The cases of two patients with severe intoxication with organophosphorus pesticides and heavy gastroduodenal complications are presented. In the first patient an acute gastric ulcer with perforation and diffuse peritonitis developed and the patient died on the third day. In the second patient two post bulbar duodenal ulcers develops with a hemorrhage from one of them. The patient was successfully operated but after 4 days a relaparotomy was performed because of obstructive ileus. He was discharged from the hospital in a good general condition after an 80 day treatment. The possibility of such severe gastroduodenal complications of acute intoxication with organophosphorus pesticides is pointed out. Urgent fiber gastroduodenoscopy and timely operation are essential for the favourable outcome of the disease.
Pancreatitis secondary to organophosphate insecticide toxicity is extremely rare. The few previously reported cases have resulted from oral ingestion of these agents. We present the first reported case of acute pancreatitis occurring after cutaneous exposure to an organophosphate insecticide. Symptoms of pancreatitis persisted for 6 months, followed by total resolution. The proposed mechanisms of pancreatic injury caused by organophosphate compounds are discussed.
