[The pharmacokinetics of Kefzol in patients with acute pancreatitis when administered intravenously and endolymphatically].

Pharmacokinetics was studied of kefsole administered by intravenous and endolymphatic routes to patients (n = 23) with acute pancreatitis. The studies made showed that intravenous route for the drug administration makes for a quicker entering of the antibiotic into the peritoneal exudate. Apart from these reasons, endolymphatic antibacterial therapy does not appear to avert the development of complications involving pus-formation/discharging in acute pancreatitis and does not seem to be essential in the complex of therapeutic measures to be applied for treating the above patients.

[Endolymphatic introduction of preparations and surgical treatment of patients with wide spread destructive lung tuberculosis].

Comparative analysis of application effectiveness of endolymphatic and conventional ways of preparations introduction at the time of preoperative schedule and the patients treatment with a wide spread destructive lung tuberculosis was conducted an original schemes of endolymphatic therapy application before and early after the operation are adduced. Endolymphatic application of preparations permitted to reduce the postoperative pleural empyema with bronchial fistula occurrence by 20.1%, without bronchial fistula-by 4.8%, the specific process exacerbation-by 8%, operative wound suppuration-by 10.5%, to reduce the general mortality by 18.8%.

[A trial of applying an oily suspension of bleomycin endolymphatically in patients with Hodgkin's disease of the extraperitoneal lymph nodes].

This study summarized results of treatment of the extraperitoneal lymphatic nodes in patients with Hodgkin's disease with infusion of oily form of bleomycin into the lymphatic vessels. Sixty patients aged 17-69 yr. (mean 43 yr.), 24 women and 36 men with Hodgkin's disease staged IIIA-IVB after 6 cycles of chemotherapy with MOPP or MOPP/ABVD were investigated. Three months after the last cycles of chemotherapy neoplastic changes in the extraperitoneal lymphatic nodes were found in 18 patients only. These patients were treated with bleomycin injected within 30 min. in a daily dose of 30 mg up to total dose 60 mg. Total remission was found in 7 patients, in 4 patients the nodes were shrunken and in the remaining 7 patients there was no visible sings of the disease. Serum level of bleomycin was the highest a few minutes after injection (217-680 ng/ml) and was followed rapid decrease. The obtained results have led to the following conclusions: 1. Oily suspension of bleomycin is a method of chose in the treatment of patients with Hodgkin's disease with changes in the extraperitoneal node. 2. High dose of bleomycin applied directly to the lymphatic vessels leads to total or subtotal remission of the nodal changes. 3. The highest serum concentration of bleomycin was found after the injection. 4. The method in painless and safe the patients, fever may occur for a few hours only.

[The reasons for the application of endolymphatic therapy in the treatment of purulent lung diseases, complicated by empyema of the pleura].

Klaforan pharmacodynamics was studied up in pleural exudate and blood serum of the patients with purulent-destructive pulmonary diseases after its single endolymphatic and intravenous introduction in the dose 60 mg/kg of the body mass. Convalescence frequency in control group was 61.6%, in principal-82.9%, lethality-18.6 and 9.3% accordingly.

Mechanisms of acquired thymic tolerance in vivo: intrathymic injection of antigen induces apoptosis of thymocytes and peripheral T cell anergy.

Intrathymic injection of Ag induces Ag-specific tolerance in several clinically relevant experimental autoimmune and transplantation models. However, the exact mechanisms of acquired thymic tolerance in vivo remain unclear. We investigated the mechanisms of acquired thymic tolerance in mice that are transgenic for the TCR specific for peptide 323-339 of OVA. Intrathymic injection of OVA leads to apoptosis of thymocytes starting as early as 3 h after injection and persisting up to 7 days. Double positive thymocytes undergo apoptosis earlier than single positive thymocytes, and significantly higher percentages of double positive thymocytes ultimately die as compared with single positive cells. Apoptotic cells show decreased surface expression of CD4. In the periphery, T cells from intrathymically injected animals had suppressed proliferation and IL-2 production to OVA compared with T cells from control Ag-injected mice. We conclude that intrathymic injection of Ag induces apoptosis of immature thymocytes and a subpopulation of mature thymocytes and induces prolonged anergy in peripheral T cells in vivo. Understanding the mechanisms of acquired thymic tolerance may lead to development of novel clinical strategies to prevent autoimmune disease and transplant rejection.

[Hemostatic therapy of gastroduodenal hemorrhage of non-ulcer genesis by the use of reserve mechanisms of hemostasis].

The article presents summarized results of treatment and examinations of 216 patients with gastro-intestinal bleedings due to acute errosive hemorrhagic gastritis and gastroduodenitis, cancer and polyposis of the stomach, liver cirrhosis and portal hypertension, hemorrhage against the background of atherosclerosis and arterial hypertension by administration of hemostatic medicine into the lymphatic system for activation and reinforcement of the reserve monocyte-macrophage mechanism of hemocoagulation. The results obtained confirm the expedience of this method of influencing the monocyte-macrophage hemostasis, having no complications and giving better medical effects.

[Endolymphatic therapy in the treatment of primary delimited melanoblastoma of the skin].

The results of treatment of 1128 patients with the body and extremities skin melanoma were analyzed. The increase of five-year survival index in patients with the tumor of T1-4 N0M0 stage by 11.4% was promoted by the postoperative adjuvant endolymphatic chemotherapy application. The dacarbasin endolymphatic infusion together with recombinant interferon laferon have permitted to increase the survival index of patients by 15.4%.

[Use of adjuvant therapy in patients with metastasis of melanoma to regional lymph nodes].

The results of treatment of 276 patients with cutaneous melanoma of T1-4N1M0 stage were analyzed. The irradiation therapy conduction did not influence the result of surgical treatment. The trustworthy improvement of three-year survival index of patients after the radical operation conduction was noted under the influence of the endolymphatic cytostatic therapy. The result of preoperative endolymphatic chemotherapy and interferon therapy conduction is quite similar.

[Changes in systemic and local immunity in patients with acute and chronic purulent sinusitis in response to regional lymphotropic immunostimulating therapy].

We studied the effects of regional lymphotropic immunostimulating therapy on alterations in immunoglobulins levels both in blood serum and paranasal sinuses mucosal secretion in patients with acute and chronic purulent sinusitis. Solutions of tactivin, lidase and novocain were administered into the area of the submaxillary lymph nodes. The effect was compared to that obtained after subcutaneous administration of tactivin and conventional therapy. No reliable alterations in systemic humoral immunity in response to lymphotropic immunostimulating therapy was reported in acute sinusitis, while in chronic sinusitis an increase in initially low IgA occurred. Regional lymphotropic administration of tactivin promoted a pronounced decrease in the initially elevated levels of SIgA, IgA and IgG in the secretion of paranasal mucosa in acute purulent sinusitis, and an increase of basaline lowered SIgA, a fall in the IgG concentration in chronic purulent sinusitis. This is indicative of effectiveness of this method of immunostimulator administration on local immunological reactivity of the mucous membrane.

[Recombinant interferon alfa-2b in the treatment of patients with skin melanoma].

An analysis was performed of early and remote results of inauguration in patients with primary-circumscribed skin melanoma of extremities and trunk of adjuvant endolymphatic interferon therapy. The results showed a statistically insignificant tendency to improve as per comparison of indices for 3-yr survival. Comparative analysis of the above indices among male patients of the study group and controls revealed statistically significant augmentation of the survival index following postoperative interferon therapy.

[Endolymphatic therapy of peritonitis].

Effectiveness of lymphological methods of treatment (LT) of endogenic intoxication (EI) developed by the author (endolymphatic anticoagulatory, improving rheology and microcirculation, antibacterial, detoxicating, enzyme-inhibiting, myo- and lymphostimulating, immunomodulating and hepatotropic) was studied in 210 patients with peritonitis divided into two groups of 105 patients each: in the control group a conventional postoperative treatment was carried out, in the study group--LT. Toxicity of the blood was determined by the level of medium-size molecules and leukocytic index of intoxication on postoperative day 1, 3, 5, 7 and 14. It was established that LT which creates conditions for simultaneous sanation of blood and lymphatic bed, interstitial space, the inflammation focus promotes total detoxication of the body with almost complete elimination of EI on postoperative day 5-7. In conventional treatment EI persisted for 7-14 days after operation.

[Regional lymphotropic chemotherapy in breast cancer--a new approach in the therapeutic procedure for the parasternal lymph nodes].

A method of regional lymphotropic chemotherapy in breast cancer is presented, aimed at attaining higher cytostatic concentration in the region of parasternal lymph nodes. In twenty-three women with centrally and medially situated tumors preoperatively 1 mg Mitoxantrone is applied around the 4 tumor poles each. In ten cases (IIb and IIIa stage) given 3 courses of nonadjuvant chemotherapy, part of the Cyclophosphamide dose is administered as regionally lymphotropic subxiphoid bilaterally. The clinical response, distribution of the Mitoxantrone blue stained regional lymph nodes and the morphological changes in the latter are analyzed. The early clinical observations indicate that the method proposed is technically easily executed, with a good local tolerance, and has a definite place in the complex treatment of breast cancer.

[Results of application of endolymphatic polychemotherapy and local supra-high frequency hyperthermia in comprehensive treatment of rectal cancer].

The treatment efficacy of a widespread cancer of recti using neoadjuvant endolymphatic polychemotherapy and local suprahigh frequency hyperthermy in a radically operated patients was studied up. In patients without regional metastases the five-year survival index have increased from 63 to 81%, and the recurrencies occurrence frequency have lowered from 20 to 10%. The method proposedhas small efficacy in patients with metastases in regional lymph nodes.

[Prophylaxis of purulent complications in operations on large joints by endolymphatic infusion of drugs].

2 groups of patients operated on knee joints for traumatic injuries were examined. In the study group endolymphatic drug therapy was carried out in postoperative period, in control group of patients conventional way of intramuscular administration of preparations was used. During the treatment, plasmatic cells, macrophages and tissue basophiles in synovial liquor were studied as well as synthesis of nucleic acids (DNA and RNA) in plasmatic cells. It was shown that immune homeostasis of synovial liquor in postoperative period normalized faster in endolymphatic administration. The studies of tissue basophiles have shown that inflammatory process in the knee joint is completed faster when endolymphatic route of drug administration was used. Clinical results evidence of higher effectiveness of endolymphatic therapy in comparison with conventional method of treatment in postoperative period. In the former case the inflammatory process was completed by the 5th day, in the latter--by the 7th-9th day. The endolymphatic therapy in all the patients resulted in healing of operation wounds by primary intention. In the group of patients treated by standard methods, suppuration of the wound was detected in 6 from 27 patients and postoperative synovitis in 7-9 days was observed in 6 patients.

Acquired thymic tolerance: role of CTLA4 in the initiation and maintenance of tolerance in a clinically relevant autoimmune disease model.

Injection of Ag into the thymus of adult animals induces specific systemic tolerance. The mechanisms of acquired thymic tolerance include anergy and the deletion of Ag-specific T cells. Here, we report that anergy to nominal Ag induced via acquired thymic tolerance requires CTL-associated Ag 4 (CTLA4) engagement. The role of CTLA4 in the induction and maintenance of tolerance was then investigated in the murine experimental autoimmune encephalomyelitis model. CTLA4 blockade abrogated the induction but not the maintenance phase of acquired thymic tolerance induced by intrathymic injection of myelin Ags. In addition, CTLA4 blockade had a restricted window of action after priming with Ag, which is consistent with the expression patterns of CTLA4 in vivo. We conclude that: 1) the induction of acquired thymic tolerance requires signaling through CTLA4 and 2) CTLA4 does not appear to be required for the maintenance of acquired thymic tolerance. This is the first report documenting the role of a CTLA4 negative signaling pathway in the induction of tolerance in an autoimmune disease model.

Persistence of autoreactive T cell drive is required to elicit anti-chromatin antibodies in a murine model of drug-induced lupus.

Long-term treatment with procainamide and numerous other medications is occasionally associated with the development of drug-induced lupus. We recently established a murine model for this syndrome by disrupting central T cell tolerance. Two intrathymic injections of procainamide-hydroxylamine (PAHA), a reactive metabolite of procainamide, into (C57BL/6 x DBA/2)F1 mice resulted in the appearance of chromatin-reactive T cells and anti-chromatin autoantibodies. The current study explores in this model the role of autoreactive T cells in autoantibody production and examines why autoantibodies after a single intrathymic drug injection were much more limited in isotype and specificity. Injection of as few as 5000 chromatin-reactive T cells into naive, syngeneic mice induced a rapid IgM anti-denatured DNA response, while injection of at least 100-fold greater number of activated T cells was required for induction of IgG anti-chromatin Abs, suggesting that small numbers of autoreactive T cells can be homeostatically controlled. Mice subjected to a single intrathymic PAHA injection after receiving splenic B cells from an intrathymic PAHA-injected syngeneic donor also developed anti-chromatin Abs, but adoptive transfer of similarly primed T cells or of B cells without intrathymic PAHA injection of the recipient failed to produce an anti-chromatin response. However, anti-chromatin Abs developed after a single intrathymic PAHA injection in Fas-deficient C57BL/6-lpr/lpr mice, suggesting that activation-induced cell death limited autoimmunity in normal mice. Taken together, these results imply that chromatin-reactive T cells produced by intrathymic PAHA created a B cell population primed to somatically mutate and Ig class switch when subjected to a heavy load or second wave of autoreactive T cells.
