Suppurative pylephlebitis and pylethrombosis: the role of anticoagulation.

Suppurative thrombophlebitis of the portal vein resulting from inflammatory intra-abdominal conditions is a rare complication that may result in pylethrombosis and portal hypertension. A case is presented with documented pylethrombosis caused by diverticulitis. Color flow Doppler scanning was used to establish the diagnosis. Systemic anticoagulation therapy was added to the antibiotic regimen because of postoperative propagation of the clot. Anticoagulation therapy prompted resolution of the episode. Long-term follow-up studies demonstrated recanalization of the portal vein. Anticoagulation should be instituted with documented acute pylethrombosis caused by inflammatory disease of the abdomen.

Bacteriology of chronic suppurative otitis media: ofloxacin susceptibility.

Fifty-four patients with active chronic suppurative otitis media (CSOM) were prospectively studied for the bacteriology and their in vitro antibiotic susceptibility to ofloxacin. Thirty-nine patients (72%) had positive cultures. The commonest organism was Pseudomonas sp., which was found in 18 (33%) patients. The second commonest organism was Staphylococcus aureus, which was found in 15 (28%) patients. Bacteroides sp. was found only in 3 (6%) patients, and all were in association with the aerobes. Of all 42 aerobes found, 35 (83%) were susceptible to ofloxacin. The in vitro susceptibility results indicate that ofloxacin can be an effective antibiotic in the treatment of active CSOM.

[Organ-preserving surgeries and nephrectomy in pregnant women with severe forms of acute suppurative destructive pyelonephritis].

The results of surgical treatment of pyodestructive pyelonephritis in 105 (4.2%) pregnant women and puerperae out of 2506 ones treated in the urology department from 1986 to 1994 are analyzed. Uni- and bilateral apostematous nephritis, carbuncle and abscess of the kidney were diagnosed in 89 and 16 patients, respectively. Nephrostomy was the principal method of treatment of focal suppurative forms of acute pyelonephritis. Bilateral nephrostomy was carried out in 12 pregnant patients, in 3 of them secondary nephrectomy was performed. Nephrectomy is indicated in total destruction of the kidneys, it was made in 37 (35.2%) pregnant women and puerperae. Introduction of radical surgery into practical treatment of neglected forms of pyodestructive pyelonephritis led to almost total liquidation of postoperative mortality in pregnant women. The efficacy of treatment of pregnant women is provided by varying in extension surgical interventions on the kidneys. Organ-sparing and radical operations in grave purulent pyelonephritis were effective in 97.2% of pregnant patients.

Pathological profile in chronic suppurative otitis media--the regional experience.

Peroperative findings in 145 consecutive cases of chronic suppurative otitis media, operated at Civil Hospital, Karachi were recorded. The mean age was 24 years. More than half of the patients (51%) had subtotal perforations and majority had damage of more than one ossicle. Involvement of all the three ossicles was seen in 30 (40%) cases. The frequency of damaged malleus along with incus was higher than other series. Granulations and subtotal perforations were commoner and could account for it. Extensive cholesteatoma was present in 30 cases.

[Role of immune disorders in the pathogenesis of suppurative inflammatory diseases of the adnexa uteri].

Clinical and immunological studies were carried out in 60 patients with tubal pyoinflammatory diseases and in 20 healthy controls of a reproductive age. The immunity status was studied using IKO monoclonal antibodies to differentiation antigens of peripheral blood mononuclears. An acute purulent process was found to be characterized by an adaptive increase in the count of mononuclears carrying the markers of B lymphocytes (CD22, IgM mu-chain), of activated cells (CD38, HLA-D(r)), and of the total number of positive correlations between immunocompetent cells. A persistent course of tubal inflammation is connected with hyperproduction of antibodies and immune complexes, increased expression of HLA-Dr antigens in combination with congenital or acquired insufficiency of T-helper/inductors and suppressors/cytotoxic T-cells. An important factor in the development of a chronic suppurative process is triggering of immunocomplex mechanisms associated with fixation of IgG, IgM, and the complement in the walls of small vessels in the uterine tubes, with a high level of circulating immune complexes, and appearance of plasma cells containing IgG and IgM in the tissues of uterine tubes.

[Acute suppurative parotitis caused by Pseudomonas aeruginosa].

Acute suppurative parotitis is a rare infection which usually occurs in hospitalized patients. Risk factors are dehydration, old age, the postoperative state and immunocompromised states. We describe a 66-year-old woman with acute myeloid leukemia who developed this condition. Pseudomonas aeruginosa was cultured from the blood and the orifice of Stensen's duct. Recovery followed a combination of antistaphylococcal medication with clindamycin, ofloxacin and gentamicin. We stress the need to recognize Gram-negative bacteria as possible pathogens in this condition.

[Use of non-medicamentous methods of treatment with the aim to improve the immune status of patients with suppurative diseases and malignant neoformations of the lungs].

A test group of 40 patients and a control group of 20 matched controls with pulmonary cancer and suppuration were exposed to preoperative and postoperative electromyostimulation (EMS) with adjuvant intravascular laser radiation of the blood (ILRB) to prevent thromboembolism versus conventional chemotherapy, respectively. At the preoperative examination all the patients exhibited secondary immunodeficiency. It was found that introduction of EMS and ILRB corrects immunity and activates fibrinolysis. A positive trend in coagulation is also attributed to immunomodulating effects of EMS and ILRB.

Tympanomastoidectomy for chronic suppurative otitis media of irradiated ears of nasopharyngeal carcinoma patients.

Chronic suppurative otitis media (CSOM) and osteoradionecrosis of the temporal bone are common sequelae of nasopharyngeal carcinoma (NPC) after radiotherapy. Tympanomastoidectomies were carried out for CSOM in 16 patients who had completed radiotherapy for NPC. Eleven (69%) patients had successful tympanoplasty; in eight of them, the otorrhoea was controlled, and the other three patients had persistent otorrhoea due to exposed osteoradionecrotic bone in the external ear canal. Localized osteoradionecrosis of the external ear canal is often associated with CSOM in NPC patients. Adequate removal of necrotic bone is essential in addition to successful tympanoplasty to eliminate otorrhoea. Therefore, we recommended tympanomastoidectomy for NPC patients with active CSOM who fail to respond to conservative treatment.

Treatment of chronic suppurative otitis media.

Chronic suppurative otitis media is a common disease process in children with tympanostomy tubes or tympanic membrane perforations. Although the underlying etiology is still unclear, the most commonly isolated organism is Pseudomonas aeruginosa. Management may be medical, surgical, or both, and depends on the patient's age, associated conditions, and clinical course.

[Pathological changes of the pyramidal eminence in chronic suppurative otitis media].

Temporal bones of patients with chronic suppurative otitis media were used for evaluation of possibility of penetration of the infection from the cavum tympani into the facial nerve via the pyramidal eminence. The infection can spread to the facial nerve in two ways: via the natural opening on the pyramidal eminence through which tendon of the musculus stapedius is passing, without bone lesions of the eminence and via the sites of bone destruction on the eminence.

[Acute suppurative thyroiditis].

A woman aged 54 years with acute suppurative thyroiditis is described. The patient had a well-known non-toxic diffuse goitre with a cyst in the left lobe. She was treated with levothyroxine (Eltroxin) for three years before actual hospitalization. Four weeks previously she had been treated with sulphamethizole for a bladder infection. Antibiotic treatment, surgical drainage and, finally, lobectomy of the left thyroid lobe cured the patient, including regression of paraesthesia in the left arm. E. coli was found in urine, blood and pus bonum from the abscess.

[Effect of some drugs on the intensity of lipid peroxidation in surrounding tissues in the local treatment of suppuration].

The effect of some drugs in their local use for treatment of purulent wounds on the intensity of peroxide oxidation of lipids in the perifocal tissues of an experimental suppurative focus was studied. The increased antioxidant and antiinflammatory effect in use of lyposomal form of gentamycin with the ectericide-modified membranes versus ectericide solution as well as more pronounced antioxidant and antiinflammatory effect of ferecterisole vs. cimesole was revealed. The results obtained indicate that the use of ferecterisole and lyposomal forms of some antibiotics for local treatment of purulent wounds has good prospects.

Cochlear implants in chronic suppurative otitis media.

Chronic suppurative otitis media (CSOM) in profoundly deaf patients is a contraindication for cochlear implantation. Eight (6%) of the 126 patients referred to cochlear implantation at this center between 1986 and 1992 became deafened as a result of bilateral CSOM but were otherwise suitable candidates. This study details the methods used in four patients to prepare the septic ear for a sterile device. Two patients had wet radical cavities with residual cholesteatoma, and two had discharging safe perforations resistant to surgical repair. Obliteration of the middle ear cleft with blind pit closure of the ear canal was attempted in all four patients, and cochlear implants were installed at a second operation 3 to 6 months later. The hearing results were as good as in implanted patients without CSOM, and the only complication has been the finding of a cholesteatoma pearl at the second operation in one patient. Fat obliteration of the mastoid and middle ear with blind pit closure of the ear canal can be adapted to make most chronic ears fit for implantation, if the patient is prepared to undergo two operations.

[Suppurative disease of the ano-rectal tract].

Suppuration of the anal-rectal tract usually requires nothing more than ordinary surgical management. In many cases, however, particularly when the fistula lies entirely above the anal sphincter, the picture is complicated, with recurrences and exacerbations. In addition, incontinence following unsuitable surgery may constitute a possible risk. Statistical assessment of a personal series is primarily directed to analysis of factors leading to chronic evolution of the disease and its complications. Eclectic management founded on the surgeon's experience, but nevertheless based on well-defined techniques, will be found to offer the best results.

[Primary suppurated splenic cysts. A case report].

The authors describe a case of an epidermoidal cyst of the spleen, which they were able to observe in an emergency case, due to suppuration of the cyst. Primary cysts of the spleen represent a rare dysontegenetic pathological condition which is, for this reason, often underestimated. The general clinical summary is related here, with particular reference to supporative complications, which cause problema of differential diagnosis with patients suffering from abscess of the spleen. The diagnosis can be made as a result of standard X-ray procedures and computed tomography and above all, following a simple scan. The treatment recommended in this case is an open splenectomy, in order to check the allarming clinical symptoms of the patient, which can deteriorate. The use of laparoscopical techniques or trans-cutaneous draining is reserved for salected cases.
