DNA content of macrophages in pulmonary granuloma induced in rats by complete Freund's adjuvant.

Pulmonary granulomas were induced in rats with complete Freund's adjuvant. Histologic and cytologic alterations and Feulgen-DNA amounts determined by a cytophotometric method were studied. Fourteen (hours after injection, the pulmonary interstitia were thickened by cell infiltration. After 3 days; small granulomatous nodules developed containing macrophages, epithelioid and giant cells. The Feulgen-DNA amounts increased significantly in the course of the first day and reached peak values on the 3rd day. Then they showed a tendency to decrease after the 7th day. The present results confirm the previously obtained data, revealing the stimulating effect of the complete Freund's adjuvant on cell metabolism, DNA synthesis included.

[Granuloma formation following intracutaneous administration of procaine-polyvinylpyrrolidone].

After intracutaneous application (dermojet) of procaine polyvinylpyrrolidone (PVP) because of backpain in a 60-year-old woman, multiple brownish-red nodules developed at the injection site within a few days. The histology showed signs of a sarcoid granuloma. There was no clinical evidence for sarcoidosis in this patient. An allergic reaction of the granulomatous type is suspected.

Role of polymorphonuclear leukocytes in the pulmonary clearance of arrested cancer cells.

Treatment of mice with trypan blue led to an accelerated clearance of 125I-5-iodo-2'-deoxyuridine-radiolabeled B16 melanoma cells arrested in the lungs of tumor-bearing mice and the numbers of pulmonary nodules developing after intravenous injection of nonradiolabeled melanoma cells was reduced. Although macrophage function was normal or depressed in trypan-blue-treated mice, there were increased numbers of circulating polymorphonuclear leukocytes which had increased cytotoxicity for B16 melanoma cells and which produced increased levels of toxic oxygen radical. These findings suggested that polymorphs were responsible for accelerated pulmonary clearance of arrested melanoma cells and this conclusion was supported by the decreased clearance of cancer cells from the lungs of mice treated with antipolymorphonuclear leukocyte antiserum. These studies expand the repertoire of effector cells responsible for determining the extent to which hematogenously disseminated cancer cells are retained in the pulmonary vasculature prior to their extravasation and secondary growth.

Sequential morphologic changes during methapyrilene-induced hepatocellular carcinogenesis in rats.

The pathogenesis of hepatocellular tumors induced in F344 rats by the antihistaminic methapyrilene was investigated by light and electron microscopy in a serial sacrifice study. Eosinophilic foci of altered hepatocytes were found in portal areas after 1 week of treatment, the eosinophilia being caused by proliferation of mitochondria. Eosinophilic neoplastic nodules developed from such lesions after 16 weeks of treatment. Hepatocellular carcinomas developed after 26 weeks of treatment. Mitochondrial proliferation, which had been found as a marker for hepatocytes altered by this compound at 1 week of treatment, was still present in the hepatocellular carcinomas, which therefore met the morphologic criteria of oncocytomas.

Canine cutaneous lymphoma.

Cutaneous lymphoma (mycosis fungoides) was diagnosed by multiple biopsies of nonulcerated cutaneous lesions in 13 dogs. The disease was initially characterized by erythema and alopecia of the trunk. Subsequently severe scaling, pruritus, and skin nodules developed. Lymph nodes were usually normal in size during early stages of the disease but became enlarged as the disease became extensive. Four dogs were treated with cytotoxic drugs. Marked improvement of skin lesions occurred in 3 dogs, but treatment did not prevent development of extra-cutaneous lesions.

[Human subcutaneous dirofilariasis: 8 new cases in northern Italy].

Eight new cases of Human Dirofilariasis in people living in Northern Italy were recorded: 3 from the province of Asti and 2 from Alessandria (Piedmont), 1 from Corsico (Miland, Lombardy) and 2 from Ravenna (Emilia-Romagna). All the patients were adults, 42 to 66 years old, 4 men and 4 women. The localization of the nematode in all the cases was subcutaneous. Signs and symptoms were always mild and consisted in the growth of a 1 to 2 cm large nodule associated with occasional localized itching and oedema, sometimes with reddening. In one case, the sting of a non-specified winged insect was recorded in the anamnesis, in the same spot where the nodule developed. In another case, the nematode came out spontaneously from the lesion. The aetiological agent was always identified as Dirofilaria repens. These 8 new cases confirm the comparatively frequent presence of Human Dirofilariasis in regions of Northern Italy, namely in the Po Valley.

NonHodgkin's lymphoma of the male urethra.

Ten cases of malignant lymphoma of the female urethra have been reported. To our knowledge we report the first such case in a man who presented in acute urinary retention with a mass protruding from the urethral meatus. Multiple subcutaneous nodules developed over the anterior abdominal wall and a 3 x 3 cm. mass developed above the umbilicus. Wedge resection of this mass was consistent with large cell lymphoma. Treatment consisted of 2 courses of arabinoside C, doxorubicin and prednisone. Followup 6 months later showed no urethral or other recurrence. Local excision, radical excision, radiotherapy (external beam and intracavitary) and chemotherapy have been used with success in other cases.

Coccidioidomycosis with necrotizing granulomatous conjunctivitis.

A patient with a steroid-resistant conjunctival nodule developed severe necrotizing granulomatous conjunctivitis during aggressive treatment with topical, subconjunctival, and systemic steroids. Culture of the tissue yielded Coccidioides immitis. A cavitary lesion was noted on a chest radiograph, and the same organism was recovered from a lung biopsy specimen. The granulomatous conjunctivitis was controlled only after aggressive debridement of the affected area and months of treatment with topical amphotericin B and oral fluconazole. A review of the literature suggests that subclinical ocular involvement from pulmonary coccidioidomycosis may be more common than generally believed. Conditions that blunt the host immune response may lead to a higher incidence of clinically significant ocular involvement.

[Pathological features of neuroleukemia. Changes and lessons].

Between 1971 and 1992 neuropathological examinations were performed in 316 patients having died of leukaemia. Among them 73 had acute lymphoid leukaemia, 134 acute myeloid leukaemia, 49 chronic lymphoid leukaemia and 60 chronic myeloid leukaemia. Meningeal leukaemia, which had been a frequent pathological complication at the beginning of the examination period, later had become rarer. The study made it possible to define further neurologic complications, the characteristic neuropathological changes of the different types and some subtypes of leukaemia and to draw clinical consequences. The incidence of cerebral "leukaemic nodules" was twelve times higher in the myeloid than in the lymphoid leukaemias, this may be the explanation for the fact that central nervous system haemorrhage was more frequent in the myeloid types. Subdural haematoma can primarily be expected in acute myeloid leukaemia. The neuropathologic features of chronic lymphoid leukaemia were particular, as no "leukaemic nodules" developed, and meningeal leukaemia was a rare event; at the same time, it was related with the highest incidence of infiltration of the dura mater and the sciatic nerve.

The immunology of scabies.

Individuals with scabies react to bites by generating a cell-mediated immune response at the bite site. This elicits a very itchy papule that is often excoriated. Sometimes ulcerated papules, vasculitis, and nodules develop as a result of other immunologic reactions in skin. Immunologic reactions mediated by antibodies of the immunoglobulin G (IgG), IgM, and especially, IgE classes may also be involved. None of these reactions have been shown to eliminate all mites from the skin surface, but locally these reactions may prevent the epidemic multiplication of scabies' organisms on the skin surface, as observed in some patients with crusted scabies.

Isolated tuberculosis of the pancreas masquerading as a pancreatic mass.

A 65-yr-old woman presented for evaluation of a pancreatic mass. She had been suffering from severe constitutional symptoms for 18 months; those symptoms included weight loss, increasing fatigue, night sweats, and recurrent fever attacks up to 40 degrees C. Later, bluish subcutaneous nodules developed on her lower limbs. Laboratory tests yielded signs of chronic inflammation and impaired glucose tolerance with elevated serum insulin and glucagon concentrations. Skin biopsy revealed lobular panniculitis. Ultrasonography and a CT scan demonstrated enlargement of the pancreas, and endoscopic retrograde pancreaticography disclosed displacement and stenosis of the main pancreatic duct. The patient was referred for explorative laparotomy, which was highly suggestive of a malignant pancreatic tumor. However, histological examination of the resected pancreatic and peripancreatic mass revealed tuberculous pancreatitis. This form of isolated tuberculous pancreatitis, associated with lobular panniculitis and laboratory features consistent with a tumor of the endocrine pancreas, has not been reported previously. Active tuberculosis should be a leading differential diagnosis in a patient with an enlarged pancreas when the usual diagnostic reasoning does not yield conclusive results.

Melioidosis in a goat.

Melioidosis was diagnosed in an adult Boer goat ewe. The owner initially noticed swelling of one half of the udder. Several firm nodules developed in the affected part of the udder 2 weeks later. At necropsy, abscesses containing a creamy to yellowish-green exudate were found in the mammary gland and one of the kidneys. Pure cultures of Pseudomonas pseudomallei were isolated from the abscesses in the udder and inoculation of male guinea-pigs with the causative bacterium, produced a Shwartzman reaction in the testis and serositis as well as necrotic lesions, in parenchymatous organs. The isolate showed sensitivity to amoxycillin-clavulanate, but resistance to a number of other antimicrobials. This appears to be the first case of melioidosis in a domestic animal in South Africa.

[A primary observation on the effect of cell fusion on metastatic potential of tumor cells].

Mouse spleen cells transfected with pSV2 neo by CaPO4 precipitation were fused with highly metastatic cell clone (PLA801-D95) from human large cell lung cancer cell line. Hybrid cell clone PMS-2 was obtained after G418(400/ml)selection. After injection of 7 x 10(6) PMS-2 cells into nude mice, there was a tumor nodule developed, but the metastatic foci could not be found while 3 x 10(6) PLA801-D95 cells would metastasize to lung and lymph nodes after they were injected into nude mice. It might indicate that sometime mouse spleen cells could not suppress tumor formation but the metastatic potential could be suppressed by the fusion of mouse spleen cells with the lung cancer cells. The results of growth curve, serum independence and incorporation rates of H-thymidine all showed that the growth rates of parental cells were higher than those of PMS-2. Our data suggest that suppression of tumorigenicity and metastatic potential could be controlled by different kinds of genes, and the cloning of metastasis suppressor gene by subtractive hybridization is ongoing in our laboratory.

Accelerated nodulosis in a patient with psoriasis and arthritis during treatment with methotrexate.

A 66-year-old woman with longstanding psoriasis involving the skin presented with asymmetrical polyarthritis. Methotrexate (MTX) was given initially intramuscularly and orally. Intramuscular MTX was discontinued, and a few months after she had been taking only oral MTX she developed nodules, first in surgical incisions, and subsequently in her buttocks, thighs, legs, and arms. Reduction of the dose of oral MTX was followed by gradual diminution in size of the nodules and then total disappearance.

Infantile myofibromatosis: a case with unusual features and review of the literature.

A three-month-old boy was admitted for a slowly progressing nodule in his right chin, first recognized at birth and recently accompanied by facial paralysis. It was found to be a soft tissue mass arising in subcutaneous tissue and extending deep into the temporal muscle, causing temporal bone erosion without infiltrating the dura. Initially interpreted as mesenchymal chondrosarcoma, combined chemotherapy (PULSE-VAC) was given. Eighteen months later an additional nodule developed in the paraspinal skin. Evaluation of both lesions showed vimentin and alpha-smooth-muscle-action positivity. As the final diagnosis was multicentric infantile myofibromatosis, the child was followed up without therapy. Thirty months later, multiple osteolytic lesions appeared on the skull and long bones of the extremities. Some lesions remained stable and some regressed during two years of follow-up.

[Prolonged use of moxisylyte chlorhydrate (Icavex) by intracavernous self-injections in the treatment of impotence. Evaluation of long-term tolerance].

Moxisylyte chlorhydrate is a selective alpha-blocker of the post-synaptic alpha 1-adrenoreceptors. It has been used since 1992 for self-injections to induce erection. Tolerance has been studied in open trials in 143 men using 20 mg per intracavernous injection. Among these subjects, 104 were followed for 11 months. Self-administration was performed 7,509 times, i.e. 49.1 injections per subject over a mean period of 307 days. No severe side effects were observed. A total of 1,041 undesirable effects were reported by 90 subjects (75.1%), including mechanical disorders (28.2%) such as pain and burning sensation at injection, hematomas and ecchymoses. Nodules developed in 0.08% of these cases but always regressed. In 71.8% of the cases, the undesirable events was imputable to moxisylyte: dry mouth (2.73%), somnolence (1.9%), sinus congestion (0.71%). No case of priapism was reported. Long-term evaluation showed a reduction in the main undesirable effects with time. This fall off in the mechanical events could be explained by the subjects become for familiar with the technique. Pharmacological effects remain to be analysed. Added to the good tolerance, this result suggests that self-injection can be proposed as a first intention treatment for impotency.
