Colonoscopic treatment of colon cancers.

Recent advances in endoscopic technology have enabled conservative treatment for patients with carcinoma in situ. The treatment of submucosally invasive carcinomas, or malignant polyps, is still controversial, however. The use of widely advocated histologic criteria, such as poorly differentiated histology (Grade III cancer), level 4 invasion or involved margin status, or lymphatic venous invasion as risk factors for adverse outcome, should be examined by multivariate analysis. Unfavorable histology at the invasive margin, PCNA, MUC-1 expression, and chromosomal abnormalities may be new candidates for prognostic indicators in patients with submucosally invasive carcinoma.

Abnormal lymph drainage in patients with chronic venous leg ulcers.

BACKGROUND: Edema often occurs in association with venous leg ulcers. OBJECTIVE: Our purpose was to assess lymphatic function in the legs of patients with venous leg ulcers. METHODS: Quantitative lymphoscintigraphy was performed in 22 normal control subjects, 32 patients with venous leg ulcers, and 7 patients with varicose veins only. RESULTS: Lymph drainage declined with age in the normal controls (r = -0.57; p = 0.0001). Lymphatic function was reduced in the ulcerated legs compared with normal limbs (age < 65 years; p < 0.0001) and in edematous legs compared with nonedematous legs (age > 65 years; ulcerated legs, p = 0.016; nonulcerated legs, p = 0.021). Lymph drainage was lower in the legs of patients with varicose veins only (p = 0.004). CONCLUSION: Impaired lymphatic function is associated with both venous leg ulcers and chronic venous hypertension.

Colonization resistance of the digestive tract and the spread of bacteria to the lymphatic organs in mice.

After oral contamination of conventional mice with high doses of Escherichia coli, Klebsiella pneumoniae or Pseudomonas aeruginosa the contaminant was recovered in abnormally high concentrations from the duodenum and caecum during the first few days. In this initial colonization phase, evidence of spread was obtained by culturing the cervical and mesenteric lymph nodes and spleen. Longer after contamination the intestinal concentration decreased to normal and spread stopped. In orally antibiotic-treated mice, the situation seen during the initial colonization phase in conventional mice occurred after a much lower oral contamination dose and persisted during the entire observation period of 2 weeks.

[Expression of Ezrin and E-cadherin in invasive ductal breast cancer and their correlations to lymphatic metastasis].

BACKGROUND & OBJECTIVE: Ezrin, a cytoskeleton linker protein, is actively involved in regulating the growth and metastasis of cancer cells. This study was performed to detect the expression of Ezrin and E-cadherin in primary invasive ductal breast cancer in order to evaluate their possible roles in lymphatic metastasis. METHODS: The expression of Ezrin and E-cadherin in 60 specimens of primary invasive ductal breast cancer (23 with metastasis, 37 without metastasis) was detected by SP immunohistochemistry. RESULTS: The abnormal expression rates of Ezrin and E-cadherin were significantly higher in the cases with metastasis than in the cases without metastasis (73.91% vs. 51.35%, P=0.039; 65.22% vs. 40.54%, P<0.001). The abnormal expression of Ezrin was positively correlated to that of E-cadherin (r=0.898, P=0.038). CONCLUSION: Ezrin and E-cadherin are closely related to invasion and metastasis of ductal breast cancer, suggesting that they are important tumor markers in predicting lymphatic metastasis of invasive ductal breast cancer.

Cardiovascular failure in mouse embryos deficient in VEGF receptor-3.

Vascular endothelial growth factor (VEGF) is a key regulator of blood vessel development in embryos and angiogenesis in adult tissues. Unlike VEGF, the related VEGF-C stimulates the growth of lymphatic vessels through its specific lymphatic endothelial receptor VEGFR-3. Here it is shown that targeted inactivation of the gene encoding VEGFR-3 resulted in defective blood vessel development in early mouse embryos. Vasculogenesis and angiogenesis occurred, but large vessels became abnormally organized with defective lumens, leading to fluid accumulation in the pericardial cavity and cardiovascular failure at embryonic day 9.5. Thus, VEGFR-3 has an essential role in the development of the embryonic cardiovascular system before the emergence of the lymphatic vessels.

Estimation of lymphatic conductance. A model based on protein-kinetic studies and haemodynamic measurements in patients with cirrhosis of the liver and in pigs.

A model of lymphatic conductivity (i.e. flow rate per unit pressure difference = conductance) based on protein-kinetic and haemodynamic measurements is described. The model is applied to data from patients with cirrhosis and from pigs with different haemodynamic abnormalities in the hepatosplanchnic system. In cirrhotic patients without ascites the estimated thoracic duct conductance (gthd) was three times higher than normal whereas this value was close to normal in patients with tense ascites. The estimated conductance of the right lymphatic duct was ten times below that of the thoracic duct in patients with ascites. In pigs gthd was similar to that in normal humans and no change was seen during acute congestion of the liver. In ascitic pigs gthd was low. The estimated conductance of the liver blood-lymph barrier was similar in normal humans and pigs, but decreased in cirrhosis and was thus compatible with increased sinusoidal wall tightening and fibrosis in the interstitial space of the liver. The model presented supports the so-called 'lymph-imbalance' theory of ascites formation according to which a relatively insufficient lymph drainage is important in the pathogenesis of hepatic ascites.

Detecting lymphatic metastases from prostatic carcinoma: superiority of CT.

CT scanning was performed on 29 consecutive patients with clinically localized prostatic carcinoma (stage A or B). Bipedal lymphangiography was performed in 12 cases. Histologic confirmation was obtained in 15 cases (pelvic lymphadenectomy in 12 and positive percutaneous needle biopsy in three). In these 15 proven cases, the overall accuracy of CT was 93% with one false positive and no false negatives. Lymphangiography was far less accurate (55%) with two false positives and three false negatives in 11 proven cases. The greater accuracy of CT resulted primarily from its ability to detect abnormal nodes in the pelvis, particularly hypogastric nodes, which are rarely opacified by lymphangiography. Preliminary experience suggests that CT is superior to lymphangiography in detecting early lymphatic spread from prostatic carcinoma in the pelvis. In the future, CT-guided percutaneous needle biopsy should be useful for documenting metastases in these patients.

Long-term survival of patients with stage IV gastric carcinoma.

BACKGROUND: Survival of patients with Stage IV (based on general rules established by the Japanese Research Society for Gastric Cancer) gastric carcinoma often is unfavorable. Among patients with a poor prognosis, a few do survive > 5 years. The authors examined pathologic and biologic features of tumors of long term survivors. METHODS: The authors analyzed data from 442 patients with Stage IV gastric carcinoma, including 20 surviving for > 5 years after gastrectomy (Group A) and 422 who died of gastric carcinoma within 5 years (Group B). Mutant p53 was immunohistochemically stained using the monoclonal antibody PAb1801. Proliferative activity was estimated by argyrophilic nuclear organizer region (AgNOR) staining and proliferating cell nuclear antigen (PCNA) staining. RESULTS: Group A had smaller and more localized tumors than Group B (P < 0.05 and P < 0.01, respectively). Lymphatic or venous invasion and peritoneal dissemination were less frequent in Group A than in Group B (P < 0.01). Abnormalities of p53 expression were found in 3 of the 14 tumors in Group A (21%), a value significantly lower than the 58 of 118 tumors in Group B (49%; P < 0.05). AgNOR count and percentage of PCNA labeling were not significantly different between Groups A and B. A multivariate analysis showed that lymph node dissection, liver metastasis, gastric resection, venous invasion, and tumor size were independent prognostic factors. CONCLUSIONS: Even in patients with Stage IV gastric carcinoma, radical gastrectomy and extensive lymph node dissection can lead to long term survival. The authors believe that combination analysis of pathologic features and p53 overexpression predict length of survival for patients with Stage IV gastric carcinoma.

Evidence of functional lymphocytes in some (leaky) scid mice.

Although the majority of severe combined immune deficiency (scid) mice lack functional lymphocytes, some (2-23%) appear to develop a limited number of B and T cells between 3 and 9 mo old. Most of these leaky scid mice were shown to contain very few clones (less than or equal to 3) of Ig-producing plasmacytes. Clonal progeny were distributed unevenly in the lymphatic tissues and appeared as discrete plasmacytic foci. In many cases, individual clones persisted for several months and produced abnormally high concentrations of Ig that included multiple isotypes. Functional T cells were inferred from the ability of leaky mice to reject allogeneic skin grafts, a T cell-dependent reaction. Interestingly, approximately 40% of leaky mice developed thymic lymphomas. In other respects, leaky mice resembled regular scid mice; e.g., their splenic cells failed to express common lymphocyte antigens (Ly-5[B220], Ly-1) and to proliferate in response to lymphocyte mitogens. Histologically, their lymphoid tissues retained the same general pattern of severe lymphocytic deficiency as scid mice.

Deletion of 14q in non-Hodgkin's lymphoma.

6 patients with non-Hodgkin's lymphoma [3 with small cell lymphocytic lymphoma of B-cell type (SL), and 1 each with follicular centroblastic/centrocytic, centroblastic, and immunoblastic lymphoma] and with the acquired cytogenetic abnormalities del(14) (q22) or del(14) (q24) are described. An evaluation of these 6 cases and 41 other lymphatic neoplasms with 14q deletion known from the literature revealed that 37 had a breakpoint in bands q22 to q24. The deletions occur significantly more often in lymphomas of SL morphology and in the leukemic counterpart, chronic lymphocytic leukemia, than in other types of lymphatic malignancies (p less than 0.001).

[Reactive lymphadenopathy-enlargement-pseudolymphoma--prelymphoma].

In this first of two articles on reactive and lymphomatous lymph-node enlargement, based several own cases an overview of reactive lymphadenopathies is given. A short presentation of the subpopulations of the lymphatic system and its function is followed by tables and case reports illustrating the individual reaction forms. The include abnormal storage, inflammatory and infctious processes, immunological disorders and hamartomatous lesionsl. Certain reactive disease entities which clinically and pathologically closely resemble lymphomas are designated as pseudolymphomas. Prelymphomas include those processes which in part may lead to a malignant lymphoma. Finally, practical aspects of lymphode diagnostics (localization factor, clinical-pathological correlation) are discussed.

Clinical significance of serum carcinoembryonic antigen, carbohydrate antigen 19-9, and squamous cell carcinoma antigen levels in esophageal cancer patients.

Serum carcinoembryonic antigen (CEA), carbohydrate antigen (CA) 19-9, and squamous cell carcinoma (SCC) antigen levels were assessed to determine if their levels are useful for staging esophageal cancer preoperatively and for predicting patient survival after esophagectomy. Hence their seropositivity was investigated for a correlation with resectability, clinicopathologic parameters of tumor progression, and treatment outcomes in patients with unresectable esophageal cancer ( n = 63) and those undergoing esophagectomy for resectable disease ( n = 267). Abnormal elevation of serum SCC antigen levels showed a significant correlation with resectability ( p< 0.0001), depth of tumor invasion ( p < 0.0001), lymph node status ( p = 0.0015), TNM stage ( p < 0.0001), lymphatic invasion ( p = 0.0019), blood vessel invasion ( p = 0.0079), and poor survival after esophagectomy ( p = 0.0061). A significant relation ( p = 0.0145) was found between elevated serum CEA levels and distant metastasis, whereas the seropositivity of CA 19-9 showed no association with resectability, tumor progression, or patient survival. These results indicate that abnormal elevation of serum SCC antigen is a useful predictor of advanced esophageal cancer associated with poor survival after esophagectomy.

Lymphangioleiomyomatosis.

BACKGROUND: Lymphangioleiomyomatosis (LAM) is a rare disease that is characterized by proliferation of abnormal smooth muscle-like cells (LAM cells), which leads to the formation of lung cysts, fluid-filled cystic structures in the axial lymphatics, and abdominal tumors. It primarily affects women. METHODS: The authors present a review of large series, registries, and protocols to highlight the prevalence, pathology, clinical features, diagnosis, and treatment options for patients with LAM. RESULTS: LAM commonly presents with progressive breathlessness or with recurrent pneumothorax, chylothorax, or sudden abdominal hemorrhage. Computed tomography (CT) scans show numerous thin-walled cysts throughout the lungs, abdominal angiomyolipomas, and lymphangioleiomyomas. Pulmonary function tests show decreased forced expiratory volume in 1 second (FEV1) and diffusion capacity for carbon monoxide (DLCO). Exercise testing shows gas-exchange abnormalities, ventilatory limitation, and hypoxemia that may occur with near-normal lung function. CONCLUSIONS: No effective treatment currently exists for this progressive disorder. However, recent progress in cancer and smooth muscle cell biology and a better understanding of the factors regulating angiogenesis and lymphangiogenesis may provide a foundation for the development of new therapeutic strategies.

Fine structure of intrascrotal lymphatic vessels infected by Wuchereria bancrofti adult worms.

Lymphangiectasia represents a basic phenomenon of acute and chronic pathology in lymphatic filariasis, and the prevalence or degree of lymphatic dilation caused by filarial worms is considered an indirect measurement of the altered lymphatic function. We examined the morphological alterations of intrascrotal lymphatic vessels surgically removed from a volunteer infected by adult worms of Wuchereria bancrofti. Scanning electron microscopy revealed lymphatic vessels with an irregular endothelium and adherent flattened lymphocytes and macrophages in variable proportions. On transmission electron microscopy the lymphatic vessels showed a thin endothelium which had an irregular contour and projected several cytoplasmic processes into the lumen. Numerous micropinocytotic vesicles and collagen fibers were abundant and disorganized. The hyperplastic endothelial cells and the subendothelial fibrosis suggest that abnormal changes in these cells may play a crucial role in the development of lymphangiectasia.

[The basal cell nevus syndrome. Case report of a 14-year-old girl].

"Basal cell naevus syndrome" (Gorlin's syndrome) is a sporadic autosomal dominant hereditary precancerous condition which affects several organ systems. The dominating clinical manifestations are multiple basal cell naevi which develop into malignant basocellular carcinoma. Further abnormalities include abnormalities of the vertebrae and ribs, odontogenic keratocysts, calcification of the falx cerebri, a special facial appearance with progeny and macrocephaly. Affected patients may develop also ovarian fibromas, fibrosarcomas, cardiac fibromas, medulloblastomas and meningiomas. Lymphatic and chylous mesenteric cysts are also frequent. In the submitted paper the authors present the case-history of a 14-year-old girl with multiple naevi, histologically specified as solid, superficial and tricho-epithelial basalioma. Phenotypic manifestations, multiple keratocysts, bone abnormalities and calcifications of the falx cerebri which are detected in the girl led to the diagnosis of Gorlin's syndrome. The authors discuss the problem of cytogenetic findings (structural abnormalities, markers of mutagenicity) and possible therapy.
