whole blood and platelet rich plasma by high-performance liquid chromatography with fluorometric detection].

An improved high-performance liquid chromatographic method with fluorometric detection (Ex 285nm, Em 345nm) for the assay of serotonin in whole blood and platelet rich plasma was developed for the routine clinical examination. Clinical sample was successfully deproteinized with perchloric acid in the presence of a reducing agent (ascorbic acid). Precision (CV < 5.0%), accuracy (recovery > 95%), lower assay limit (20ng/ml) and assay time (15min/sample) were allowable for the routine clinical examination. The level of serotonin (ng/ml, mean +/- SD) in whole blood and platelet rich plasma from normal volunteers (n = 53) was 143 +/- 47 and 180 +/- 60, respectively.

Ultrastructural localization of calcium around the membrane of the surface connected system in the human platelet.

The localization of calcium in the membrane system of human platelets was determined by ultrahistochemical methods equipped with an electron probe x-ray microanalyzer. After potassium oxalate-glutaraldehyde treatment large amounts of electron opaque precipitates were observed around the membrane of the surface connected system. Electron probe x-ray microanalysis clearly defined that the precipitates were composed of calcium oxalate. The localization of calcium on the membrane of the surface connected system was also confirmed even after treatment of the platelets with potassium antimonate-OsO4. These results support a model which depicts the surface connected membrane system taking part in the store and the transport of calcium.
