[Neurofibroma plexiforme of the small pelvis: a case report].

There are reports of rare cases of nerve tissue tumors of the small bowel, ovary, urinary bladder or retroperitoneal space. We report a case of neurofibroma plexiforme of pelvis minoris in 42-year-old woman. The first diagnosis was adnexal tumor. Laparotomy was performed and a big, solid tumor of left parametrium was found. The histology of the tumor was described as neurofibroma plexiforme. There was also an inflammatory tumor of appendix involving right Fallopian tube. The described by other authors cases suggest, that diagnosis of nerve tissue tumors in pelvis minoris is very difficult. Surgery is an adequate way of final diagnosis and management.

[Evaluation of utility of MR T2-weighted images using multishot echoplanar imaging of female pelvis: comparison with fat-suppressed fast spin echo].

The purpose of this study was to apply multishot echoplanar imaging (EPI) to the female pelvis and to compare the results with respiratory triggered fast spin echo with fat-suppression (fFSE). Twenty-seven patients with pelvic disease were examined. EPI images were obtained using 8 shots with breath-holding (bhEPI) and 16 shots without breath-holding (bEPI), while the FSE sequence was fat-suppressed respiratory-triggered FSE. Signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) and contrast to uterus or ovary (Contrast) were compared between EPI and FSE images. Identification of uterus, ovary, and tumors was carried out simultaneously. In SNR, CNR, and Contrast, EPI could not provide image quality superior to that of fFSE. Moreover, on EPI images, identification of uterus, ovary, and tumors was judged to be inferior or equal. In conclusion, multishot EPI cannot replace fFSE sequences in imaging of the female pelvis. However, because EPI has heavily T2-weighted contrast, the EPI sequence can be a valuable adjunct to routine examination.

[Choriocarcinoma of the renal pelvis: a case report].

A 68-year-old male presented to our hospital complaining of gross hematuria. Intravenous pyelography subsequently demonstrated a left non-visualized kidney, and he was admitted for further evaluation. Cystoscopy revealed a bladder tumor around the left ureteral orifice and retrograde pyelography showed a filling defect in the left renal pelvis. The urinary cytology from the left renal pelvis indicated class IV, and the microscopic findings of a bladder biopsy demonstrated grade 1 transitional cell carcinoma. We performed transurethral resection of bladder tumor (TUR-BT) followed by left nephroureterectomy with bladder cuff. The pathological diagnosis was high grade transitional cell carcinoma with choriocarcinomatous component. After the operation, the serum human chorionic gonadotropin-beta (hCG-beta) level was slightly elevated, and the combination chemotherapy with methotrexate, vinblastine and cisplatin (MVC) was administered. Although the serum hCG-beta level fell to 0.1 ng/ml, after two courses of MVC chemotherapy, bilateral pulmonary metastases appeared in the chest X-ray with increasing hCG-beta levels. Salvage chemotherapy with cisplatin, etoposide and bleomycin (PEB) was performed. After two courses of PEB chemotherapy, the serum hCG-beta level fell to within the normal range and all pulmonary metastases disappeared.

[Solitary pulmonary nodule in a patient with treated carcinoma of the renal pelvis: metastatic disease?].

OBJECTIVE: To describe a case of a solitary pulmonary nodule in a patient that had been treated for carcinoma of the renal pelvis. METHODS: A solitary pulmonary nodule was detected on the chest film of a patient that had been treated for carcinoma of the renal pelvis. The characteristics of the nodule are described and its diagnosis, with special reference to metastasis and primary pulmonary carcinoma, is discussed. RESULTS: The anatomopathological study demonstrated a chondroid hamartoma. CONCLUSIONS: A metastatic or a primary tumor is suspected when a solitary pulmonary nodule is detected in a patient that has been previously treated for urothelial carcinoma. However, other types of lesions with a more favourable outcome cannot be discarded, such as chondroid hamartoma as in the case described herein.

[Spiral computed tomography in the assessment of vascular lesions of the pelvis due to blunt trauma].

PURPOSE: We investigated the role of Helical Computed Tomography (CT) in the evaluation of low or high flow vascular injuries in patients with blunt pelvic trauma. MATERIAL AND METHODS: From May 1998 to December 1999, forty-nine patients (32 men and 17 women, ranging in age 14-59 years) with acute symptoms from blunt pelvic trauma were submitted to Computed Tomography (CT). A conventional radiography of the pelvis had been performed in all cases. CT was performed with a helical unit (thickness 8 mm, reconstruction interval 8 mm, pitch 1.5) after intravenous contrast agent (150-180 mL) rapid infusion (4-5 mL/s, 60 s acquisition delay from bolus starting) and using a power injector. A second spiral acquisition was performed in all cases from the iliac roofs to the inferior border of the pubic symphysis. Vascular hemorrhage was considered as low flow when the hematoma appeared as a focal homogeneous density area and as high flow when associated with contrast agent extravasation. Moreover, traumatic assessment included evaluation of the hematoma, of the leakage site and of the involved vessel. RESULTS: Radiologic examination of the pelvis revealed fractures in 35/49 patients (71.4%). Helical CT allowed us to identify low flow hemorrhage in 37 patients, affected with hematomas from fracture of the iliac wing or of the sacrum (14 cases), tear of the pelvic (3 cases) or extrapelvic (4 cases) muscular structures, or injury of the venous plexus (20 cases). In four patients two vascular injuries were detected. High flow hemorrhage was seen in 12 patients, who had Helical CT findings of contrast agent extravasation along the common iliac vein (3 cases), external iliac artery (3 cases), internal iliac artery (4 cases), internal pudendal artery (1 case), obturator artery (1 case), inferior epigastric artery (2 cases), superior gluteal artery (2 cases), inferior gluteal artery (1 case), cremasteric artery (1 case). In 6 patients with high flow hemorrhage, two vascular injuries were shown. In all these patients, an extraperitoneal hematoma was associated with the contrast agent extravasation. DISCUSSION AND CONCLUSIONS: Fractures of the pelvic ring generally result from severe trauma. Management of these injuries must include not only treatment of the skeletal trauma but also of the associated shock and complications. Major blood loss usually occurs as a result of bleeding from the branches of the internal iliac artery. With respect to pelvic plain radiography, CT provides superior detailing of fractures, position of fracture fragments and extent of diastasis of the sacroiliac joints and pubic symphysis. Moreover CT provides diagnostic information regarding the presence or absence of pelvic bleeding and can identify the site of bleeding. In our experience, Helical CT allows us to distinguish high flow hemorrhage, where vascular injuries must be treated first, from low flow hemorrhage which can be managed differently.

[Epidermoid cancer of the kidney pelvis].

Squamous cell carcinoma of the renal pelvis is a rare tumour, mainly occurring in elderly patients with neglected and secondarily infected pyelocaliceal stones. It has a poor prognosis. The authors report the case of a 74-year old man with a destroyed and painful kidney containing a staghorn calculus for 30 years. The preoperative diagnosis was pyonephrosis, but nephrectomy, justified by the symptoms, showed that the kidney was invaded by a squamous cell carcinoma of the renal pelvis invading the psoas. The patient died 1 month after the operation. Treatment is essentially preventive by removal of staghorn calculi and management of chronic urinary tract infections.

CA19-9-producing transitional cell carcinoma of the renal pelvis: a case report.

We report a case of CA19-9-producing transitional cell carcinoma of the renal pelvis. A 59-year-old male patient with left hydronephrosis was referred to us from a local physician. Retrograde pyelogram revealed irregular filling defects involving calices, pelvis and proximal ureter. The serum CA19-9 level was elevated. Under the diagnosis of renal pelvic tumor, we performed radical left nephroureterectomy. The tumor was histologically diagnosed as transitional cell carcinoma. The tumor cells showed positive immunostaining for CA19-9. The serum CA19-9 level was normalized after the operation. To our knowledge, this is the 28th case of a CA19-9-producing tumor in the Japanese literature.

[Improvement of focal dose determination in the small pelvis].

An easily applicable auxiliary method in order to register exactly the topographic relations on horizontal sections in the small pelvis is indicated. Only the body contour, pelvis pictures on two planes, diapositives with the corresponding pictures of the cross-section and a photographic enlarger are needed. A sagittal tomogram in the median plane, and for radium therapy the localization pictures on two planes are other helps.

Staged closure of the pelvis in secondary repair of cloacal exstrophy in the older patient.

The authors describe a staged approach for pelvic closure applied in preadolescents and adolescents with previously failed closure of cloacal exstrophy. Closure of the pelvis was performed in four patients, 11 to 16 years old, with history of previous closures for their cloacal exstrophy that had resulted in either dehiscence or severe disfigurement. They were treated with pelvic osteotomies and external fixation; three later underwent internal fixation of the pubic rami. Genitourinary reconstructions were performed with the final stage of pelvic ring closure. Genitourinary closure was successful in all four patients despite a recurrent pubic diastasis resulting from sacroiliac joint instability in the patient not receiving internal fixation. Staged closure of the pelvis with iliac osteotomies, external fixation, gradual correction, and subsequent internal fixation of the approximated pubic rami can facilitate soft tissue closure of preadolescents and adolescents with unacceptably closed cloacal exstrophy.

[Carcinosarcoma of the renal pelvis and ureter: a case report].

We report a case of carcinosarcoma of the renal pelvis and ureter arising in an 89-year-old man who presented at our hospital with gross hematuria. Abdominal computed tomography, excretory pyelography, and retrograde pyelography demonstrated that left hydronephrosis was caused by an ureteral tumor. Left urine cytology indicated transitional cell carcinoma. The patient underwent chemotherapy and radiation therapy. However, gross hematuria recurred, and the patient underwent left nephroureterectomy. The surgical specimen showed carcinosarcoma in the renal pelvis and ureter histologically. He has been free of cancer for 1.5 years.

[Sarcomatoid carcinoma of the renal pelvis: a case report].

A 65-year-old man visited our hospital with the complaint of left hypochondrial pain. Since he had left giant hydronephrosis due to ureteral stone, we performed left nephrectomy. Unexpectedly, macroscopic examination of the resected kidney revealed multiple yellowish nodules located in the renal pelvis and calyces. Histopathologically the nodules consisted of two pattern of malignancy, transitional cell carcinoma and spindle sarcomatous tumor. Immunohistochemical examination showed that spindle cells were stained positive for cytokeratin, and the final diagnosis was sarcomatoid carcinoma of left renal pelvis. Postoperatively, the patient underwent two courses of adjuvant chemotherapy, but metastases to retroperitoneal lymph nodes were noted two months after operation. He died of the disease eight months postoperatively.

[Mucinous adenocarcinoma of the renal pelvis in the horseshoe kidney: a case report].

A 41-year-old man with macroscopic hematuria and abdominal fullness was referred to our hospital. Computed tomography (CT) revealed a left renal pelvic tumor in the horseshoe kidney. We performed left heminephrectomy and ureterectomy. The pathological diagnosis was the mucinous adenocarcinoma in the renal pelvis. He received postoperative adjuvant chemotherapy (CAP therapy). He died of retroperitoneal recurrence 8 months postoperatively. In the literature we found 95 cases of primary adenocarcinoma in the renal pelvis including our case.

Intestinal metaplasia with dysplasia in pelvis of a horse-shoe kidney.

Intestinal metaplasia of renal pelvis occurs uncommonly and is usually secondary to persistent chronic irritation or inflammation. It is considered to be precursor of adenocarcinoma. A case of horse-shoe kidney which showed extensive intestinal metaplasia and focal severe dysplasia in the renal pelvis is being presented. The metaplastic change in this case is being attributed to persistent inflammation due to more chances of stasis in horse-shoe kidney coupled with renal calculi.

Physiological asymmetry of trunk ranging and pelvis motility: an anatomo-functional study in 80 healthy subjects.

A key feature in physiotherapeutic treatment of patients with motion disturbances is the appropriate ranging of the trunk and pelvis motility. Eighty subjects randomly selected and free from known pathology of the muscular-skeletal and/or of the neurological system classed into four groups according to the age and the sex have been assessed, by using a new, simple and easy administrable tool. Our results demonstrate that the new measurement tool showed a very low intra- and inter-observer variability, that healthy subjects showed a more adduced and elevated right scapula if compared to the contralateral one and, as regard as the pelvic motion, a broader joint excursion in passive motion compared with active motion in the overall group, a broader joint excursion in young subjects compared with elderly ones, and a broader joint excursion in female subjects compared with males subjects. In conclusion our study allowed to identify a range of physiological asymmetry and pelvis motility. Such a range of physiological asymmetry might be useful as a reference for the physiotherapists.

Low frequency of human polyomavirus BKV and JCV DNA in urothelial carcinomas of the renal pelvis and renal cell carcinomas.

Human polyomaviruses BKV and JCV establish a persistent infection in the kidney and possess oncogenic potential in experimental animal models. Their possible involvement in the pathogenesis of human urogenital tumors has been suggested by several studies but is not conclusively resolved. For the present study, highly sensitive quantitative TaqMan PCR assays were developed to detect BKV and JCV DNA sequences in matched pairs of human tumors and surrounding normal tissue. Samples from 55 patients with renal pelvic urothelial carcinomas and from 83 patients with renal cell carcinomas of all histologic subtypes were investigated. Human polyomavirus DNA was detected in renal pelvis samples with a frequency of 16% (BKV) and 15% (JCV), and in kidney samples with a frequency of 1% (BKV) and 23% (JCV), respectively. However, viral sequences were not present more often in tumors than in normal tissue. There was no integration of the potentially oncogenic viral large T-antigen in the tumors, and the viral concentration of both BKV and JCV was not higher in DNA extracted from tumors compared to normal tissue. These data provide no evidence for a causative role of the human polyomaviruses BKV and JCV in the development of tumors of the renal pelvis and the kidney.

[Current aspects of radiographic diagnosis of pelvis and pelvic organ traumas in peace and wartime conditions (review of the literature)].

The injuries of pelvis and pelvic organs remain one of the difficult problems of surgery in military and peace-time. The detection of pathologic changes in the cases of pelvic traumas depends on the time, quality and volume of radial investigations. The rule that any pelvic injury is considered as the complicated one until the opposite will be proved determines the diagnostic algorithm of roentgenologic examination, and the contrast methods are of great importance. The perspective method of for investigating the casualties with severe combined pelvic trauma is the spiral computed tomography. The investigation of casualties with pelvis and pelvic organ traumas must be conducted at the shortest periods after the trauma using the whole complex of radial investigative methods both the conventional roentgenologic and imaging methods.
