Absence of extracellular enzyme activity and cytotoxicity in cell-free culture filtrates of Haemophilus ducreyi.

Ten isolates of Haemophilus ducreyi, including seven clinical isolates and three laboratory reference strains, were assessed for their ability to produce extracellular enzymes which might contribute to their pathogenicity. Protease, elastase, lecithinase, lipase or collagenase enzyme activity were not detected in culture filtrates from any of the isolates tested using either plate or spectrophotometrical assays. Furthermore, cell-free culture filtrates did not exhibit cytotoxic activity in vitro when tested using either an established tissue culture cell line (Vero) or a primary cell culture of human keratinocytes. These results suggest that extracellular products do not play a role in host invasion or production of tissue damage by H. ducreyi.

Sexually transmitted diseases causing genital lesions in adolescents.

Evaluating the adolescent with a genital lesion requires a thorough knowledge of the common and less-common STDs that cause genital findings. A thoughtful history and complete physical examination are always in order, as is testing for a particular suspected STD and for coexisting STDs, especially HIV.

Expression of OmpP2A and OmpP2B is not required for pustule formation by Haemophilus ducreyi in human volunteers.

Haemophilus ducreyi express two porin proteins, termed OmpP2A and OmpP2B. To test whether expression of OmpP2A and OmpP2B was necessary for virulence in humans, eight volunteers were experimentally infected with the parent (35000HP) in one arm and a double OmpP2A OmpP2B mutant (35000HP::P2AB) in the other arm. The pustule formation rates were 58.3% (95% CI, 33.2-83.5%) for the parent and 41.7% (95% CI, 19.3-64.0%) for the mutant (P=0.25). Biopsy of 35000HP and 35000HP::P2AB-infected sites yielded similar amounts of bacteria in quantitative culture. These results indicate that expression of OmpP2A and OmpP2B is not necessary to initiate disease or to progress to pustule formation in humans.

Immunoglobulin M and G antibody response in rabbits after experimental Haemophilus ducreyi infection.

The enzyme-linked immunosorbent assay and immunoblot technique were used to investigate the humoral immune response following intradermal infection of rabbits with Haemophilus ducreyi. Basically, the kinetics of IgM and IgG antibody synthesis was the same as in other bacterial infections. However, antibody titres in the enzyme-linked immunosorbent assay were remarkably low. In the immunoblot experiments, antibodies of both immunoglobulin classes were mainly directed against Haemophilus ducreyi antigens with relative molecular masses of 79,000, 62,000, 55,000, 49,000, and 26,000, suggesting that these antigens may play a dominant role in the humoral immune response after infection.

Treatment of sexually transmitted diseases in women.

Guidelines for the treatment of patients with sexually transmitted infection are developed by the Centers for Disease Control and Prevention after consultation with a group of professionals knowledgeable in the field. This article briefly introduces various infections, reviews new diagnostic information, and presents the latest guidelines for therapy. All recommended and alternative regimens are drawn from the most recent treatment guidelines. Although this article focuses primarily on therapy, it also emphasizes the importance of counseling and prevention. Clinicians have the opportunity and obligation to provide education and counseling to patients. Prevention messages should be tailored to the individual patient with consideration given to her specific risk behaviors.
