[The autonomy of women: some reflections-statements on the goal].

The author reflects on women's reproductive autonomy and raises questions on the true significance of advancements in feminism in recent years. Does today's context of social interaction between sexes not promote a biologistical femininity characterized by a negative perception of the feminine body and of its reproductive potential? Does women's "liberation" not imply the rejection of the feminine body and of its difference? In order to avoid the perverse effects of the search for autonomy, it is not more appropriate to redefine the feminine body with reference to women instead of with reference to the male universe, as it seems to be the case today?

[And if we speak about men?].

This article raises questions about some of the perverse effects of the reasoning behind correlations between sex and health in our socio-cultural context. Such a reasoning strongly denounces the psychosocial problems of women, but tends to forget the vulnerability of men which is nonetheless clearly evident in official statistics on suicide, dependence on alcohol and other drugs, violence and itinerancy.

[Clinico-immunological parallels in non-coronary diseases of the myocardium].

Profound disturbances of the immunologic homeostasis system are a characteristic feature of cardiac muscle inflammations. Depressed rosette formation in patients with non-coronarogenic diseases of the myocardium is associated with impaired maturation of immunocompetent cells and with severe inhibition of suppressor function, this resulting in perversed proliferative response to phytohemagglutinin. In patients with dystrophic and functional diseases of the cardiac muscle the activity of short-lived suppressors is unchanged; therefore examination of this parameter may be helpful in the differential diagnosis between inflammatory and dystrophic diseases of the myocardium.

[Can ventricular dysfunction be prevented or delayed in course?].

Left ventricular dysfunction is asymptomatic or paucisymptomatic for a long time. It would be beneficial if progression to the symptomatic stages could be prevented. This is only possible when the abnormal conditions of cardiac load or perfusion which cause the myocardial disease, can be corrected. The treatment of hypertension or valvular disease is an example of this approach. Another approach is to reduce the perverse effects of the neurohormonal adaptations to cardiac failure. The treatment of chronic myocardial infarction by angiotensin converting enzyme inhibitors is an example of this approach.

[Changes in respiration and hemodynamics in patients with cranio-cerebral injury after hyperbaric oxygenation].

Respiration, central hemodynamics, gas and acid-alkaline balance of the blood were examined in the course of hyperbaric oxygenation (HBO) sessions in 30 patients in the acute period of severe craniocerebral injury, including 15 patients with a favourable outcome and 15 patients with a subsequent lethal outcome. In the first group, HBO gave rise to a decrease in the minute respiratory volume (MRV) and, in particular, in the minute blood volume (MBV). Acid-base and gas balance of the blood remained unchanged or improved. This attests to intactness of oxygen reactivity of the brain and body on the whole and allows establishment of a favourable prognosis of the further course and outcome of disease. The second group manifested a different dynamics: the initial MRV was greater, further increasing in the course of HBO sessions, acid-alkaline and gas balance of the blood worsened towards the development of non-compensated respiratory alkalosis which reflects ineffective hyperventilation and disturbed perversed oxygen reactivity.

Incest perpetrators in group therapy: a psychodynamic perspective.

Patients who have been sexually abusive toward children in their family often have guarded prognoses. However, the legal system's promotion and support of treatment elicits cautious optimism; even limited progress can make a significant difference in the lives of these persons and their families. The authors report on their work with incest perpetrators in a psychoanalytically oriented psychotherapy group with 20 incest offenders (selected after evaluation of 50), describing major obstacles (i.e., patients' lack of motivation and countertransference) and frequent clinical issues (i.e., perversion, psychopathic tendencies, low self-esteem, and substance abuse).

The effects of a scientifically objectifying (self-) understanding on the psychosexual development.

The question as to whether a spiritualized technical-mechanical world and human image can play a role in the development of sexual perversions was posed. Sexually deviant behaviour could then be understood as being an escape from a mechanical self-experience, poor in sensitivity. The presence of concept contents in psychological tests and anamnestic details which indicate such a self-experience was examined in children and adolescents with disturbances in sexual identity, object choice and sexual behaviour and was analysed for patients with fetishistic or violent tendencies. In fetishistic patients the results came up to our expectations more than in those with tendencies to violence, but not all single results supported our hypothesis.

[Monoclonal gammopathies of undetermined significance called "MGUS"].

While the discovery of a monoclonal gammopathy in the blood of a patient sometimes leads to the diagnosis of multiple myeloma, immunocytoma or other B-cell lymphoproliferative syndromes, "secondary" monoclonal dysproteinemia is seldom considered. In most cases "monoclonal gammopathy of undetermined significance" (MGUS) is diagnosed. After ten years about 25% of these cases evolve to true multiple myeloma or Waldenström's disease. Other MGUS usually remain asymptomatic, though sometimes a complication such as amyloidosis or cryoglobulinemia may occur as a result of the "perverse" effect of the monoclonal protein. Some cases are described.

The classic and the romantic vision in psychoanalysis.

Psychoanalysis is characterized by a tension to be found in intellectual history at least since the eighteenth century. The classic vision of man is that of distrust of the idiosyncratic and subjective and the emphasis on the need for objectivity and rationality. In psychoanalysis this is reflected in the attitude of benevolent suspicion which seeks the traces of the pleasure principle in order to allow maturation. It is exemplified here through Freud's work. The romantic vision sees man as essentially striving for full selfhood, and mental suffering is the result of the thwarting influence of the environment. Kohut, who is taken to exemplify this vision, takes an attitude in which he seeks for the healthy striving behind the seemingly ill and perverse. He emphasizes the human need for idealization as a normal phenomenon. The tension between classicism and romanticism expresses itself in clinical problems no less than in theory. It is argued that this tension is not to be resolved, as it reflects the tension between the human ability and need for full experience and the capacity for self reflection which is essential to maturity and wisdom.

The income--fertility relationship: effect of the net price of a child .

This article examines the relationship between income and completed family size in empirical fertility models. The relationship, which is hypothesized to be positive, often is negative in empirical studies. This perverse result is thought to occur because of the many correlations between income and other factors that affect fertility. In this research, these other factors--such as the net price of a child, the opportunity cost of the wife's time, and supply factors--are statistically controlled, and the income effect is positive and significant. When the net price of a child is not controlled, however, the income effect becomes negative and significant.

[Representative study of morbidity in the Berlin population].

Representation of the frequency of crowding in a sample of 657 Berlin residents aged between 18 and 30 years. Sex and age distribution of the examined individuals with regard to the severity and location of crowding. Relationship of the crowding with the symptoms of Class III malocclusion, perversion in the position of the canine and cross-bite.

Prevention of child sexual abuse.

Child sexual abuse prevention programs are a growing phenomenon addressing an expanding number of topics raised by a variety of violent and perverse acts. Currently programs tend to focus on a fairly narrow age group and use an educational model for children. There is a need for broad-based programs with a strong focus on the adults around children. Perinatal prevention programs require an adult focus and must take advantage of a special time in child and family development to address lesson of health and intimacy in a way that may decrease the incidence of future child sexual abuse.

[The anthropological phenomenon of strangeness in psychotherapy].

Strangeness is conceived as an existential phenomenon, by which a perception is antithetical facing the perceptions hitherto, and incomparable as well concerning the present imaginations. In order to perceive strangeness, it requires to face an interpersonal encounter. It is the essence of strangeness that the encounter, which usually compensates the lack of experiences, is not compensated in a perception as strangeness. Strangeness as a perception does not compensate the lack of experiences by means of the present imaginations. Strangeness is not conceivable out of its own. Strangeness requires the precondition of active daring towards an interpersonal encounter, which promotes the appearance of the absolutely unexpected in form of an intrusion. Such as a psychic achievement of separation. This will be illustrated by the incestuous coping of the hero, as a typical involvement in strangeness. Finally perversion will be mentioned, as an absolute impairment concerning the perception of strangeness. Two case-reports are presented.

The fetishistic use of speech.

The question which has been addressed in this paper is that of how broad or narrow our definition of fetishism should be. Can speech and words be included in the category of fetishistic phenomena? A detailed clinical case has been presented of an obscene telephone perversion in which obscene words, seen, heard and spoken, were utilized to deal with conflicts involving dread of the sight of the 'castrated' female genital and to ward off intense castration anxiety by supporting the illusion of a female phallus. The clinical observations have been utilized to illustrate the conclusion that although fetishism most often employs concrete objects for the defensive purposes involved, there are other instances in which abstract words and speech can be used concretely in place of or in addition to the more familiar concrete items. A brief discussion has been made of certain aspects of language development that are pertinent in this regard.

[Clinical and social characteristics of persons accused of committing sexual offenses].

The results of clinical statistical examination of 1.405 men responsible for sexual offences are presented. On the basis of the results obtained some data are given (which were unknown before) concerning unlawful sexual behaviour of subjects with psychic pathology, frequency and character of sexual offences in the given population; then the effect of alcoholisation and general recidivation of crimes is specified and stressed; most criminogenic social and clinical groups are singled out; and knowledge on distribution and representation of sexual pathology (perversion) in nosologic groups and in practical work of forensic psychiatrists is outlined and supplemented.

[Principles of the forensic psychiatric assessment of exhibitionism].

Incompetence of the existing approach to forensic psychiatric assessment of exhibitionism as well as the necessity for conducting the analysis of direct manifestations of perverse behaviour are shown. Dissimilar clinical manifestations of this kind of paraphilias are presented and its fixed, compulsive and impulsive variants are described. The necessity for singling out differential forensic psychiatric criteria depending on psychopathologic characteristics of exhibitionism is stressed.

[Isolated mitral valve insufficiency in comparison with mitral-tricuspid insufficiency: various mechanisms of compensating for the defect and functional status of the myocardium].

A combined analysis of ventricular contractility and intracardiac hemodynamic compensatory mechanisms was carried out, on the basis of angiocardiographic findings, in 37 patients with rheumatic mitral incompetence. Atrial fibrillation aggravates essentially the defect's hemodynamics, while added tricuspid incompetence is accompanied by a certain off-loading in the lesser circulation network. A grossly perversed phasic structure of intramyocardial stress was noted, apparently being a compensatory mechanism. Reduced specific coronary flow and diastolic perfusion gradient in intact coronary arteries are shown to be causes of clinical angina.

Comparative tolerability of two formulations of Rhinalar (flunisolide) nasal spray in patients with seasonal allergic rhinitis.

This double-blind, randomized, crossover study compared the incidence of nasal burning and stinging, as well as overall tolerability of the currently marketed formulation of Rhinalar (original formulation) to a new formulation of Rhinalar containing less propylene glycol. In addition, patient and investigator subjective evaluations were used to compare the effectiveness of the test medications in controlling the nasal symptoms of seasonal allergic rhinitis. A total of 122 patients were enrolled in this 4-week trial. Each patient received one formulation of Rhinalar for 2 weeks and then crossed over to receive the alternate formulation for an additional 2 weeks. Eighteen patients withdrew from the trial prematurely. Ten patients were lost to follow-up and eight withdrew due to side effects and/or inadequate therapeutic response. Statistical comparisons of patient evaluations of nasal burning and stinging with the two formulations of Rhinalar showed a very significant difference in terms of severity (P less than .001), duration (P less than .001), and tolerability (P = .006) in favour of the new formulation. A reduction in severity of throat irritation with the new formulation was also shown to be statistically significant (P = .006). Nausea, headache, and other side effects including watery eyes, taste perversion, and runny nose were seldom reported with either test medication. Both formulations were shown to be equally effective in relieving the nasal symptoms of seasonal allergic rhinitis. The considerable reduction in nasal burning and stinging and throat irritation with the new formulation of Rhinalar was shown to enhance patient acceptability and may lead to better compliance.
