[Domestic violence in lesbians: the experience of a community group in Quebec].

Intervention and research in same-sex relationship domestic violence is a newly emerging field. The Groupe d'intervention en violence conjugale chez les lesbiennes (GIVCL) is the first francophone community organization in Quebec to provide support for lesbians who are victims of domestic violence. Realizing that there were very few resources and services adapted to the specific needs of lesbians, the GIVCL provides support groups for victims, offers training sessions, and participates in research in this area. The GIVCL seeks to share its experience and concerns with other groups. This article outlines the GIVCL's program, discusses the growth and limitations of the program, and suggests approaches that can lead to the development of services that are better adapted to the needs of the lesbian community.

Educational needs of palliative care nurses in Quebec.

Education appears to be a useful strategy for improving quality of care and work-related stress management. The present study assesses the educational needs of palliative care nurses working in acute care hospitals (n=88) and CLSCs (Centre Local de Services Communautaires, home-based, n=109) of the larger Quebec City area. It also describes relationships between educational needs, psychological distress, and self-efficacy. Nurses were surveyed and compared on training needs, preferred learning format, as well as barriers and factors facilitating attendance to workshops. Similar educational needs were observed in both work settings. Crisis, stress, and emotion management were rated as the most useful topics, followed by nonpharmacological ways to manage pain. Preferred educational formats were lectures and group discussions. Attending free training sessions during work hours was described as the best way to promote participation. Educational needs were positively related to psychological distress, and negatively related to perceived self-efficacy in providing good palliative care.

Health promotion and disease prevention for older adults: Intervention themes and strategies used in Quebec local community health centres and seniors' day centres.

OBJECTIVE: Despite the considerable potential of disease prevention and health promotion (DPHP) among older adults, extant data suggest that this field of intervention is still underdeveloped. To shed further light on this issue, this paper presents the results of an inventory of DPHP interventions for older adults conducted in local community health centres (CLSCs) and seniors' day centres in the province of Quebec. METHODS: All CLSCs (N=147) and day centres (N=124) were invited to participate (response rates: 74% and 79%). Data were collected through telephone interviews. Interventions were coded according to type of intervention strategies and target themes. RESULTS: Awareness-raising and health education strategies emerged as the most frequently-cited type of intervention strategies, reported by 77% of CLSCs and 95% of day centres, respectively. The two themes reported by a majority of CLSCs were physical health (87%) and community issues (58%). Lifestyle habits (92%) and social issues (92%) were the two most frequently-cited themes in day centres. DISCUSSION: DPHP for older adults is substantially well developed in terms of intervention offerings in the two types of organizations under study. However, the range of available interventions requires expansion to increase the potential of DPHP programs to tackle the numerous challenges posed by the aging of the population.

[Prevalence and concurrent forms of violence against children in the Quebec population].

OBJECTIVE: To date, the phenomenon of concurrent forms of violence against children has received little consideration from research. This study sought to determine the prevalence and cooccurrence of various forms of violence (physical, sexual and psychological). METHOD: A telephone survey was conducted with a representative sample of 822 Quebec adults. RESULTS: More than one in three adults (36%) reported having experienced one form of violence in childhood. Ten percent (10%) of the adults experienced two forms of violence while 3% of the respondents reported having experienced all three forms of violence in childhood. Physical violence (22%) was the form most frequently reported, followed by psychological violence (16%) and sexual violence (14%). The different prevalence rates did not vary as a function of age. However, women were more likely to report having been sexually victimized (rape and fondling) and less likely to report having experienced physical violence. DISCUSSION: These results, including both the global rates and those particular to each gender, are comparable to findings in similar North American studies. The co-occurrence noted is salient enough to necessitate particular attention to diverse clinical clientele and to be considered in future research exploring the risk factors of violence and its subsequent sequelae.

[Usefulness of a vaccination registry and the management of consent: the case of Quebec].

CONTEXT: The Quebec Public Health Act provides for the creation of a central vaccination registry. However, details of how such a registry would be used have not been established. This study seeks to document the opinion of both the public and of health workers about the usefulness of the registry, consent for enrolment, access to the data and the right of withdrawal. METHODS: Individual interviews and discussion groups were held with parents, senior citizens and health workers. The material collected was recorded, transcribed and analyzed qualitatively using MAXQDA software. RESULTS: The usefulness of a vaccination registry seems to meet with general agreement, as does the idea of obtaining broad-based consent at enrolment. However, this consensus starts to crumble on questions of determining the rules governing access to the registry. Some favour limited consent, authorizing each access, while others, more numerous, prefer a broad-based consent giving access to all individuals authorized under the law. Opinions are even more divided on the question of giving the user the right to not record certain vaccines, particularly when they may be considered sensitive and a possible cause of prejudice. DISCUSSION: The more the information is considered sensitive by users, the more they seek to have access to the register and entries into it controlled and to apply some kind of specific consent, limited in scope, rather than a broad-based consent. This study lets us draw lessons applicable to the computerization and networking of medical records.

Spatio-temporal distribution of hypothyroidism in Quebec.

This study estimates the incidence and prevalence of hypothyroidism in Quebec, based on a data bank produced by the Régie de l'assurance maladie du Québec (RAMQ) on the use of thyroid hormones by persons insured under RAMQ's public drug insurance plan between 1992 and 2001. In 2001, the prevalence of thyroid hormone use in women and men respectively was 10.8 and 2.9 percent. Prevalence increases with age, reaching, among those aged 65 and over, 21.9 percent in women and 8.0 percent in men in 2001. Incidence is highest in women between the ages of 45 and 64 and in men aged 65 and over. Age-related incidence is relatively stable in women but tends to increase in men. On a regional and local basis (by Centre local de services communautaires [CLSC]), incidence rates up to 2.4 times higher than anticipated on the basis of provincial incidence rates were observed.

[Pulmonary transplantation in Quebec].

Quebec performed the first lung transplantation in 1963 which was unsuccessful in term of survival rate at that time. Since 1985, as the advancement of knowledge of pulmonary pathologies and of surgical techniques, 18 heart lung transplantations and 30 lung transplantations have been performed.

[Physical child abuse in Quebec. What is the role of the dentist?].

The role that the dentist can play in screening for physically abused children is often underestimated. The importance of his young clientele and the fact that the oro-facial area is a primary site associated with child abuse, allows the dentist to intervene in these matters. This article gives a Quebec perspective of the situation. The main points in the Youth Protection Act are discussed and local statistics confirm the extent of the problem. The importance of a proper examination to assist in screening such cases and protocol on what procedures to follow is suggested.

[Evaluation of maxillofacial injuries in a Quebec hospital].

This article describes a study done on patients with maxillofacial traumas at the Hôpital Christ-Roi in Quebec City. The patients were selected from those treated during the first three years after the Oral and Maxillofacial Surgery Department was set up. The hospital has 200 beds. In this study, the different epidemiological data and traumatic type are examined. Patient treatment is also discussed. The data analysis is supported by existing literature on the subject.

[Mortality due to cancers of the brain and lymphatic tissues and leukemia as a function of agriculture pesticide use in Quebec (1976-1985)].

Potential hazards associated with the use of agricultural pesticides are a growing concern for society. We describe the relationships between the mortality data for cancers of the brain, the lymphatic tissues and leukemia, and the spatial distribution of agricultural pesticide use for 34 drainage basins in Quebec, from 1976-1985. The basins were grouped into three categories (low, intermediate, and high exposure) according to the level of sales of pesticides. For cancers of the lymphatic tissues among women 35 to 64 years of age, a high relative risk (RR) was observed (RR = 1.91, 95% Confidence Interval = 1.14, 3.18) in basins highly exposed to pesticides compared to those with low exposure. Analysis of correlation for this cancer at 34 basins showed significant associations between geographical distributions of the Standardized Mortality Ratio (SMR) and those of numerous variables indicative of pesticide use in agriculture. Agreement between the results for mortality data in this exploratory study and the results from another study on cancer incidence data for 1982-1983, supports the hypothesis of a potential relationship between the use of pesticides in agriculture and cancer of the lymphatic tissues.

Evaluation of a community health representative program among the Cree of northern Quebec.

The James Bay Cree Community Health Representative (CHR) program was implemented in 1984 to train persons from the Cree population of northern Quebec to act as health care advocates and educators, as intermediaries between the Cree population, health services and local organizations, and as participants in assessing health needs. A formative evaluation was initiated which included quantitative analysis of the daily tasks of CHRs and a qualitative component based on documentary research, observation and semi-structured interviews. The evaluation revealed that CHRs actively participated in the ongoing community health programs mainly through health education; the people interviewed showed a high level of satisfaction. However, direct supervision, sufficient continuing education for the CHRs and better integration into health care teams are long-term goals which need to be emphasized in the program.

Screening for HBsAg in pregnant women: a cost analysis of the universal screening policy in the province of Quebec.

We carried out a cost analysis of a universal prenatal screening policy for hepatitis B virus infection in pregnant women. A universal screening policy in the province of Quebec (87,000 births per year) would cost about $473,000 per year and the prevention of one chronic carrier, $8,915. The cost varied greatly according to the ethnic origin of the mother and the cost of the serologic test. Strategies to reduce the cost of the serologic test could greatly reduce the cost of this screening policy.

[Psychogeriatrics in the year 2000: a medical and ethical challenge for society in Quebec].

During the past fifteen years, remarkable developments have occurred in Psychogeriatrics. Major advances have taken place in the biology of aging and the care of the elderly. Because of the demographic expansion of the geriatric population, psychiatrists and family physicians need to be familiar with the specific interventions which are available to tract the psychiatric problems of the elderly. Economic concerns should not distract attention from fundamental ethics issues connected with Psychogeriatrics. The intrinsic dignity of every human being must be recognized especially in the case of more vulnerable elderly patients. Such a conviction could prove essential as medicine and ethics face the new challenges that Psychogeriatrics will bring to Quebec int the next decades.

Jets over Labrador and Quebec: noise effects on human health.

OBJECTIVE: To determine whether the noise from low-level flights over Labrador and Quebec is harmful to human health. DATA SOURCE AND SELECTION: Search of MEDLINE for articles on the effect of noise, particularly impulse noise associated with low-level flights, and a search of the references from identified articles. DATA SYNTHESIS: The noise levels from low-level flights could affect hearing acuity. However, the more important consequences appear to be stress-mediated physiologic effects, especially cardiovascular ones, and psychologic distress, particularly in children. Subjective perception of control over the noise has been found to mitigate some physiologic effects. CONCLUSION: There is sufficient evidence to show that the noise from low-level flights is harmful to human health.

First coronary heart disease event rates in relation to major risk factors: Quebec cardiovascular study.

The incidence of first coronary heart disease (CHD) events was evaluated prospectively in relation to the baseline measurements of systolic and diastolic blood pressure, serum cholesterol, smoking status and education in a cohort of 4576 Quebec men aged 35 to 64 and free from CHD at entry in 1974. From 1974 to 1986, 603 first CHD events were documented. The most frequent first manifestation was angina (6.7/1000 person-years) followed by nonfatal myocardial infarction (4.7/1000) and CHD death 2.2/1000). There was a positive relationship between the first CHD event and systolic (Z = 4.67) and diastolic (Z = 6.50) blood pressure. This relation was observed for angina, nonfatal myocardial infarction and CHD death. Serum cholesterol was also related to all events (Z = 4.99) but more specifically to angina and nonfatal myocardial infarction. Cigarette smoking was significantly related to first CHD manifestations. This relationship for specific CHD events was observed in men who smoked more than 20 cigarettes per day. Men who discontinued smoking one year before the study had a risk not different from those who never smoked. No relationship was observed between years of schooling and CHD events. Blood pressure, cholesterol and smoking constituted nearly two-thirds of the attributable risk of first CHD events.

Effects of patient, physician and hospital characteristics on the likelihood of vaginal birth after previous cesarean section in Quebec.

Repeat cesarean section is a major factor contributing to the rising cesarean section rate. Although vaginal birth after a previous cesarean section (VBAC) is advocated in most cases, it has not yet been adopted as widespread policy. In a case-control study we compared 400 women in Quebec who underwent VBAC with 1600 women who had a repeat cesarean section from 1985 to 1987 in an attempt to identify factors that favour vaginal delivery. Using both simple and multiple logistic regression analyses we examined the effect of independent variables linked to the patients (two variables), the attending physicians (seven) and the hospitals (two) on the dependent variable (type of birth) with the use of odds ratios. We found that the physician characteristics related to type of practice and the degree of hospital specialization were significant factors in predicting the type of delivery. Women who gave birth vaginally were more likely than those who had a repeat cesarean section to be attended by a physician with a specialized practice and to give birth in a hospital providing an intermediate or high level of care. This suggests that VBAC is still perceived as a high-risk option and is managed by only a minority of specialized obstetricians.

Restriction endonuclease analysis of porcine Pasteurella multocida isolates from Quebec.

We have used restriction endonuclease analysis (REA) of genomic DNA to classify porcine Pasteurella multocida isolates with similar capsular and somatic serotypes, and to monitor the distribution of isolates from 12 different herds in Quebec. Within herds, P. multocida isolates of similar capsular and somatic serotypes showed similar REA fingerprints. Between herds, some isolates had similar REA fingerprints. However, differences in REA enabled subtyping of many P. multocida isolates with the same antigen types. Our data indicate that REA would enable accurate epidemiological typing of P. multocida in conjunction with classical capsular and somatic typing.

Multielectrode cochlear implantation in children: the Quebec experience.

The Quebec Cochlear Implant Research Program has implanted three children since August 1987. This paper reports the results obtained with these children. All of them improved their auditory abilities with cochlear implantation and training. However, the abilities which spontaneously recovered following implantation, the speed of improvement, and the overall level of performance are different from one child to the other. Our data suggest that the age at onset of hearing loss, and possibly the number of years of deafness, may be related to the differences in performance.
