A formalised and closer strategic partnership between a health authority and the healthcare industry has brought benefits to both sides. Joint working with seven companies has been used to develop an osteoporosis framework for the district, resulting in distribution of a comprehensive resource pack. Lessons have been learned on the way, and the project is being evaluated.
Peduncular hallucinosis is usually caused by infarction of the mesencephalic portion of the brainstem, and is characterised by acute onset of vivid, colorful hallucinations often consisting of moving patterns, people or animals, which the patient is aware are unreal. Charles Bonnet's syndrome is a similar condition, caused by impaired version. Both syndromes may be explained by visual release phenomena caused by lesions in different parts of the visual system. The article consists in two case reports of peduncular hallocinosis and a review of partinent literature.

Although "managed care" means different things to different people, it has become an icon for health care change and is commonly cited as the principle cause for the physician's lament. Successful leaders are finding ways to address the signs, symptoms, and causes of the physician grief reaction to the tumultuous marketplace changes affecting how they view themselves as professionals and how they practice medicine. Health care CEOs continue to place the search for solutions, physician-hospital integration, and the alignment of system and physician incentives as their number one strategic priority. This article reviews common sources of anger and identifies potential solutions. Strategies for dealing with the anger include: (1) recognizing the issues, causes, signs, and symptoms; (2) restoring balance and power; (3) developing new economic partnerships; (4) managing the organizational culture; and (5) educating one another and developing leadership.

This article is a follow-up to an interview with Charles Dwyer, PhD, which appeared in the 1999 March/April issue of The Physician Executive. He described how physician executives can change the perceptions of today's beleaguered physicians and help them cope with change. We then asked him for some hands-on strategies to deal with physician anger, fear, and resentment. After much contemplation on providing a list of "fixes" that will restore each of us to a state of greater satisfaction, Dr. Dwyer concludes that there are no generalizable solutions because there are too many variables that come into play in each organization, individual, or group. Attending to the self can provide both individual rescue from these turbulent times and the best hope for changes in the system from which patients and health care providers can benefit. If physicians are to regain their power and maintain, or even improve, their quality of life, clearly changes are called for. And these are changes that require persistent effort and uncomfortable adjustments.
