[A case of pulmonary moniliasis following the misuse of antibiotics].

Our patient is a 30 year old, married female, with one daughter. She has been coughing about 200-250 cc purulent, blood stained sputum per day for the last three years. In the microscopic examination of sputum, abundant leukocytes, a small quantity of red blood cells and epithelia were found. Elastic fibres were absent. In cultures C. albicans and beta hemolytic streptococci were isolated. Coughing, sputum, and scattered moist rales at the bases (see the chest x-rays) were found clinically. Other systems were normal. An amelioration began on the third day of mycostatin treatment. There was complete healing by the tenth day of the treatment.

Biochemical and physiological parameters on the healthy skin surface of persons with candidal intertrigo and of persons with tinea cruris.

Biochemical and physiological tests were carried out on the skin surface of 20 patients with candidal intertrigo and 27 patients with tinea cruris. In all patients the test areas were free of efflorescences. The same tests were performed in 39 and 27 resectively healthy test persons of the same age and sex. The following striking findings came to light: 1. There was a significant decrease in the percentage amount of squalene in the skin surface lipids of the moniliasis group as compared with the control group. 2. There was a significant decrease in the reducing substances in the so called water solubles obtained with the phenol sulfuric acid method in the moniliasis group. The same results were obtained when only those moniliasis patients who were definitely not suffering from diabetes mellitus were taken into account. This is presumably a question of a reduction in the bound carbohydrates. 3. There were significantly more amino acids extractable from the skin surface of the tinea cruris patients than of the control persons. These results point to important predisposing factors for the susceptibility to candidal intertrigo and tinea cruris respectively.

Keratitis with hypoparathyroidism.

Chronic kiratitis developed in two children with nonsurgical hypoparathyroidism, as part of an autosomal recessive syndrome that included adrenal insufficiency and moniliasis in what was postulated to be an autoimmune disease. The corneal changes may also have been caused by autoimmune mechanism. Activity of the keratitis diminished once the hypoparathyroidism had been brought under control. However, these patients were thought to be at risk for adrenal insufficiency: neither had moniliasis or adrenocortical insufficiency at present, but the features of the hypoparathyroidism adrenal insufficiency-moniliasis syndrome appeared at different ages and in differing sequences.

Pulmonary mycotic infections; allergic and immunologic factors.

The mechanisms of immunity and allergy, at play in every infectious disease, must be comprehended before the pathogenesis of an infection can be appreciated.Immunity, allergy and serology are concerned with specific antigen-antibody reactions. In immunity the principal concern is with the final disposition of antigen (agglutination, lysis, and phagocytosis). In allergy attention is focused upon tissue damage resulting from antigen-antibody union. In serology interest is devoted to the presence of antibody as evaluated by certain visible in vitro reactions-precipitin, agglutination, opsonization and complement fixation tests. There are two types of allergic reaction-the immediate or anaphylactic type and the delayed type or the allergic disease of infection. Neither kind takes part in the mechanism of immunity. At this time the allergic antibody and the immune antibody must be considered as two different and distinct antibodies. Skin and serologic tests are important diagnostic aids in certain pulmonary mycotic infections-for example, coccidioidomycosis, blastomycosis, histoplasmosis and moniliasis. Clinical expressions of allergy may appear in coccidioidomycosis, histoplasmosis and moniliasis. Pulmonary mycoses are divided into three groups, that is, the endogenous mycoses (actinomycosis, moniliasis, geotrichosis), the endogenous-exogenous mycoses (cryptococcosis, aspergillosis, mucormycosis) and the exogenous mycoses (nocardiosis, coccidioidomycosis, histoplasmosis, North American blastomycosis). The diagnosis and treatment of the important mycotic infections that invade lung tissue are discussed.

Variety of determinants in an adrenal antigen common to man and some animals.

Precipitating adrenal antibodies are specifically associated with the moniliasis-polyendocrinopathy syndrome. This may include several simultaneous autoimmune disorders such as Addison's disease, hypoparathyroidism, thyroiditis, pernicious anaemia and ovarian failure often combined with moniliasis and alopecia. The antibodies react with two adrenal specific antigens present in man and in different animals. One of these antigens designated S-(soluble) antigen is present in all subcellular fractions and the other designated P-(particulate) antigen was found in precipitable form only in the mitochondrial fractions. Rabbits immunized with bovine adrenal homogenate and different subcellular fractions, produced three to four adrenal specific antibodies, which were qualitatively identical. The corresponding antigens were designed as S1-S4, one of which (S4) was strictly specific for bovine tissue. S1-, S2- and S3-antigens were identical in bovine and ovine adrenals, whereas a partial reaction of identity was found between ruminants and the S1- and S2-antigens of all other species tested. The S1- and S2-antigens seem to contain a variety of determinants, some common to all the species studied and some limited to certain species only. Of the four S-antigens only one (S1) in rabbits, was found to contain autoantigenic determinants. Comparative studies with sera from patients with the moniliasis-polyendocrinopathy syndrome indicate, that this molecule is the one with which human and rabbit antisera react.

Oral complications associated with bone marrow transplantation in a pediatric population.

The effect of dental evaluation and treatment prior to bone marrow transplantation in 11 pediatric patients was assessed. Oral complications associated with marrow ablative therapy and the immediate posttransplant period (Days 0-35) were also studied. Oral complications during marrow ablative therapy included: xerostomia (2/11 patients) and parotitis (1/11 patients). Oral complications in the immediate post-transplant period (Days 0-35) included: mucositis (11/11 patients); moniliasis (9/11 patients); and stomatitis associated with acute G.V.H.D. (3/9 patients receiving allogenic marrow transplants). The onset of the mucositis in the posttransplant period usually occurred at the nadir of the white blood cell count and resolved when the absolute neutrophil count was approximately 500/m3. The onset of moniliasis in the posttransplant period was usually 1-2 days after the onset of the mucositis. Resolution of the moniliasis usually paralleled resolution of the mucositis. No patient developed an infectious and/or hemorrhagic complication of odontogenic origin.

[Histochemical characteristics of experimental candidiasis with specific sensitization].

The authors studied histochemically the significance of specific sensitization in the pathogenesis of specific moniliasis. The activity of various enzymes of the fungus and of the cells of the inflammatory infiltrate with the subsequent quantitative assessment was determined. In visceral moniliasis tissue changes were in a definite correlation with the changes of the active agents in the cells of the fungus and in the cells of the inflammatory inflitrate. An increase in the activity of the fungus enzymes was noted at the early periods of parasitic infestation; this was later replaced by its reduction, coinciding with the reduction of the amount of the vegetative forms. Preliminary administration of the moniliasis allergen induced an increase of the sensitivity to the subsequent infection. Changes in the specifically sensitized rats developed against the background of marked vascular disturbances and coursed by the hyperergic type.

Topical treatment for vaginal candidiasis in pregnancy.

BACKGROUND: Vaginal candidiasis (moniliasis or thrush) is a common and frequently distressing infection for many women. It is even more common in pregnancy. OBJECTIVES: The objective of this review was to assess the effects of different methods of treating vaginal candidiasis in pregnancy. SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group trials register. In addition, the Cochrane Controlled Trials Register (CENTRAL/CCTR) was searched. Date of last search: April 1999. SELECTION CRITERIA: Randomised trials of any treatment for vaginal candidiasis in pregnancy. DATA COLLECTION AND ANALYSIS: Two reviewers assessed trial quality and extracted data. MAIN RESULTS: Twelve trials were included. Based on five trials, imidazole drugs were more effective than nystatin when treating vaginal candidiasis in pregnancy (odds ratio 0.21, 95% confidence interval 0.16 to 0.29). Nystatin,in turn was more effective than hydrargaphen in one trial.A trial of clotrimazole was more effective than placebo (odds ratio 0.14, 95% confidence interval 0.06 to 0.31). Single dose treatment was less effective than three or four days treatment when assessed by culture and by symptoms in three different trials and treatment lasting for four days was less effective than treatment for seven days (odds ratio 10.6, 95% confidence interval 4.01 to 28.05). Based on two trials, treatment for seven days was no more or less effective than treatment for 14 days (odds ratio 0.41, 95% confidence interval 0.16 to 1.05). REVIEWER'S CONCLUSIONS: Topical imidazole appears to be more effective than nystatin for treating symptomatic vaginal candidiasis in pregnancy. Treatments for seven days may be necessary.

Burning mouth: an analysis of 57 patients.

The burning mouth complaint of each of fifty-seven patients was thoroughly studied. Psychogenesis was found to be the most frequent cause, followed by geographic tongue and moniliasis. Multiple causative factors, such as psychogenesis and moniliasis and psychogenesis and geographic tongue, were found in some patients. The purely psychogenic group was composed mostly of postmenopausal women. The tongue and palate were the most frequently affected sites. There were some similarities among patients in the geographic tongue and psychogenesis groups. A diagnostic protocol for patients with burning mouth is described.
