Nursing experience of redesigning a schedule to consider gender identity disorder in recruits' physical examination.

The military service in Taiwan currently uses a recruiting system. With the duty of helping examinees get through their physical examination, we examiners have to consider their individual statements and privacy so as to adjust where necessary while undertaking the examination. The purpose of this case study is to explore the nursing experience of handling the gender identity disorder patient in the field of physical examination. Through their participation, and by considering their personal circumstances, we can execute a thorough examination effectively and smoothly but not influence other examinees, and avoid unexpected disputes. In particular, private items including chest X-ray, electrocardiogram, and gender organ examinations must be executed with care. The nursing designed alternative schedule may provide a reference for the participants involved in the field of recruits' physical examination.

The impact of information technology on nursing practice and knowledge.

The impact of information technology on nursing has been a subject of discourse and dissertation for the latter half of the 20(th) century and the early part of the 21(st). That this burgeoning technology has impacted the way nurses nurse can be without doubt. Whether this technology has and will have a negative or positive outcome on nursing practice is where the debate centres. The purpose of this paper is to analyse the debate that surrounds the issues of the impact of Information Technology (IT) on nursing practice as well as to briefly assess the types of Clinical Information Systems (CIS) that may be beneficial as opposed to detrimental to the practice of nursing in the 21(st) Century and beyond.

Attitudes of nurses towards End-user Development.

The purpose of this study is to explore nurses' attitudes toward developing self-made information tools. In order to understand nurses' attitudes, we studied the factors that affect nurses' attitudes toward End-user Development (EUD). The investigation of nurses in Taiwan hospitals showed that nurses had positive attitudes toward EUD. A framework of nurses' attitudes toward EUD is presented. A gap was found between the functions provided by the nursing information system and self-assessment required by the job. The nurses were inclined to be dissatisfied with support provided by the information centers. Nursing managers were advised to enhance on-the-job training for computer applications, and to include computer competencies in the nursing ladders program. The results indicated that nurses were willing to develop their own tools. We believe that nurses with computer application skills who are provided with appropriate tools can become nursing application developers. The results encourage nurses to develop software by themselves. Further research and development is required to enhance the impact of EUD and establish a sharing environment for EUD applications.

Revision of the Belgian Nursing Minimum Dataset: From data to information.

The Ministry of Public Health commissioned a research project to the Catholic University of Leuven and the University Hospital of Liège to revise the Belgian Nursing Minimum Dataset (B-NMDS). The study started in 2000 and will end with the implementation of the revised B-NMDS in January 2007. The study entailed four major phases. The first phase involved the development of a conceptual framework based on a literature review and secondary data analysis. The second phase focused on language development and development of a data collection tool. The third phase focused on data collection and validation of the new tool. In the fourth phase the validity and reliability of the dataset was tested. The new dataset is without avail if it is not leading to new information. Four applications of the dataset has been defined from the beginning: evaluation of the appropriateness of stay (AEP) in the hospital, nurse staffing, hospital financing and quality management. The aim of this paper is to describe how the B-NMDS can contribute to each of these applications.

eHealth in nursing--Already routine? Results of two case studies from Germany.

eHealth activities are beginning to emerge from the research domain and many industrialized countries are embarking on national eHealth programs. This does not mean, however, that nurses will automatically benefit from the new technology. The aim of this paper therefore is to identify key issues for a successful integration of nursing into eHealth scenarios. Two eHealth case studies from Germany serve to illustrate the importance of focussing on strategic applications, integrating users and managing change and innovation properly. eHealth - supported applications embrace (i) all multidisciplinary processes enabling patient safety through continuity of care, (ii) nurse-led discharge, case and disease management and (iii) cross-sector communication including the electronic nursing summary. We found that the German eHealth program does not meet these criteria sufficiently. We therefore conclude that there is an urgent need to incorporate existing results of research and demonstration projects and a need for further research. eHealth initiatives in other countries must be evaluated by the same criteria for their capacity to include nurses in eHealth--supported patient care.

Workplace response to virtual caregiver support and remote home monitoring of elders: The WIN project.

Research has demonstrated the health and financial cost to working caregivers of older adults and the cost to business in lost productivity. This paper describes the implementation of the Worker Interactive Networking (WIN) project, a Web-based program designed to support employed caregivers at work. WIN innovatively linked working caregivers via the Internet to home to monitor elders' status using wireless sensor technology and included an online information and support group for a six-month period. Twenty-seven employees from thirteen business sites participated. Despite problems with wireless carrier service, feasibility outcomes were achieved. We were able to collect six months of continuous real time data wirelessly from multiple types of homes across 4 states. This model demonstrates that businesses can offer a similar program and not be overwhelmed by employee demand or abuse of technology access. Reluctance to consider home monitoring was apparent and was influenced by familial relationships and values of privacy and independence.

Content coverage of SNOMED-CT toward the ICU nursing flowsheets and the acuity indicators.

Nursing flowsheets contain discrete and detailed patient data that can be reused in various clinical decisions including patient acuity estimation. To be reused, patient data should be represented consistently and unambiguously with standardized terminologies. This study investigated the domain completeness of SNOMED-CT toward the ICU nursing flowsheets and the acuity indicators used at Mayo Clinic. We first paraphrased the terms and the semantic relations appeared in the source materials into short clinical statement. Then, the entire prepared statement was mapped to SNOMED-CT and validated by expert reviewers. SNOMED-CT provided complete matches and partial matches to 78.13% and 14.1% of the flowsheets statements respectively. It provided complete matches and partial matches to 60.8% and 39.2% of the acuity statements respectively. The average observed agreement among the reviewers ran around 85%. Lack of appropriate terms was the major cause of the incomplete matches with the flowshee statements. Majority of the incomplete matches with the statements from the acuity indicators were caused by limitations in the concept models.

Developing a cancer nursing information system: Determining core nursing diagnoses for the six most common cancers in Korea.

The purpose of this study was to determine core nursing diagnoses (CNDs) from NANDA Taxonomy II for a cancer nursing information system supporting the nursing process. The main objective for determining CNDs was to increase the perceived usefulness and the perceived ease of use of the information system by supporting the process of making nursing diagnoses. The CNDs were for each of the patient groups of the six most common cancers in Korea. For the determination of CNDs, a survey and an analysis of patient records were performed. Two hundreds and nineteen clinical nurses participated in the survey and 72 patient records were analyzed. Based on the findings of the survey and analysis of patient records, 15 CNDs for each of the populations were determined.

Nursing minimum data set: A literature review.

UNLABELLED: Essential information for nurses may be obtained from information technology systems, for this reason it is fundamental the adoption of standard language which translates the essence of nursing. The Nursing Minimum Data Set and Nursing Management Minimum Data are relevant in description, register and retrieval of these data. A literature review on the use of these data sets for research on the subject, the trends and nurses' practice was accomplished. METHOD: Selected articles from the index of Medical Literature and Analysis and Retrieval System on Line (1993 - 2005), from the integrated library archives of Regional Medicine Library, in English and Portuguese. FINAL CONSIDERATIONS: The findings permitted the identification of the total number of publications which show the necessity o dissemination and discussion of the subject among the Brazilian nurses; description of current health trends, the elderly and chronic disease patients care; nursing home care evaluate trends, justifying costs, comparing and evaluating cares. The increasing interest on the use of these data sets can be explained by the necessity of consistent and available information at the shortest length of time possible.

Design and development of the pressure ulcer nursing information system for clinical risk management.

Since 2002, hospitals have to report to Nursing Service Department, Hospital Authority Head Office on the pressure ulcer trends for risk management in Hong Kong. In line with the strategy, hospitals have designed their own patient observation records and reporting forms for monthly analysis and reporting of in-patient hospital acquired pressure ulcers. The incidence rates of individual hospital and its specialties are then calculated manually or using electronic spread sheets. However, the diversity in data definition and vocabulary use generates difficulties in communication among professionals and hospital managers. The development of the system would help to standardize the requirements and to reduce the time required for generating trends and ulcer information. The system also lays the foundation for future systems integration with the changing information system infrastructure.

Soprano Specialty Forms Project--A solution that empowers clinicians in the management of research and audit data.

Throughout the world, clinicians (including nurses) create ad-hoc stand-alone databases in order to meet their individual research and audit requirements. These databases often exist in large numbers within an institution. They meet individual requirements and capture a large quantity of valuable clinical information, but it is not possible to pool or share that information because of a lack of common data structures and coding. The Soprano Specialty Forms solution developed by Orion Health in collaboration with Auckland District Health Board resolves this problem. The solution provides a common data storage and coding infrastructure within which clinicians can create their own forms for data collection, enter data through an integrated portal used as part of their everyday clinical practice, and easily query the data collected regardless of its source.

Apply creative thinking of decision support in electrical nursing record.

The nursing process consists of five interrelated steps: assessment, diagnosis, planning, intervention, and evaluation. In the nursing process, the nurse collects a great deal of data and information. The amount of data and information may exceed the amount the nurse can process efficiently and correctly. Thus, the nurse needs assistance to become proficient in the planning of nursing care, due to the difficulty of simultaneously processing a large set of information. Computer systems are viewed as tools to expand the capabilities of the nurse's mind. Using computer technology to support clinicians' decision making may provide high-quality, patient-centered, and efficient healthcare. Although some existing nursing information systems aid in the nursing process, they only provide the most fundamental decision support--i.e., standard care plans associated with common nursing diagnoses. Such a computerized decision support system helps the nurse develop a care plan step-by-step. But it does not assist the nurse in the decision-making process. The decision process about how to generate nursing diagnoses from data and how to individualize the care plans still reminds of the nurse. The purpose of this study is to develop a pilot structure in electronic nursing record system integrated with international nursing standard for improving the proficiency and accuracy of plan of care in clinical pathway process. The proposed pilot systems not only assist both student nurses and nurses who are novice in nursing practice, but also experts who need to work in a practice area which they are not familiar with.
