[Management of old neglected posttraumatic acromioclavicular joint instability and arthrosis].

UNLABELLED: Resection arthroplasty of the AC joint was performed in 42 cases of osteoarthrosis and residual instability of traumatic origin including 26 shoulders with horizontal instability of more than half of the width of the clavicula and lesions of the deltotrapezoid fascial complex (Rockwood type II: 7; type III: 9; type IV: 17; type V: 9). 23 cases were treated with a sole Weaver-Dunn procedure. 26 cases with horizontal instability and lesions of the deltotrapezoid fascia (Rockwood IV and V type) were treated in 7 cases with the standard Weaver-Dunn procedure and in 19 cases with a modified Weaver-Dunn procedure in combination with a coracoclavicular (3 x 1) and acromio-clavicular (1 x 1) 1 mm PDS string augmentation and double breasting fascioplasty of the deltotrapezoid complex. RESULTS: Successful results (in Patte-Score) after a minimum follow-up of 2 years (mean: 32 months) were reached in 88.4% of cases with only Weaver/Dunn procedure with a significant difference of good and excellent results in the horizontally stable group (93.8%) versus the horizontally unstable group (57.2%). In the group with horizontal instability and Weaver-Dunn procedure and complex additional stabilization with fascioplasty and PDS augmentation, 89.5% excellent and good results were found. CONCLUSION: Cases with horizontal instability (type Rockwood IV and V) seem to be over-represented among patients with failed conservative treatment. Resection arthroplasty with ligament transposition after Weaver/Dunn gives excellent results in posttraumatic osteoarthrosis with mainly vertical and moderate horizontal instability. In cases with advanced horizontal instability after Rockwood IV and V injuries, almost equal results can be reached by an additional coracoclavicular and acriomioclavicular PDS augmentation with deltotapezoid fascioplasty.

Differentiation of benign and malignant tumors of the parotid gland: value of pulsed Doppler and color Doppler sonography.

To establish criteria for the differentiation of benign and malignant tumors of the parotid gland using color Doppler sonography (CDS) and pulsed Doppler sonography (PDS) we examined 37 patients with parotid tumors by gray-scale ultrasound, CDS, and PDS. Tumor vascularization displayed by CDS was graded subjectively on a 4-point scale (0 = no vascularization, 3 = high vascularization). From the Doppler spectrum, the highest systolic peak flow velocity, the resistive index (RI), and the pulsatility index (PI) were calculated. There were 11 malignant and 26 benign tumors. Tumor vascularization by CDS was grade 0 or 1 in 88.5% of benign lesions, whereas it was grade 2 or 3 in 82% of malignant lesions (P < 0.0001). The highest systolic peak flow velocity was statistically significantly higher in malignant lesions than in benign lesions. Using a threshold systolic peak flow velocity of 25 cm/s, sensitivity was 72% and specificity was 88% for the detection of a malignant tumor. Evaluation of tumor vascularization by CDS and PDS cannot differentiate between benign and malignant parotid tumors with certainty. However, high vascularization and high systolic peak flow velocity in tumor vessels should raise the suspicion of malignancy, even if tumor morphology on gray-scale sonography indicates a benign lesion.

Long-term follow-up in patients with Pendred syndrome: vestibular, auditory and other phenotypes.

Fourteen patients with a Pendred syndrome gene (Pds) mutation and three patients without the mutation were studied to evaluate long-term vestibular and auditory manifestations among patients with bilateral enlarged vestibular aqueducts (EVA). Charts were reviewed retrospectively for age, gender, otological history, presence or absence of vertigo, results of magnetic resonance imaging, relevant gene mutations and perchlorate discharge test. A missense mutation, His 723 Arg (H723R), was identified in the homozygous state in six patients and in the heterozygous state in seven patients. Another missense mutation, Tyr 410 Met (T410 M), was identified in the heterozygous state in one patient. Patients with vertigo tended to have hearing fluctuation, compared with the patients without vertigo. Patients homozygous for H723R tended to have more episodes of vertigo than the heterozygous individuals. Only one patient who was homozygous for H723R had goiter. The long-term outcome of hearing level in patients with the H723R mutation was significantly better compared with published results for patients with a Pds mutation, but not for those with the H723R mutation. Whether environmental factors or a subtype of the Pds mutation H723R are related to the long-term outcome for these patients must be clarified.

External septal reconstruction with the use of polydioxanone foil: our experience.

We present our experience and results after using polydioxanone (PDS) foil in septal reconstruction. In a period of 2 years, 12 patients who were admitted in our department with severe septal deviation and breathing problems underwent septoplasty under general anaesthesia. The nasal septum was approached via an external approach. In all patients, after resecting and exposing the septum, the removable piece after being divided into straight pieces, was sutured onto an appropriate sized PDS foil and reimplanted together between the mucoperichondrium flaps. Sutures were placed to fixate the "new septum" to the nasal dorsum and to the anterior nasal spine. The immediate postoperative course was unremarkable and in a follow-up appointment 6 months and 1 year postoperatively, one complication occurred, with septum subluxation noted in one patient. Use of PDS foil in septal reconstruction is an important surgical option for the correction of the markedly deviated nasal septum. Fixation of the straightened and replanted septum at the nasal dorsal septum border with the upper lateral cartilages and at the nasal spine is essential.

Stimulation of carotenoid metabolism in arbuscular mycorrhizal roots.

Development of arbuscular mycorrhizal roots is correlated with accumulation of various isoprenoids, i.e. acyclic C(14) polyene 'mycorradicin' and C(13) cyclohexenone derivatives. We present data indicating a strong stimulation of carotenoid metabolism in such roots. Carotenoid profiling revealed mycorrhiza-specific accumulation of zeta-carotene in Zea mays and Medicago truncatula. Precursor accumulation after inhibition of phytoene desaturase (Pds) activity by norflurazon indicated an increased phytoene biosynthetic capacity in mycorrhizal roots of all species analyzed. Nicotiana tabacum plants transformed with a PDS promoter- GUS construct showed a cell-specific induction of PDS promoter activity in root cells containing arbuscules. Mycorradicin biosynthesis and, partially, mycorrhization were impaired in maize mutants deficient in carotenoid biosynthesis. These data indicate that (1) mycorradicin is probably synthesized via a C(40) precursor carotenoid, (2) carotenoid biosynthesis is induced in mycorrhizal roots, (3) induction occurs, at least partially, at the transcriptional level, and (4) that this may play a functional role during mycorrhization.

A longitudinal study of psychiatric symptoms in primiparous women: relation to personality disorders and sociodemographic factors.

OBJECTIVE: To examine the prevalence of personality disorders (PDs) and elucidate the importance of PDs, sociodemographics and health-related factors for the development of psychiatric symptoms in primiparous women. METHOD: 625 primiparous women were assessed during pregnancy, three and 18 months following delivery. The Symptom Checklist-90 (SCL-90), the modified SCID-screen questionnaire and the Global Assessment of Functioning Scale (GAF) were used. RESULTS: The prevalence of PDs was 6.4%. PD was strongly associated with psychiatric symptoms during and after pregnancy. The most important factor predicting long-lasting mental problems was having a PD. A higher level of psychiatric symptoms was found in pregnancy than postpartum. Psychiatric caseness during pregnancy was strongly associated with caseness postpartum. In addition, socio-economic status, younger age and previous treatment for mental problems were identified as risk factors. CONCLUSION: The study indicates a strong association between long-lasting psychiatric illness and personality disorders in childbearing women. A strong association was also found between psychiatric symptoms during pregnancy and the postpartum period.

Optimum surgical suture material and methods to obtain high tensile strength at knots: problems of conventional knots and the reinforcement effect of adhesive agent.

BACKGROUND: Strong suture material and a firmly tight knot that never loosens are necessary for tendon suture, but the appropriate combination of suture materials and the methods to make a knot are matters of uncertainty. METHODS: The tensile strength of four conventional tendon suture materials (Surgilon, Ethibond, Ethilon, PDS II) and newly developed FiberWire were examined. An experienced orthopedic surgeon formed surgical knots with USP2 wire (0.5-0.599 mm in diameter) by making loops and then pulled them at 20 mm/min using Instron. RESULTS: With the conventional surgical suture method (the addition of one throw on a surgeon's knot), Surgilon proved to have the highest tensile strength (163.6 +/- 6.5 N). Other suture materials, when tied under the same conditions, slipped and did not reach the ultimate tensile strength. When four additional throws were made, FiberWire provided the highest tensile strength (316.6 +/- 12.2 N) among the five suture materials tested (others were 140-200 N). When an adhesive agent, cyanoacrylate, was applied to a knot, PDS II provided the highest tensile strength (182.0 +/- 10.1 N). CONCLUSIONS: Our study suggests three suitable combinations of suture materials and methods for suture knot formation, depending on the site of surgery, that provide optimum treatment outcomes. Surgilon provides the most stable strength for general suture techniques. FiberWire is the strongest suture material for a site where a large number of throws is clinically possible. PDS II provides a strong suture when combined with cyanoacrylate reinforcement.

[Stability of osteosyntheses for condylar head fractures in the clinic and biomechanical simulation].

BACKGROUND: Fractures of the condylar head are traditionally managed by closed techniques, despite a considerable rate of dysfunctional problems. PDS pin osteosynthesis (presented by Rasse 1992) via a preauricular approach failed to become established as a standard procedure due to a lack of stability. Alternatively, mini- or microplating, as performed in our patients (101 condylar head fractures between 1993 and 2000), showed high-grade limitations of translatory movements in about 30% due to scarification after loosening of osteosynthesis materials. The aim of this study was to establish an efficient procedure for achieving a functionally stable and atraumatic osteosynthesis. METHODS: For the definition of a suitable procedure, models of the mandible (standardized fractures, types A, B, and C) were osteosynthesized (six samples for each type of fracture and type of screw), each with three PDS pins, 2.0 mm resorbable, 2.0 cortical, 1.7 and 1.2 mm small fragment screws, and exposed to increasing loads in centric (0-20-35 mm opening) and eccentric (35 mm opening) condylar positions. A computerized biomechanical test stand allowed a dynamic simulation of chewing forces (16 hydraulic drives). The resulting fracture gaps were measured without contact by a motion capture system. RESULTS: Within physiological limits, only 1.7 small fragment and 2.0 mm cortical screws were able to bear occlusal loadings up to 200 N (1.2 mm small fragment screws up to 150 N, resorbable 2.0 mm screws up to 100 N, and PDS-pins up to 50 N). In a pullout experiment (condylar spongious bone of young pigs, aged 4-6 months), 1.7 mm small fragment screws showed superior retention. A consecutively developed small fragment screw-system has been applied clinically in 74 condylar head fractures (58 patients). After removal of osteosynthesis material, 41 of 49 TM joints have so far shown complete restitution. CONCLUSIONS: The newly developed osteosynthesis system using a retroauricular approach based on 1.7 mm small fragment screws makes maxillomandibular immobilization unnecessary. The extra-articular position of the screw heads prevents scar-induced articular limitations. Preexisting degenerative alterations of the TMJ soft tissues, however, will affect functional results adversely.

Growth of tracheal anastomoses: advantage of absorbable interrupted sutures.

Growth of the trachea after complete transection and anastomosis was studied in four groups of 1-month-old New Zealand white rabbits. The trachea was transected at the fifth cartilaginous ring and then anastomosed with continuous 6-0 polypropylene (Prolene) (group 1), interrupted 6-0 polypropylene (group 2), continuous 6-0 polydioxanone (PDS) (group 3), or interrupted 6-0 PDS (group 4). The animals were followed up for 90 to 103 days (mean follow-up, 95 days). At the time the animals were killed, body weight had increased 125% (1.2 to 2.7 +/- 0.18 kg). Growth of the trachea was assessed at the time of death. Results from this study suggest that growth of a tracheal anastomosis is retarded in a growing animal model. The degree of resultant stenosis was significantly less when an absorbable suture material (PDS) and an interrupted suturing technique were used.

Epileptiform events induced by GABA-antagonists in entorhinal cortical cells in vitro are partly mediated by N-methyl-D-aspartate receptors.

The effects of the GABA-antagonists, picrotoxin and bicuculline on responses of medial entorhinal cortical cells to subicular stimulation were tested in vitro. On every cell tested either antagonist caused a profound enhancement of synaptically evoked depolarizations to the point where paroxysmal depolarizing shifts (PDS) were recorded, although only a minority of cells showed evidence of inhibitory potentials in the control situation. The initial PDS was followed by either a long afterdepolarization or a series of afterdischarges. The afterpotentials were always reduced or blocked by the N-methyl-D-aspartate (NMDA)-receptor antagonist, 2-amino-5-phosphonovalerate (2-AP5). The amplitude of the initial PDS in many cells was also reduced by 2-AP5. Thus, entorhinal cortical cells are susceptible to epileptogenesis induced by a reduction of GABAergic inhibition and the paroxysmal events contain a large, NMDA-receptor mediated component.

Sodium valproate decreases synaptic potentiation and epileptiform activity in hippocampus.

The actions of sodium valproate (NaVP) were studied in the in vitro hippocampus using extracellular, intracellular and voltage-clamp recording techniques. In the CA1 region, concentrations of 30-200 microM NaVP reduced the amplitude but not the time course of post-tetanic potentiation (PTP) of dendritic field excitatory postsynaptic potentials (EPSPs). Epileptiform discharges were studied intracellularly in CA3 cells after pharmacological blockade of synaptic inhibition and repeated tetanic stimulation. NaVP (100 microM) blocked evoked paroxysmal depolarizing shift (PDS) discharges through a mechanism of increasing the threshold for burst-firing. When the PDS current was studied under voltage-clamp, application of NaVP (100 microM) resulted in a graded reduction of the PDS waveform. All of the actions of NaVP may result from inhibition of excitatory synaptic transmission following repetitive cell firing. A hypothesis is proposed that NaVP may act to decrease excitatory synaptic potentiation necessary for network synchronization.

Morphological aspects of microarterial anastomoses: a comparison of nylon with polydioxanone.

The morphological appearance of longitudinally sectioned rat femoral arteries was determined in intact arteries and from 3 to 435 days after vessel division and anastomosis with either 9/0 gauge nylon or polydioxanone (PDS) in 26 animals. The purpose of the study was to establish the mechanisms and compare the quality of healing after microarterial anastomosis and to determine whether PDS was degraded before sufficient anastomotic healing had taken place. The results revealed that there was no difference in the process of healing or quality of anastomosis with either suture material. From 3 to 21 days post anastomosis, there was a progressive separation of the ends of vessels within the developing scar. Anastomotic patency was established and maintained at first by an adventitial overgrowth of fibroblasts and undifferentiated adventitial cells and later by the growth of a smooth myocyte scar that stretched between the cut ends of the vessel and over the intima in the form of elongated circumferential plaques. The vessel was morphologically healed by the 21st day. The sutures served little or no purpose in maintaining anastomotic integrity after the 5th day, being situated in the scar forming between the separating vessel ends. PDS was present within the vessel wall up to 120 days post anastomosis and was certainly intact at the time of morphological healing, suggesting that this material is safe as a microvascular suture.

The antiproliferative effect of interferon and the mitogenic activity of growth factors are independent cell cycle events. Studies with vascular smooth muscle cells and endothelial cells.

We studied the antagonistic effects of interferon (IFN) and growth factors in G0/G1-arrested normal bovine aortic smooth muscle cells (SMC) which were stimulated by serum, or purified platelet derived growth factor (PDGF), supplemented with plasma-derived serum (PDS). The growth response, measured as [3H]thymidine incorporation into DNA, was dependent on the concentration of the mitogen. Human IFN alpha, recombinant human IFN alpha 2, or a crude bovine-IFN preparation prepared from virus-infected bovine aortic endothelial cells, inhibited SMC growth induced by either serum or PDGF with PDS. The extent of IFN inhibition was inversely related to the concentration of the mitogenic stimulus. We also investigated whether IFN inhibited the early events in G1 phase, stimulated by the competence factor PDGF, or the progression of the cell into the S phase induced by PDS. The results indicated that IFN inhibited these two stages of the G1 phase independently. In addition, we investigated the antiproliferative effect of IFN on bovine aortic endothelial cells (BAEC), which do not respond to PDGF but to the mitogenic activity of fibroblast growth factor (FGF). IFN inhibited the mitogenic activity of FGF in a dose-dependent manner. The results indicate that the anti-proliferative activity of IFN and the mitogenic effects of different growth factors are independent.

Anticonvulsant effects of localized chronic infusions of GABA in cortical and reticular structures of baboons.

We studied the effects of chronic (7 day) infusions of GABA (100 and 20 micrograms/microliter, 10 microliter/h) applied in different cerebral structures of baboons made photosensitive by a subconvulsant dose of allylglycine. The GABA infusion has partial anticonvulsant effects when applied to the motor cortex, reticular magnocellular nucleus (RMC), or substantia nigra (SN), but when directed to the prefrontal cortex (area 8) it has no effect. These anticonvulsant effects of GABA infusion are more important when GABA is infused into the motor cortex, where paroxysmal discharges (PDs) originate, than when it is infused into the RMC. In contrast, the anticonvulsant effects on light-induced generalized seizures are more pronounced when GABA is infused into the RMC than when it is infused into the motor cortex. GABA infusion into the SN has no effect on PDs and myoclonia and blocks seizures less effectively than the RMC infusion. These results are in accordance with the role of the motor cortex as a generator of PDs and of RMC in the generalization of seizures. Focal paroxysmal EEG and clinical activities, previously reported to appear at the end of the motor cortex GABA infusion, were not observed after RMC or SN infusions. However, behavioral hyperactivity occurring at the end of subcortical GABA infusions was observed. These behavioral signs could correspond to the clinical expression of a GABA withdrawal syndrome.

Age differences in the proliferative response of cultured arterial smooth muscle cells induced by sera of hypothalamically-stimulated rats.

In an earlier report, it was shown that arterial smooth muscle cells (ASMC) cultured from normal rat aorta, proliferated in response to homologous sera from young rats which had received hypothalamic stimulation (HS), in contrast to the effect of sera from non-stimulated age-matched controls (sham-operated). In the present study, this proliferative response was compared in young and old rats. Isolated target cells were subcultured from primary explants of aortic tissue obtained from young, male, Fischer 344 rats, which were electrode-implanted in the hypothalamus but not stimulated. After an initial quiescence period (growth arrest), target cells were exposed to plasma derived serum (PDS) from 4 experimental groups: young stimulated rats; young sham-operated controls; aged stimulated rats and aged sham-operated controls, at PDS concentrations of 2.5% and 5.0% and counted at days 2 and 5. Proliferative responses of ASMC were found to be influenced by concentration of the PDS, age of the donor animal contributing the serum and the presence of HS. The greatest responses were observed in relation to sera derived from aged stimulated rats, especially at the higher concentration, suggesting an interaction of HS with advanced age. These results are discussed in reference to their possible bearing on the pathogenesis of atherosclerosis.

Cardiovascular periodicities in healthy humans in the absence of breathing and under reduced chemical drive of respiration.

In 20 healthy human subjects (aged 20-24 years) power density spectra in 1-min time series were computed by modified MATLAB procedures from inter-beat heart intervals and systolic and diastolic finger blood pressure. High- (HF), mid- (MF) and low-frequency (LF) peaks (0.16-0.37 Hz, 0.08-0.12 Hz and 0.02-0.05 Hz, respectively) were distinguished in power density spectra (PDS). During 1-min voluntary apneas MF and LF power increased and in 18 out of 20 subjects HF peaks disappeared in PDS. Breath holding performed in hyperoxia or in hyperoxia accompanied by 1-min hyperventilation preceding apnea augmented power at MF and LF range and abolished marginal HF peaks present in the spectrum in two subjects. It is concluded that apneic PDS is characterized by augmented power of MF and LF rhythms, and represents endogenous free running cardiovascular periodicities released from feed-back mechanical, reflex and feed-forward central entrainment to respiratory rhythm.

An investigation of skin deformation around percutaneous devices.

According to the literature one of the primary failure models of percutaneous devices (PDs) in soft tissue implantation sites is that of mechanically induced trauma along the implant/tissue interface. In order to avoid such avulsion, subcutaneous flanges have been incorporated into PD designs to provide a better distribution of stresses along the implant/tissue interface. Tissue necrosis and inflammation were then observed to be most pronounced around the rim of the subcutaneous flange. It was the goal of this study to develop and test PDs with subcutaneous flanges of varying stiffnesses in order to allow the flange to distribute stresses into the surrounding tissues more evenly, thereby reducing the likelihood of failure due to avulsion. PDs were tested with flanges of constant thickness or varying linearly, reducing toward the flange rim. They were evaluated using an in-vitro testing method which was designed to simulate the situation of an implanted PD subjected to external loading. The results indicated that the stiffness characteristics of the subcutaneous flange had a substantial effect on the behavior of the devices under loading. A PD having a radial stiffness decrease would improve the likelihood of implant survival in the clinical situation due to the reduction of mechanically induced trauma along the implant/tissue interface.

Comparing uterine electromyography activity of antepartum patients versus term labor patients.

OBJECTIVE: The purpose of this study was to compare uterine electromyography of patients delivering>24 hours from measurement with laboring patients<or=24 hours from measurement. STUDY DESIGN: Fifty patients (group 1: labor, n=24; group 2: antepartum, n=26) were monitored using transabdominal electrodes. Group 2 was recorded at several gestations. Uterine electrical "bursts" were analyzed by power-spectrum from 0.34 to 1.00 Hz. Average power density spectrum (PDS) peak frequency for each patient was plotted against gestational age, and compared between group 1 and group 2. Frequency was partitioned into 6 bins, and associated burst histograms compared. RESULTS: Group 1 was significantly higher than group 2 for gestational age (39.87+/-1.08 vs 32.96+/-4.26 weeks) and average PDS peak frequency (0.51+/-0.10 vs 0.40+/-.03 Hz). Histograms were significantly different. A correlation coefficient of .41, with significance, was found with PDS vs gestation. CONCLUSION: Uterine electromyography in antepartum patients is significantly lower than in laboring patients delivering<or=24 hours from measurement.
