Comparison of use of cerebrospinal fluid, serum, and throat swab specimens in diagnosis of enteroviral acute neurological infection by a rapid RNA detection PCR assay.

A PCR assay for detection of enterovirus RNA in multiple specimen types from patients with neurological infections was evaluated. Combined PCR assay of cerebrospinal fluid and serum (systemic specimens) was more sensitive than assaying either specimen alone in children but not in adults. Compared with PCR in systemic specimens, detection of enterovirus RNA in throat swabs showed a sensitivity of 62.5% and a specificity of 75.6%.

Coryneform bacteria in throat cultures of healthy individuals.

Throat swabs from 113 healthy individuals from Hamburg, Germany, and Zurich, Switzerland, were investigated for coryneform bacteria with nonselective and selective media. Ninety specimens contained 123 strains. Surprisingly, 76% of them were strains of Corynebacterium durum (47%) and Rothia dentocariosa (29%). Only two were strains of Corynebacterium pseudodiphtheriticum, and none were strains of C. striatum, C. amycolatum, or C. diphtheriae.

Retrospective diagnosis of diphtheria by detection of the Corynebacterium diphtheriae tox gene in a formaldehyde-fixed throat swab using PCR and sequencing analysis.

The tox gene of Corynebacterium diphtheriae was detected in a formaldehyde-fixed throat swab taken from a 68-year-old man who was reported to have died of suffocation due to a pharyngeal tumor. DNA templates prepared from bacterial cells fixed with 10% formaldehyde were subjected to a PCR analysis with tox gene-specific PCR primers. The resultant 112-nucleotide-long PCR product was sequenced using a dye terminator method, and an expected 57-nucleotide-long internal sequence of the tox gene was detected. This method is applicable for retrospective diagnosis in diphtheria cases in which only a formaldehyde-fixed clinical sample is available.

Community-acquired bacteremic Acinetobacter pneumonia in tropical Australia is caused by diverse strains of Acinetobacter baumannii, with carriage in the throat in at-risk groups.

Acinetobacter isolates from eight subjects with community-acquired Acinetobacter pneumonia (CAAP), a major cause of fatal community-acquired pneumonia in tropical Australia, were phenotypically and genotypically confirmed by pulsed-field gel electrophoresis analysis to be broadly diverse Acinetobacter baumannii strains. Wet-season throat carriage of A. baumannii was found in 10% of community residents with excess levels of alcohol consumption, the major at-risk group for CAAP.

Immunofluorescence assay for detection of the nucleocapsid antigen of the severe acute respiratory syndrome (SARS)-associated coronavirus in cells derived from throat wash samples of patients with SARS.

An antigen detection assay for severe acute respiratory syndrome (SARS) coronavirus was established in this study by an indirect immunofluorescence test, which utilized cells derived from throat wash samples of patients with SARS and a rabbit serum that recognized the nucleocapsid protein of SARS-associated coronavirus (SARS-CoV) but not that of other human coronavirus tested. It detected SARS-CoV in 11 of 17 (65%) samples from SARS patients as early as day 2 of illness but in none of the 10 samples from healthy controls. Compared with other diagnostic modalities for detecting SARS-CoV, this assay is simpler, more convenient, and economical. It could be an alternative for early and rapid diagnosis, should SARS return in the future.

Comparison of media and techniques for detection of group A streptococci in throat swab specimens.

Detection of group A streptococci in primary throat cultures was compared by using aerobic and anerobic incubation with selective nonselective media. Sheep blood agar plates incubated anaerobically detected 98% of the group A streptococci, whereas aerobically incubated blood agar plates which had been stabbed at the time of inoculation detected only 63%. Blood agar plates containing sulfamethoxazole and trimethoprim (23.75 and 1.25 mirograms per ml, respectively) detected only 70% of group A streptocci when incubated aerobically and 84% when incubated anaerobically.

Measles viruses on throat swabs from measles patients use signaling lymphocytic activation molecule (CDw150) but not CD46 as a cellular receptor.

Both CD46 and signaling lymphocytic activation molecule (SLAM) have been shown to act as cellular receptors for measles virus (MV). The viruses on throat swabs from nine patients with measles in Japan were titrated on Vero cells stably expressing human SLAM. Samples from all but two patients produced numerous plaques on SLAM-expressing Vero cells, whereas none produced any plaques on Vero cells endogenously expressing CD46. The Edmonston strain of MV, which can use either CD46 or SLAM as a receptor, produced comparable titers on these two types of cells. The results strongly suggest that the viruses in the bodies of measles patients use SLAM but probably not CD46 as a cellular receptor.

Varicella zoster virus DNA in throat swabs.

Eighty-one throat swabs from 18 patients infected with varicella were tested for varicella zoster virus DNA by the polymerase chain reaction. The positive rates were 26.2% (11/42) during the incubation period and 89.7% (35/39) after clinical onset. The results indicate the pharynx is a site for early replication of the virus.

Varicella zoster virus DNA in throat swabs of vaccinees.

Ninety throat swabs from 30 healthy children who received varicella vaccine were tested for varicella zoster virus DNA by the polymerase chain reaction. Seroconversion was observed in 27 of 30 (90%) vaccinees. The positive rates were 7.4% (2/27) at one week and 0% (0/27) at four weeks after vaccination. These rates were considerably lower than those in normal children with varicella.

"I have a bone stuck in my throat".

Any patient claiming to have swallowed a bone and to have it stuck in his throat should be believed. The bone will probably not show in a lateral radiograph of the neck. The bone must be looked for and removed: otherwise the patient may die of septic complications.

Fish bones in the throat.

Patients presenting with sharp pain in their throat after eating fish were studied to determine the site and frequency of impacted fish bones. In 79% no fish bone was demonstrated despite careful follow up. In those proven to have a fish bone 93% were in the oropharynx. The role of X-rays and the indications for referral for endoscopy are discussed.

Essential dataset for ambulatory ear, nose, and throat care in general practice: an aid for quality assessment.

OBJECTIVE: To describe the documentation of care for the usual range of ear, nose, and throat (ENT) problems seen in primary care as a basis for developing a computerised information system to aid quality assessment. DESIGN: Descriptive study of the pattern of ENT problems and diagnoses and treatment as recorded in individual case notes. SETTING: The primary health care centre in Mjölby, Sweden. PATIENTS: Consultations for ENT problems from a 10% sample randomly selected from all consultations (n = 22,600) in one year. From this sample 375 consultations for ENT problems (16% of all consultations) by 272 patients were identified. MAIN MEASURES: The detailed documentation of each consultation was retrieved from the individual records and compared with the data required for a computer based information system designed to help in quality management. RESULTS: Although the overall picture gained from the data retrieved from the notes suggested that ENT care was probably adequate, the recorded details were limited. The written case notes were insufficient when compared with the details required for a computerised system based on an essential dataset designed to allow assessment of diagnostic accuracy and appropriateness of treatment of ENT problems in primary care. CONCLUSION: There is a gap between the amount and the type of information needed for accurate and useful quality assessment and that which is normally included in case notes. More detailed information is needed if general practitioners' notes are to be used for regular quality assessment of ENT problems but that would mean more time spent on keeping notes. This would be difficult to justify. IMPLICATIONS: The routine information systems used at this primary healthcare centre did not produce sufficient documentation for quality assessment of ENT care. This dilemma might be resolved by specially designed desktop computer software accessed through an essential dataset.

Gonorrhoea of the throat at a venereological clinic. Incidence and Results of Treatment.

Among 3,271 consecutive cases of gonorrhoea seen during the years 1971 to 1974, the incidence of tonsillar gonorrhoea was 3-6% in men and 5-8% in women. Oro-genital intercourse was admitted by 34% of men and 33% of women. Among these patients the incidence of tonsillar gonorrhoea was 7% in men and 16% in women. Only two cases of gonococcal septicaemia were seen, and there were no other complications. Few patients experienced symptoms from the throat. Tonsillar cultures was positive on the first examination in only 74%, it was negative until after single-dose treatment had been given in 18%.

Ear, nose, and throat cancer: ultrasound diagnosis of metastasis to cervical lymph nodes.

We present the results of a study on the value of ultrasound in the detection of metastasis to the cervical lymph nodes in connection with cancer of the ear, nose, and throat. Comparison of clinical, ultrasound, and histological findings for 100 patients who underwent surgery revealed that clinical examination had a sensitivity of 78% versus 92.6% for ultrasound. All 18 cases of thrombosis of the internal jugular vein were detected by ultrasound. Clinical staging of the disease was modified in 28 of these patients based on ultrasound findings, including three false positive findings. Ultrasonographic follow-up at three months for a second group of 110 patients who did not undergo neck dissection provided prognostic information, since lesion stability or progression was correlated with death in less than one year in 41 of 43 patients. Ultrasound is of primary value in providing information of an anatomic nature, including the detection of subclinical lymph nodes, volumetric evaluation, and determination of vascular connections, particularly detection of internal jugular venous thrombosis. Furthermore, for patients whose necks have been thickened as a result of radiotherapy, ultrasound allows assessment of local status.

Daily rhythms of lipogenesis in fat and lean white-throated sparrows Zonotrichia albicollis.

Lipogenesis was assayed at 6 times a day in the migratory white-throated sparrow Zonotrichia albicollis by measuring the incorporation of label in liver and body lipid 4 h after injection of [3H] acetate. There were daily rhythms of 3H incorporation by liver lipid both in fat birds in the vernal migratory condition and in lean birds in the summer photorefractory condition. Most of the liver lipid incorporation of 3H was restricted to the late afternoon and early evening in both the fat and lean birds. In addition, the total liver incorporation was similar for both groups. On the other hand, nearly twice as much 3H was incorporated in the body lipid of fat birds as in that of lean birds, and a daily rhythm was present only in fat sparrows. Injections of prolactin given simultaneously with [3H] acetate early during the day (at a time when daily injections of the hormone can cause severe losses in body fat stores) drastically reduces liver and body lipid incorporation of 3H. However, injections of prolactin during the afternoon (when daily injections of the hormone produce large increases in body fat stores) have no apparent influence on the incorporation of label. Our results indicate that a principal difference between lean and fat birds resides in a greater capacity in fat birds to transport triglycerides from the liver, in which they are produced, to the body storage depots. The daily rhythm of prolactin in some temporal relations may cause fattening by increasing the transport capacity or cause loss in fat stores in other temporal relations by directly inhibiting hepatic lipogenesis.
