A 31-year-old man was referred to our hospital because of a chest X-ray abnormality. Computed tomography showed an extra-pulmonary mass along the left 8th rib. T1-weighed magnetic resonance imaging (MRI) revealed a mass lesion with intermediate signal intensity. T2-weighed MRI showed a high signal intensity in peripheral and multiple central portions with intermediate signal intensity, which yielded a bead-like appearance. Gadopentetate dimeglumine-enhanced T1-weighted MRI showed marked contrast enhancement in the central zone of the mass. En bloc resection of the mass with the left 8th rib was performed. The tumor resembled a rosary in gross appearance, and histological examination confirmed the diagnosis of a benign schwannoma arising from the intercostal nerve. The bead-like appearance in the MRI was unique and had never been reported before for such a tumor. The postoperative course was uneventful, and the patient has been well for eighteen months.
Fibromatosis is the proliferation of connective tissue with local infiltrative growth and a tendency to relapse in contrast with fibrosarcoma which never produces metastases. Two patients, a mother and son diagnosed with fibromatosis of initial cervical location in whom surgical extirpation and complementary radiotherapy were performed. The mother underwent polychemotherapy (adriamicine plus dacarbazine) because of extensive relapse. Definitive resolution was achieved in both cases.

A 60-year-old man was admitted for investigation of an abnormality detected in chest radiography: a giant tumor with calcification in the right middle lung field. A computed tomography (CT) scan revealed multiple tumors with calcification on the posterior chest wall. Histological analysis of the tumor specimen obtained by surgical biopsy demonstrated an increasing number of plasma cells accompanied with the deposition of amyloid. A bone marrow biopsy contained over 50% of plasma cells. We therefore diagnosed these tumors as multiple myeloma. It has been reported that multiple myeloma is usually characterized by osteolytic lesions; osteosclerotic changes are rare. Multiple myeloma should be taken into account as one of the causes of a chest wall tumor even if it is diffusely calcified.
Surgery used to treat chest wall sarcomas requires preoperative evaluation in order to document their margins, and to reveal any nodules of recurrence. Because of its considerable sensitivity and specificity ultrasound was associated with routine imaging techniques. The results obtained are useful in defining superficial and lateral tumor margins, but particularly in determining micro-nodules of recurrence which would otherwise go unrecognized. Based on the data obtained using routine methods of diagnosis and ultrasound, we submitted patients to excision that each and every time proved to be wide on histological examination. A particular surgical method was used for reconstruction, involving a polypropylene mesh fixed to one or two mouldable plates. This method exploits the elasticity of polypropylene, which is indispensable for the correct respiratory dynamics of the rib cage, thus avoiding paradoxical or harmful movement. Oncological and functional results were satisfactory.

In six of 91 cases of Hodgkin's disease observed over a three-year period, a tumor mass filling the infraclavicular hollow was noted. It was on the left side in all instances. Although in four cases it was the only superficial manifestation of Hodgkin's disease for a long period, in all cases there were ultimately other areas of involvement. The lesion did not occur in any of 81 cases of lymphosarcoma observed concurrently.

AIMS: to describe the incidence and thickness of a representative collection of melanomas occurring in recent years in New Zealand. METHODS: all pathology reports of primary malignant melanoma of the skin, in defined periods in 1987-9, were collated for four regions of New Zealand. RESULTS: nonMaori incidence rates were 52 and 58 melanomas per 100,000 person years for men and women, respectively. These rates are much higher than the 24 per 100,000 person years for melanoma registrations in 1983-4. The age standardised invasive melanoma rate of 35 per 100,000 person years was higher than those of most Australian states and close to the rate for Queensland in 1986. Preinvasive melanomas comprised 26% of all melanomas. Of invasive melanomas, 58% in men and 55% in women were less than 0.76 mm in thickness; 7% of invasive melanomas in men and 5% in women were at least 3.5 mm thick. CONCLUSIONS: increasingly early detection of malignant melanomas in successive years is likely to have contributed to the recent increase in incidence, perhaps to a major degree, but an environmental cause cannot be excluded.
