[A case of renal cell carcinoma metastasizing to the contralateral kidney and renal pelvis].

Extremely rarely renal cell carcinoma metastasizes to the contralateral renal pelvis or ureter. A 42-year-old man had undergone left radical nephrectomy for renal cell carcinoma (pT1b, grade 2) in March, 2000. Fifteen months later, he complained of macroscopic hematuria. Computed tomographic scanning and retrograde pyelography showed a right renal pelvic tumor. Enucleation of pelvic tumor was performed and a parenchyma mass incidentally identified in the right kidney was also resected. Histopathological examination of each tumor revealed renal cell carcinoma identical to the primary tumors in the left kidney suggesting metastasis to renal pelvis and de novo tumor or metastasis in the right kidney.

[The treatment of nonorganic neoplasms of the lesser pelvis].

In 1967-1989, 330 patients with non-visceral tumors of the small pelvis (children--110 and adults--220) were registered. One hundred and five patients, 17 children included, presented with benign tumors whereas 225 (children--93 and adults--132)--with cancer. Absolute and relative, local and general contraindications to surgery were outlined. The most important criteria are the extent of tumor, patient's status, equipment and surgical skills. Tumors were approached through 9 different accesses depending on their size and site. Surgery wat performed in 242 patients, viz. 89.5% of patients with benign tumors and 65.7% of those with cancer. The percentage of operated children (37.6%) was much lower than that of adults (85.6%) since the former presented with more advanced tumors. Radical surgery was performed in 192 (79.3%) patients; benign tumors were removed in 86.7% whereas cancer--in 44.9%. Complications were encountered in 56.2% of cases, bleeding being the most frequent one (47.0%). Lethality following radical surgery was 3.1%. Radiation and chemotherapy alone proved ineffective. Complex treatment assured better clinical course. One-ten-year survival rate in adults with malignant tumors was 51.2%-31.8%. As few as 17.2% of children survived one year whereas only 6.5%-2 years which points to more aggressive clinical course of malignant non-visceral tumors of the small pelvis in children.

Temperature gradients at various points within the human pelvis as measured during laparoscopy.

Temperatures have been measured during pelvic laparoscopy, under general anesthesia, in 59 women. Temperatures on the serosal surface of the rectum, the mesovarium, the ovarian arteries and the surface of the external iliac artery have been compared with the temperature deep in the rectal cavity on the rectal mucosa. The mean temperatures recorded at all sites within the pelvic cavity were lower than the mean temperatures recorded on the rectal mucosa. The mean differences between rectal mucosa and the other pelvic sites are: rectal mucosa -0.20 +/- standard error of the mean (SEM) 0.03C, mesovarium -0.28 +/-SEM 0.04C, ovarian artery -0.35 +/- SEM 0.05C and external iliac artery -0.44 +/- SEM 0.06C. All differences are highly significant from zero and do not appear to be related to the time from the onset of anesthesia, the temperature of the gas used to fill the abdominal cavity, the time from the last menustrual period and any pathology present in the pelvis. The results are consistent with a previous suggestion, by Grayson,2 that the human rectal mucosa may be an organ of heat production and that arterial blood within the pelvis may be precooled by counter current heat exchange with veins draining cold blood from the extremities.

[The diagnosis and treatment of chronic venous insufficiency of the pelvis of non-occlusive genesis].

The clinical, ultrasonic duplex scanning and phlebographic (retrograde pelvis renoovaricography) examinations of 89 patients with chronic venous insufficiency of the pelvis were analyzed. Two forms of the clinical course of the disease were established: parietal and visceral. A detailed clinical, ultrasonic and phlebographic characteristics of these forms are described. The authors have developed two original methods of sclerotherapy: 1. varicose dilated superficial veins in the zones of atypical localization without using the elastic compression; 2. left ovarian vein with its following pharmacological spasm. The effectiveness of the sclerozing treatment was 86.6% with the first method and 100%--with the second method.

[Squamous cell carcinoma of the renal pelvis with giant hydronephrosis].

We report a case of squamous cell carcinoma of renal pelvis associated with giant hydronephrosis. A 71-year-old woman presented to our hospital with a complaint of abdominal fullness due to the right giant hydronephrosis. Although the diagnosis of her hydronephrosis was made about 20 years ago at another hospital, it had been left untreated. Computed tomography showed the right hydronephrosis of 20 x 20 x 25 cm in diameter and no evidence of tumor or calculus in the right urinary tract. For relief of her complaint, right nephrectomy was performed. The fluid content was bloody and 4,200 ml in volume. Histological examination revealed a flat type squamous cell carcinoma of the renal pelvis. This is the 30th case of renal pelvic malignant tumor associated with giant hydronephrosis reported in Japan. The literature was reviewed and the management of giant hydronephrosis was discussed.

Digital angiography using hand-operated table movement for vascular disease of the pelvis and lower extremities.

We developed a new technique using hand-operated table movement during digital angiography (DA) for the study of vascular disease of the pelvis and lower extremities. Digital video memory (DVM) is used in this technique to digitize, record, and display fluorographic images without subtraction in real time. This allows free table movement and the imaging of a large area with a single injection of contrast medium. Forty-five examinations for vascular disease of the pelvis and lower extremities were performed by this DA technique. Good images of an extensive area from the lower abdomen to the lower leg were obtained in a single study.

[Sclerotherapy of small pelvis varicosis].

This paper analyses the results of sclerotherapy provided to 89 patients with small pelvis varicosis. Selection of the treatment technology depended on the clinical variant of disease. In patients with damage to the parietal venous system of the pelvis, the treatment was realized by multiple repeat injections of a sclerosing agent to the pelvic veins in a minimal dose which may induce only proliferative processes in varicose vein walls. Elastic compression was not applied. Women with the syndrome of blood overfilling of the pelvic organs (visceral pattern of lesion) were provided sclerotherapy of the left ovarian vein followed by its pharmacological spasm. The treatment results were evaluated by clinical examination of patients, duplex scanning end estimation of the quality of life based on the patients' self-estimation according to the recommendations of the International Association of Phlebologists. Excellent results were obtained in 32.6%, good in 46.1%, satisfactory in 19.1% and unsatisfactory results in 2.2% of patients.

Recurrent carcinosarcoma of the renal pelvis. Case report.

Carcinosarcoma is a malignancy that occurs very rarely in the renal pelvis; thus, only a very limited number of cases has been documented. These tumors are composed of both carcinomatous and sarcomatous elements. We report here a case of a carcinosarcoma composed of well-differentiated, keratinizing, squamous carcinoma cells and high-grade sarcoma cells. Differently from the cases described so far, this carcinosarcoma of the renal pelvis was characterized by the quick recurrence, following complete surgical resection, as a highly disseminated high-grade sarcoma.

3D reconstruction of the pelvis using the NSCP technique.

Many authors have already pointed out the importance of the three dimensional aspect when dealing with pelvic and spinal pathologies. The purpose of the present study is to verify the feasibility on the pelvis and the accuracy of a recent 3D reconstruction technique based on biplanar X Rays with regard to direct measurements. The results on 8 dry non-pathologic pelvises show a mean error of 3.9 mm for the global geometry of the pelvis, with local maxima of 26 mm and 95% of errors inferior to 14.5 mm.

An unusual cause of pelvi-ureteric junction obstruction.

Congenital lumbar hernia is rare in the pediatric age group. This report describes an infant with a superior lumbar hernia with herniation of the kidney that resulted in pelvi-ureteric junction obstruction. Surgical repair of the hernia and reduction of the herniated kidney resulted in resolution of the pelvi-ureteric junction obstruction. The literature on the subject is also reviewed.

Data mining of the E-pelvis simulator database: a quest for a generalized algorithm for objectively assessing medical skill.

Inherent difficulties in evaluating clinical competence of physicians has lead to the widespread use of subjective skill assessment techniques. Inspired by an analogy between medical procedure and spoken language, proven modeling methods in the field of speech recognition were adapted for use as objective skill assessment techniques. A generalized methodology using Markov Models (MM) was developed. The database under study was collected with the E-Pelvis physical simulator. The simulator incorporates an array of five contact force sensors located in key anatomical landmarks. Two 32-state fully connected MMs are used, one for each skill level. Each state in the model corresponds to one of the possible combinations of the 5 active contact force sensors distributed in the simulator. Statistical distances measured between models representing subjects with different skill levels are sensitive enough to provide an objective measure of medical skill level. The method was tested with 41 expert subjects and 41 novice subjects in addition to the 30 subjects used for training the MM. Of the 82 subjects, 76 were classified correctly (92%). Moreover, unique state transitions as well as force magnitudes for corresponding states (expert/novice) were found to be skill dependent. Given the white box nature of the model, analyzing the MMs provides insight into the examination process performed. This methodology is independent of the modality under study. It was previously used to assess surgical skill in a minimally invasive surgical setup using the Blue DRAGON, and it is currently applied to data collected using the E-Pelvis.

Primary squamous cell carcinoma of the renal pelvis masquerading as pyonephrosis: a case report.

Squamous cell carcinoma of the renal pelvis is a rare tumor, which is usually associated with nephrolithiasis. It is rarely associated with pyonephrosis. We report the case of a 69-year-old man who presented with features of pyonephrosis and underwent nephrectomy. The postoperative histological evaluation revealed an unsuspected squamous cell carcinoma of renal pelvis with a concomitant pyonephrosis. The rarity of this tumor in the absence of renal calculi and its association with pyonephrosis is highlighted.

Balkan endemic nephropathy and papillary transitional cell tumors of the renal pelvis and ureters.

Papillary transitional cell tumors of the renal pelvis and ureters occur more frequently in the Balkan countries than in other areas in the world. The populations involved are the same as those who are at risk to develop Balkan endemic nephropathy (BEN). Seventy-seven patients were treated in the Urology Clinic for papillary cell tumors of the renal pelvis and ureters (UTT) from 1986 to 1988. Forty-four of the patients were from areas where BEN is endemic. The at-risk population is estimated to be less than 2% of the control population. Eleven of the 77 were on hemodialysis when UTT were detected, and all but one of those were from BEN areas. This finding emphasizes the need for continual monitoring of end-stage renal disease patients for evidence of these tumors. Comparing the frequency of the tumors from our study (1986 to 1988) with that of early studies indicates that the risk of developing the tumors remains very high in the BEN areas. We conclude that the causative agent(s) is still prevalent in the involved areas.

[The anatomy of the pelvis].

The pelvis consists of the sacrum, the ossa coxae, and additionally the ligaments between these bones. According to mathematical calculations the bony structure as well as the arrangement of ligaments result in optimal engineering principles, i.e. maximal strength is achieved by minimal principles, i.e. maximal strength is achieved by minimal material. The muscles inside the pelvis are covered by a fascial layer. The latter represents a continuation of the abdominal transversal fascia which extends to the pelvic floor. The connective tissue fibres around the pelvic viscera interweave with those of the pelvic floor and laterally with the connective tissue around vessels and nerves. Connective tissue around the neurovascular sheaths is nearly transversely arranged and forms the lateral ligaments of bladder, uterus or prostate gland and the rectum. Several vertically arranged connective tissue ligaments accompany either the superior rectal artery and hypogastric nerves or the ureter and the ovarian vessels. Therefore, the lymphatic fluid of the pelvic organs passes the aortic lymph nodes and/or to the lateral pelvic wall along the branches of the internal iliac vessels.

[Neoplasm invasion of the renal vein by transitional cell carcinoma of the kidney pelvis].

We present two cases of renal pelvis transitional cell carcinoma with invasion of the renal vein. We emphasize the importance of preoperative diagnosis and review the diagnostic methods available for revealing this uncommon eventuality. Owing to the prognostic importance and subsequent surgical treatment, this possibility should be evaluated in those cases of renal pelvis transitional cell carcinoma with extensive parenchymatous invasion or in those cases in which kidney function cancellation occurs in the excretory urograph.

[Diagnosis and surgical treatment of primary tumors of the renal pelvis and ureter].

The incidence of primary tumors of the renal pelvis and ureter is progressively increasing; now they account for about 10 per cent of all urothelial tumors with predominance of papillary ones. They metastasize early, most frequently in the liver, the bones and the lung. For a period of 10 years, 46 patients with upper urinary tract tumors of epithelial origin have been examined and treated. In 30 of these patients (65.2 per cent) the cancer was localized in the renal pelvis and in 16 (34.8 per cent) in the ureter. In 40 patients (87 per cent) the initial symptom was hematuria with dull or colic-like pain in the lumbar area. Excretory urography and retrograde ureteropyelography are essential for the exact diagnosis. Treatment should be early and radical--nephrectomy with total ureterectomy and excision of part of the bladder wall near the ureter orifice. Organ-preserving operations are indicated only in cases of single kidney or accompanying disease of the other kidney, which after some time may require its removal.

[Metastasis of cancer of the renal pelvis and ureter to the adrenal gland].

A 67-year-old man was found to have a metastatic tumor of left adrenal gland originating from the left renal pelvic and ureteral cancer. Histopathological findings of the renal pelvic and ureteral cancer indicated transitional cell carcinoma, which was the diagnosis for the adrenal tumor. Diagnosis of cancer of the renal pelvis and ureter with metastasis to the adrenal gland before operation is very rare. A review of the literature on the metastasis from cancer of renal pelvis and ureter was made.
