Coronary atherosclerosis in transplanted mouse hearts. I. Time course and immunogenetic and immunopathological considerations.

An experimental system is described in which coronary arteries of mouse hearts transplanted heterotopically develop obstructive lesions by 4 weeks. Transient immunosuppression permits graft survival. Donor/recipient antigenic differences may be either class I or class II major histocompatibility antigens (H-2) or non-H-2 antigens. An infiltrate including CD4+ and CD8+ T lymphocytes and macrophages concentrates early in the intima and adventitia of larger coronary arteries, with little in the myocardium. Subsequently, the intima expands with cells of donor origin and the infiltrate invades the media. Endothelial and intimal cells express ICAM-1, leukocytes LFA-1: Endothelium expresses class I, but not class II, antigens. As class II disparity alone suffices, the endothelium can apparently be an indirect target of immune injury. We propose that graft atherosclerosis is T cell initiated and elicited by heterogeneous antigens in the endothelium or media. It is separable from rejection of the myocardium.

[Herpes gestationis: immunologic and immunogenetic aspects].

Herpes gestationis or pemphigoid gestationis (PG) is a bullous disease developing in association with pregnancy. It is believed to be an immunologically mediated disorder. Antibody binding by specific autoantibodies was detected in placental tissue and in the area of the hemidesmosomes and the basal membrane zone of the skin. These autoantibodies react with antigens of 180 kDa and 230 kDa relative molecular mass, which are presumably identical to the bullous pemphigoid antigen BPAG1 and BPAG2. The herpes gestationis factor (HG factor) preferentially binds to the 180-kDa antigen (BPAG2). The involvement of specific autoantibodies against adhesion molecules suggests the involvement of an aberrant immune response. Several groups have reported an association of pemphigoid gestationis to alleles of the human leukocyte antigens; HLA-B8 (class I) and HLA-DR3 and HLA-DR4 (class II), encoded on the short arm of chromosome 6. The unique feature of pemphigoid gestations is compared with other bullous dermatoses the association with the hormonal regulation during pregnancy and the presence of allogenic tissue. The hormonal regulation at the level of gene expression, possibly including gene expression of non-classic HLA class I molecules such as HLA-G, may be a cofactor in the pathogenesis of pemphigoid gestationis. This account, together with a case report, discusses the association of pemphigoid gestationis with HLA and a putative pathophysiological role of the newly described non-classical HLA molecules encoded in the MHC.

[Immunogenetic findings in herpes gestationis].

A 27-year-old woman suffered from herpes gestationis in her second pregnancy. The pruricy urticarial and bullous skin lesions were exacerbated immediately after birth, and premenstrually after the onset of menstruation. The diagnosis was confirmed by the determination of anti-basement membrane antibodies of the IgG class both in the serum (herpes gestationis factor) and in lesional skin by immunofluorescence techniques. The newborn had no skin lesions and no basement membrane zone antibodies were detected. The HLA (human leucocyte antigen) typing revealed the HLA-A11/1, B18/w63, C-/-, DR4/w13, DQw3/w1 haplotype in the mother and the HLA-A3/31, B7/35, Cw7/w4, DR2/3, DQw1/w2 haplotype in the father. Comparison with the first child (HLA-A11/31, B18/35, -/Cw4, DR4/3, DQw3/w2) showed that the second child (HLA-A11/3, B18/7, -/Cw7, DR4/2, DQw3/w1) had a different HLA haplotype. The pattern of the HLA haplotype of the mother and the father seems to be important in the immunopathogenesis of herpes gestationis.

Immunogenetic aspects of juvenile chronic arthritis.

The HLA associations of Juvenile Chronic Arthritis are reviewed in the light of the newest results. The most convincing data are available about the early onset pauciarticular Juvenile Chronic Arthritis (EOPA-JCA), where there are highly significant associations with 3 different regions of the HLA system: the HLA-A locus (HLA-A2), the HLA-DR/DQ region (HLA-DR5 and DR8 haplotypes) and the DP region (DPB1*0201). All these associations are independent of each other and not brought about by linkage disequilibrium. There are significant interactions between the associated alleles DPB1*0201 and DR5 haplotypes, DPB1*0201 and DR8 haplotypes as well as between A2 and DR5 and DR8 haplotypes, and between A2 and DPB1*0201. The association with the DR/DQ haplotypes reveal that most likely the DQA1 gene locus is primarily associated with JCA. There is a common motif which is present on all susceptible DQA1 alleles (0401, 0501, 0601) and not present on all the others (12). Taking all these information together, the following hypothesis is proposed: the observed HLA associations are a reflection of the direct involvement of the HLA genes in pathogenesis. The normal function of HLA molecules, namely the presentation of peptides of the T-cell receptor, is assumed to be a key mechanism in pathogenesis where an arthritogenic peptide is specifically bound by the DQ molecules, with binding specificity determined by the common motive on the DQA chain. It is possible that some of the arthritogenic peptides may be derived from self-histocompatibility antigens such as HLA-A2 and/or DPB1*0201.

[Immunogenetic studies of spina bifida--sibling cases in three families].

We reported sibling cases of spina bifida in three families. In the first family, three boys were the products of one mother and different two fathers. They all suffered from spina bifida cystica with various degrees of hydrocephalus. Cyst closure and ventriculo-peritoneal shunt were performed. Elder boys had more severe neurological deficits. In the second family, two boys and one girl were affected. The elder boy had minimum neurological deficit but the younger boy had a severe neurological deficit and hydrocephalus. Cyst closure was performed on both of them but the shunting procedure was not. The girl was normal. In the third family, three girls were affected. The second girl suffered from spina bifida cystica with hydrocephalus and cyst closure was performed. After that ventriculo-peritoneal shunt was done. Other two girls were spina bifida occulta without any neurological deficit. HLA typing, red blood cell antigen typing and chromosome test were studied in three families. There was no evidence of linkage between spina bifida and HLA antigens. Also no relationship between spina bifida and red blood cell antigens. They all had normal chromosomes. We could not make as one material for genetic counseling from above studied three methods.

Clinical, laboratory and immunogenetic aspects of arthritis associated with chronic lymphocytic thyroiditis.

OBJECTIVE: Synovial fluid effusions and/or arthritis may be found in patients with chronic lymphocytic thyroiditis (CLT), even in the absence of hypothyroidism. Sometimes these arthropathies can be attributed to the rheumatic diseases frequently associated with CLT, but in some instances the arthritis seems to be independent of any of the diseases known to cause arthritis, and therefore remains unclassified. This study was carried out in an attempt to characterize the type and outcome of arthritis associated with CLT. METHODS: We performed a prospective study with a follow-up of 6.42 years (range 4-13) on 33 patients affected with CLT and presenting with arthritis. All conditions known to cause arthritis were previously excluded. Investigations included HLA typing, x-ray of the affected joints and, when possible, synovial fluid (SF) analysis with an interleukin (IL)-1 beta determination. Patients were divided based on their clinical presentation into two groups: those with polyarthritis and those with oligoarthritis. RESULTS: During the follow-up, 8 out of 16 patients with polyarthritis developed severe rheumatoid arthritis, characterized by bone erosions, high levels of SF IL-1 beta and an increased frequency of HLA DR4. The other 8 patients had polyarthritis in a mild, non-erosive form, which responded well to symptomatic drugs. Oligoarthritis, found in 17 patients, also showed a mild evolution, with frequent spontaneous remissions. The non-rheumatoid polyarthritis and oligoarthritis patients were characterised by the absence of bone erosions, low levels of SF IL-1 beta and an increased frequency of HLA DR3. CONCLUSIONS: We conclude that it is possible to find in association with CLT a type of inflammatory arthritis characterized by a mild, non-erosive evolution, low SF levels of IL-1 beta and an increased frequency of HLA-DR3. This arthritis seems to be independent of thyroid dysfunction and shows a clinical pattern similar to the arthritis usually found in connective tissue diseases.

[Further research on the immunogenetic G blood group system in swine].

As a result of several cycles of isoimmunization, two variants of antisera dose-dependently reacting with antigens of the porcine G blood group were obtained. It is suggested that the specific response of immunized pigs to antigens inherited from different wild boar subspecies may be associated with allele Ga and probably with Robertsonian translocations 15/17 and 16/17.
