Use of company stock ownership records to locate subjects lost to follow-up.

When 10,766 members of a refinery retrospective cohort study were assessed for vital status using conventional company and Social Security Administration records, the status of 370 remained unknown. Computerized corporate files of shareholder information yielded current addresses for 243 of those lost to follow-up, and through Directory Assistance, telephone numbers were obtained for 152. Vital status was ascertained by telephone for 146 of the 370 members of the cohort whose status had been unknown.

After the internal market: public ownership or poisoned chalice?

This article examines the reasons why the internal market failed to gain public acceptance and looks beyond the popular view which argued the non-acceptance was a result of the "two tier" service and increased bureaucracy. It compares the experience of customers in the internal market with those of other public services that have undergone privatization. The article concludes that differences in the range and pattern of the services provided by the NHS led the public to a lower level of understanding regarding improvements and efficiency. The benefits of the market in other once public services were more transparent and there were greater incentives to accept the change. The article recommends that the clinicians involved in the management of primary care groups should learn the lessons of the internal market because users are ill-equipped for the next stage of healthcare reform as they were for the last.

An entomological investigation of the likely impact of cattle ownership on malaria in an Afghan refugee camp in the North West Frontier Province of Pakistan.

Field trials were undertaken to determine the effect of close proximity of humans to livestock on the human biting rates exhibited by various anophelines. The results indicate that proximity to cattle and to goats increases the subject's chances of being bitten by anophelines. Man-biting by Anopheles stephensi rose by 38% (8-68% CI) in the presence of a cow, and by 50% (16-84% CI) in the presence of two goats. Other species exhibited similar trends. These findings explain the results of an earlier trial which revealed that malaria prevalence was higher amongst families that kept cattle than those that did not. The findings are discussed in relation to existing information regarding the host preferences of local anophelines and classic theories regarding zooprophylaxis. Our findings suggest that animals are only likely to have a worthwhile prophylactic affect when the vector is zoophilic, and then only when the animals are deployed to form a barrier between that vector and man. In situations where deployment of livestock to form a zoobarrier is impractical, the livestock should be located as far from man as possible.
