[Alcohol intoxication of children (author's transl)].

This study is based on 116 blood alcohol estimations of intoxicated children up to 14 years of age; all these children recovered after several hours of clinical treatment. In 84 cases the data about the anamnesis, the neurological symptoms, and the clinical course could be evaluated in detail. The average age of children was 10 years; most of them (60%) were 11--14 years old. Sex distribution: 4 male: 1 female. The average blood alcohol concentration was 1.4% at the moment of blood sampling; it ranged between 0.5 and 2.9%. No constant correlation was found between the blood alcohol concentration of the children, their age, sex, and the intoxication symptoms. As a result of this study it may be stated that there seems to be no special intolerance for alcohol during infancy. Specific aspects of the alcohol intoxication of children, i.e., the rapid passage through the stadium of excitation may be explained by the very short drinking time and the relatively large amount of alcohol ingestion combined with no experience about the effects of C2H5OH.

[Indicators for alcohol abuse in female drunk drivers. (Comparative study with alcohol intoxicated male automobile drivers)].

Women account for about 12% of all cases of drink driving in German cities. Little is known about their drinking behaviour and the extent of their alcohol misuse. In order that these questions may be clarified, three major studies were evaluated. In these studies, additional indicators for alcohol misuse such as GGT or methanol and acetone+isopropanol or indeed CDT, GGT, methanol and acetone+isopropanol were measured in blood samples. The results were compared with studies of the same kind on male car drivers. There were no significant differences in the ages of males and females. However, there were no differences at all regarding the frequency of the blood alcohol levels or the CDT. Indicators levels of chronic alcohol abuse, such als GGT levels above 70 U/L, methanol levels above 10.00 mg/kg or acetone+isopropanol concentrations above 9.00 mg/kg were approximately the same with women as they were with men. This demonstrates an increasing similarity amongst male and female DWI drivers regarding the drinking behaviour and alcohol abuse.

[The effect of polyamine on the structure of the free amino acid pool in the blood plasma and in the liver of animals with chronic alcoholic intoxication].

The effect of polyamine on the total content of free amino acids and the structure of the amino acid fund in blood plasma and hepatic tissue was studied in rats with chronic alcoholic intoxication. Rapid utilization of exogenic introduced amino acids and a normalizing effect of the drug on the structure of the amino acid fund were demonstrated. The high concentration of aromatic amino acids in the drug may be among the causes of the increased content of this fraction of amino acids in the tissues studied after administration of the drug.

Studies on the size of human male gonad in biomorphosis, alcohol intoxication, and cirrhosis--a review and own findings.

The size of the testicle (absolute weight, relative weight as the index X, length, and width was measured in 142 late patients aged 17 to 87 a (mean age = 54 a) serving as controls, in 193 deceased patients aged 20 to 81 a (mean age = 47 a) with a history of alcoholism, and in 136 defunct patients aged 30 to 92 a (mean age = 59 a) with cirrhosis. A significant difference in the mean size of the testicles was demonstrated between the cirrhosis group and both the control and alcoholism groups. No statistically significant differences was observed between the control group and the alcoholism group. The degree of acute alcoholic intoxication has no significant effect on the degree of testicular atrophy. Chronic alcoholism exerted a more significant effect on the degree of testicular atrophy than acute alcoholism. Coexistent cirrhosis increased this difference even more. The type of cirrhosis and the degree of its inflammatory activity had no significant influence on the testicular size. Such effect was demonstrated, however, if diffuse fatty infiltration of the cirrhotic liver was present. In the biomorphosis, the greatest mean testicular size was observed in the 4th and 5th decade of life. From the 6th decade on, the testicular size decreased statistically significantly. The curve of the values of the mean testicular size in alcoholism was more flat than in the control group and was situated below it. No statistically significant differences among the values of the testicular size in this curve were found. The testicular size curve in cirrhosis was below both these curves, and a statistically significant difference was found between the 7th and the 9th decades. The mean weight of the testicules in controls, alcoholics, and patients with cirrhosis was 17.58, 16.49, and 13.25 g, respectively, the mean volume was 16.37, 15.34, and 12.21 ml, respectively, the value of the index X was 2.47, 2.52, and 1.97, respectively, the testicular length was 5.05, 4.08, and 3.61 cm, respectively, the width was 2.93, 2.87, and 2.70 cm, respectively.

IGF-I induced phosphorylation of S6K1 and 4E-BP1 in heart is impaired by acute alcohol intoxication.

BACKGROUND: The purpose of the present study was to determine whether acute alcohol (EtOH) intoxication impairs the signal transduction pathway used to coordinate insulin-like growth factor (IGF)-I stimulation of myocardial protein synthesis. METHODS: Rats were injected intraperitoneally with EtOH or saline. After 2.5 h, IGF-I or saline was injected intravenously and the heart was excised at 2 min or 20 min. Additional rats were pretreated with RU486 or tumor necrosis factor (TNF) binding protein (BP) to assess the importance of elevations in glucocorticoids or TNF, respectively, as endogenous modulators of IGF-I signal transduction. RESULTS: EtOH did not alter the total amount or tyrosine phosphorylation of the IGF-I receptor, IRS-1 or PKB under basal or IGF-stimulated conditions. However, EtOH attenuated the ability of IGF-I to phosphorylate ribosomal S6 kinase (S6K)-1 on residues T389 ( approximately 62%) and T421/S424 ( approximately 40%), and also reduced ribosomal protein S6 phosphorylation. Under basal conditions, EtOH altered the distribution of eukaryotic initiation factor (eIF) 4E, as evidenced by a decreased amount of the active eIF4E.eIF4G complex (53%), an increased amount of inactive eIF4E.4E-BP1 complex ( approximately 3-fold), and decreased phosphorylation of 4E-BP1 (56%). EtOH also impaired the ability of IGF-I to reverse the above-mentioned changes in the eIF4E system. Pretreatment of rats with RU486 or TNFBP was unable to attenuate the EtOH-induced changes in either eIF4E distribution or the phosphorylation state of 4E-BP1, S6K1 or S6. CONCLUSIONS: These data indicate that acute EtOH intoxication alters selected aspects of translational control in the heart under basal conditions. Furthermore, despite appropriate stimulation of IGF-I receptor, IRS-1 and PKB, EtOH impairs IGF-I signaling via S6K1 and 4E-BP1 pathways, and this defect is regulated in a glucocorticoid- and TNF-independent manner. This IGF-I resistance may represent a participating mechanism by which alcohol limits protein synthesis in heart.

Isopropyl alcohol intoxication: a diagnostic challenge.

Isopropyl alcohol (IPA) is an ingredient of commonly used household solutions. Accidental and suicidal ingestion of IPA sometimes can be fatal if it goes unrecognized and untreated. There are few published reports on IPA intoxication. We describe a case of repeated IPA ingestion in a single individual, followed by a review of the literature on the subject. The differential diagnosis, diagnostic pitfalls, and therapeutic interventions in patients with IPA intoxications are discussed.

[Issues regarding alcoholic intoxication and driving].

After recalling the risks deriving from alcohol abuse at work, the paper discusses the complex problems (medical, social, juridical) related to drinking and driving. Acute intoxication may be adequately identified (also in the medico-legal setting) through direct or indirect measurement of blood ethanol concentration, whereas the diagnosis of alcohol abuse and binge drinking (useful to assess fitness to work and/or driving) is complicated by the scarce efficiency of the currently available biomarkers. Alcohol abuse control and prevention among Italian Police Forces are ruled by the Ministerial Decree 30 June 2003, n. 198.
