Maxillary distraction osteogenesis: a two-dimensional mathematical model.

Patients with cleft lip and palate with severe maxillary retrusion usually have a mandible with anterior-superior autorotation and subsequent overclosure and loss of the vertical facial dimension. Maxillary distraction osteogenesis can correct the sagittal maxillomandibular relationship and should simultaneously reestablish vertical dimension through maxillary vertical height increase and clockwise rotation of the mandible to restore facial balance. We present a two-dimensional mathematical model in the sagittal plane, which reestablishes sagittal and vertical skeletal deficiencies and proper occlusal alignment for planning maxillary advancement with distraction osteogenesis in patients with cleft lip and palate. The model is illustrated in a case of a 13-year-old boy with a complete bilateral cleft lip and palate and severe maxillary retrusion. The two-dimensional mathematical model described in this article allows the surgeon and orthodontist to calculate in a simple and accurate way the ideal distraction vector to advance the maxilla to its desired position.

Transient retention of endochondral cartilaginous matrix with bisphosphonate treatment in a long-term rabbit model of distraction osteogenesis.

UNLABELLED: Bisphosphonates induce major increases in strength of callus in distraction osteogenesis in the short term. Poor understanding of the underlying mechanism, however, raises concerns about long-term consequences. In this long-term study in 32 rabbits, zoledronic acid transiently increased trabeculae by delayed temporal progression of endochondral bone remodeling but did not prevent radiographic completion of bone repair. INTRODUCTION: We hypothesized that bisphosphonate inhibition of osteoclast-mediated resorption would retain bone during repair, producing a larger callus in the short term. However, if remodeling was not restored, completion of the bone repair process in the long term could be jeopardized. MATERIALS AND METHODS: Juvenile rabbits underwent right tibial osteotomy and 2 weeks of distraction, followed by a period of consolidation. Animals received saline (controls) or zoledronic acid (ZA; 0.1 mg/kg at surgery and again 2 weeks later), and distracted tibias were examined by radiograph, DXA, histology, and histomorphometry at 2, 4, 6, 18, and 44 weeks after surgery. RESULTS: Regenerated bone in ZA-treated animals was denser than controls on radiographs at 6 weeks and had more distinct radiodense trabeculae and retention of original cortices at 18 weeks. By 44 weeks, controls and ZA-treated animals were radiographically healed and indistinguishable. Regenerate BMD and BMC increased between 2 and 4 weeks in all animals, with a greater effect in ZA. At 6 weeks, BMD and BMC in ZA-treated animals were 1.6- and 2-fold greater, respectively, than controls (p < 0.01). From 6 to 44 weeks, the control values gradually increased and approached the ZA-treated values. Regenerate bone volume and trabecular number by histomorphometry were from 1.6- to 2-fold greater in ZA-treated animals at 6 and 18 weeks (p < 0.05). Endochondral cartilaginous matrix volume was up to 2.4-fold greater in ZA-treated animals at 2 and 4 weeks (p < 0.05). TRACP+ cells in ZA-treated animals were larger with more nuclei. Mineral apposition rate and osteoblast number and surface were lower in ZA-treated animals at 6 weeks (p < 0.01) but not at later times. CONCLUSIONS: Disruption of TRACP+ cell function by ZA during bone regeneration seems to lead to an accretion of cancellous bone built on a larger endochondral cartilaginous matrix and increased bone mass, consistent with reported increases in short-term callus strength. This increase in bone mass, caused by a delay in remodeling, provided a transient advantage without preventing radiographic completion of the bone repair process in the long term. Noncontinuous treatment with nitrogen-containing bisphosphonates thus can have short-term beneficial effects without preventing long-term bone repair.

Vertical distraction osteogenesis of fibula transplant for mandibular reconstruction: a case report.

Bone continuity defects in the mandible are caused by tumor surgery, trauma, infection, or osteoradionecrosis. Today, reconstruction of long-span mandibular defects with a free fibular flap is a routine procedure. However the bone height of the mandible after reconstruction is about half that of the dentulous mandible. Therefore, the deficiency in bone height makes implant placement impractical. In our case, because it was necessary to restore the mandibular height, a vertical distraction osteogenesis was performed on the grafted mandible of the patient who was referred to our clinic with a reconstructed mandible owing to a gunshot injury. As a result, the vertical discrepancy between the fibula and the native hemimandible of the patient was corrected. And the placement of dental implants was performed without any complications. In conclusion, we believe that the vertical distraction osteogenesis of free vascularized fibula flaps is a reliable technique that optimizes implant positioning for ideal prosthetic rehabilitation.

Muscle adaptation during distraction osteogenesis in skeletally immature and mature rabbits.

The lack of adaptation of muscle is thought to be a major source of complications during distraction osteogenesis (DO). Although adaptation to DO varies with the regimen (lengthening rate >1 mm/day and increase in bone length >20%) muscle contractures associated with DO may be a function of age. We tested this idea by subjecting skeletally mature and skeletally immature rabbits to an aggressive regimen of DO (1.4 mm/day with a 20% increase in tibial length). By using immunofluorescence to assess the presence of neonatal myosin heavy chain in sections from the tibialis anterior, we observed that the generation of new muscle tissue in response to DO was vigorous in young animals (27% positive fibers), whereas it was more muted in adult animals (9.9% positive fibers). This adaptive response was associated with a pronounced proliferation of myoblasts in the young but not in the mature animals. Adult tibialis anterior subjected to DO showed a 50% loss in tetanic and twitch tension whereas those in young animals did not. This correlated with partial denervation of adult but not young muscle, as judged by morphological criteria. These experiments indicate that adaptation to DO depends not only on mechanical variables but also on skeletal maturity.

Effect of ED-71 on modeling of bone in distraction osteogenesis.

We investigated the effect of 2-beta-(3-hydroxypropoxy)-1alpha,25-dihydroxyvitamin D3 (ED-71) on the modeling of bone in distraction osteogenesis. The tibiae of 30 rabbits were lengthened by 10 mm in 10 days. Following osteotomy, ED-71 (0.05 microg/kg) was administered subcutaneously twice a week to the ED-71 group until necropsy. The bone mineral content (BMC) of the lengthened callus was measured by dual-energy X-ray absorptiometry (DXA). Five rabbits per group were killed at 1, 3, and 8 weeks after completion of lengthening, and the lengthened callus was examined histologically and histomorphometrically. Bone volume of the lengthened callus was measured by peripheral quantitative computed tomography (pQCT) at 8 weeks after the completion of lengthening. At all timepoints the BMC in the ED-71 group was significantly higher than that in the untreated group. The mineral apposition rate and bone formation rate were higher in the ED-71 group than in the untreated group at 1 and 3 weeks after the completion of lengthening on the coronal section. In cross sections, the cortical area and width in the ED-71 group showed significantly higher values than in the untreated group at 8 weeks after the completion of lengthening. Both the endosteal osteoid surface and endosteal eroded surface showed no differences between groups. However, the endosteal mineral apposition rate and endosteal bone formation rate were significantly higher in the ED-71 group. At 8 weeks after completion of lengthening, the intracortical area and intracortical BMC were significantly greater in the ED-71 group than in the untreated group, but no significant difference was noted in intracortical BMD. These findings indicate that ED-71 increases callus volume during the early period after the completion of lengthening, resulting in thick cortical bone formation.

Use of distraction osteogenesis techniques in skeletal dysplasias.

The authors report the results of 63 operations performed on patients with skeletal dysplasias using the techniques of distraction osteogenesis. There were a total of 26 patients (16 males, 10 females) with a mean age of 12.5 years. The group had 13 different skeletal dysplasias. In 55 of 63 (87%) of the procedures, the objectives of surgery were achieved. The authors review their results with reference to the type of fixator used and the underlying diagnosis. Circular frames were associated with improved results. Spondylometaphyseal dysplasia and Jeune syndrome were the only dysplasias treated that were associated with significant complications.

Distraction osteogenesis in maxillofacial surgery using internal devices: review of five cases.

PURPOSE: The purpose of this report is to show the feasibility and potential advantages of using internal devices for distraction osteogenesis in the management of maxillofacial skeletal deficiencies. PATIENTS AND METHODS: Distraction osteogenesis was used to correct a variety of maxillofacial skeletal deformities in five patients. One patient underwent bilateral Le Fort III advancement aided by distraction, three patients underwent mandibular ramus lengthening, and one patient underwent segmental alveolar reconstruction by distraction. The devices were activated by either a transcutaneous or transmucosal pin. After achievement of the desired skeletal transport, the activating pins were disengaged and removed from the distraction device. This allowed the distraction device to remain submerged and to stabilize the site of the consolidating bone. RESULTS: All patients achieved lengthening of their jaws. However, premature consolidation was noted in two patients, and one patient had significant relapse. CONCLUSIONS: Development of internal distraction devices is important to address the limitations of currently available biphasic systems. Potential benefits of internal devices include 1) elimination of skin scarring caused by translation of transcutaneous fixation pins, 2) improved patient compliance during the fixation or consolidation phase because there is no external component, and 3) improved stability of the attachment of the device to the bone.

[Design and application of internal devices for automatic elasticity distraction osteogenesis].

OBJECTIVE: To improve the design of the devices used for autoatic elasticity distraction osteogenesis, and to preliminarily testify their feasibility for clinical application. METHODS: The internal automatic distraction devices were constructed by three parts, including sinusoid shape memory alloy distractor, reconstructive plate and mini-plate. The animal model of trifocal bone transport reconstruction of canine mandibular mentis was duplicated, in which a 6 cm mandibular mentis segment was removed and the transport disc was created by osteotomy in both ends of the mandibular defect. The plates were used for internal fixation and shape reconstruction of the mandible. They also served as transport distraction guidance for the elasticity distractor. RESULTS: The new devices could complete bone transport as expected. Regenerated bone segments were formed in two months and both sides of the mandibular body were lengthened. Because of the narrow angle and obstruction from the thick soft tissues in the mentis area, the ultimate symphysis mentalis was not reconstructed. CONCLUSION: The design of the internal elasticity distraction devices showed potential feasibility for clinical practice. But further studies are still needed for better control of the strength and balance of distraction forces.

Distraction osteogenesis assisted by tissue engineering in an irradiated mandible: a case report.

Distraction osteogenesis (DO) can provide predictable bone regeneration without grafting procedures but requires long treatment time and forms less bone transverse to the direction of distraction. To promote 3-dimensional bone formation and shorten the consolidation period, tissue-engineered osteogenic material (injectable bone) was applied in a patient who was being treated with vertical DO with an osteocutaneous fibular flap to reconstruct the mandible. The material, which comprised autologous mesenchymal stem cells culture-expanded then induced to be osteogenic in character and platelet-rich plasma (PRP) activated with thrombin and calcium chloride, was infiltrated into the distracted tissue at the end of distraction and injected into a space created labially with a titanium mesh at implant placement. The infiltration contributed to full consolidation of the regenerate for 3 months, and the injection thickened the regenerated ridge and bridged a gap between the native mandible and distracted fibula. The reconstructed mandible was expanded from 10 mm to 25 mm in height despite a lacerated and opened labial periosteum in the distracted area. Six implants 18 mm in length were placed and subsequently achieved osseointegration. The cutaneous flap covering the implants was trimmed, and the palatal mucosa was transplanted to the regenerated ridge for vestibuloplasty. These raw surfaces were covered with PRP; within 3 weeks, they had attained an epithelium. The implants have supported a fixed prosthesis with adequate surrounding bone and attached mucosa. DO was assisted by tissue engineering and became effective in restoring the compromised mandible.

Use of distraction osteogenesis for the correction of deviated nasal septum and premaxilla in a horse.

A 13-month-old female Andalusian was evaluated because of a congenital left deviation of the maxilla and nasal septum, which had resulted in a gross malocclusion of the maxillary incisor teeth. Surgical correction of a deviated nasal septum and premaxilla in a horse was first reported in 1978 and involved a pre-maxilla osteotomy and use of an autogenous rib graft, with a nasal septum osteotomy performed in a second surgery; to the authors' knowledge, no alternative surgical treatments have since been described. In this horse, a new surgical treatment by distraction osteogenesis without nasal septum osteotomy was attempted. After the procedure, the premaxilla and nasal septum deviations were corrected; however, an overjet lesion (rostral projection of maxillary arcade over the mandibular arcade) was evident, for which the owners declined treatment. Compared with previously recommended procedures, distraction osteogenesis appears to be a less invasive treatment for congenital facial deformities and may be considered an option in the treatment of congenital deviation of the nasal septum and premaxilla (wry nose), head scoliosis, brachygnathism, and prognathism in horses.

Molecular signaling in bone fracture healing and distraction osteogenesis.

The process of fracture healing has been described in detail in many histological studies. Recent work has focused on the mechanisms by which growth and differentiation factors regulate the fracture healing process. Rapid progress in skeletal cellular and molecular biology has led to the identification of many signaling molecules associated with the formation of skeletal tissues, including members of the transforming growth factor-beta (TGF-beta) superfamily and the insulin-like growth factor (IGF) family. Increasing evidence indicates that they are critical regulators of cellular proliferation, differentiation, extracellular matrix biosynthesis and mineralization. Limb lengthening procedure (distraction osteogenesis) is a relevant model to investigate the in vivo correlation between mechanical stimulation and biological responses as the callus is stretched by a proper rate and rhythm of mechanical strain. This model also provides additional insights into the molecular and cellular events during bone fracture repair. TGF-beta 1 was significantly increased in both the distracted callus and the fracture callus. The increased level of TGF-beta 1, together with a low concentration of calcium and an enhanced level of collagen synthesis, was maintained in the distracted callus as long as mechanical strain was applied. Less mineralization is also associated with a low level of osteocalcin production. These observations provide further insights into the molecular basis for the cellular events during distraction osteogenesis.

Temporal and spatial increases in blood flow during distraction osteogenesis.

Ten adult mongrel dogs underwent left tibial lengthening at the proximal metaphysis. They were divided into three groups on the basis of distraction period: 14, 28, and 56 days. Quantitative technetium scintigraphy was used to measure regional blood flow within the tibiae during distraction, and periodically during consolidation, up to 17 weeks after operation. Blood flow was measured from regions of interest during the flow phase of the scan and expressed as a ratio of lengthened side to control side. At the distraction site, the flow increased to nearly ten times control, peaked at two weeks postoperatively, then decreased to four to five times the control for the remainder of the distraction period. During the consolidation period, significantly increased flow persisted at levels of two to three times control. The distal tibiae, away from the distraction gap, showed similar amplitude and temporal patterns of increased flow. No significant differences were found between the groups tested or when compared with similar fracture models. These findings lend credence to Ilizarov's hypothesis that distraction osteogenesis may contribute to healing of chronic osteomyelitis or hypovascular nonunions at distant sites by inducing a prolonged hypervascular state. Whether distraction osteogenesis transforms the normal reparative response of bone injury to a regenerative response remains conjectural.

The effect of low intensity pulsed ultrasound applied to rabbit tibiae during the consolidation phase of distraction osteogenesis.

The purpose of this study was to determine if low intensity pulsed ultrasound (LIPU) accelerated the maturation of regenerate bone when applied after distraction in a rabbit model. A mid-tibial osteotomy was performed in 26 New Zealand white rabbits and an external fixator applied anteromedially. After a seven day latency period, the tibiae were distracted 0.5 mm every 12 h for 10 days. Thirteen of the rabbits received LIPU for 20 min/day (treatment group) and 13 received sham LIPU (control group) from day 17 until sacrifice on day 37. Radiographs were taken weekly after distraction and the total and mineralized areas of the callus were measured. After sacrifice, dual-energy X-ray absorptiometry, torsional testing to failure, and histomorphometry were performed. Ultrasound-treated tibiae were a mean of 68.8 +/- 3.8% as stiff as and 68.2 +/- 6.0% as strong as the contralateral tibiae. Control tibiae were 78.7 +/- 7.0%, as stiff as and 70.2 +/- 7.9% as strong as the contralateral tibiae. The differences in stiffness and strength were not significant (p = 0.39 and 0.81, respectively) with the number of the animals tested in the study. The treatment group was 91.6% as dense as the contralateral side and the control group was 88.5% as dense as the contralateral side (p = 0.84). Radiographs revealed a significantly larger callus in the LIPU-treated tibiae at 1, 2 and 3 weeks after distraction compared to control tibiae (p < 0.01, 0.008 and 0.05, respectively). Histomorphometry revealed significantly less fibrous tissue in the LIPU-treated tibiae (p < 0.05) and a strong trend towards more bone in the LIPU-treated tibiae compared to controls (p = 0.06). LIPU was found to increase the size of the distraction callus and it might alter the composition of regenerate bone but it did not have a positive effect on the mechanical properties or density of regenerate bone when applied during the consolidation phase of distraction osteogenesis.

[Correction of deformities and length discrepancies of the forearm in children by distraction osteogenesis].

OBJECTIVES: To review the results of correction of deformities and length discrepancies of the forearm in children. METHODS: We evaluated the results of distraction osteogenesis in 10 forearms of nine patients with forearm shortening and/or deformity. The mean age at the time of surgery was 10.2 years (range 5 to 16 years). Etiologies included congenital radioulnar synostosis and preaxial longitudinal deficiency of the forearm (n=1), congenital radioulnar synostosis and shortening of the forearm (n=1), multiple hereditary osteochondromas (n=1), distal radial physeal arrest (n=1), Madelung's deformity (n=2), bilateral congenital forearm shortening due to postaxial acrofacial dysostosis syndrome (n=1), and defect nonunion of the radius alone (n=1) and with the ulna (n=1) with shortening. Ilizarov type circular external fixators and monoplanar Orthofix fixators were used in seven and three patients, respectively. The mean follow-up was four years and four months (range 1 to 9 years). RESULTS: The mean length gain was 36.7 mm (range 25 to 60 mm), which meant a mean increase by 31.5% (range 14% to 66%). A satisfactory functional and cosmetic improvement was achieved in all patients. Bone consolidation occurred in three to 13 months without a need for bone grafting. The most common complication was callus deformity seen after the removal of the fixator. CONCLUSION: Lengthening of the forearm results in improved upper extremity function, especially in activities requiring equal arm length as well as better cosmetic appearance if adequate soft tissue is preserved.

Mandibular distraction osteogenesis: effects on articulation and velopharyngeal function.

Mandibular distraction osteogenesis has gained popularity for the treatment of mandibular hypoplasia and asymmetry. With this technique, significant skeletal and soft tissue changes in the lower face can be produced in a relatively short time period without the need for extensive surgery. Despite general acceptance, the long-term sequelae of the procedure have not been well documented. In this report, we describe two patients who experienced changes in their articulation and velopharyngeal function after unilateral mandibular distraction. The first patient underwent 35 mm and the second patient 45 mm of mandibular lengthening. In both cases, articulation skills declined, and there was velopharyngeal inadequacy immediately after distraction. However, in both cases, the velopharyngeal inadequacy was transient; one patient recovered near-normal velopharyngeal function within 1 month after distraction and the second patient recovered function within 8 months. The relations among mandibular distraction, articulation, and velopharyngeal function are discussed.

Mandibular distraction osteogenesis: a historic perspective and future directions.

Although orthognathic surgery has gained a generalized acceptance for maxillomandibular deformity correction, several limitations are associated with acute advancement of osteotomized bone segments. Furthermore, large skeletal discrepancies, such as those seen in syndromic patients, require such extensive bone movements that the surrounding soft tissues will not adapt to their new position, resulting in relapse or compromised function and esthetics. Recently, a number of experimental and clinical investigations have demonstrated that gradual mechanical traction of bone segments at an osteotomy site created in the craniofacial region can generate new bone parallel to the direction of traction. This phenomenon, known as distraction osteogenesis, opens up new possibilities in the correction of craniofacial deformities by orthodontists and maxillofacial surgeons. Hence, the purpose of this article is to review the historic development and biologic foundation of mandibular distraction osteogenesis, critically evaluate the current mandibular distraction devices with their clinical applications, and predict the future evolution of mandibular osteodistraction techniques.

Distraction osteogenesis of basal mandibular bone for reconstruction of the alveolar ridge.

Alveolar distraction is being used increasingly for alveolar bone reconstruction in patients with severe mandibular defects. When there has been total loss of alveolar bone, distraction of the mandibular basal bone is necessary. Distraction osteogenesis is considerably more challenging in mandibular basal bone than in alveolar bone. The low level of the cut increases the technical difficulty and may result in a poor outcome. We describe three cases in which more than 10 mm of distraction of mandibular basal bone was required. Semirigid distraction devices were used to reconstruct the alveolar structures in each case.

A new device to quantify regenerate torsional stiffness in distraction osteogenesis.

We present a newly developed torsional stiffness measurement device with the potential to quantitatively assess the in vivo torsional stiffness of bone regenerate during distraction osteogenesis. We describe the form and function of this device and its application in a model of regenerate consolidation. The device was able to produce data to assess stiffness of the regenerate with an accuracy between +/- 3 and +/- 9% for material stiffness ranging between 0.1 and 2.4 Nm/o and with a precision of +/- 3.6%. This method provides advantages over similar methods of bone fracture healing assessment with guaranteed maintenance of bone axis, minimized risk of bone misalignment during the bone healing process and a close relation to the functional loading pattern in torsion of bones such as tibia and femora.
