Textile rental companies are poised on the edge of a boom in the healthcare marketplace. The detonator is outpatient care. Between 1984 and 1990, outpatient visits increased from almost 233 million to more than 310 million, or about 33 percent. The shift from traditional hospital care to outpatient or ambulatory service is creating a new market opportunity for textile rental services. Innovative medical services are cropping up in suburban malls and neighborhoods, and demand is high for high-quality linen supply or contract laundry services.

Hospital linen managers can reduce costs most effectively by devising and imposing controls on linen operations rather than by merely limiting linen usage. Such controls include use of work load timetables for the laundry department, an itemized charge system for each linen-using department, standards for daily usage, and immediate investigation of deviations from the standards.
Total quality improvement (TQI) advocates that all staff members in an organization develop their own ideas on job improvement about their own specific jobs. This process helps to improve staff performance and to build continually on those improvements. This article will describe how the TQI process was used successfully by quality management staff members at a federal medical center to investigate a problem with linen.
Materials Requirements Planning (MRP) has been used extensively in manufacturing and other industries to improve on-time delivery and to reduce costs. In this paper, we illustrate how an MRP-type system was developed to monitor surgical linen at a large teaching hospital. We also describe a bar-code scanning 'tag and recapture' study to estimate total inventory. The hospital implemented several changes based upon our recommendations that resulted in time savings and a smoother flow of materials throughout the surgical linen supply chain.

An investigation into two cases of post-operative Bacillus cereus meningitis revealed that hospital linen laundered by a batch continuous washing machine was heavily contaminated by B. cereus spores. The washing machine, detergents, other chemical additives and the water supply were eliminated as the source of contamination. It was found that the linen introduced into the washing machine had a high B. cereus spore content and that this was still present after the wash process. The spores were not killed by either the heat disinfection stage of the wash or the addition of chemical disinfectants and were not removed by the dilution in the process. The multiplication of B. cereus was thought to have occurred on used, damp linen stored in plastic bags, particularly when ambient temperatures were high. An increase in the water flow through the washing machine was the only measure associated with a decrease in B. cereus on laundered linen.
