Congenital radio-ulnar synostosis: surgical treatment.

The results of an operative approach to the problem of radio-ulnar synostosis were assessed in thirteen patients, ten to twenty-five and one-half years after the procedure was performed. We concluded that in a patient with bilateral synostosis one hand, the one not used in writing, should be shifted to a position of 20 to 35 degrees of supination. With one hand in this position, the other may be left in considerable pronation. Often after such a shift it is not necessary to rotate the second arm. However, if the pronation is marked in the second forearm, and if function is impaired unduly by this position, surgical correction is indicated. The arm should be placed in a position of 30 to 45 degrees of pronation. In unilateral radio-unlar synostosis, the ordinarily ideal position of the radius is between 10 and 20 degrees of supination. In an adult, the patient's occupation should be considered in deciding on the rotatory positions of the forearms. We usually prefer a method of transverse osteotomy through the conjoined mass of the radius and ulna. Careful observation of the effect on the vascular status of the limb during and immediately after surgery is important.

Quantitation of radio-labelled vitamin K1 and vitamin K1 epoxide in plasma by high pressure liquid chromatography.

A convenient, accurate and reproducible high pressure liquid chromatographic method for the quantitation of radio-labelled vitamin K1 and vitamin K1 epoxide in plasma is described. The method involves the determination of total ether extractable radioactivity, and a chromatographic separation to determin the relative quantities of radio-labelled vitamin K1 and vitamin K1 epoxide. The method is useful over a wide range of ratios of the two compounds, and has a coeffcient of variation of approximately 5%.

[Radiation resistance: a concept in need of review or a stubborn problem for radio-oncology?].

Many tumors are considered to be radio-resistant. Reviewing recent literature on clinical results, this notion of radio resistance needs to be revised. With new technics and improved dosimetry it has become possible to control locally even well differentiated carcinomas and achieve clear survival benefits. Three examples illustrate the importance of radiation therapy of prostatic cancer as a well established procedure. It would seem that radiation therapy in colorectal tumors of certain stages is becoming part of initial treatment. A case is made out for meningeoma, where surgery undoubtedly remains the first approach; after incomplete removal or in locally inoperable patients, radiation therapy may be of value.

Sensitive direct radio-immunoassay for human renin.

A sensitive direct human renin radio-immunoassay has been developed for clinical use. The antigen source was human renal renin purified from Haas' preparation by pepstatin-C6-sepharose affinity chromatography, this was used to prepare a specific human renin antibody. The radio-immunoassay was performed by the double antibody technique using the delayed tracer addition method. Standard curves were obtained over the range 0.2-8.0 ng/ml. Dilution curves of human renal renin and human plasma were superimposable on the standard curve. Both active and inactive renin were detected by this method, and measurements correlated well with total renin activity after trypsin activation. Intra- and inter-assay coefficients of variance were 4.6% and 5.1%, respectively. Renin concentration was higher in patients with renovascular hypertension (1.97 +/- 0.38 ng/ml, mean +/- s.d., n = 10, P less than 0.01), but lower in primary aldosteronism (0.66 +/- 0.16 ng/ml, n = 13, P less than 0.01) compared with essential hypertension (1.38 +/- 0.34 ng/ml, n = 12). This method provides a new tool for the investigation of the renin-angiotensin system in man.

Computed tomographic diagnosis of distal radio-ulnar joint disruption.

Distal radio-ulnar joint dislocation may be a difficult diagnosis using standard wrist radiographs. We report a patient in whom standard films did not convincingly demonstrate a dislocation which was well seen with computed tomographic scans of the wrist. Computed tomography may be the method of choice for demonstrating distal radio-ulnar joint dislocation when this is suspected clinically but not seen with standard films.

[Quantitative analysis of bone tissue. Correlation of several radio-morphometric indices with gamma-absorptiometric mineralometry].

According to results of experimental study on 70 women, either normal or osteoporotic, the authors come to a critical valuation of known radio-morphometric index concerning the measure of amount of bone and propose the cortical planimetric percent area of the second right hand metacarpal as original radio-morphometric index, greatly correlated (r = 0,879) to gamma-absorptsiometric mineralometry.

[Glutathione--an essential component of radio-modifying action of serotonin].

5-HT applied prior to, immediately or 5 min after irradiation decreased equally the radiation damage to Ehrlich ascite tumor cells. A decrease in the endogenous glutathione content by N-ethylmaleimide (NEM) led to reduced the 5-HT radio-modifying effect. Its radio-modifying effect decreased irrespective of whether NEM was applied prior to or after irradiation.

[Therapeutic ratio in combined radio-chemotherapy--radiobiological considerations].

Although much effort has been directed for obtaining a more effective way of combination in combined radio-chemotherapy, radiobiological rationales for improving the therapeutic ratio have not yet been established. Biological characteristics of solid tumors, target normal tissues in acute and chronic injuries, and also their implications in radio-chemotherapy were discussed in this review. These include cellular kinetics, pH gradient and 4 R's during the treatment course. It was considered that experimental model systems in vitro suitable for solid tumors and slowly proliferating normal tissues are required for obtaining rationales to improve therapeutic ratio.

[Radiolysis of acrinol in radio-sterilization of acrinol pharmaceuticals].

Acrinol in dry solid state after gamma irradiation with 80 kGy (8 Mrad) undergoes 8% decomposition, with G(-M)=30, and in wet solid state (10% moisture content) undergoes 15% decomposition, with G(-M)=56, as shown by absorbed spectrometric measurement. From these results, it is estimated that the irradiation dose of 25 kGy (2.5 Mrad) prefered by many countries as sterilization dose will achieve the radiolysis rate of 2.8% or 4.7% for dry solid state or wet solid state acrinol, respectively. The stickiness power of plaster tape made of raw rubber and polyterpene resin don't decrease with irradiation dose of less than 40 kGy (4 Mrad). Then, the radio-sterilization on the dose of 25 kGy (2.5 Mrad) may be applied to solid pharmaceuticals such as commercial rubber adhesive plaster with acrinol pad. Besides, acrinol in 0.1% aqueous solution after irradiation of 10 kGy (1 Mrad) at room temperature undergoes 20% decomposition, with G(-M)=0.5. The radiolysis rate in this state is estimated 45% with the dose of 25 kGy (2.5 Mrad). Then, the radio-sterilization with this dose must not be applied to liquid pharmaceuticals such as 0.1% acrinol aqueous solution. While, it was reported by Hosobuchi and Sato that the antimicrobial effect of irradiated acrinol to Staphlococcus aureus increased with irradiation dose. Then, it is expectable that some materials with antimicrobial activity are prodused by gamma irradiation.

[Justification of the indications for radio-frequency electrostimulation of the atria and the results of its use in paroxysmal supraventricular tachycardias].

Re-entry tachycardia was found in 182 patients of a total of 275 subjected to electrophysiological investigation of heart function; in 26 of those, a radio-frequency atrial electrostimulation system was implanted. Early postoperative complications were noted in 3 patients: subcutaneous hematoma in 2, and skin decubitus in 1. Long-term follow-up (as long as 7 years) confirmed the efficiency of atrial radio-frequency pacing.

Nonspecific inhibition of in vivo tumor growth by allo-sensitized lymphocytes - requirement of radio-sensitive T-lymphocytes.

In previous papers we demonstrated with the Winn assay the WKA rat spleen cells sensitized to allogeneic tumor (AH-66 from Donryu strain) inhibited in vivo tumor growth of a syngeneic tumor (KMT-17) when early spleen cells (taken one week after the last immunization) were used. For the tumor inhibition by late spleen cells (2 weeks after the last immunization), further addition of inactivated homologous allogeneic cells was required. In the present study we examined with the Winn assay the nature of allo-sensitized spleen cells which nonspecifically inhibit in vivo growth of syngeneic tumors. Tumor inhibitory activity of early spleen cells was decreased by anti-rat T serum treatment. The same was true in tumor inhibition by late spleen cells stimulated with mitomycin-C treated AH-66 cells. Both of early and late spleen cells responsible for tumor inhibition are radio-sensitive, whereas KMT-17 sensitized T cells which specifically inhibited growth of homologous tumor cells belong to radio-resistant subpopulations.

[Radio-immunological testing of progesterone for diagnosis of corpus luteum insufficiency (author's transl)].

Radio-immunological assay was used to determine progesterone concentrations in serum during the luteal phase in 34 patients with treated and untreated corpus luteum insufficiency. Twenty patients with normal menstruation cycles were used as controls. Progesterone levels were found to be in agreement with the histological pattern of the endometrium. Progesterone values obtained from serial radio-immunological assay thus are high-accuracy criteria for diagnosis and therapeutic monitoring of corpus luteum insufficiency.

[Treatment results in radio-iodine therapy of the autonomous adenoma of the thyroid gland (author's transl)].

In order to control the results of radio-iodine therapy, the autonomous adenomas treated in our clinic from 1961 to 1975 with radioactive iodine on 105 patients have been subjected to follow-up examinations and compared with the results obtained by other authors, especially Swoboda et al. (23), who treated 185 patients with corresponding follow-ups. In both groups, efforts were aimed at achieving successful treatment via high single-dose radio-iodine therapy. The article discusses the indication, method, results and posttherapeutic care involved. Both the examined groups demonstrate good treatment results (in 95 or 98%, respectively, of our cases and of the cases treated by Swoboda et al., the autonomous adenoma was eliminated). This proves that high single-dose therapy with radioactive iodine (I131) is a successful method of treating an autonomous adenoma, thus representing a therapeutic alternative to surgery.

[Serum gastrin: interests and limitations of radio-immunoassay (author's transl)].

Radio-immunoassay of serum gastrin may now be carried out in all laboratories of radio-immunology. Comparison of two commercial kits A: Schwartz-Mann and B: CEA-SORIN according to criteria of specificity, sensitivity and reproducibility within and between systems, shows that they both permit the detection of pathological hypergastrinemias (Zollinger Ellison and atropic gastritis). Both kits have an identical intra-system coefficient of variation (10 p. cent and 7 p. cent for A, 5 p. cent and 8 p. cent for B) on the other hand, the inter-system coefficient of variation is better for kit B (22,4 p. cent and 37 p. cent for kit A, and 11,5 p. cent and 14,2 p. cent for kit B). The normal values for each kit are quite different: 97 + 64 pg.ml-1 for A and 51 . 23 pg.ml-1 for B preventing one from comparing estimations carried out with two different kits.

Detection of reverse transcriptase in culture medium for mammary tumour cell lines: a comparison of an established radio-labelling technique and a contemporary non-isotopic technique.

Classically, radio-label techniques have been employed to analyse biological samples for reverse transcriptase (RT) activity. More recently, however, non-isotopic kits have been developed for retroviral quantification. Nevertheless, until the present investigation it has not been known if these contemporary methods are more sensitive at detecting reverse transcriptase activity. In our study, a non-isotopic ELISA method was shown to be considerably more sensitive than the radio-label technique at detecting reverse transcriptase in growth medium used to culture the murine breast cancer cell line GR/A. Using the ELISA, less reverse transcriptase activity was demonstrated in growth medium from human mammary adenocarcinoma MCF-7 cells than the murine source. This ELISA did not detect reverse transcriptase activity from a pure source of Moloney murine leukaemia virus. In light of this, the broad applicability of this ELISA for reverse transcriptase from different viral sources must be investigated before it can be used to monitor biological supernatants for the presence of retroviruses.

[IgE and the significance of the radio-allergo-sorbent-test (RAST)].

In 1966 a new immunoglobulin was found in persons with allergies and in non-typical myeloma proteins. Normally this immunoglobulin E is present only in nanogramms and rests with predilection on the membrane of mast-cells. There is a reaginic-anaphylactic reaction after re-exposure of antigens to the antigen-antibody reaction followed by denudation of mediators of the anaphylactic reaction. With the Radio-Immuno-Sorbent-Test (RIST) the IgE can be quantitatively determined. Elevated IgE-blood levels are typically found in atopic eczema. With the Radio-Allergo-Sorbent-Test (Rast) the allergen specific IgE can be defined. A conformity with appropriate patchtests can be achieved in 60-80% of the cases. In this review advantoses and problem of RIST- and RAST-diagnoses are described. RAST represents a valuable aid in diagnosis of allergies beeing not burdensome and risky, as it is easy to perform and bears no risk to the patients. At the present time, however, patch tests are necessary in the diagnosis of allergies.

Hydrogel embolic agents. Theory and practice of adding radio-opacity.

RATIONALE AND OBJECTIVES: Spheres of hydrogel have been developed as embolic material with the ability to incorporate radio-opaque materials. To optimize particle design for radiographic or fluoroscopic visualization, we have examined the theoretical determinants of particle contrast. In addition, loaded hydrogel particles were tested in a rabbit model. METHODS: Computer simulations of particle subject contrast were examined regarding particle composition, particle size, patient thickness, and x-ray beam kilovoltage. Embolizations in the rabbit kidney were used to test the practical aspects of the materials. RESULTS: Tantalum and tungsten offer some theoretical and practical advantages over other materials. With this particular hydrogel preparation, contrast material loading was limited to 20% of the volume as loaded contrast agent. The soft particles passed through catheters as small as 3 French; they were usually injected as a suspension of saline/contrast material. Tantalum/hydrogel particles as large as 2 mm could be forced through the 140 cm/3-Fr catheter with a guide wire. CONCLUSIONS: Radio-opacity of embolic material should add an element of control in embolization procedures that is lacking with the current agents. The heavy metals, tungsten and tantalum, are suitable additives for radiopaque material for hydrogel emboli. The input relationship appears predictable with computer monitoring techniques. Initial results in a study of these radiopaque particles are very encouraging. Further studies are underway to evaluate the long-term effects in renal and hepatic circulations.

[Non-hysteroscopic thermodestruction of the endometrium using radio waves in the treatment of menorrhagia].

OBJECTIVE: To test non-hysteroscopic thermodestruction of the endometrium for safety and efficiency. DESIGN: Prospective pilot study from February 1991 to July 1992. SETTING: University Hospital Nijmegen. PATIENTS AND METHOD: There were 68 patients aged 30-55 years with menorrhagia, who did not want children but wished to retain the uterus. There were two patients with a subserous myoma but with a normal uterine cavity. Patients with hypergonadotropism, a markedly enlarged uterus, abnormalities at cytological examination of the cervix, adnexal lesions, prolapse, intrauterine lesions and coagulation disorders were excluded. By way of preparation, danazol was administered for 4 weeks before the intervention (36 patients). Three patients with side effects were given lynestrenol. From January 1992, an LH-RH analogue was administered (29 patients) for 6 weeks prior to the intervention or in the patients with a subserous myoma for 12 weeks. Thermodestruction of the endometrium by means of radio waves was carried out according to a standard protocol under general or epidural spinal anaesthesia. The intracavitary temperature measured was 62-65 degrees C, mean duration of the treatment 20 minutes. RESULTS: The patients were discharged 10-24 hours after the intervention. A non-disturbing watery discharge of 3-6 weeks' duration was reported. Normal activities were resumed after one week. Success (duration of follow-up 3-21 months) was defined as amenorrhoea (6 patients; 9%) or a markedly reduced menstruation or duration of menstruation (48 patients; 70%). Six patients (9%) reported no improvement and of eight patients (12%) with slight, unsatisfactory improvement three were treated again, with success (4%). During the trial period the method was further adjusted and perfected. There were no complications. CONCLUSION: This preliminary experience shows that thermodestruction with radio waves is simple, safe and efficient.
