Bovine farcy in the Accra Plains of Ghana.

Bovine farcy is described for the first time in Ghana in herds kept in the Aveyime area of the Accra Plains. Reasons why the disease seems confined to a small geographical area are not yet clear. Different breeds of cattle are equally affected. Mineral deficiencies were not observed in the samples taken for analysis (Ca, Mg, P, Na, K, Cl). Limited examination of tick population on cattle showed a predominance of A. variegatum. Factors enhancing the pathogenicity and persistence of the Mycobacterium are still not clear. A study is soon to start.

Nocardia asteroides keratitis: a case associated with soft contact lens wear.

Ocular nocardiosis is rare. Most cases of nocardial keratitis have resulted from corneal trauma by objects contaminated with the organism. We describe the first case of Nocardia asteroides keratitis associated with an extended wear soft contact lens. We also describe its successful treatment with topical trimethoprim-sulfamethoxazole in combination sulfacetamide.

Nocardia asteroides keratitis presenting as a persistent epithelial defect.

An elderly male presented with epithelial defect which resisted all medical and therapeutic approaches until Gram stain and cultural report documented the presence of Nocardia asteroides of the offending organism. A 3-month course of chronic keratitis with persistent epithelial defect resolved in 48 h following the use of topical trimethoprim-sulfamethoxazole.

Nocardia asteroides keratitis associated with extended-wear soft contact lenses.

Chronic unilateral keratitis developed in a 34-year-old woman who wore extended-wear contact lenses. A clinical response was not obtained until Nocardia asteroides was correctly identified as the causal agent. The response to 30% sulfacetamide was dramatic. We review the clinical presentation of Nocardia keratitis and recommendations for management. Use of corticosteroids should be avoided in Nocardia keratitis. This opportunistic organism should be considered in patients who wear contact lenses in whom infectious keratitis develops.

[Nocardial brain abscess: case report].

A 25-year-old man, who was slightly immunosuppressed, presented headache and right motor weakness due to multiple brain abscesses disseminated from lung abscess. They were diagnosed, by bacteriological examination, as nocardial brain abscesses (nocardia asteroides) 4 weeks after the first operation. In spite of delay in the diagnosis, he was relieved by operations (three times) and chemotherapy including high doses of Sulfamethoxazole-Trimethoprim. He was eventually discharged. We stress the necessity of early diagnosis and the efficacy of Sulfamethoxazole-Trimethoprim for nocardial brain abscess.

Nocardia bursitis.

Nocardia asteroides was recovered from the olecranon bursa in a healthy young man with no predisposing illness. Diagnosis was made on bursal fluid culture. Clinical features and bursal fluid analysis were misleading. Our patient responded to sulfonamide therapy.

Endogenous Nocardia endophthalmitis: report of a case in a patient treated for lymphocytic lymphoma.

A patient with pulmonary, cerebral, and intraocular nocardiosis is reported. The intraocular disease was first recognized at autopsy. Only 7 patients with intraocular nocardiosis have been described previously. The intraocular lesions were central and paracentral, and the distribution resembled those described in 5 of the previous patients with metastatic intraocular disease. The ocular histology differed from those cases previously reported in that a granulomatous reaction accompanied the suppurative component. Malignant lymphoma treated with radiation therapy, cytotoxic drugs, and steroids formed the background against which the disease occurred in this patient.

Monoclonal antibodies against isoelectrically focused Nocardia asteroides proteins characterized by the enzyme-linked immunoelectro-transfer blot method.

Sera from rabbits immunized with culture filtrates and homogenates of Nocardia asteroides B1042 gave at least eight precipitin bands by immunoelectrophoresis. At least 20 proteins with isoelectric points (pls) in the pH 4 to 5.4 range were observed in isoelectric focusing patterns. The enzyme-linked immunoelectro-transfer blot (EITB) assay showed that several of the isofocused proteins reacted with rabbit antisera and with sera from nocardiosis and tuberculosis patients. Antibodies against three proteins with pls of 4, 4.43, and 4.68 (antigenic factors 1,6,8) were present in nocardiosis patients' sera. The proteins were excised from isofocused gels, and IgG monoclonal antibodies (MAbs) were produced by the hybridoma method. MAbs against factors 1 and 6 did not crossreact with cytoplasmic antigens of Mycobacterium chelonae, M. intracellulare serotypes 4B and 8A, M. fortuitum, M. gordonae, or M. kansasii in the EITB method. Factor 8 (MAb) crossreacted with antigens of M. intracellulare and M. fortuitum.

Lymphocutaneous nocardiosis caused by Nocardia asteroides. Case report and literature review.

A 22-year-old man developed erythematous nodules and an ulcer mimicking sporotrichosis on his right hand and forearm, which was subsequently diagnosed as lymphocutaneous nocardiosis caused by Nocardia asteroides. To our knowledge, 21 cases of lymphocutaneous nocardiosis have been reported, with the majority of those cases, except for a few that remain uncertain, being due to Nocardia brasiliensis. The case reported herein, the first known definitive case caused by N asteroides (to our knowledge), showed a more chronic course and less inflammatory manifestation compared with that caused by N brasiliensis. This is probably because N brasiliensis has a stronger virulence than N asteroides.
