[Pleomorphic adenoma of the parotid gland in childhood].

BACKGROUND: The authors report the results of ten years' activity at the Pedodontic Surgery Unit in the Institute of Maxillofacial Surgery at the Second University of Naples and analyse the possibility of conservative treatment of pleomorphic adenoma in childhood. METHODS: The study includes 614 cases of salivary gland pathology. The authors illustrate the epidemiological, clinical and therapeutic characteristics of this type of lesion in childhood, emphasising that the data emerging from this study do not differ from the clinical and therapeutic management of pleomorphic adenoma in young patients reported in the literature. RESULTS: In particular, they emphasise that satisfactory therapeutic results can be achieved using conservative techniques, like conservative superficial parotidectomy. CONCLUSIONS: Conservative superficial parotidectomy has the advantage of being less radical and traumatic for a young patient, without provoking a significant increase in recidivation.

[Tumour suppressor p53 gene sequence mutations in pleomorphic adenoma of salivary gland].

OBJECTIVE: To detect p53 exon 7 and 8 mutation and to study genesis and biological characteristics of pleomorphic adenoma of salivary galand. METHODS: Polymerase chain reaction- single-strand conformational polymorphism (PCR-SSCP) and DNA sequencing analysis were used. RESULTS: PCR-SSCP analysis showed that in 11 patients with positive SSCP, 7 showed abnormal exon 7 and 3 abnormal exon 8. Comparison of SSCP results with DNA sequencing showed that patients all patients with p53 exon 7 and 8 mutations were SSCP positive. Exon 7 mutation pattern was point mutation(T-->C). Exon 8 mutation pattern included base substitution (point mutation) (C-->A) and frame-shiftmutation (base insertion and base loss). CONCLUSIONS: Polycodon and poly-pattern mutations appeared in p53 exon 7 and 8. Pleomorphic adenoma of salivary galand was similar to malignant tumor in p53 gene mutation. Poly-codon mutation could occur in the same case.

[Pleomorphic adenoma with bilateral pulmonary metastasis].

We report a new case of pleomorphous adenoma of the submaxillary glands with multiple lung metastases. Histological proof was obtained on the thoracoscopic surgical specimen. Clinically, this benign tumor presents as a malignant tumor. Diagnosis has been a subject of debate; surgical resection is indicated. Diagnosis is achieved by elimination in a patient with one or several nodules occurring in a context of recurrent pleomorphous adenoma.

Multiple primary pleomorphic adenomas in a single parotid gland: report of a new case.

The development of multiple primary pleomorphic adenomas in a single parotid gland is extremely rare in previously untreated patients, as only nine cases have been previously reported. In this article, we report the tenth such case, which occurred in an 87-year-old Japanese women. We also report the results of our 7-plus-year review of the types of parotid tumors seen at our institution. We identified 98 tumors in 89 patients; pleomorphic adenomas were the most common tumors, accounting for 45.9% of the total.

[The case of gigantic pleomorphic adenoma of the parotid gland].

The authors describe case of big size pleomorphic adenoma of the parotid gland. This tumor lead to cachexia and to elimination of life in society. Attention was paid on the surgical treatment of pleomorphic adenomas by superficial or total parotidectomies. Possibility of the post operative complications avoidance, even in such big cases was emphasized. Recurrence was not observed during 10 months follow up. Use of surgical treatment make that the patient was able to return to her previous job.

[A case of giant pleomorphic adenoma of the cheek with two malignant centers].

The authors present the case pleomorphic adenoma of a patient aged 77 of atypical location with two malignancy center. After performed diagnostic for an operation the tumor resected. The histopathologic examination proved in the pleomorphic adenoma the cylindromatosum and cancer solid vertens.

Pleomorphic adenoma in the lung.

Pleomorphic adenoma arising from right middle lobe bronchus is reported in an 18-year-old female. Right middle lobectomy was performed and the histologic pattern of the resected tumor was indistinguishable from that commonly described for pleomorphic adenoma of the salivary glands. Mitotic figures were not present. There has been no recurrence two years following surgery.

[Pleomorphic adenoma of minor salivary glands].

Pleomorphic adenoma, a benign tumor often seen in ENT practice, arises either from minor as well from major salivary glands. 5-14% of tumors occur in minor glands. Sixteen cases of minor salivary gland pleomorphic adenomas are studied. Eight of them originate from the oral cavity: 6 from the palate, one from the lower lip and one from the cheek. Two oral adenomas were malignant. In 4 patients tumors occur in the nasal cavity and in two other patients--in the neck. The clinical and pathological features of these patients are presented. Special attention is given to malignant transformation and the rate of recurrence of the tumors.

[The cancerous recurrence of a pleomorphic adenoma of the parotid. Comments on a case].

After taking the clinical case examined as a starting point and supporting the data obtained with a literature analysis of the cancerous relapse of a parotid pleomorphic adenoma, the Authors suggested a suitable surgical operation (total parotidectomy with conservation of the facial nerve) in the treatment of parotid pleomorphic adenoma. Doing so, it is possible to avoid a relapse of neoplasm and a possible transformation into malignancy. Furthermore it might prevent the formation of parotid carcinoma.

Pleomorphic adenoma of the epiglottis.

Pleomorphic adenoma is the most common benign tumor of the major salivary glands. A pleomorphic adenoma in the larynx constitutes a rarity. A small number of cases have been reported in the literature. We report on a case seen in our hospital, and have reviewed those cases published in the medical literature during the last 25 years.

[Pleomorphic adenoma of the epiglottis. Case report and review of the literature].

A rare case of a pleomorphic adenoma of the epiglottis in an 82-year-old patient is presented. Resection of the tumor was accomplished by using the CO2-laser and required a partial epiglottectomy. Postoperatively there were no functional impairments. This is the third case of a pleomorphic adenoma of the epiglottis, published in literature, but the first case in which epiglottectomy was performed by endoscopic laser resection.

[Metastasizing pleomorphic adenoma].

A case of metastasizing pleomorphic adenoma is reported with a review of the literature. It's a enigmatic lesion of the parotid gland in with both the primary tumor and metastasis were composed of benign pleomorphic structures. We think that some intra or extra cellular factors mut be discovered to recognize the malignant potential of this tumors and further to understand all cancerous pathology.

[The pathologic basis of partial parotidectomy in parotid pleomorphic adenoma treatment].

OBJECTIVE: The purpose of this study was to offer pathological reasons for partial parotidectomy adopted in parotid pleomorphic adenoma treatment. METHODS: Continuous slices from 25 cases of primary parotid pleomorphic adenoma were carefully examined and the extra-envelope infiltration, the budding and the primary multi-center were measured and recorded. RESULTS: The range of extra-envelope infiltration and budding was limited in 0.09-0.285 mm out of tumor capsules, which was far less than the safe boundary(1 cm) of the partial parotidectomy. Two cases with multi-centers were found, but all the centers were aggregated together and encapsulated in a united envelope. CONCLUSION: Partial parotid parotidectomy is a safe method and can remove the tumor completely in parotid plemorphic adenoma treatment.

[Giant pleomorphic adenomas of the parotid gland].

The authors describe four cases of giant pleomorphic adenomas of the parotid gland with the diameter greater then 15 cm in patients treated in the ENT Department, Medical University of Gdańsk in years 1989-2002. Three of the patients were treated surgically--parotidectomy was performed, in first case removing tumor of 28 cm in diameter and 3.2 kg, in second case 15 cm and 0.7 kg and in third case 18 cm and 1.3 kg. The forth patient was disqualified from the surgical treatment because of the metastases to the lungs from the giant malignant pleomorphic adenoma involving half of the head. The authors presented opinions on the diagnostic and therapeutic methods in that disease and also present case reports from Poland and other countries.

[Enucleation of a pleomorphic adenoma of the palate: a conservative choice].

The benign mixed tumor (pleomorphic adenoma) is the most common intraoral salivary gland tumor: more than 50% occurs on the hard and soft palate. The epidemiological data, histopathological and clinical features, preoperative diagnostic methodologies, treatment options, recurrence rates and prediction published in the literature for this neoplasm are discussed. A case of minor salivary gland tumor of the palate in a 26 year old man is reported. The therapy chosen was a surgical conservative enucleation of the lesion. Histological examination showed pleomorphic adenoma; there is no recurrence after 2 years.

Parotid gland cancer: a case report of a primitive adenocarcinoma arising from a multirecurrent pleomorphic adenoma.

Parotid tumours represent a wide group of tumours which are mostly localized in the major salivary glands. We report a case of a 69-year old female with a parotid tumour history who was referred to us with a prominent lump and swelling localized in the right parotid area. In the period between 1985-2002, she was operated 5 times elsewhere for a recurrent pleomorphic adenoma of the right parotid (mixed tumour until 1991), which was histologically confirmed. Sixteen years after the primitive tumour, she underwent mastectomy and axillary dissection for a ductal carcinoma. Routine follow-up has been conducted on both malignancies. Pleomorphic adenoma is the most frequent tumour of the parotid. The potential risk of a malignant transformation can increase over the years with an incidence of 1% to 7%. Management of these recurrences is complex and controversial because of the different treatment options advocated such as radical resection with possible facial nerve sacrifice, or postoperative radiotherapy in non radical cases.

The immunohistochemical profile of luminal epithelial neoplastic component from pleomorphic adenomas of salivary glands.

The immunohistochemistry study made on 30 cases of salivary glands pleomorphic adenomas aims to establish the antigen profile of luminal epithelial neoplastic component proliferates into these tumors. We ascertain that luminal epithelial neoplastic cell were almost exclusively positive to antibody peculiar to epithelial differentiation (AE1/AE3, MNF 116, CK 8, CK 19, CK20, EMA) and occasional to S-100 protein and negative to vimentin, alpha-smooth muscle actin, calponin, Glial Fibrillary Acidic Protein (GFAP). This kind of immunoreactivity was similar to ductal cells from striated channels of salivary glands, suggesting the origin of such pleomorphic adenomas from this place.

Recurrent pleomorphic adenoma of epipharynx following irradiation therapy.

We present a case of a 67-year-old female patient having a pleomorphic adenoma of epipharynx with intact fibrous capsula, which despite its benignity recurred and, moreover, responded quite well to irradiation therapy. The patient, at first, refused any therapy. Three years later, when the CT showed tumor enlargement she accepted only irradiation therapy, resulting in significant reduction of tumor size. Five years later the tumor recurred but without malignant transformation. She still refused surgery, but accepted re-irradiation therapy. After a three-year follow-up the patient has no symptoms of the tumor enlargement. This case shows that the irradiation therapy might be quite effective in the treatment of pleomorphic adenoma of epipharynx, but it is clear that much better results could be accomplished in combination with the surgery. In cases when the surgery is rejected or not possible because of the vicinity of the vital structures, we recommend the irradiation therapy as a therapy of the choice.
