A perceptive individual time trial performed by triathletes to estimate the anaerobic threshold. A preliminary study.

AIM: The aim of the study was to test the ability to estimate the power output (PO) and heart rate (HR) associated with ''anaerobic threshold'' levels for triathletes by means of a 30-min perceptive individual time trial (PITT30). METHODS: Thirteen triathletes (8 males and 5 females) performed an incremental exercise test to estimate maximal parameters such as oxygen uptake, power output and heart rate. From this incremental exercise test, the individual anaerobic threshold (IAT) and ventilatory threshold (VT) for all subjects were estimated. Then, the subjects completed a PITT30 at self-selected work intensity on a stationary ergometer equipped with the SRM Training System. Mean values of PO, HR, and pedalling cadence were recorded continuously between the 5th and the 30th min of the test. RESULTS: Significant correlations were observed between the mean PO recorded during PITT30 and PO measured at IAT (r=0.88; p<0.0001) and at VT (r=0.89; p<0.0001). Furthermore, bias and limits of agreement confirm the degree of association between the 3 METHODS: However, PITT30 over-estimated HR values compared to the values obtained at IAT and VT. CONCLUSION: It was concluded that, for triathletes, mean PO measured with PITT30 allows a partial valid estimation of PO associated with 2 known methods of ''anaerobic threshold'' determination. The application of PITT30 may offer a useful tool for athletes and coaches to estimate the ''anaerobic threshold'' in order to control accurately the training effects.

[A case of Addison's disease presented with depression as a first symptom].

This report describes a 52-year-old male patient with idiopathic Addison's disease presenting depression as a first symptom. His psychomotor inhibition, depressive mood, sleep disturbances, general fatigue, muscular pain, and arthralgia were considered to be due to intense work in a stressful environment. Neither his physician nor his orthopedist found any physical disease. Therefore, he was diagnosed with endogenous depression by a psychiatric clinic, and antidepressants were prescribed. Antidepressants were not sufficient for improving his symptoms, and he was admitted to our hospital. Endocrine blood examination revealed primary adrenocortical insufficiency. Treatment with glucocorticoid induced rapid improvement in both the psychiatric and physical symptoms. It is well known that psychiatric symptoms occur in the progressive stage of Addison's disease. At present, however, the occurrence of psychiatric symptoms is very rare, mainly because of a decrease in the incidence of this disease or an increase in mild cases. In addition, Addison's disease presenting with psychiatric features in the early stage has the tendency to be overlooked and misdiagnosed. Thus, we suggest the necessity of blood work for ACTH and cortisol in the field of psychiatry.

The expert site visitor chairperson: supportive, effective, efficient.

In much of nursing academe the words "self-study" and "accreditation site visit" are enough to squeeze the coronary arteries of nurse administrators and faculty. Such words conjure up images of months of labor intensive work, anxiety and concerns that all might not go well and that the program's accreditation will be placed in jeopardy. Both the completion of a self-study, designed as a self-assessment of program strengths and weaknesses, and preparation for the on-site visit are an addition to the normal tasks of nurse administrators and thus often result in overtaxing resources allotted to maintenance of the program.

Substance use and occupational injuries among high school students in South Texas.

This cross-sectional study examined the association between self-reported nonfatal occupational injuries among high school students in South Texas and their self-reported general use of substances. Data were collected using a self-administered, anonymous questionnaire (n = 3365). The independent variables included alcohol, marijuana, cocaine, inhalant, and steroid use. The dependent variable was occupational injury. Adjusting for grade, sex, ethnicity, and work intensity, odds ratios were calculated using logistic regression. Supporting an association with each substance, odds ratios consistently strengthened as use increased. Additional research is merited given the scant number of previous studies. The associations for inhalant (OR, 5.61) and steroid (OR, 7.63) use argue for their inclusion in future research.

Hormonal cut-offs of partial androgen deficiency: a survey of androgen assays.

While the typical symptomatology of severe hypogonadism in young adults permits the clinical diagnosis of hypoandrogenism, the diagnosis of partial androgen deficiency of the aging male (PADAM), as occurs relatively frequently in elderly males, is much more difficult. This is due to its clinical picture being subtle and aspecific, and to the fact that both clinical and biochemical evidence of androgen deficiency are required for the diagnosis. In the absence of a practical, clinical useful parameter of androgen activity, we have to rely upon bio-active plasma testosterone levels. Although the requirements of androgens in elderly males may be different from those in young, healthy adults, the lower limit of levels observed in the latter is generally used to define biochemical hypoandrogenism. Based on these data, we consider 11 nmol/l of testosterone, 0.225 nmol/l of free testosterone (FT) and 5.3 nmol/l of biotestosterone (bio-T) as the lower normal limits. As even when using the same kits, values for plasma testosterone and SHBG may differ significantly between laboratories, each laboratory should define its own normal values. As to the methodology, neither direct measurement of free testosterone by analog assay, nor the FT index (T/ SHB) can be recommended, only values obtained by dialysis, ammoniumsulfate precipitation or calculation yielding reliable estimates of androgen bio-activity. Dialysis and ammoniumsulfate precipitation are however work intensive and not widely used.

[Predictive equations and analysis of influence factors of lung ventilation based on a large occupational population in North China].

OBJECTIVE: To establish suitable predictive equations of lung function for occupational population in North China. METHODS: A total of 5 002 on the job or retired healthy adults from five enterprises in North China with category of mild or moderate work intensity underwent spirometry using a Chest HI-198 spirometer and the procedures recommended by the American Thoracic Society, were a sample. RESULTS: The data of 3 913 subjects were used. A normal distribution of our data was shown using the normality test and distribution curve. Univariate analysis showed that both age and height were significantly correlated with FVC, FEV(1), FEV(1)/FVC (%) and MMF. Further multiple linear stepwise regression analysis indicated that the levels of FVC, FEV(1), FEV(1)/FVC (%) and MMF were highly influenced by age, height, and weight rather than chest circumference. Thus, only age, height, and weight were introduced into our regression equations. Data from the studied subjects and other source were utilised to examine the validity of the equations and a high accordance rate (> 90%) was obtained. No significant difference (P > 0.05) was found in the predictive values between the simplified equations and equations in which more variables were included. CONCLUSION: The studied predictive equations for male non-smokers, female non-smokers, and male smokers were established based on data from a large occupational population. These equations should be more applicable for evaluating lung ventilatory function of occupational populations in North China.

[A physician demand and supply forecast model for Nova Scotia].

RATIONALE: There is well-founded concern about the current and future availability of Health Human Resources (HHR). Demographic trends are magnifying this concern -- an ageing population will require more medical interventions at a time when the HHR workforce itself is ageing. The lengthy and costly training period for most health care workers, especially physicians, poses a real challenge that requires planning these activities well in advance. Hence, there is definite need for a good HHR forecasting model. OBJECTIVES: To present a physician forecasting model that projects the Full-Time Equivalent (FTE) demand for and supply of physicians in Nova Scotia to the year 2020 for three specialties: general practitioners, medical, and surgical. The model enables gap analysis and assessment of alternative policy options designed to close the gaps. METHODOLOGY: The methodology for estimating demand fo physician services involves three steps: (i) Establishing the FT for each physician. To this end we calculate the income of each physician using Physician Billings Data and then identify the 40th and 60th percentile income levels for each of the 40 specialties. The income levels are then used to calculate the FTE using a formula developed at Health Canada; (ii) Calculating the FTE for each service by distributing the FTE of each physician at the service level (i.e., by patient age, sex, most responsible diagnosis, and hospital status group); and (iii) Using Statistics Canada's population projections to project future demand for three broad medical disciplines: general practitioners, medical specialist, and surgical specialists. The supply side of the model employs a stock/flow approach and exploits time-series and other data for variables, such as emigration, international medical graduates (IMGs), medical school entrants, retirements, mortality, and so on, which in turn allow us to access a host of policy parameters. RESULTS: Under the status quo assumption, demand for physician services will outstrip the growth in supply for all three specialties. CONCLUSIONS: The model can simulate supply-side policy changes (e.g. more IMGs, delayed retirements) and can also reflect changes in demand (e.g. a cure for leukemia; different work intensities for physicians). The model is highly parameterized so that it can accommodate shocks that may influence the future requirements for physicians. Once a future requirement is determined, the supply model can identify the policy levers (new entrants, immigration, emigration, retirement) necessary to close the gap between demand and supply. The model is a user-friendly tool made for policy makers to formulate appropriate physician workforce planning.

Attenuation of exaggerated exercise blood pressure response in African-American women by regular aerobic physical activity.

INTRODUCTION: A hyperreactive blood pressure response to exercise is a predictor of developing hypertension. The present study determined the influence of physical activity on an exaggerated exercise blood pressure response (EEBPR) in normotensive African-American women. METHODS: We screened 36 women 18-26 years of age for EEBPR defined as a > or = 50 mm Hg difference in systolic blood pressure at rest and during exercise at 50% peak oxygen uptake (VO2peak). Seven subjects demonstrated an EEBPR and participated in the study. Study participants trained for eight weeks on a bicycle ergometer at a work intensity of 70% VO2peak. Blood pressure, heart rate, cardiac output (CO), stroke volume (SV), and total peripheral vascular resistance (TPR) were determined at baseline and during submaximal exercise at power outputs of 30 W and 50% VO2peak. Subjects served as their own controls, and data were evaluated by using a paired t test at P<.05. RESULTS: Effectiveness of the intervention was shown by a significantly greater VO2peak associated with significant decrements in systolic and mean arterial pressures at power outputs of 30 W and 50% VO2peak. A significant decrement in heart rate was observed during exercise at 30 W. Significant increments in CO and SV and decrement in TPR were found during exercise at 50% VO2peak. CONCLUSION: The reduction in TPR associated with regular aerobic physical activity may attenuate the EEBPR and decrease the risk for hypertension in normotensive, young-adult, African-American women.

Carpal tunnel syndrome: survey of an occupational hazard.

Carpal tunnel syndrome (CTS) is one of a number of muscle-tendon and nerve related disorders that affect people performing intensive work with their hands. Hand and wrist disorders are receiving most of the attention although their symptoms are reported less often by dental workers than symptoms of neck/back disorders. The lack of data pertaining to the behaviour of Indian general dental practitioners towards carpal tunnel syndrome was the basis behind this descriptive study to evaluate the prevalence of carpal tunnel syndrome symptoms.

[Analysis of actual consumption of food substances and energy in the USSR and Union Republics (data for 1987)].

The analysis has been presented of the actual nutrition of the population of the USSR as a whole, and of all the Union Republics during 1985-1987. It has been found that the nutrition of the population in the country (on an average per capita) is imbalanced with respect to the main food substances. The food rations meet the requirements of the energy value, but do not provide physiological needs of the body in the chemical composition including the essential factors of nutrition. The main cause of such state is the irrational structure of nutrition. The analysis of the actual nutrition of the adult population capable for work has not revealed any specific disorders in the principles of the rational nutrition related to a certain group of work intensity. The nutrition deficiencies in separate occupational groups of subjects have reflected the general tendencies in the character of nutrition of the population in the country as a whole, as well as in the republic and region. Certain shortcomings of the actual nutrition common to varying occupational groups of the population have been established.

Alcoholism and other drug addictions: a primary disease entity, 1991 update.

Intensive work needs to continue on the genetic characteristics of the X-Y chromosome. There needs to be further clarification of the allele penetrants and the oncogenes, as well as determination of the specific chromosomes. It is now apparent that the neurotransmitters, particularly dopamine, seratonin, and norepinephrine, as related to the MFB receptor sites, are a determining factor in the presence or absence of this disease. However, the exact manner in which these neurotransmitters and receptor sites act, particularly in terms of the feed-back systems, remains obscure. New receptor sites are also being discovered and additional neurotransmitter systems, such as the second messenger system, uncovered. It would be naive at this point to attempt to delineate specific receptor sites or neurotransmitter systems as the absolute determinant of the disease. However, the basic heuristic model accepted by the Georgia Impaired Health Professionals Program remains valid. The heuristic model proposed in the past 15 years remains basically intact and valid. In this model, it is apparent that the essential disease criteria are fulfilled by alcohol and other drug addictions. Chemical dependency is a primary, psychosocial, biogenetic disease. The American culture, however, often fails to appreciate the difference between controlled abusive use and the uncontrolled addictive user. They do not understand that controlled abusive use can lead to severe physical changes, even death.(ABSTRACT TRUNCATED AT 250 WORDS)

Heat exchange after cholinolytic and oxime therapy in protective clothing.

The effect of the currently fielded therapeutic antidotal (DRUG) combination (a cholinolytic, 2 mg of atropine sulfate, and an oxime, 600 mg of pralidoxime chloride) in combination with chemical protective Mission Oriented Protective Posture clothing (MOPP IV) was studied. Eight healthy male subjects participated in intermittent light physical activity (1.4-2.1 kcal/minute) in two distinct environments: 35 degrees C, 60% rh (95 degrees F, HOT) and 13 degrees C, 44% rh (55 degrees F, COOL). Subjects were exposed once to HOT wearing MOPP (CON) and once wearing MOPP after DRUG. Similarly, each subject was exposed to COOL wearing MOPP and MOPP after DRUG. Rectal temperature (Tre) and mean weighted skin temperature (Tsk) were not different between DRUG and CON during COOL. Exposure time during COOL was 350 minutes. Tre averaged .5 degrees C higher in DRUG than CON in HOT. The rate of core temperature increase was 2 times faster in DRUG than CON in HOT. Tsk was 1.0 degrees C higher in DRUG experiments in HOT. Whole-body sweating rate was 40% lower (p less than .05) in DRUG than CON experiments in HOT. Heart rate was 27 beats/minute higher by 30 minutes post-injection in DRUG at 35 degrees C. Exposure time was 213 +/- 30 minutes in CON and 190 +/- 38 minutes in DRUG at 35 degrees C. These data indicate the currently fielded therapeutic antidotal drug combination increases thermal strain in subjects exposed to a hot environment when wearing protective clothing. The results are applicable to subjects performing light, intermittent work. At higher work intensities, these findings of increased thermal strain would be exacerbated.

Changes of the oxidative phosphorylation in mitchondria of rat skeletal muscle following strenous exercise.

The rate of oxygen consumption of mitochondria from rat muscles at pH 7.4 is elevated by 1-lactate. The respiratory control ratio and the ADP/O-ratio are decreased under these conditions. Acidification to pH 6.5 in the absence of 1-lactate does not change the interpreted mitochondrial functions. The experimental data are discussed as a partial uncoupling effect of 1-lactate on the oxidative phosphorylation. Similar changes in those mitochondrial functions are found after short-time intensive swimming exercise of rats. These variations might be a reason for the sometimes described reduced aerobic performance after intensive work.

[Complex evaluation of the work of physician-endoscopy specialists].

The presented system of quantitative criteria for assessing hospital physicians' work is based on the results of comparative evaluation of actual labour activity along with the standard and mean indicators. Physician's work is assessed by three main factors: amount of work (intensity); complicacy; quality with further introduction of an integral indicator. Complex approach, relative simplicity of labour accounting, derivation of estimates and availability of objective (standard) limits enable one to use this system as the basis for improving labour management of both separate medical workers and health establishments as a whole.

[Traumatism among the personnel of fishing boats].

The purpose of the study was to analyze accident rates on board the ships of the North Basin Fish Fleet along with the data on surgical interventions during a 10-year period. High accident rates among the members of the deck crew were caused by intensive working conditions characterized by cargo moving, monitoring operations, insufficient lighting of the deck and different mechanisms. Such kinds of work as maintenance and repair of the mechanisms in the machine room, trawling, fishing and fish processing were regarded as highly traumatic. Hand trauma is an occupational injury in sailors because of the great amount of manual labor on board the ships. It was pointed out that control of nonoccupational accidents and alcohol abuse among sailors was also of great importance.
