An attempt was made, by means of test strips for leucocytes, eruthrocytes, protein, ketone bodies, nitrate and ascorbic acid to distinguish urine samples with abnormal sediments. The urinary sediment was, at the same time, quantified with the Kova system. It was demonstrated that the teststrip system can separate out nearly all samples containing significant amounts of sediment. In addition, lysed erythrocytes and leucocytes are detected. It is, therefore, suggested to restrict microscopic examination to those samples which have been positive to at least one test strip. It is important that genital cleansing is undertaken before micturition.

With the isolation of the human immunodeficiency virus (HIV), numerous assays have been developed to detect antibodies to the retrovirus. The manufacturers' recommend that all sera exhibiting a positive enzyme immunoassay be confirmed by the more specific Western blot test (WBT). Recently, a new WBT strip was released for detection of antibodies to HIV. Four hundred patients' sera and 22 CDC proficiency samples were each tested with these strips and with a WBT strip prepared within the laboratory using the CDC techniques. The strips were then evaluated for banding pattern with the reactions recorded on a scale of 0 to 4+ for each band consistent with HIV response. Criteria for positive results were based on the ASTPHLD/CDC criteria. Primary emphasis was on antibody banding to gp41 (envelope protein) and p24 (core protein). The new strips showed appropriate changes in level of antibody, especially for declining antibody to p24. When the banding pattern for antibody to gp41 was compared to the standard strips, the new strips showed a sensitivity of 99.6% and specificity of 99.4%. The presence or absence of antibody to p24 on the new strips versus the standard strips showed a sensitivity of 93.7% and specificity of 99.5%.
More and more frequently, diabetics use portable blood glucose meters to assess their own glycemic control. New meters are constantly being introduced commercially. We evaluated three recently marketed meter/strip systems in terms of accuracy, precision, and the stability of the colors on the test strips. We compared the results from the meters with those from a Beckman Glucose Analyser and calculated the correlation and regression coefficients (Diascan, r = 0.93, y = 0.64x + 41.76; Glucometer II, r = 0.94, y = 0.86x + 2.32; and Reflolux II, r = 0.99, y = 0.99x + 5.15). Both the Diascan and Glucometer II meters tended to underestimate blood glucose whereas Reflolux II gave unbiased measurements throughout the clinical range. Reflolux II was also the most precise of the three. Furthermore, the Reflolux test strips kept their color over 6 days of storage at 4 degrees C, while strips from the other two systems faded. All three meters are compact, simple to calibrate, and convenient. They remain, however, relatively expensive, as do the test strips.
PURPOSE: To compare the repeatability of fluorescein break-up time (FBUT) measurements determined with either a standard fluorescein strip or the Dry Eye Test (DET) modified fluorescein strip methods. METHODS: This was a prospective, randomized contralateral study of 100 patients, in which FBUT measurements were determined with a standard FUL-GLO fluorescein strip (Akorn, Inc., Buffalo Grove, IL, U.S.A.) in one eye and a DET strip (Akorn, Inc., Buffalo Grove, IL, U.S.A.) for the contralateral eye. Three consecutive measurements were made immediately after fluorescein instillation. The second eye was evaluated 1 minute after completion of the first eye. Data from patients with FBUT values less than 20 seconds were included in the data analysis, because measurements greater than 20 seconds are not diagnostically significant. RESULTS: Seventy-five patients met enrollment and FBUT measurement criteria. For three consecutive FBUT measurements, the DET values were within 3 seconds for 72 of the 75 patients (96%). Eighty percent of patients reported no sensation with the DET strip, 20% reported mild sensation, and no patient reported moderate sensation. With FUL-GLO strips, measurements were within 3 seconds for 53 of the 75 patients (71%) (p <0.005). The standard fluorescein strip method elicited reports of no sensation from 31% of patients, mild sensation from 60%, and moderate sensation from 9% (p <0.001). CONCLUSIONS: The DET strip provides a significant reduction in sensation upon application, improved single measurement reliability, and enhanced measurement precision, compared with a conventional fluorescein strip.
The Boehringer Mannheim test (BM test) for cystic fibrosis has been applied to 15 734 neonates. A positive result occurred in 130, seven of whom were subsequently shown to have the disease. To our knowledge no cases have been missed. Tests have been performed centrally by experienced laboratory staff and the false-positive rate of 0.83% is considered acceptable. It is important to follow up weak as well as strongly positive reactions to the BM test strip.
