Profile validity standards for MMPI and MMPI-2 F scales.

Although numerous indices of validity have been developed for the MMPI and MMPI-2, interest in the F scale and its variants continues, especially among practicing clinicians. The use of the binomial for assessing standards for random answering and possibly for judgments of malingering offers another approach for the interpretation of F-scale scores. The theoretical binomial distribution and Monte Carlo data are in accord. Cut-off scores of 24 for the MMPI and 23 for the MMPI-2 suggest random responses, and scores of 40 and 37, respectively, suggest clinical interpretation rather than randomness of responding.

Usefulness of subjective ovine milk scores: II. Genetic parameter estimates.

Genetic parameters for a subjective milk score given to ewes within 24 h of parturition were estimated to determine the usefulness of milk score as a selection trait to improve milk production, which influences total litter weight weaned. Heritability of milk score and the genetic correlation of milk score with litter weight weaned were estimated by REML separately for four sheep breeds, Rambouillet (n = 1,731), Targhee (n = 1,638), Columbia (n = 1,731), and Polypay (n = 1,129). Litter weight weaned was the total weight of lambs weaned at approximately 120 d of age under a western range production system. Observed heritability estimates for milk score at first parity were moderate and similar among breeds, ranging from 0.18 to 0.32. Heritability estimates adjusted for a binomial distribution of milk scores at first parity were high (Columbia, 0.43; Polypay, 0.35; Rambouillet, 0.50; Targhee, 0.84). Estimates of observed heritability for second-parity milk score were moderate to high, ranging from 0.23 to 0.46. Milk score at first or second parity was genetically correlated with milk score records at maturity (third parity and greater), with estimates ranging from 0.69 to 1.00. Milk score and litter weight weaned were genetically correlated at first or second parity in Rambouillet (r(g) = 1.00) and Targhee breeds (r(g) = 1.00 and 0.61, respectively), but not in the Columbia and Polypay breeds. Estimates of heritability for lifetime records for milk score ranged from 0.16 to 0.26 across breeds. Estimates of genetic correlations of annual lifetime milk score records with litter weight weaned were high (Columbia, 1.00; Polypay, 0.81; Rambouillet, 1.00; and Targhee, 0.77). Repeatability estimates for milk score were similar across breeds, 0.23 for Columbia, Rambouillet, and Targhee ewes and 0.28 for Polypay ewes. Milk score measured at first or second parity may be a good predictor of future potential milking ability. Further, milk score can be used as a selection trait to improve maternal ability for increasing litter weight weaned. The need for increasing ewe milking performance and lamb growth rate at first parity in commercial range sheep production systems may be addressed by selection for milk score at first parity.

Between-breed variability of stillbirth and its relationship with sow and piglet characteristics.

Litter characteristics at birth were recorded in 4 genetic types of sows with differing maternal abilities. Eighty-two litters from F(1) Duroc x Large White sows, 651 litters from Large White sows, 63 litters from Meishan sows, and 173 litters from Laconie sows were considered. Statistical models included random effects of sow, litter, or both; fixed effects of sow genetic type, parity, birth assistance, and piglet sex, as well as gestation length, farrowing duration, piglet birth weight, and litter size as linear covariates. The quadratic components of the last 2 factors were also considered. For statistical analyses, GLM were first considered, assuming a binomial distribution of stillbirth. Hierarchical models were also fitted to the data to take into account correlations among piglets from the same litter. Model selection was performed based on deviance and deviance information criterion. Finally, standard and robust generalized estimating equations (GEE) procedures were applied to quantify the importance of each effect on a piglet's probability of stillbirth. The 5 most important factors involved were, in decreasing order (contribution of each effect to variance reduction): difference between piglet birth weight and the litter mean (2.36%), individual birth weight (2.25%), piglet sex (1.01%), farrowing duration (0.99%), and sow genetic type (0.94%). Probability of stillbirth was greater for lighter piglets, for male piglets, and for piglets from small or very large litters. Probability of stillbirth increased with sow parity number and with farrowing duration. Piglets born from Meishan sows had a lower risk of stillbirth (P < 0.0001) and were little affected by the sources of variation mentioned above compared with the 3 other sow genetic types. Standard and robust GEE approaches gave similar results despite some disequilibrium in the data set structure highlighted with the robust GEE approach.

LS-CAP: an algorithm for identifying cytogenetic aberrations in hepatocellular carcinoma using microarray data.

Biological techniques such as Array-Comparative genomic hybridization (CGH), fluorescent in situ hybridization (FISH) and affymetrix single nucleotide pleomorphism (SNP) array have been used to detect cytogenetic aberrations. However, on genomic scale, these techniques are labor intensive and time consuming. Comparative genomic microarray analysis (CGMA) has been used to identify cytogenetic changes in hepatocellular carcinoma (HCC) using gene expression microarray data. However, CGMA algorithm can not give precise localization of aberrations, fails to identify small cytogenetic changes, and exhibits false negatives and positives. Locally un-weighted smoothing cytogenetic aberrations prediction (LS-CAP) based on local smoothing and binomial distribution can be expected to address these problems. LS-CAP algorithm was built and used on HCC microarray profiles. Eighteen cytogenetic abnormalities were identified, among them 5 were reported previously, and 12 were proven by CGH studies. LS-CAP effectively reduced the false negatives and positives, and precisely located small fragments with cytogenetic aberrations.

Sex ratio of male sex preselected children born to couples with exclusively female offspring.

The present study was undertaken to determine if couples with exclusively female children had a decreased probability of a male child after using male sex preselection. Selection criteria for subjects necessitated that couples have had only female children previously and had produced a child at one of 14 centers after using protocol 3 (n = 70) and modified 3 (n = 28) male sex preselection. Prior to sex selection, protocol 3 couples produced a combined total of 135 female children for an average of 2.01 (range 1-4) females per couple; modified 3 couples produced a combined total of 62 female children for an average of 2.21 (range 1-4) females per couple. The normal approximation to the binomial distribution was used to determine significant differences between the sex ratios prior to and after male sex preselection. For couples using protocol 3 there were significant differences in the sex ratio prior to sex preselection (0%) and after sex preselection (73.0%) (p less than .00003). There were also significant differences between the sex ratio prior to modified 3 (0%) and after sex preselection (86%) (p less than .00003). Couples using male sex preselection do not have a decreased probability of a male child if they have had exclusively female children.

Normal and hearing-impaired word recognition scores for monosyllabic words in quiet and noise.

The effects of noise on word recognition scores were assessed with normal-hearing and hearing-impaired subjects. Fifty-one normal-hearing subjects were tested at 50 dB HL using signal-to-noise ratios (S/Ns) of 5, 10, and 15 dB. Thirty subjects with mild-to-moderate sensorineural hearing losses were tested in quiet and in noise at S/Ns of 10 dB and 15 dB. Monosyllabic words in a Multitalker Noise were selected for testing. Mean scores for the normal-hearing subjects were 45% at the 5 dB S/N, 74% at the 10 dB S/N, and 87% at the 15 dB S/N. For the hearing-impaired subjects, scores were 85% in quiet, 60% at the 15 dB S/N, and 40% at the 10 dB S/N. These results suggest that background noise which is mildly disruptive for normal hearing subjects can be highly disruptive to hearing-impaired subjects. Moreover, these findings indicate that subjects with mild-to-moderate sensorineural hearing loss require a more favourable S/N than normal listeners to achieve comparable word recognition scores. Test-retest differences for word recognition scores revealed variability that agreed closely with predictions based on the binomial distribution for both groups of subjects. Speech-in-noise abilities must be measured directly because regression equations revealed that speech-in-noise scores cannot be predicted accurately from either puretone thresholds or speech-in-quiet scores. Word recognition functions are presented from several hearing-impaired subjects and demonstrate the value of testing in noise.

Evaluation of overall reproductive performance of dairy herds.

Reproductive efficiency was assessed in 106 commercial dairy herds in Ontario. Herds that were similar in historical calving interval or projected calving interval varied widely in reproductive culling rate. The correlation between the reproductive culling rate and the historic calving interval and between the reproductive culling rate and the projected calving interval were -0.23 and were not significant. The coefficient of variation associated with the reproductive culling rate (75.7%) was much higher than that of the historical calving interval (3.8%) or the projected calving interval (4.5%). The repeatability of the reproductive culling rate (0.26) was much lower than that of the historical calving interval (0.81) or the projected calving interval (0.60) because culling rate was not only influenced by the overall herd reproductive performance, but also by the differences in the maximal allowable days open for rebreeding and the estimation of which cows were culled for reproductive failure. Additionally, a high coefficient of variation for the reproductive culling rate is unavoidable because this variable has a binomial distribution and a low mean value (7.5%). These disadvantages are not sufficient to exclude this measure from a comprehensive assessment of overall herd reproductive performance. The projected calving interval and the reproductive culling rate can be combined into the adjusted calving interval and thus interpreted jointly. On 40.5% of the farms providing data for this study, fertility assessment based on the projected calving interval indicated better reproductive performance than that based on adjusted calving interval. Hence, the assessment of herd fertility on the basis of projected calving interval can lead to a different and erroneous conclusion regarding the level of herd fertility than that obtained with the evaluation based on adjusted calving interval.

Egg Discharging Patterns Of Ascaris Lumbricoides In Low Worm Burden Cases.

In Ascaris lumbricoides infection, the faecal examination, undertaking for ova detection, is not always diagnostic. It is just because some of the infected cases has Ascaris worm(s) which do not produce eggs. In the present study, the authors attempted to analyse quantitatively the egg discharging patterns in Ascaris infected cases with low worm burden. The following results were obtained: 1. In 1,861 Ascaris worms collected from 853 cases, the sex ratio was 1: 0.74 (female: male). Sex combinations in each burden of case were always fitted with theoretical values from the binomial distribution; (m+f)(n)= (0.426+0.574)(n). 2. In each worm burden, their sex combination indicated different egg discharging patterns; false negative cases, unfertilized ova passers and fertilized ova passers. When the relative frequency of the above three egg discharging patterns was plotted to worm burden per case, a definite relationship was found. The cases with six or more worms have nil probability to be false negative case or unfertilized ova passer. 3. Out of 853 cases, we found 129 false negative cases. The collected worms from 125 cases were morphologically analysed. It was found that 52% of them were infected with only male worm(s) and 24% were infected with young female worm(s). And in 8.8%, old female(s) with empty uterus were infected. The cause of 15.2% was remained unexplained, even though the collected worms were scrutinized.

Selection Criteria Of Subjects For Drug Evaluation In Enterobiasis.

When an antipinworm drug was evaluated in human being, the results are still variable according to authors. Among the various factors involved in these variable results, the selection of subjects must be one of them. In the past, in evaluating a drug, the subjects were simply anal swab positive cases in a population. By that kind of selection method, especially in low endemic population, anal swab positives spontaneously turned to egg negatives at random. Moreover, the time, frequency of negative conversion cannot be predicted. By the results, the cure rates obtained from such ill-defined subjects are variable according to the subjected population. Another problem is that evaluation should represent the efficacy in heavily infected cases. To reinforce and overcome such shortcomings, the evaluator shoud select the cases wha are infected with whole developmental stages of worm. And the evaluator should have a method selecting such ideal cases only with anal swabs. To find out such method the results of anal swabs of long-term follow-up without any antipinworm treatment were reevaluated. In 41 children from 6 orphanages of different endemicity, swabs were taken for 8 or 21 times during 36 or 91 days. The positive rate of follow-up swabs was compared with that of already obtained in 4 preliminary swabs. And we collected other data on the combination of 4 anal swab results in different endemicity. The results are as follows: 1. The positive rate of follow-up swabs was significantly different by the consecutiveness of egg positivity in preliminary swabs as well as the endemicity of subjected population. The consecutively positive cases in higher endemicity over 60% showed the highest, and the least variable positive rate in follow-up swabs. 2. The observed frequency of consecutively positive/consecutively negative combination was always higher than the expected one obtained by binomial distribution equation. This indicates that individual hygiene and previous infection are important factors in determining the actual occurrence of subsequent reinfection. Summarizing these results, the subjects for drug evaluation should be those of consecutively positive cases in population over 60% endemicity. These subjects have the lowest chance of spontaneous negative conversion at random period of follow-up swabs. Eligible cases for such criteria would be sufficiently secured in highly endemic population.

Seasonal variations of metacercarial density of Clonorchis sinensis in fish intermediate host, Pseudorasbora parva.

The seasonal variations of the rate and intensity of metacercarial infection of C. sinensis in P. parva were observed. The fish were collected at Sun-Am river which located in Kim-Hae City, Kyong-Sang-Nam Do(=Province), Korea, from March 1983 to February 1984 every month. A total of 788 fish was examined. The number of metacercariae in each fish was individually counted after the individual digestion by artificial gastric juice. The result was as follows: During one year, 513(65.1%) out of 788 fish were infected with metacercariae. In May, June, July and September, the infection rates ranged from 82. 0 % to 98. 6% whereas the rates was relatively low in March, April, November and February raning from 11. 4% to 64.7%. The intensity of infection was similar with those of infection rates. The mean intensity per infected fish was 103.0 and standard deviation was 118.9 throughout one year. The highest mean intenstiy was in June(294. 8) and the lowest in Novebmver(11.1). The observed frequency of fish with certain intensities of metcercariae were fitted to theoretical equations derived from negative binomial distribution in March, April, November and February(p > 0.05). Meanwhile, the equation of lognormal distribution were fitted with the observed frequencies in May, June, July and September(p > 0.05, p > 0.75). The variance/mean ratio varied by month. The value was the highest in July(814.3) and the lowest in November(158.8). Unlike our hypothesis, the metacercarial density of Clonorchis sinensis in its the most favourable fish host, Pseudorasbora parva showed considerable seasonal variations in the hyperendemic area. The possible factors were discussed.

Prevalence, worm burden and other epidemiological parameters of Ascaris lumbricoides infection in rural communities in Korea.

The epidemiological status of ascariasis was analyzed in 8 rural villages in Korea, through observation of its epidemiological parameters such as prevalence, worm burden and basic reproductive rate. Total 978 inhabitants were subjected to stool examination and recovery of worms after chemotherapy with pyrantel pamoate. The results were as follows: The worm positive rate in each village was 16.5~79.5%, while the egg positive rate was 9~18% lower, 3.3~66.7%. The average worm burden(among all inhabitants) ranged from 0.21 to 8.44 by villages and the frequency of cases with each worm burden showed negative binomial distributions with 'k' values of 0.38-0.54. The prevalence rates (worm) in each village was almost identical with the theoretical ones from Anderson and May's equation; p=1-(1+M(*)/k)**(-k), where 'p' is worm prevalence and 'M(*)' equilibrium average worm burden. The basic reproductive rate 'R' was calculated from 1.03 to 2.11. It is suggested that, although 'R' in lower endemic areas is approaching to the breakpoint of reinfection (R=1), control programs of ascariasis in Korea should be continued until it becomes below the level nationwidely.

Second Generation Binocular Polaroid Test.

BACKGROUND: The aim of the study was to introduce a new test to detect small areas of suppression in the binocular visual field and to lower the minimum age at which a suppression test could be identified correctly. Two tests were compared: the Binocular Polaroid Test (poltest), introduced in clinical practice a few years ago and used in screening programs for vision impairment; and the Second Generation Binocular Polaroid Test (polstar), more recently designed to have simpler answers than the poltest. METHODS: One hundred and eighty-two children were examined consecutively. To assess the validity of the two tests, sensitivity, specificity, predictive value, and phi coefficient were calculated. The applicability of the two tests and the speed of the responses was assessed by means of the test of probability of binomial distribution. RESULTS: The polstar has appeared to be simpler and easier in younger patients (age range 20 to 36 months, P = 0.078), and has shown 100% of specificity with a sensitivity of 84% for the polstar 1 and of 100% for the polstar 2. CONCLUSION: The results appear to demonstrate that the poltest has been improved, and that the polstar may be considered an interesting investigation device for early detection of monocular visual impairment.

Variations in fecal Schistosoma japonicum egg counts.

Variations in fecal Schistosoma japonicum egg counts were studied in ZhuXi administrative village, JiangXi Province, China. Population stool examinations were collected with duplicate, standard, 41.5-mg Kato-Katz thick smears on seven consecutive days for 570 individuals from two natural (individual) villages: village I with high endemicity and village II with low endemicity. The proportion of individuals with at least one positive count increased from 42.4% after a single measurement to 68.3% after seven measurements in village I (n = 356), and from 17.0% to 36.0% in village II (n = 214), respectively. This demonstrates a very high variation in repeated S. japonicum egg counts and a considerable lack of sensitivity of the Kato-Katz technique; light and moderate infections are especially missed with a single or a few measurements. The observed day-to-day variation in individual egg counts is highly aggregated (variance higher than the mean) and suggestive of a negative binomial distribution. For five individuals on three days, repeated sampling from different locations of a stool specimen shows a clear trend with egg counts decreasing from the beginning of the stool to the end and from the outside layer to the center. Ten multiple samples from a particular subsection (10-30 g) of a stool specimen for 44 positive individuals still showed aggregation in egg counts, particularly for high intensities of infection. This means that the aggregation in repeated daily S. japonicum egg counts cannot be explained alone by a specific day-to-day component and variation in the concentration of eggs at different locations in the stool. There also exists clustering of eggs within parts of the stool.

Association between pleural plaques and coronary heart disease.

OBJECTIVE: The aim of this study was to verify a clinical impression that patients with coronary heart disease disproportionately frequently have calcified pleural plaques. METHODS: Chest X-rays were collected from 148 patients referred consecutively to the Helsinki University Central Hospital for coronary angiography and from 100 consecutive lung cancer patients seen at the same hospital. The radiographs were analyzed for the presence of calcified pleural plaques according to the classification the International Labour Office. A generalized linear model with binomial distribution and log link was used to estimate the relative risks and their 95% confidence intervals (95% CI). RESULTS: The prevalence of calcified pleural plaques was 35% for the coronary patients and 19% for the lung cancer patients. Calcified pleural plaques were more common among the men than the women, and the risk increased with age. The relative risk of calcified pleural plaques, adjusted for age and gender, was 2.19 (95% CI 1.44-3.32) for the coronary patients as compared with the lung cancer patients. CONCLUSIONS: Further studies with better information on past exposure to asbestos and other potential risk factors are warranted to confirm the observations and to examine whether the association between coronary heart disease and calcified pleural plaques is related to an etiologic or an individual susceptibility factor common to both of these conditions.

NPO after midnight before elective surgery is no longer common practice for the majority of anesthesiologists.

One thousand three hundred thirty-seven anesthesiologists randomly selected from the current American Society of Anesthesiologists' directory were surveyed to discover if their attitudes toward the policy of allowing preoperative patients nothing by mouth (NPO) have changed. One thousand forty surveys were returned. For each response to a survey question, the percentage of respondents giving that response was calculated. The 95% confidence intervals for these percentages were calculated by using the normal approximation of a binomial distribution. Chi-square analyses were used to explore possible relationships between demographic factors and NPO orders. Nearly all anesthesiologists surveyed used less than an 8-hour fast for elective pediatric surgery. At least 50% of the anesthesiologists surveyed no longer use NPO after midnight as an order for elective surgery for adults. Of the demographic variables, only geography and the question of whether or not a hospital was a teaching institution were statistically related to NPO practice.

[Hand clasping--an overview].

The literature concerning the asymmetry clasping hands is reviewed based on 192 studies. This paper describes the incidence, sex differences, age differences and genetical problems including twinning. The incidence of left hand clasping ranges from 30% to 75% (mean 43%). The review confirms the so-called east-west-gradient and there is a predominance of the left type in Europe. Age and sex differences are only small. There is only a small relationship between hand clasping and handedness. 18 authors examined hand clasping in families and 4 in twins. The family data suggest that hand clasping may be under genetic control, yet it is clear that no simple genetic model for the inheritance can be applied. Both monozygotic and dizygotic twins show a low concordance and the R-R, R-L and L-L pairs in monozygotic and dizygotic twins are in binomial distribution.

Length of pregnancy in African Americans: validation of a new predictive rule.

This study evaluated whether a new predictive rule is more accurate for estimating the length of pregnancy in African Americans than Nägele's rule, the accepted standard. After identifying women in early pregnancy, telephone interviews were conducted to obtain information about 16 previously established determinants of gestational length. Based on these data, a linear multivariate regression model was used to predict an estimated delivery date (EDD) for each mother. In addition, the EDD was determined using Nägele's rule. Later, the actual delivery date was compared with the EDD predicted by the new rule and with the EDD predicted by Nägele's rule. Each pregnancy was assigned to its better prediction group, either the new rule's group or the Nägele's rule group. Fifty-seven pregnancies were identified prospectively and monitored. The new rule predicted the actual delivery date more accurately in 66% (37/56) of pregnancies, Nägele's rule was a better predictor in 34% (19/56) of pregnancies, and both rules were equally accurate in predicting the delivery date for one pregnancy. The new rule was more precise than Nägele's rule (P = .022) when the binomial distribution was used. When using the linear regression model rule, a more accurate EDD can be determined for African-American women. Moreover, it is possible to predict the risk of preterm delivery (those occurring > 3 weeks earlier than the EDD).

The parasites of the badger (Meles meles) in the north of Mugello (Florence, Italy).

During the period January 1993-June 1994, a parasitological survey was carried out on 19 badgers (Meles meles) road killed in Northern Mugello (Florence). The following helminths (together with their prevalence) were isolated and classified: Uncinaria criniformis (84.2%); Capillaria sp. (31.6%); Molineus patens (21.1%); Mesocestoides melesi (21.1%); Aelurostrongylus falciformis (52.6%); Crenosoma melesi (21.1%). According to results, only sex related differences in prevalence were studied. The parasite biocenosis is composed exclusively by dominant and codominant species. Mesocestoides melesi represents the first record for Italy. All the species found fitted a negative binomial distribution.
