Manometric artefacts suggesting compression of the duodenum by the superior mesenteric artery in healthy humans.

Multi-channel manometry offers the opportunity to study intestinal motor activity with high spatiotemporal resolution. We report tonic and phasic intraluminal pressure changes in the mid-portion of the horizontal part of the duodenum. In 10 healthy volunteers, we recorded 2 h of interdigestive duodenal motility using a water-perfused catheter. The assembly incorporated 12 duodenal sideholes at 1.5-cm intervals (D1-D12). Measurement of the antral and duodenal transmucosal potential difference (TMPD) was used to maintain a correct position of the catheter. The incidence of pressure waves (PWs) increased gradually from proximal (D1) to distal (D12) (P < 0.0001), while the mean amplitude of PWs decreased (P < 0.0001). In eight of 10 subjects, the signals recorded from D9 showed tonic pressure elevations with superimposed phasic pressure changes at heart-rate frequency, comprising 13.8% of total recording time. In the other two subjects, this phenomenon occurred in D8 (9.9% of time). D10 showed a lower incidence of PWs compared with neighbouring sideholes (D6-D9/D11-D12) (P < 0.035), with normal amplitudes. Fluoroscopy was performed in three subjects and showed that D9 was located at the midline. In healthy subjects manometric signals recorded from the horizontal part of the duodenum showed localized artefacts, presumably caused by compression by the superior mesenteric artery. In addition, a 'silent' region was present just distal to this site, the origin of which is uncertain.

Contribution of bile and pancreatic juice to the control of pH in the pig duodenum.

To evaluate the respective contributions of biliary and pancreatic secretions in the formation of intraduodenal pH, 10 pigs were chronically fistulated in either the common bile duct (5 pigs) or the pancreatic duct (5 pigs). They were all fitted with a permanent catheter into the duodenum for the continuous return of secretion and with a duodenal T-cannula allowing the introduction of a combined glass electrode. pH was recorded over 8-hour periods in fed or fasted pigs in which both secretions were returned or in which either the bile or the pancreatic juice was not restituted. In fed pigs with secretions normally flowing into the duodenum, the pH fluctuated between 4 and 6 during 57% of the recording time; it exceeded 6 during 29% of this period. The rest of the time, it remained between 2 and 4. Bile deprivation did not change the duration of the 4 less than pH less than 6 and 2 less than pH less than 4 ranges, although a slight decrease of the pH greater than 6 time-course was recorded. Deprivation of pancreatic secretion did not induce significant variations of the pH pattern. In fasted pigs with restituted biliary and pancreatic secretions, a pH greater than 6 was recorded for 70% of the 8-hour period. The pH was lower in short periods: 19% (4 less than pH less than 6), 8% (2 less than pH less than 4) and less than 1% (pH less than 2) of the time.(ABSTRACT TRUNCATED AT 250 WORDS)

Displacement of occluded plastic endoprostheses into the duodenum during percutaneous biliary drainage: description of an under-reported technique.

AIM: Percutaneous transhepatic biliary drainage (PTBD) is occasionally requested in patients with an occluded plastic stent in situ. We describe a technique for dislodging the stent into the duodenum during PTBD. MATERIALS AND METHODS: Twelve patients (M:F, 9:3 mean age 70 years) with plastic stents in the common bile duct (CBD) underwent PTBD. Eleven patients had malignant obstruction and one patient had Mirizzi's syndrome and a large duodenal diverticulum. PTBD was performed using right lobe access in nine patients and a left hepatic access in three patients. The level of biliary obstruction was at the lower CBD in five patients, mid-CBD in four patients and upper CBD in three patients. After standard percutaneous biliary access was established, an 8mm by 4 cm balloon catheter was gently inflated alongside the upper end of the plastic stent and advanced over a stiff guidewire, towards the duodenum, to dislodge the plastic stent. RESULTS: Plastic stents were successfully dislodged in all 11 patients with malignancy and metallic stents were deployed for palliation. In nine of 11 patients with malignant lesions the PTBD dislodgement of the plastic stent and insertion of a metallic stent was carried out as a single-step procedure. In two patients with biliary sepsis, a two-step procedure was necessary. In the patient with Mirizzi's syndrome the plastic stent could not be dislodged. No complications were observed. CONCLUSION: Plastic stents, inserted at ERCP, can be dislodged in the majority of cases at PTBD using a balloon catheter inserted alongside the failed stent without complication.

Epidermal growth factor promotes rapid response to epithelial injury in rabbit duodenum in vitro.

BACKGROUND & AIMS: Growth factors are mainly involved in the regulation of intestinal epithelial barrier function. This study investigated the effect of epidermal growth factor (EGF) and insulin-like growth factor 1 (IGF-1) on epithelial restitution of rabbit duodenum in vitro. METHODS: Rabbit duodenal mucosal strips mounted in an Ussing chamber were luminally exposed to 10 mmol/L HCl for 10 minutes and then incubated with buffer alone or luminal buffer containing various concentrations of EGF and IGF-1 for 3 hours. Resistance was calculated from potential difference and short-circuit current. Damage was assessed by morphometry on H&E-stained sections. RESULTS: HCl caused resistance to decrease from 112 +/- 2 to 51 +/- 4 ohms x cm2 10 minutes after injury (n = 6; P < 0.05). Postinjury treatment with 25 or 50 ng/mL luminal EGF for 3 hours stimulated resistance to recover to 94 +/- 3 and 104 +/- 3 ohms x cm2, respectively, vs. 81 +/- 3 omega x cm2 in controls (P < 0.05). Ten minutes after injury, 62% of the mucosa was damaged; 3 hours after injury, damage was reduced to 24% +/- 1.09% and 10% +/- 1.42% in the 25 and 50 ng/mL EGF group, respectively, vs. 38% +/- 0.93% in controls (n = 6 per group). EGF stimulated enterocyte migration. IGF-1 did not impair epithelial restitution. CONCLUSIONS: EGF, but not IGF-1, promoted epithelial restitution of rabbit duodenum in vitro.

Carbonic anhydrases and mucosal vanilloid receptors help mediate the hyperemic response to luminal CO2 in rat duodenum.

BACKGROUND & AIMS: The duodenal mucosa is exposed to PCO(2) >200 mm Hg due to the luminal mixture of gastric acid with secreted bicarbonate, which augments mucosal protective mechanisms. We examined the hyperemic response to elevated luminal PCO(2) in the duodenum of anesthetized rats luminally exposed to high CO(2) saline to help elucidate luminal acid-sensing mechanisms. METHODS: Blood flow was measured by laser Doppler, and intracellular pH of epithelial cells by measured by ratio microimaging. The permeant carbonic anhydrase (CA) inhibitor methazolamide, relatively impermeant CA inhibitor benzolamide, vanilloid receptor antagonist capsazepine, or sodium-hydrogen exchanger 1 (NHE-1) inhibitor dimethyl amiloride were perfused with or without the high CO(2) solution. RESULTS: The high CO(2) solution increased duodenal blood flow, which was abolished by pretreatment with methazolamide or capsazepine or by dimethyl amiloride coperfusion. Sensory denervation with capsaicin also abolished the CO(2) effects. Benzolamide dose-dependently inhibited CO(2)-induced hyperemia and at 100 nmol/L inhibited CO(2)-induced intracellular acidification. The membrane-bound CA isoforms IV, IX, XII, and XIV and cytosolic CA II and the vanilloid receptor 1 (TRPV1) were expressed in duodenum and stomach. Dorsal root ganglion and nodose ganglion expressed all isoforms except for CA IX. CONCLUSIONS: The duodenal hyperemic response to luminal CO(2) is dependent on cytosolic and membrane-bound CA isoforms, NHE-1, and TRPV1. CO(2)-induced intracellular acidification was inhibited by selective extracellular CA inhibition, suggesting that CO(2) diffusion across the epithelial apical membrane is mediated by extracellular CA. NHE-1 activation preceding TRPV1 stimulation suggests that luminal CO(2) is sensed as H(+) in the subepithelium.

[4199 biopsies from the endoscopic normal lower duodenum].

Lower duodenal biopsies (LDB) are not taken at every oesophago-gastro-duodenoscopy (EGD). In the present study, biopsies from the endoscopic normal lower duodenum were checked as a measure of quality assurance. From 1996 to 2000, 9,955 EGD were performed and 4,199 LDB were taken (42.2 %). Of these, 667 showed pathological histology (15.9 %). A non-specific inflammation was seen in 537 cases and lymphangiectasia in 30 cases. Signs of indigenous sprue were described histologically in 6 LDB. In 4 of the 6 first diagnoses, the LDB was taken owing to clinical suspicion of malabsorption syndrome. Giardia lamblia could be detected in 22 patients. Only 6 of the 22 patients had diarrhoea. A total of 18 clinically relevant first diagnoses were made by LDB in asymptomatic patients with normal endoscopic findings in the duodenum. In order to make a relevant first diagnosis, 233 LDB had to be taken. LDB can be dispensed within EGD when there is neither diarrhoea nor loss of weight, and no anemia, iron deficiency, vitamin deficiency, macrocytosis, hypoproteinaemia, meteorism, joint symptoms or fever.

[Acute gastrointestinal bleeding caused by perforation of a Greenfield caval filter into the duodenum].

HISTORY AND CLINICAL FINDINGS: A 77-year-old woman was admitted to a nearby hospital because of acute upper gastrointestinal bleeding and collapse. A Greenfield caval filter had been implanted nine years before admission because of pulmonary embolism. INVESTIGATIONS: Gastroduodenoscopy showed two hooks of the caval filter having penetrated the duodenum. The diagnosis was confirmed by an abdominal CT scan. TREATMENT AND COURSE: The patient was transferred to our hospital for surgical removal of the cava filter, which was done through the right-flank retroperitoneal approach. She had an uneventful recovery and was discharged from the hospital on the 7th postoperative day. CONCLUSIONS: Acute upper gastrointestinal bleeding caused by by a Greenfield cava filter perforating the duodenum is an extremely rare complication. But in case of acute gastrointestinal bleeding in a patient with an implanted caval filter or vascular prosthesis this should be considered and the filter removed surgically.

Effects of aqueous extracts of six marine microalgae on smooth muscle contraction in rat duodenum and vas deferens.

We report the effects of aqueous extracts of six marine microalgae on responses to various contractile agents in isolated rat duodenum (acetylcholine, calcium chloride, barium chloride) and vas deferens (noradrenaline). Our results suggest that only extracts of Phaeodactylum tricornutum, Skeletonema costatum, and Chlorella stigmatophora may have pharmacological potential. Extracts of these three species had spasmolytic effects on responses to the three contractile agents in duodenum. In vas deferens, P. tricornutum and C. stigmatophora inhibited the contractions induced by noradrenaline, whilst S. costatum showed a non-dose-dependent potentiation. With the aim of elucidating their mechanisms of action, we are attempting to isolate the active principles present in these extracts.

Giant Brunneroma of the duodenum.

A case of giant hyperplasia of Brunner's glands in the duodenum is reported. Recurrent vomiting due to stenosis of the duodenum was the leading symptom. Endoscopy, ultrasound and histological examination were employed as complementary methods to establish the diagnosis. The 5 X 7 cm large tumor was surgically excised. Brunneromas are always benign and their incidence is about 0.08% in upper GI-tract endoscopy.

Hydrocortisone and 1,25(OH)2D3: role in proliferative responses of the embryonic chick duodenum in organ culture.

The present studies were conducted to determine if hydrocortisone (HC) and/or 1,25-dihydroxycholecalciferol (1,25(OH)2D3) alter proliferative responses of the cultured duodenum, and if such alterations could be related to the known augmentation of 1,25(OH)2D3-induction of a specific, intestinal calcium-binding protein (CaBP) by glucocorticoids. HC stimulated proliferation in the duodenal epithelium, as indicated by increased DNA synthesis (3H-thymidine uptake), increased cell number/villus, and increased mitotic index after colchicine treatment. Goblet cell numbers were not significantly increased with any hormone treatment. The presence of 1,25(OH)2D3 alone did not affect proliferative responses. CaBP concentration as a function of tissue weight was 50% greater in HC stimulated intestine, indicating that the proliferogenic action of HC on the duodenum alone could not account for the glucocorticoid-1,25(OH)2D3 interaction in CaBP synthesis.

[A case each of complete duplication of the stomach and of the duodenum (author's transl)].

Previous reports of the very rare observations concerning complete duplication of the stomach and of the duodenum are reviewed and such a case is described. Radiological examination and gastroscopy showed complete duplication of the stomach, the abnormal organ being situated dorsal to the normal stomach and communicating with it at three separate points--fornix, body and antrum, and also through a duodenal diverticulum. In addition the authors describe complete duplication of the bulb and descending part of the duodenum, connecting through a duodenal diverticulum. The pathogenesis of these duplications and their relationship to duodenal diverticula is discussed.

Association of HFE protein with transferrin receptor in crypt enterocytes of human duodenum.

In hereditary hemochromatosis (HH), intestinal absorption of dietary iron is increased, leading to excessive iron accumulation in tissues and resultant organ damage. The HFE protein, which is defective in HH, normally is expressed in crypt enterocytes of the duodenum where it has a unique, predominantly intracellular localization. In placenta, the HFE protein colocalizes with and forms a stable association with the transferrin receptor (TfR), providing a link between the HFE protein and iron transport. In the present study, we examined the relationship of the HFE protein to the TfR in enterocytes of the human duodenum and measured the uptake of transferrin-bound iron and ionic iron by isolated crypt and villus enterocytes. Immunocytochemistry showed that the HFE protein and TfR both are expressed in the crypt enterocytes. Western blots showed that, as was the case in human placenta, the HFE protein in crypt enterocytes is physically associated with the TfR and with beta2-microglobulin. The crypt cell fraction exhibited dramatically higher transferrin-bound iron uptake than villus cells. On the other hand, the villus cells showed 2-3 times higher uptake of ionic iron than crypt cells. We propose that the HFE protein modulates the uptake of transferrin-bound iron from plasma by crypt enterocytes and participates in the mechanism by which the crypt enterocytes sense the level of body iron stores. Impairment of this function caused by HFE gene mutations in HH could provide a paradoxical signal in crypt enterocytes that programs the differentiating enterocytes to absorb more dietary iron when they mature into villus enterocytes.

The contribution of protozoa to the protein entering the duodenum of sheep.

1. Four sheep, each fitted with a rumen fistula and a re-entrant cannula at the proximal duodenum were fed a semi-purified diet containing urea as the only nitrogen source. The quantities of total protozoal amino acid-N (TPAN) present in the rumen and entering the duodenum were determined when the mean rumen dilution rate (D) was low (0.034/h) and when D was increased to 0.078/h by the intraruminal infusion of artificial saliva. 2. Increasing the dilution rate had no significant effect upon the proportions of TPAN present in the total microbial amino-acid-N (TMAN) of the rumen fluid and duodenal digesta. With both dilution rates the mean proportion of TPAN in the duodenal TMAN (0.24) was markedly less than the equivalent proportion (0.45) found in the rumen fluid. 3. The daily flow of TPAN, as measured at the duodenal cannula at both dilution rates were equivalent to only 41% of the flow of TPAN as predicted from measurements of rumen outflow, indicating that a substantial proportion of rumen protozoal protein was retained within the rumen.

Concurrent studies of the flow of digesta in the duodenum and of exocrine pancreatic secretion of calves. 7. Influence of milk substitutes on abomasal lipolysis and biliary secretion.

1. The abomasal hydrolysis of lipids and the flow of endogenous (biliary) lipids was studied in two Friesian calves given four milk-substitute diets, by sampling the duodenal digesta. The diets were: reconstituted, mildly preheated, spray-dried skim-milk powder with (SKF) or without (SK) margarine fat or with 500 g/kg skim-milk powder in diet SKF replaced by soy-beran flour (ASKF) or fish-protein concentrate (BSKF) together with dried whey. The diets were given ad lib. twice daily from 13 to 37 d of age, each diet being given for six consecutive days. Collections of duodenal digesta from the re-entrant cannula situated caudal to the bile duct were made for 12 h after feeding the 6th and 12th meals for each diet. Samples from one collection only were subjected to detailed analysis of the lipid classes. 2. The inclusion of non-milk protein (ASKF and BSKF) not only increased the rate of passage of lipid through the abomasum but also the proportion of the lipid present as triglyceride particularly, in the first 2 h after feeding. 3. In a 12 h period, 2.3-6.3 g 'polar' lipids (mainly biliary phospholipids) were estimated to have been secreted. The rate of flow was high during the first hour after feeding and constant thereafter. The quantity of 'polar' lipid was not related to the type of milk fed or the duodenal flow of lipid. 4. When diet SK was fed, the small amounts of lipid present were extensively hydrolysed so that free fatty acids represented 700 g/kg lipid of dietary origin passing through the duodenum. When margarine fat was included in the diets (SKF, ASKF and BSKF), the free fatty acids represented only 210 g/kg lipid of dietary origin. 5. The quantities of lipid and nitrogen passing through the duodenum were poorly related to the quantities ingested at the beginning of the 12 h experimental period but were closely related to each other.

Feeding responses to infusions of glucose solutions into the duodenum of cockerels and the influences of pre-fasting or vagotomy.

1. The duodenum of cockerels was chronically catheterised, to study the effects of 3 h infusions of glucose solutions on voluntary food intake. 2. Compared to water, solutions containing 30, 60 and 90 g glucose/l infused at 1 ml/min induced significant, dose-related depressions in intake after the first 30 min of infusion. 3. Fasting the birds for 3 h before the start of infusion resulted in higher intakes during the period of infusion than in unfasted birds. 4. Vagotomy at the level of the proventriculus prevented the inhibitory effects of glucose infusion into the duodenum, except at the highest concentration (120 g/l).

Electrogenic ion transport in duodenum, an aid in cystic fibrosis diagnosis.

BACKGROUND: Abnormality in chloride transport across epithelial tissues is a basic defect in cystic fibrosis (CF). Our aim was to compare the induced chloride secretion in duodenum in CF patients with different mutations. METHODS: Duodenal biopsies from 9 patients were investigated in a modified Ussing chamber and the secretory response to prostaglandin E2 (PGE2) and acetylcholine (ACh) were measured. RESULTS: PGE2 and ACh induced no changes in chloride secretion in the AF508 homozygotes. In heterozygotes the induced change in chloride secretion corresponded to the severity of the known mutations. CONCLUSION: The secretory response in duodenum in CF is influenced by the patients genotype and mainly related to sweat chloride secretion.

[Effect of different drinking frequencies and milk treatments on the digestive flow in the duodenum of the calf].

The digesta dry matter and N-passage rates at the duodenum were measured at 3-to-8-week-old calves with duodenal re-entrant cannulae within 24 hours. The animals received the amount of milk in 1,2 and 4 meals. The milk enriched with 2% fat was given fresh, treated with hydrochloric acid or with formaldehyde. The calves received their feed in buckets. The feeding frequency influenced the chyme- and N-passage rates. After the feeding of 7 l milk in 4, 2 and 1 portions resp. up to 1,200; 1,600 and 2,200 ml chyme resp. passed the duodenum in the first hour after the feeding. More frequent feeding thus guarantees a more continuous flow of the chyme. On average the amount of chyme reached 180 . . . 186% of the amount of milk taken in independent of the feeding frequency whereas the N-amount corresponded to that contained in the feed. Formaldehyde treatment increased the amount of chyme to more than 200% of the milk intake and disturbed the coagulation process in the abomasum. The souring of the milk supported the effective protein hydrolysis in the abomasum due to a quick decrease of the pH-value and, after the feeding in 1 portion, prevented an "overflow" of the abomasum.

[Protein and amino acid metabolism in the digestive tract of growing young bulls. 4. Apparent digestibility of NH3-free raw protein reaching the duodenum and critical evaluation of apparently digestible raw protein].

The experimental determination of the apparent digestibility (AD) of the NH3-free crude protein at the duodenum (NH3 fr. CPD) showed a value of 65.7% at a standard error of +/- 3.6% for the section of the digestive tract between proximal duodenum and end of rectum. This standard error can only be insignificantly diminished due to the existing relation between AD of NH3 fr. CPD in dependence on the CP concentration in the ration (y = 56.4 + 0.598 x +/- 3.17; r = 0.501xx) so that this relation has no practical significance. The criticism of AD CP is correct from a point of view of digestion physiology. There is only agreement between AD PC and AD NH3 fr. CPD in a CP concentration range between 12.5 and 14.0% CP in the DM, which, however, applies to a large range of the production sphere in the rearing of young bulls. It excludes, however, the low live weight range with high live weight gains.
